การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อเพื่อบรรยายประสบการณ์ของพยาบาลเด็กในการจัดการการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพ ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่มีประสบการณ์ในการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 คัดเลือกผู้ให้ข้อมูลแบบเจาะจงจำนวน 9 ราย เก็บรวบรวมข้อมูลด้วยวิธีการสัมภาษณ์เชิงลึก การบันทึกเทป การสังเกต และการบันทึกภาคสนาม นำข้อมูลทั้งหมดมาวิเคราะห์เนื้อหาตามวิธีการของ van Manen ผลการศึกษาประสบการณ์ของพยาบาลเด็กในการจัดการการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 ประกอบด้วยประเด็นหลักและประเด็นย่อยดังต่อไปนี้ 1. การเตรียมความพร้อมในการปฏิบัติงานดูแลผู้ป่วยเด็กโควิด-19 ประกอบด้วย 3 ประเด็นย่อย 1.1) การจัดเตรียมอุปกรณ์ทางการแพทย์ให้เพียงพอ 1.2) การปรับปรุงพื้นที่เพื่อรับผู้ป่วยติดเชื้อ 1.3) การเตรียมความรู้และทักษะในการป้องกันการติดเชื้อ 2. กระบวนการแรกรับผู้ป่วยเด็กติดเชื้อโควิด 19 เข้ารับการรักษา ประกอบด้วย 3 ประเด็นย่อย 2.1) การประเมินอาการเบื้องต้นเพื่อเตรียมห้องและอุปกรณ์การแพทย์ 2.2) การเตรียมผู้ปกครองให้พร้อมสำหรับการดูแลเด็ก 2.3) การทำหัตถการตามแผนการรักษา 3. การปรับรูปแบบการดูแลผู้ป่วยเด็กติดเชื้อโควิด 19 ประกอบด้วย 4 ประเด็นย่อย 3.1) การเฝ้าระวังอาการผู้ป่วยเด็กผ่านจอมอนิเตอร์ 3.2) การรวบกิจกรรมการพยาบาลเป็นรอบเวลา 3.3) การให้ผู้ปกครองมีส่วนร่วมเป็นผู้ดูแลหลัก 3.4) การประสานการส่งต่อผู้ป่วย 4. การดูแลผู้ปกครองที่ติดเชื้อโควิด 19 ประกอบด้วย 2 ประเด็นย่อย 4.1) การเบิกจ่ายยาและส่งต่อไปรักษาหากมีอาการหนัก 4.2) การดูแลด้านจิตใจให้คลายความเครียด 5. มาตรการป้องกันการแพร่กระจายเชื้อโควิด 19 ประกอบด้วย 4 ประเด็นย่อย 5.1) การใช้อุปกรณ์การแพทย์แบบครั้งเดียวแล้วทิ้ง 5.2) ของเยี่ยมจากภายนอกได้รับการฆ่าเชื้อก่อนส่งต่อให้ผู้ป่วย 5.3) การจัดเวลาและพื้นที่ในการรับประทานอาหารของพยาบาล 5.4) การอบห้องด้วยรังสียูวีทุกวัน 6. ความท้าทายและปัญหาที่พบในการปฏิบัติงาน ประกอบด้วย 4 ประเด็นย่อย 6.1) ความยากลำบากในการทำหัตถการ 6.2) อุปกรณ์ทางการแพทย์ไม่เพียงพอต่อการใช้งาน 6.3) การสื่อสารกับผู้รับบริการต่างชาติ 6.4) ความไม่พึงพอใจของผู้ปกครองและญาติ 7. ผลลัพธ์ทางบวกที่ได้รับจากการปฏิบัติงาน ประกอบด้วย 3 ประเด็นย่อย …

