Blood lead levels are routinely measured in pediatric clinics due to the well-documented detrimental effects of lead exposure on neurodevelopment and consequently cognition. Reading difficulties, impaired detailed movement, and depressed reaction time are three areas, among others, implicated in lead toxicity (Needleman, 1990). Furthermore, lead toxicity can be of particular concern in pregnant women, as placental transmission is viable (Gundacker & Hengstschläger, 2012, as cited in Sun et al., 2018). Food allergies among children are highly prevalent. The elimination diet and fear of allergic reactions cause anxiety in many children and their parents. Immunotherapy stimulates the production of Treg cells which inhibit IgE-mediated allergic reactions. This can successfully desensitize children to their allergens. Subcutaneous immunotherapy can be used for environmental allergens while sublingual or oral immunotherapy should be used for food allergies. There are some risks associated with oral immunotherapy and research is ongoing to study the safety and efficacy of oral immunotherapy.

ความรอบรู้สุขภาพด้านอาชีวอนามัยของช่างซ่อมบำรุงอากาศยานของหน่วยซ่อมบำรุงอากาศยานแห่งหนึ่งในประเทศไทย, ปุญญิศา ผลพันธิน

การศึกษาแบบภาคตัดขวาง มีวัตถุประสงค์เพื่อศึกษาความรอบรู้สุขภาพด้านอาชีวอนามัยและปัจจัยที่เกี่ยวข้องของช่างซ่อมบำรุงอากาศยานของหน่วยซ่อมบำรุงอากาศยานแห่งหนึ่งในประเทศไทย กลุ่มตัวอย่างคือช่างซ่อมบำรุงอากาศยานจำนวน 706 คนโดยไม่มีการสุ่มตัวอย่าง เก็บข้อมูลด้วยแบบสอบถามชนิดตอบด้วยตนเอง วิเคราะห์ข้อมูลด้วยสถิติถดถอยพหุคูณ ผลการศึกษา พบว่ากลุ่มตัวอย่างมีคะแนนความรอบรู้ด้านอาชีวอนามัยเฉลี่ยอยู่ที่ 143.56±10.54 คะแนน จัดอยู่ในระดับมาก ปัจจัยที่มีความสัมพันธ์กับความรอบรู้ด้านอาชีวอนามัยที่ดี ได้แก่ เพศชาย เมื่อเปรียบเทียบกับเพศหญิง (β= - 3.52; 95%CI: -6.19 ถึง -0.85) อายุงานมากกว่า 11 ปีขึ้นไป เมื่อเปรียบเทียบกับอายุงาน 11-20 ปี (β= 9.68; 95%CI: 8.15 ถึง 11.21) อายุงาน 21-30 ปี (β= 15.45; 95%CI: 13.73 ถึง 17.16) และ และอายุงานมากกว่า 31 ปี (β= 19.43; 95%CI: 17.77 ถึง 21.09) แผนกโรงซ่อม (Hangar) เมื่อเปรียบเทียบกับแผนก Shop maintenance (β= - 4.34; 95%CI: -6.53 ถึง -2.15) และแผนกลานจอด (β= -9.17; 95%CI: -10.59 ถึง -7.76) และการรับทราบนโยบายด้านอาชีวอนามัยและความปลอดภัย เมื่อเปรียบเทียบกับการไม่รับทราบนโยบายฯ (β= -2.78; 95%CI: -5.50 ถึง -0.06) (p < 0.05) จากผลการศึกษา การสร้างเสริมความรอบรู้ด้านอาชีวอนามัยให้กลุ่มช่างซ่อมบำรุงอากาศยานควรเน้นทักษะการรู้เท่าทันสื่อ รวมถึงการประชาสัมพันธ์ถึงนโยบายให้ช่างซ่อมบำรุงอากาศยานรับทราบและมีความเข้าใจเนื้อหาและแนวทางการปฏิบัติที่ตรงกัน เพื่อเพิ่มระดับความรอบรู้ด้านอาชีวอนามัยให้มากขึ้นไปอีก

บทนำ: โรคไตเรื้อรังเป็นปัจจัยที่ทำให้เกิดโรคหลอดเลือดหัวใจ โดยความเสี่ยงของการเสียชีวิตจากโรคหลอดเลือดหัวใจจะเพิ่มขึ้นตามอัตราการกรองไต (eGFR) ที่ลดลง โดยเฉพาะอย่างยิ่งในโรคไตเรื้อรังระยะสุดท้าย การปลูกถ่ายไตเป็นการรักษาที่ดีที่สุดและช่วยลดโอกาสการเกิดโรคหัวใจและหลอดเลือดเมื่อเทียบกับผู้ป่วยที่ยังฟอกไตอยู่ อย่างไรก็ตามอัตราการเกิดโรคหัวใจและหลอดเลือด โดยเฉพาะภาวะหลอดเลือดหัวใจเฉียบพลันในผู้ป่วยปลูกถ่ายไตยังคงสูงกว่าเมื่อเทียบกับประชากรปกติ และเป็นสาเหตุการเสียชีวิตอันดับหนึ่งหลังการปลูกถ่ายไต วัตถุประสงค์: เพื่อศึกษาปัจจัยทำนายการเกิดภาวะหลอดเลือดหัวใจเฉียบพลันหลังการปลูกถ่ายไต วิธีการศึกษา: การศึกษาจากข้อมูลย้อนหลังในผู้ป่วยที่ได้รับการปลูกถ่ายไตที่โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ระหว่าง วันที่ 1 มกราคม พ.