Relative Hazard Losses Along The Atlantic And Gulf Coasts, Erin Kemp Jul 2025

Relative Hazard Losses Along The Atlantic And Gulf Coasts, Erin Kemp

Theses and Dissertations

Coastal areas face a variety of natural hazards, many of which are increasing in frequency or severity due to climate change. Continued coastal development emphasizes the need for a better understanding of the impact of these hazards on coastal communities. This study aims to improve this understanding by investigating temporal, spatial, and causal patterns in county-level relative losses (RL) from hazards along the Atlantic and Gulf Coasts from 2001 to 2021. Three research questions are addressed: 1) Are relative losses increasing over time in coastal counties? 2) Where are the highest relative losses among coastal counties? and 3) Which hazards …

A Spatial Scan Statistic For Group Testing Data, Vincent Onyame Jul 2025

A Spatial Scan Statistic For Group Testing Data, Vincent Onyame

Theses and Dissertations

Group testing involves pooling specimens from multiple individuals and offers an efficient means to surveil low-prevalence pathogens, but poses challenges for spatial cluster detection when only pooled results are observed. In this thesis, we develop a spatial scan statistic tailored to group-testing data with variable pool sizes. The statistic compares a null hypothesis of a homogeneous infection rate across all clusters to an alternative hypothesis that infection probabilities differ inside and outside a candidate cluster, with both models fitted by maximum likelihood estimation. We approximate the null distribution of the maximum likelihood ratio test via Monte Carlo simulation.

Patients with recurrent/metastatic (r/m) head and neck squamous cell carcinoma (HNSCC) have limited treatment options and a dismal prognosis, especially when their cancer is resistant to standard treatments like anti-programmed cell death protein 1 and platinum-based therapies. Petosemtamab - a human, common light chain, bispecific antibody with enhanced antibody-dependent cellular cytotoxicity targeting epidermal growth factor receptor (EGFR) and leucine-rich repeat-containing G-protein coupled receptor 5 (LGR5) - demonstrated antitumor activity in r/m HNSCC. In many tumor types, including HNSCC, EGFR is an oncogenic driver, while LGR5 is upregulated. LGR5 can potentiate the wingless-type integration site (WNT)/β-catenin signaling pathway in response to …

Faculty, Staff and Student Publications

Background: Encephalitis is characterized by brain parenchymal inflammation caused by infection or autoimmunity. There are limited recent data on how immunocompromised patients with encephalitis differ from the general encephalitis population.

Methods: This retrospective study of 2 large medical institutions compares clinical characteristics and outcomes of immunocompromised and immunocompetent patients with all-cause encephalitis.

Results: Of the 657 patients, 151 (23%) were immunocompromised. Immunocompromised patients were more likely to have an infectious etiology, comorbidities, inflammatory cerebrospinal fluid (CSF) profile, abnormal neuroimaging, and worse clinical outcomes as assessed by discharge Glasgow Outcome Scale (GOS) and in-hospital mortality (all P < .05). The most commonly identified etiologies in immunocompromised patients were herpes simplex virus (HSV) and varicella zoster virus. HSV accounted for similar proportions in the immunocompromised (18%) and immunocompetent (14%) groups, though it was more commonly associated with a CSF neutrophilia in the immunocompromised group (P = .001). Strikingly, …

Empirical Tuberculosis Treatment In Human Immunodeficiency Virus (Hiv)-Associated Fever Of Unknown Origin: A Case-Based Rationale, Matthew Antonioli, Samantha H Antonioli, Gabriel M Aisenberg

Faculty, Staff and Student Publications

Fever of unknown origin (FUO) in people living with human immunodeficiency virus (PLHIV) is clinically defined as recurrent fever lasting more than four weeks in the outpatient setting or more than three days during hospitalization, despite a thorough diagnostic evaluation. This evaluation typically includes a comprehensive medical history, physical examination, imaging studies (such as chest radiography), and an extensive range of laboratory tests, including complete blood counts, blood and urine cultures, and metabolic panels. Among the many possible causes, tuberculosis (TB) stands out as a leading concern, given its disproportionate burden in PLHIV. However, whether to initiate empirical anti-tubercular therapy …