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ ศึกษาความสัมพันธ์ระหว่างการเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ และศึกษาตัวแปรพยากรณ์ที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพระดับปฏิบัติการ โรงพยาบาลมหาวิทยาลัยของรัฐ ที่มีประสบการณ์ทำงานตั้งแต่ 1 ปีขึ้นไป จำนวน 210 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการเรียนรู้ตลอดชีวิต แบบสอบถามภาวะผู้นำเชิงนวัตกรรม แบบสอบถามบรรยากาศการส่งเสริมนวัตกรรมในองค์การ และแบบสอบถามพฤติกรรมการทำงานที่เน้นนวัตกรรม ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ได้ค่า CVI เท่ากับ .94, .89, .88 และ .93 ตามลำดับ และทดสอบค่าความเที่ยง โดยคำนวณหาค่าสัมประสิทธิ์แอลฟ่า ได้ค่าความเที่ยง เท่ากับ .90, .97, .95 และ .96 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติสหสัมพันธ์เพียร์สัน และสถิติถดถอยพหุคูณ ผลการวิจัยพบว่า พฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพในโรงพยาบาลมหาวิทยาลัยของรัฐ อยู่ในระดับปานกลาง (M = 3.49, S.D. = 0.76) การเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การ มีความสัมพันธ์ทางบวกกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .683, .524 และ .499 ตามลำดับ) ปัจจัยที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ คือ การเรียนรู้ตลอดชีวิต บรรยากาศการส่งเสริมนวัตกรรมในองค์การ และภาวะผู้นำเชิงนวัตกรรม โดยมีอำนาจในการทำนายร้อยละ 53.6 (Adjusted R² = .536) สามารถเขียนสมการทำนายในรูปคะแนนมาตรฐาน ดังนี้ ZIWB = .540 ZLLL + .192 ZOIC + .149 ZIL

งานวิจัยนี้มีวัตถุประสงค์ เพื่อศึกษาพฤติกรรมสุขภาพที่พึงประสงค์ของผู้สูงอายุไทย และเพื่อศึกษาความสัมพันธ์ระหว่างคุณลักษณะของผู้ดูแลและพฤติกรรมสุขภาพที่พึงประสงค์ของผู้สูงอายุไทย ดำเนินการวิจัยโดยใช้ระเบียบวิธีวิจัยเชิงปริมาณ และใช้ข้อมูลทุติยภูมิจากโครงการสำรวจประชากรสูงอายุในประเทศไทย พ.ศ. 2564 โดยสำนักงานสถิติแห่งชาติ กลุ่มตัวอย่างคือ ผู้สูงอายุที่มีอายุ 60 ปีขึ้นไป ที่มีผู้ดูแลในการใช้ชีวิตประจำวัน จำนวน 3,406 คน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ค่าความถี่ ค่าร้อยละ ค่าเฉลี่ย ค่าต่ำสุด ค่าสูงสุด และค่าส่วนเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมานคือ การถดถอยโลจิสติกทวิภาคผลการศึกษา พบว่า ผู้สูงอายุไทยมีเพียงร้อยละ 7.09 ที่ออกกำลังกายเป็นประจำ และมีเพียงร้อยละ 45.53 ที่กินผัก/ผลไม้เป็นประจำ และผู้สูงอายุที่ดื่มน้ำสะอาดวันละ 8 แก้วหรือมากกว่าเป็นประจำ มีเพียงร้อยละ 36.84 ในขณะที่สัดส่วนของผู้สูงอายุที่ไม่ดื่มสุรา/เครื่องดื่มที่มีแอลกอฮอล์ และไม่สูบบุหรี่ สูงถึงร้อยละ 96.96 และ 97.08 ตามลำดับ ซึ่งแสดงให้เห็นว่าพฤติกรรมสุขภาพที่ยังคงต้องส่งเสริมให้มีเพิ่มมากขึ้นคือ การออกกำลังกาย การกินผัก/ผลไม้ และการดื่มน้ำสะอาด เมื่อศึกษาความสัมพันธ์ระหว่างคุณลักษณะของผู้ดูแลกับพฤติกรรมสุขภาพที่พึงประสงค์ พบว่า เพศ อายุ ระดับการศึกษาสูงสุด รูปแบบการอยู่อาศัย การลาออกจากงาน รายได้เฉลี่ยต่อเดือน การฝึกอบรมเพื่อดูแลผู้สูงอายุ ระยะเวลาที่ใช้ในการดูแลต่อสัปดาห์ (ชม.) และทัศนคติที่มีต่อการดูแลผู้สูงอายุ มีความสัมพันธ์กับพฤติกรรมสุขภาพที่พึงประสงค์ของผู้สูงอายุไทยอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 และพบว่าความสัมพันธ์ของผู้ดูแลกับผู้สูงอายุ ไม่มีความสัมพันธ์กับพฤติกรรมสุขภาพที่พึงประสงค์ของผู้สูงอายุในทุกด้าน