ศ.2561 ถึง 31 ธันวาคม พ.ศ.2566 จำนวน 250 ราย โดยเก็บข้อมูลจากเวชระเบียนของผู้ป่วย ฐานข้อมูลของคลินิกปลูกถ่ายไต และในผู้ป่วยปลูกถ่ายไตที่เกิดภาวะหลอดเลือดหัวใจเฉียบพลันจะมีการเก็บข้อมูลเพิ่มเติมจากฐานข้อมูลผู้ป่วยห้องปฏิบัติการสำหรับการตรวจสวนหัวใจและหลอดเลือด ผลการศึกษา: ผู้ป่วยปลูกถ่ายไตจำนวน 250 ราย พบผู้ป่วยเกิดภาวะหลอดเลือดหัวใจเฉียบพลัน จำนวน 12 ราย (ร้อยละ 4.8) จากการศึกษาพบปัจจัยทางคลินิกที่สัมพันธ์กับภาวะหลอดเลือดหัวใจเฉียบพลันอย่างมีนัยสำคัญ ได้แก่ ประวัติโรคหลอดเลือดหัวใจ (adjusted OR=8.71, 95%CI 1.45-52.42, p=0.018), ระดับน้ำตาลในเลือด (adjusted OR=1.03, 95%CI 1.01-1.05, p=0.001) และอัตราการกรองของไต (eGFR) (adjusted OR0.94, 95%CI 0.93-0.98, p=0.002) ลักษณะของผู้ป่วยที่เกิดภาวะหลอดเลือดหัวใจเฉียบพลันหลังการปลูกถ่ายไตเป็น NSTEMI ร้อยละ 66.7 และ STEMI ร้อยละ 33.3 โดยผู้ป่วยร้อยละ 66.7 มีโรคหลอดเลือดหัวใจตีบตั้งแต่ 2 เส้นขึ้นไป (multivessel disease) ข้อมูลการรักษาเกี่ยวกับการเปิดเส้นเลือดหัวใจ (revascularization) พบว่าเป็น PCI ร้อยละ 66.7 CABG ร้อยละ 25.0 และการรักษาแบบอนุรักษ์ (conservative management) พบร้อยละ 8.3 สรุปผลการศึกษา: ผู้ป่วยปลูกถ่ายไตโดยเฉพาะอย่างยิ่งผู้ป่วยที่มีประวัติโรคหลอดเลือดหัวใจเดิม มีอัตราการกรองของไต (eGFR) ที่ต่ำ และภาวะน้ำตาลในเลือดสูง มีความเสี่ยงต่อการเกิดภาวะหลอดเลือดหัวใจเฉียบพลันหลังการปลูกถ่ายไต

Background: The dysbiosis of the gastric microbiota has been implicated in gastric carcinogenesis. However, previous studies showed a large heterogeneity of the gastric microbiota profiles in various sampling methods which impacted the diagnostic yield. Currently, the specific method for assessing non-Helicobacter pylori microbiota associated with gastric cancer remain unidentified. Cytokine Levels In Malignant Pleural Effusion: Comparison Between Expandable And Non-Expandable Lung, Jarudul Phocharoen

Background: Lung expandability is crucial for successful pleurodesis in malignant pleural effusion (MPE); however, pleurodesis failure can occur despite meeting expandability criteria. Objectives: To study pleural fluid cytokines as potential biomarkers for predicting lung expandability and pleurodesis outcomes in MPE. Methods: Sixty-eight patients with MPE were enrolled and classified into expandable (n = 34; pleural elastance ≤ 14.5 cmH2O/L or pleural apposition ≥ 75%) and non-expandable (n = 34; pleural elastance > 14.5 cmH2O/L and pleural apposition < 75%) lung groups. Pleural fluid cytokine levels were measured using multiplex assays and compared. Pleurodesis outcomes were assessed in the expandable lung group, and the diagnostic performance of these cytokines in predicting pleurodesis success was evaluated. Results: Pleural fluid cytokine levels, including transforming growth factor-β1 (TGF-β1), did not significantly differ between patients with expandable and non-expandable lungs, nor did they correlate with pleural elastance. However, within the expandable lung group, where 25% of patients experienced