This explanatory qualitative case study applied the Advocacy Coalition Framework (ACF) to study Detroit’s local food policy changes from 2000 to 2022. The research question and sub-questions were framed using three ACF hypotheses that addressed advocacy coalitions, collective action, and policy change. My research showed how actors in the policy subsystem worked to promote policy change during the study period. Using this Advocacy Coalition Framework approach, I explored local food policy change in the context of community resilience and system changes. The study of refugee psychology and suicidal distress is a comparatively nascent field when compared with the body of research concerning non-refugee populations. Over the past fifty years, however, the post-migration refugee experience has been the subject of growing clinical interest. In particular, the construct of post-migration living distress (PMLD) has been identified as a salient factor contributing to psychological distress in refugee populations. This dissertation explores the relationship between PMLD and suicidality in a sample of refugees resettled in a New England city. This study aimed to explore the stories of Black women in professional leadership roles. Grounded in a Black feminist perspective (Crenshaw, 1989) and informed by historical and intergenerational trauma (Brave Heart, 1999), this study utilized Black storytelling as a methodological framework and employed thematic analysis to examine the narratives of participants. A total of nine Black women in leadership positions across various industries participated in one-on-one interviews. Objective: National Guard post-deployment and reintegration processes leave servicemembers with a unique urgency to return to their families, homes, and civilian lives due to the timing and location of the post-deployment Transition Assistance Program (TAP); consequently, National Guard members have reported “checking the boxes” on mental health screeners to prioritize a quick and smooth transition (Salahi, 2023). Although researchers have called for data on the experiences of National Guard members undergoing post-deployment TAP since 2005 (Bascetta), no research to date has answered this call. This study explores the lived experiences of clinical supervisors who engage in somatically oriented supervision. Informed by relational-cultural theory and polyvagal theory, the research employs a grounded theory methodology to investigate the potential development of a somatic supervision framework. Eleven participants completed semi-structured interviews and data were analyzed using both thematic analysis and grounded theory techniques. Female veterans are currently the fastest growing population of veterans in the United States (U.S. Department of Veterans Affairs, 2022) and have a higher prevalence of posttraumatic stress disorder (PTSD) than male veterans (Na et al., 2021), yet there has been limited research done on this population. Females have also been found to experience more posttraumatic growth (PTG) than males (Kalaitzaki, 2021; Prieto-Ursa & Jordar, 2020; Yildiz, 2021). This dissertation explored the lived experiences of psychological trauma in adults diagnosed with Attention-Deficit/Hyperactivity Disorder (ADHD). Using an Interpretative Phenomenological Analysis (IPA) methodology, seven participants exhibiting clinically significant symptoms of psychological trauma completed semi-structured interviews to share their personal narratives. Following analysis, data was consolidated into five group experiential themes: (a) patterns of negative relationships, (b) dysfunctional family dynamics, (c) recognition of role in interpersonal conflict, (d) validation through ADHD diagnosis, and (e) ADHD and trauma as intertwined. Research investigating the bicultural and mental health experiences of Filipino Americans, the third largest Asian American group in the United States, are consistently overlooked, with second-generation Filipino Americans (SGFAs) facing greater risk of adverse mental health outcomes. This quantitative study addresses a critical gap in research by examining the relationship between bicultural stress and mental well-being among SGFAs living in the United States. Using a cross-sectional survey design, a multivariate regression analysis examined the relationship between bicultural stress and four mental health constructs (as measured by depression, anxiety, stress, and self-esteem) among 302 SGFA participants. The purpose of this study was to describe the experience of licensed psychologists who provided unexpected feedback to assessment clients about not meeting the criteria for neurodevelopmental disorders. Individuals are increasingly seeking formal evaluation for neurodevelopmental disorders based on anecdotal information they come across on the internet. Many people develop strong identity attachments to these diagnostic labels, and it can be incredibly dysregulating to learn that they do not meet the criteria for a diagnosis. This dissertation builds on historical and scientific assertions that Former Soviet Union immigrants survived distinct political traumas, resulting in unique, culturally adaptive behaviors and beliefs. Using constructivist grounded theory, it examined how these behaviors manifest in the evangelical Slavic community (ESC) of former Soviet Union Refugees (FSURs) in the U.S. The researcher asserts that (a) adaptations are generationally preserved in the ESC through a “cycle of authoritarianism,” and (b) survival strategies became maladaptive in new social environments. Narratives from eight young adults raised in the ESC were analyzed alongside a literature review and lived experience. Findings show that coping strategies …