N-glycosylation is a crucial post-translational modification that significantly affects immune recognition. The plant-based platform, especially Nicotiana benthamiana, offers scalable and cost-effective production of therapeutic proteins with glycosylation patterns similar to mammalian cells. However, plant-specific N-glycans, such as α(1-3)-fucose and β(1-2)-xylose (XF), have the potential to trigger allergic reactions. This study investigates how various N-glycan structures on the RBD-Fc fusion protein influence the immune response in mice. The RBD-Fc fusion protein was used for vaccine formulation, produced in both wild-type (WT) and glycoengineered (∆XF) N. benthamiana plants. Staphylococcus aureus, a gram-positive, commensal bacterium, is the cause of a multitude of opportunistic diseases and infections. While S. aureus has evolved ways to escape innate immune responses in the host, the mechanisms are not fully understood. One area of active research involves mechanisms by which S. aureus avoids detection by Pattern Recognition Receptors (PRRs), such as Nuclear Oligomerization Domain 2 (NOD2), present on the surface of and inside innate immune cells. Identification early biomarkers for severe liver dysfunction in severe dengue patients is essential. Currently, there is no any data on circulating miRNAs to predict this important complication, Therefore, this study aimed to explore the role of circulating miRNAs in severe liver dysfunction in dengue infection. Osteoarthritis (OA) is a widespread degenerative joint disease marked by cartilage destruction. This study investigates the role of monocytic myeloid-derived suppressor cells (M-MDSCs) in OA, focusing on their gene expression profiles and their relationship with body mass index (BMI) in knee OA patients. Previous research has shown a positive correlation between M-MDSCs and BMI in OA. Aims: We aimed to recruit a representative cohort of women and men with multi-morbid chronic heart disease as part of a trial testing an innovative, nurse-co-ordinated, multi-faceted intervention to lower rehospitalization and death by addressing areas of vulnerability to external challenges to their health.
Methods and results: The prospective, randomized open, blinded end-point RESILIENCE Trial recruited 203 hospital inpatients (mean age 75.7 ± 10.2 years) of whom 51% were women and 94% had combined coronary artery disease, heart failure, and/or atrial fibrillation. Objectives: To compare the supply of molnupiravir and nirmatrelvir/ritonavir in relation to patient characteristics and other co-prescribed medicines and to estimate the number of patients without contraindications to nirmatrelvir/ritonavir who were treated with molnupiravir.
Study design, setting: Retrospective observational study of patients identified in the Pharmaceutical Benefits Scheme (PBS) 10 % sample dataset who were supplied with either molnupiravir or nirmatrelvir/ritonavir between May and December 2022. We supplemented the PBS dataset with aggregated counts from published literature to determine prevalence of clinical contraindications to nirmatrelvir/ritonavir. Introduction: Coxiella burnetii (C. burnetii)-infected livestock and wildlife have been epidemiologically linked to human Q fever outbreaks. Despite this growing zoonotic threat, knowledge of coxiellosis in wild animals remains limited, and studies to understand their epidemiologic role are needed. In C. burnetii-endemic areas, ticks have been reported to harbor and spread C. burnetii and may serve as indicators of risk of infection in wild animal habitats. Background and objective: There are several benign processes that affect the mediastinum with considerable morbidity that may range from reactive entities to neoplastic disorders. This review article will focus on non-neoplastic benign mediastinal diseases which include large vessel vasculitis such as Takayasu and giant cell arteritis, mediastinal granulomas, fibrosing mediastinitis and mediastinal infections. These diseases can cause significant morbidity and mortality; therefore, we aim to familiarize readers with the pathophysiology, epidemiology and diagnosis of these mediastinal diseases and provide an update on the treatment options available.