pleurodesis failure, TGF-β1 levels were significantly higher in the failure group (median [IQR]: 5,779.83 [3,924.54–9,705.53]) than in the success group (2,598.93 [1,425.35–3,294.35]; p = 0.003). A TGF-β1 cutoff of < 3,743.04 pg/mL demonstrated 90.48% sensitivity and 85.71% specificity for predicting pleurodesis success. Conclusion: Pleural fluid TGF-β1 may predict pleurodesis success in expandable lung, but its limited correlation with lung expandability and pleural elastance suggests complex biological and mechanical interactions.

Cone beam computed tomography (CBCT) is widely used as the primary imaging method in image-guided radiation therapy. The Varian Halcyon system (Varian Medical Systems, Palo Alto, CA, USA) requires daily imaging prior to each patient’s treatment. This study focuses on comparing the radiation doses to normal organs across all available kV-CBCT protocols on the Varian Halcyon system. Radiophotoluminescent glass dosimeters (RPLGD) were used to measure doses, placed inside an Alderson Rando phantom positioned at specific anatomical sites including the head, thorax, breast, and pelvis. Background : ภาวะอ้วนเป็นสาเหตุสำคัญและมีความสัมพันธ์กับโรคทางเมตาบอลิกรวมทั้งโรคหัวใจและหลอดเลือด และจำเป็นต้องได้รับการผ่าตัดกระเพาะอาหารเพื่อลดน้ำหนักในกรณีที่ไม่สามารถลดน้ำหนักได้ด้วยการปรับพฤติกรรม และการใช้ยาลดน้ำหนักโดยเฉพาะในผู้ป่วยโรคอ้วนรุนแรง โกรทดิฟเฟอเรนชิเอชันแฟกเตอร์ 15 (GDF-15) และไฟโบรบลาสต์โกรทแฟกเตอร์ 21 (FGF-21) มีส่วนเกี่ยวข้องกับการควบคุมความอยากอาหารและเมตาบอลิซึม โดยมีระดับสูงขึ้นในภาวะอ้วน การศึกษานี้มีวัตถุประสงค์เพื่อประเมินการเปลี่ยนแปลงของระดับ GDF-15 และ FGF-21 ที่ 1, 3 และ 6 เดือนหลังการผ่าตัดกระเพาะอาหาร และความสัมพันธ์กับการลดน้ำหนัก การควบคุมระดับน้ำตาลในเลือด และการหายของโรคเบาหวาน Method : การศึกษานี้เป็นการศึกษาติดตามไปข้างหน้าในผู้ป่วยโรคอ้วนรุนแรงอายุ 18–70 ปี (ดัชนีมวลกาย ≥ 30 กิโลกรัม/ตร.ม.) ที่เข้ารับการผ่าตัดกระเพาะอาหารเพื่อลดน้ำหนักในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ตั้งแต่เดือนกุมภาพันธ์ พ.ศ. 2561 ถึงมิถุนายน พ.ศ. 2567 ได้รับการเจาะเก็บเลือดเพื่อตรวจระดับพลาสมาของ GDF-15 และ FGF-21 ร่วมกับพารามิเตอร์เมตาบอลิกอื่นๆ รวมไปถึงน้ำหนักและดัชนีมวลกายเพื่อประเมินการเปลี่ยนแปลงของฮอร์โมน เมตาบอลิกและน้ำหนักที่เปลี่ยนแปลงระหว่างก่อนการผ่าตัด และที่ 1, 3 และ 6 เดือนหลังการผ่าตัดกระเพาะอาหารเพื่อลดน้ำหนัก Result: ในผู้ป่วย 47 รายที่เข้ารับการผ่าตัดกระเพาะอาหารเพื่อลดน้ำหนัก ระดับ GDF-15 ที่หลังผ่าตัดกระเพาะอาหารเพื่อลดน้ำหนักที่ 6 เดือนไม่มีความแตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับก้อนผ่าตัด แต่มีเพิ่มขึ้นอย่างมีนัยสำคัญที่ 1 และ 3 เดือนเมื่อเปรียบเทียบกับก่อนผ่าตัด ส่วนระดับ FGF-21 เพิ่มขึ้นอย่างมีนัยสำคัญที่ 1 และ 3 เดือน โดยไม่มีความแตกต่างที่ 6 เดือน การผ่าตัดบายพาสกระเพาะอาหารแบบรู-เอ็น-วายผ่านกล้อง (RYGB) พบมีการเปลี่ยนแปลงของ GDF-15 และ FGF-21 มากกว่าการผ่าตัดกระเพาะอาหารแบบสลีฟผ่านกล้อง (LSG) โดยระดับ GDF-15 เพิ่มขึ้นอย่างมีนัยสำคัญที่ 1 และ 3 เดือน และระดับ FGF-21 เพิ่มขึ้นที่ 3 เดือนอย่างมีนัยสำคัญ …