Wild Hogs In The Water: Contested Infrastructural Ecologies Of Reservoir Storage In Texas, Sayd Randle Jul 2025

Wild Hogs In The Water: Contested Infrastructural Ecologies Of Reservoir Storage In Texas, Sayd Randle

Research Collection College of Integrative Studies

Reservoirs are developed to store water in reserve for future use. But once built, reservoir sites inevitably hold more than just water, often serving as a key habitat for a range of species. This paper examines how one such animal has transformed water storage facilities and nearby landscapes into contested ground in urbanising areas of Texas, USA. Living around the reservoirs, feral hogs complicate the process of urbanisation by degrading the stockpiled water and infrastructure at the storage sites themselves and by damaging private property throughout the surrounding landscape. Tracking local efforts to manage the hogs, the case study illustrates …

Stockpiling, Forecasting And Protecting Skills: Skilling As Technique Of Preparedness In Times Of Crises, Rebecca Ye, Yasmin Y. Ortiga

Research Collection School of Social Sciences

In this comparative and situational analysis, we examine how nation-states use skilling as a technique of preparedness during crisis moments. By analysing two strategically selected country cases, Singapore and Sweden, during a critical phase of a global pandemic, we identify three techniques: stockpiling, forecasting, and protecting skills, and how these were enacted as a means to perform preparedness. Stockpiling skills was observed to take place as a rapid response; forecasting skills was a technique enacted with a view on the longer-term horizon and protecting skills was mobilised as a short-term future technique, where boundaries were assembled around skilling. While there …

Against The Phrase "Aggressive Care", Trevor M Bibler

Center for Medical Ethics and Health Policy Staff Publications

Language is the primary technology clinical ethicists use as they offer guidance about norms. Like any other piece of technology, to use the technology well requires attention, intention, skill, and knowledge. Word choice becomes a matter of professional practice. The Brief Report offers clinical ethicists several reasons for rejecting the phrase "aggressive care." Instead, ethicists should consider replacing "aggressive care" with the adjacent concept of a "recovery-focused path." The virtues of this neologism include: the opportunity to set aside the emotion of "aggression," the phrase's accuracy when capturing the intention of the patient or their representative, and an unappreciated rhetorical …

BACKGROUND: There is no prospective, US multicenter study of COVID-19-associated pulmonary aspergillosis (CAPA). CAPA definitions do not differentiate invasive aspergillosis (IPA) from colonization. Validity of single mycologic test results is unclear.

METHODS: We performed a prospective 7-center US study of mechanically ventilated adults with COVID-19 (April 2021-May 2022). Mycoses Study Group (MSGERC) CAPA criteria include host and clinical factors, imaging and test results (histopathology; bronchoalveolar lavage [BAL] culture and/or BAL or serum galactomannan-immunoassay). Proven, putative, and unlikely IPA were defined by clinical criteria. CAPA-unlikely IPA criteria included survival or negative autopsy following no/limited antifungal treatment. IPA likelihood was estimated using …

UNLABELLED: The genetic subtypes of Burkitt lymphoma have been defined, but the role of epigenetics remains to be comprehensively characterized. We searched genomic DNA from 218 patients across four continents for recurrent DNA methylation patterns and their associations with clinical and molecular features. We identified DNA methylation patterns that were not fully explained by the Epstein-Barr virus status or mutation status, leading to two epitypes described here as HypoBL and HyperBL. Each is characterized by distinct genomic and clinical features including global methylation, mutation burden, aberrant somatic hypermutation, and survival outcomes. Methylation, gene expression, and mutational differences between the epitypes …

IMPORTANCE: Hospital-onset bacteremia and fungemia (HOB) are common and potentially preventable complications of hospital care.

OBJECTIVE: To assess whether nonindividual patient features, which summarize interactions with other patients and health care workers (HCWs), can contribute to predictive and causal machine learning models for HOB.

DESIGN, SETTING, AND PARTICIPANTS: This prognostic study included adult patients admitted to Barnes-Jewish Hospital, an academic hospital in St Louis, Missouri, in 2021. Analyses were developed between October 2023 and August 2024 and in April 2025.

EXPOSURE: Individual patient features were extracted from electronic health records and used to engineer nonpatient features, including interactions with HCWs …