The objective of this study is to evaluate the clinical presentations and adverse outcomes of Coronavirus Disease 2019 (COVID-19) in patients with systemic sclerosis (SSc) and assess the impact of SSc features on the clinical course of COVID-19.Materials and methods: In this multicenter, retrospective study, SSc patients with COVID-19 were included. Clinical features of SSc, along with detailed COVID-19 data, were extracted from medical records and patient interviews.Results: The study included 112 patients (mean age 51.4 ± 12.8 years; 90.2% female). Ensuring the completeness of IS-A relations in SNOMED CT is crucial for maintaining its accuracy in clinical applications. In this study, we propose a hybrid approach leveraging non-lattice subgraphs and pre-trained language models (PLMs) to identify missing IS-A relations in SNOMED CT. We fine-tuned four BERT-based models: BERT, DistillBERT, DeBERTa, and BioClinicalBERT, and four generative large language models (LLMs): BioMistral, Llama3, Gemma2, and Phi-4. Missing IS-A relations were identified through consensus predictions by all eight models. De-BERTa achieved the best performance (precision: 0.96, recall: 0.97, F1-score: 0.965) for IS-A relation prediction. Our approach identified 678 potential missing IS-A relations in …

Invasive candidiasis (IC) is an increasingly prevalent, costly, and potentially fatal infection brought on by the opportunistic yeast, Candida. Previously, IC has predominantly been caused by C. albicans which is often drug susceptible. BACKGROUND: Eight-hydroxy-2'-deoxyguanosine (8-OHdG), a biomarker of oxidative damage evaluated in human neurodegenerative disease, has potential to correlate with postmortem diagnosis of neuroaxonal dystrophy/degenerative myeloencephalopathy (NAD/DM) in horses.

Patients with cirrhosis that are hospitalized with COVID-19 infection have been found to have worse outcomes. No comparative study has been conducted between gastrointestinal (GI) bleeding in patients with cirrhosis who are diagnosed with COVID-19. We utilized the National Inpatient Sample (NIS) database to perform a retrospective analysis of 24, 050 patients diagnosed with cirrhosis and COVID-19. The identified patients were separated into variceal bleeding, nonvariceal bleeding, and no (or neither) GI bleeding groups. Purpose of review: Spinal cord stimulation (SCS) is a minimally invasive and reversible therapy for the treatment of severe neuropathic pain. There are several biologic complications that can arise during the implantation of SCS systems. This review aims to consolidate recent research on complications associated with SCS, specifically focusing on biologic complications.
Recent findings: Biologic complications are less frequently encountered compared to device-related complications but may have more severe consequences. Biologic complications covered in this review include infection, seromas, hematomas, dural puncture, nerve/spinal cord injury, and therapy habituation.

We report a case of a 15-year-old with refractory and relapsed AML and profound prolonged neutropenia who developed a Saprochaete capitata disseminated invasive infection while on echinocandin prophylaxis for invasive fungal disease. Azole antifungal therapies, which are often used as prophylaxis, were initially avoided due to concerns for CYP drug interactions. Treatment with a combination of liposomal amphotericin B, voriconazole, and adjuvant granulocyte transfusions was successful as he awaited neutrophil recovery.