House dust mite (HDM) represents one of the most prominent sources of allergens. The rising prevalence of HDM-induced allergic diseases such as allergic rhinitis, allergic asthma or atopic dermatitis is a major global health challenge. Allergen immunotherapy (AIT), by frequent administrations of HDM allergen extracts, is the unique treatment capable of alleviating the Th2-biased allergic response through the induction of immune tolerance. Excessive dietary intake of oxalate is a key risk factor of calcium oxalate (CaOx) nephrolithiasis. Therefore, limiting daily oxalate intake is recommended to prevent CaOx stone formation. Black tea and coffee are commonly consumed beverages that are rich in oxalate, and avoidance of tea and coffee consumption is relatively hard. ยุงลายบ้านเป็นพาหะสำคัญของโรคที่เกิดจากไวรัส เช่น โรคไข้เลือดออก โรคชิคุนกุนยา และโรคซิกา ก่อให้เกิดการเจ็บป่วยและเสียชีวิตทั่วโลก ไม่กี่ปีที่ผ่านมามีรายงานว่าสาหร่ายขนาดเล็กสามารถยับยั้งการเจริญของลูกน้ำยุงและส่งผลต่อจุลินทรีย์ในกระเพาะอาหารของยุง ซึ่งเป็นแนวทางในการพัฒนากลยุทธ์ใหม่สำหรับควบคุมพาหะนำโรค เพื่อแยก เพาะเลี้ยง และตรวจสอบความสัมพันธ์และความหลากหลายของสาหร่ายขนาดเล็กและจุลินทรีย์ในกระเพาะอาหารของลูกน้ำยุงและน้ำจากแหล่งเพาะพันธุ์ยุงลายบ้าน จากจังหวัดนครศรีธรรมราช โดยใช้กล้องจุลทรรศน์แบบใช้แสงและเทคนิคเมตาบาร์โค้ด พบสาหร่ายขนาดเล็ก 10 สกุล ด้วยกล้องจุลทรรศน์แบบใช้แสง และด้วยเทคนิคเมตาบาร์โค้ด 25 สกุล รวมถึงไดอะตอมในอันดับ Achnanthales โดยพบสาหร่ายขนาดเล็กจากทั้ง 2 วิธี 4 สกุล ได้แก่ Tetradesmus, Nannochloropsis, Monoraphidium และ Leptolyngbya และมี 2 สกุล คือ Tetradesmus และ Leptolyngbya ที่พบจากทั้งกระเพาะอาหารและแหล่งเพาะพันธุ์ของยุงลายบ้าน ส่วนจุลินทรีย์พบทั้งหมด 18 ไฟลัม โดยพบมากที่สุดทั้งในกระเพาะอาหารลูกน้ำและในน้ำจากแหล่งเพาะพันธุ์ยุงเดียวกัน ได้แก่ ไฟลัม Proteobacteria, Planctomycetota และ Actinobacteriota ตามลำดับ สะท้อนถึงความสอดคล้องขององค์ประกอบจุลินทรีย์ระหว่างสิ่งแวดล้อมและตัวอ่อนของยุง บ่งชี้ถึงความเชื่อมโยงทางชีวภาพระหว่างสิ่งแวดล้อมและลูกน้ำยุงลายบ้าน ซึ่งนำไปประยุกต์ใช้ในการพัฒนาวิธีควบคุมลูกน้ำยุงอย่างยั่งยืนและเป็นมิตรต่อสิ่งแวดล้อมได้

Background: Drug importation is critical for ensuring access to essential medicines in Thailand. However, the current system faces significant challenges, including outdated regulations, inefficient processes, and limited use of modern technology. This study aimed to identify regulatory and operational gaps and propose improvements to enhance the effectiveness of Thailand’s drug importation control. Methods: A two-phase mixed-methods study was conducted in Bangkok from 2019 to 2021. Conventional bottom-up synthesis of carbon quantum dots (CQDs) often requires extended reaction times, high energy input, and specialized equipment, limiting scalability and sustainability. In this study, we present an eco-friendly and energy-efficient method for synthesizing CQDs using H₂CO₃ as a carbon precursor at just 72 °C for 1 hour—representing one of the lowest reported synthesis temperatures and shortest reaction times using simple apparatus. Graphene oxide vacancies act as catalytic and nucleation sites, promoting the formation of CQDs under these mild conditions. Cannabidiol (CBD), a major phytocannabinoid present in Cannabis sativa, possesses multiple effective pharmacological activities, such