While subthreshold depression (subD) is relatively young for a diagnostic concept at a mere half century old, it has burdened individuals and societies for far longer. Research has established that it is a highly prevalent, clinically important entity with clear links to personal distress and impairment (including increased suicidal ideation and behavior), societal burden (e.g., increased reliance on public assistance programs), and economic costs (both personal and public). This qualitative study, utilizing an interpretative phenomenological analysis methodology, elucidates the experience of Latina and Hispanic women living in the United States who have volitionally undergone sterilization or have sought the procedure out for birth control. Understanding the rich, contextual, lived experience of Latina and Hispanic women through an intersectional framework highlights how cultural influences and contemporary policies on reproductive rights informed their personal choice to undergo a female sterilization procedure. This phenomenological study examines the literacy experiences of English-speaking Caribbean (ESC) immigrant students in public schools in New York City, as well as the structural factors surrounding their experiences and the culturally sensitive solutions to these experiences. Although in the English-speaking Caribbean context students often use English, many of them also speak Creole or a dialect. They are linguistic forms, including Jamaican, Trinidadian, Guyanese, among other Creoles, that are frequently misinterpreted or underestimated in formal education systems, resulting in confusion and a lack of appropriate instructional support. BACKGROUND: Peripheral arterial disease (PAD) is prevalent and associated with significant cardiovascular (CV) mortality. However, it remains under-recognized, particularly in Asian countries. This study aims to assess awareness of PAD, its risk factors, and perceived consequences in relation to other CV diseases to inform strategies for improving clinical care.
METHODS: A total of 1,008 adults attending outpatient clinics at a university-affiliated tertiary hospital were recruited between January-August 2020 for a phone questionnaire. Leadless pacemaker implantation offers a safe and effective option for patients with limited venous access or high bleeding risk. The procedure usually necessitates insertion of a large-bore sheath over the femoral vein. We report a case of successful MicraTM implantation via transjugular approach in a patient with inferior vena cava obstruction.

Background
This study aims at reviewing the use of the two SGLT2 inhibitors, dapagliflozin and empagliflozin, in a local hospital, Caritas Medical Centre, in terms of efficacy and safety.
Methods
This study was a retrospective, single-centered review carried out in Caritas Medical Centre. 143 adult patients who were prescribed with one of the two SGLT2 inhibitors available in CMC, between 1 Mar 2021 and 1 Mar 2022 were included in the study. The inclusive study duration were 12 months pre- and post- SGLT2 inhibitor initiation.

Background
Primary prevention of stroke using oral anticoagulants (OACs) is indicated for clinical atrial fibrillation (AF) documented on standard electrocardiogram (ECG) in those patients with a high risk of stroke. The recent availability of both external ECG monitors (either using consumer electronic or medical devices) and implantable devices have enabled earlier AF episodes to be detected before clinical AF become manifest. These episodes, often termed subclinical AF (SCAF), represent an area of controversy for the application of OAC.

Will Conduction System Pacing Replace Biventricular Pacing For Treating Heart Failure In Patients With Wide Qrs Duration?, Chan Yat Sun Joseph

Conduction system pacing CSP) is an emerging therapy for resynchronization of patients with heart failure and wide QRS duration. It has been proposed to replace cardiac resynchronization therapy with biventricular pacing (CRT with BVP) mainly because of shortcoming of CRT with BVP including failure to implant coronary sinus leads and unsatisfactory response in certain subset of heart failure patients. The current evidence for CSP in heart failure patients with wide QRS duration is reviewed and the gap in evidence in supporting CSP to replace CRT with BVP is discussed.

Introduction: Blood pressure variability, in addition to blood pressure itself, has been used as a predictor for mortality. This study examined the predictive power of baseline/latest/mean/median blood pressure and blood pressure variability measures for all-cause mortality and adverse cardiovascular outcomes.
Methods: The retrospective observational study analyzed patients who presented to family medicine clinics between 1st January, 2000 and 31st December, 2001. Double Outlet Right Ventricle (DORV) is a rare cyanotic congenital heart defect often associated with polycythemia and bleeding risk. A 7-year-old boy with uncorrected DORV presented with fever, melena, and shock. He was cyanotic, tachypneic, and tachycardic, with SpO₂ of 68% and a harsh ejection systolic murmur. Labs showed anemia, thrombocytopenia, leukocytosis, and prolonged APTT. Tubex TF IgM was strongly positive. A diagnosis of typhoid fever with gastrointestinal bleeding and uncorrected DORV was made. Management included oxygen, inotropes, and fluid resuscitation. Propranolol was withheld. Severe infection with underlying cyanotic heart disease and coagulopathy presents a critical management dilemma.

Background
Currently there are two strategies in Percutaneous Coronary Intervention (PCI) of the true left main (LM) bifurcation lesions: provisional stenting and two-stent technique. The optimal treatment strategy for true LM bifurcation lesions is undetermined. In addition, the percentage of use of imaging-guiding in these studies was low. We would like to undertake a retrospective cohort study to compare the clinical outcomes of provisional stenting versus two-stent technique in treating the true LM bifurcation lesions with the help of intravascular imaging.

An Unusual Cause Of Acute Constrictive Pericarditis, Yue Hin Tang, Raymond Chi Yan Fung

Constrictive pericarditis, a condition where the pericardium thickens and constricts, has long been considered as a chronic sequela of prior insults to the pericardium such as heart surgery, repeated pericarditis, tuberculosis infection, radiation exposure, etc. Susceptible populations are thus expected to be middle-aged.
We present a case in which a young gentleman was diagnosed with acute constrictive pericarditis due to Adult-onset Still’s disease with macrophage activation syndrome. This case illustrated that constrictive pericarditis could happen in acute settings and thus the importance of cardiologists to maintain open-minded in search of a diagnosis