as neuroprotective, cardioprotective and anti-inflammatory properties. However, its hydrophobic nature and rapid degradation mechanisms limit its clinical usefulness. Encapsulation of lipophilic compounds inside modified polymers with hydrophilic properties can improve the aqueous solubility and stability of drugs in specific environments. Cancer remains the leading cause of global mortality, with conventional chemotherapy drugs often limited by poor selectivity, side effects, and drug resistance. As a promising alternative and adjuvant, curcumin offers potent anticancer properties, but its low bioavailability highlights the need for targeted delivery systems to enhance its therapeutic potential. This study designs a targeted delivery system for curcumin using hyaluronic acid-grafted pNIPAM (HA-g-pNIPAM) modified liposomes, characterized both experimentally and computationally. This thesis challenges the dominant discourse surrounding the so-called "boy crisis" in education by critically examining gender performativity in Belgian classrooms. Rather than accepting the narrative that boys are inherently disadvantaged in contemporary schooling, this study interrogates how gender is constructed and performed within educational settings. Drawing on Judith Butler’s theory of performativity, the research explores how teachers, curricula, and peer interactions contribute to the reinforcement or subversion of traditional gender norms. Accurate and reliable gemstone classification is critical in gemology for authentication and identification purposes. A one-class classifier using self-organizing maps (SOMs), combined with Monte Carlo permutation testing, was successfully developed for gemstone classification based on Raman spectroscopy data. The developed SOM model was optimized by adjusting the map size and training iteration counts, and Monte Carlo permutation tests were conducted to evaluate prediction score through empirical p-values by generating random permutations of the data, allowing for a statistically robust assessment of classification reliability. Climate change is one of the major factors influencing environmental changes. The study of proxies from geological records can extend climate data into the past, enabling the reconstruction of past environmental changes. This study collected sediment cores from three points at the deepest part of Nong Thale Pron, Nakhon Si Thammarat Province. The sediment core NTP-CP6 was selected for analysis using X-ray fluorescence (XRF), radiocarbon dating, grain size analysis, total organic carbon, total nitrogen, stable carbon isotopes, and n-alkanes. These particles can adversely impact marine organisms and infiltrate the food chain, potentially posing risks to human health. Due to their small size and ubiquitous distribution, the removal of microplastics from water presents a significant technical challenge. This study focuses on the separation of microplastics using hydrocyclones. A three-dimensional Eulerian–Eulerian computational fluid dynamics (CFD) model was used to investigate this process.
