Introduction - The majority of active Mycobacterium tuberculosis cases are pulmonary, but extrapulmonary tuberculosis (TB) can account for up to 30% of active cases. Cases of tuberculosis myositis are rare and account for less than three percent of active cases and diagnosis is often delayed due to its rarity. Cancer treatments are known to introduce cardiotoxicity, negatively impacting outcomes and survivorship. Identifying cancer patients at risk of heart failure (HF) is critical to improving cancer treatment outcomes and safety. This study examined machine learning (ML) models to identify cancer patients at risk of HF using electronic health records (EHRs), including traditional ML, Time-Aware long short-term memory (T-LSTM), and large language models (LLMs) using novel narrative features derived from the structured medical codes. Achieving and maintaining the therapeutic range in vancomycin treatment is important for optimal outcomes. While guidelines and best practices based on empirical studies exist, the theoretical best dosing strategies under various conditions remain illusive. We developed an RL-based simulation framework using a deep learning two-compartment pharmacokinetic model (PK-RNN-2CM) and introduced the area under the time-concentration curve (AUC) reward score, which translates clinical guidelines into an RL reward. Ground truth time-concentration curves were generated from patient-specific data, and simulated curves were produced under different dosing strategies with optional noise perturbations to mimic real-world settings. Microbiota restoration therapy has become a standard treatment for recurrent Clostridioides difficile infection (rCDI). In this article, we review the studies supporting the licensure of two live biotherapeutic products (LBPs) designed to prevent rCDI and to provide clinicians with a perspective on their differences. Context: Unicentric Castleman disease (UCD) is a dynamic entity with a wide spectrum of morphologic findings. UCD can be further subdivided into hyaline-vascular and mixed/plasmacytic variants. Hyaline-vascular UCD has both follicular and interfollicular (stromal) changes, and occasionally these lesions show a skewed representation of either the follicular or stromal compartments. Plasmacytosis is usually minimal in the hyaline-vascular variant. The mixed/plasmacytic variant of UCD is composed of sheets of plasma cells often associated with a variable number of follicles with regressive changes.

Background: A useful clinical biomarker requires not only association but also a consistent temporal relationship. For instance, chemotherapy-induced neutropenia and epidermal growth-factor inhibitor-related acneiform rash both occur within weeks of treatment initiation, thereby providing information prior to efficacy assessment. Although immune checkpoint inhibitor (ICI)-associated immune-related adverse events (irAE) have been associated with therapeutic benefit, irAE may have delayed and highly variable onset. To determine whether ICI efficacy and irAE could serve as clinically useful biomarkers for predicting each other, we determined the temporal relationship between initial efficacy assessment and irAE onset in a diverse population treated with ICI.

Purpose of review: Oral squamous cell carcinoma (OSCC) survival rates have remained stagnant due to a lack of targeted therapies and diagnostic tools. Patient risk is currently determined solely through clinicopathologic features, primarily tumor staging, which lacks the necessary precision to stratify patients by risk and accurately dictate adjuvant treatment. Similarly, conventional OSCC therapies have well-established toxicities and limited efficacy.

Recent findings: Recent studies show that patient risk can now be assessed using non-invasive techniques, at earlier time points, and with greater accuracy using molecular biomarker panels. Additionally, novel immunotherapies not only utilize the host's immune response to combat disease …

Background: Traditional vaccine development, often a lengthy and costly process of three separated phases. However, the swift development of COVID-19 vaccines highlighted the critical importance of accelerating the approval of vaccines. This article showcases a seamless phase 2/3 trial design to expedite the development process, particularly for multi-valent vaccines.

Research design and methods: This study utilizes simulation to compare the performance of seamless phase 2/3 design with that of conventional trial design, specifically by re-envisioning a 9-valent HPV vaccine trial. Patients with leukemia experience profound immunosuppression both from their underlying disease as well as chemotherapeutic treatment. Little is known about the prevalence and clinical presentation of nontuberculous mycobacteria (NTM) in this patient population. We identified six cases of NTM infection from 29,743 leukemia patients who had acid-fast bacilli (AFB) cultures. Four cases had bloodstream infections and five had disseminated disease, including one who presented with an unusual case of diffuse cellulitis/myositis. All patients were lymphopenic at time of diagnosis, and two patients ultimately died from their NTM infection. Although most follicular-derived thyroid cancers are well differentiated and have an overall excellent prognosis following treatment with surgery and radioiodine, management of advanced thyroid cancers, including iodine refractory disease and poorly differentiated/undifferentiated subtypes, is more challenging. Over the past decade, better understanding of the genetic drivers and immune milieu of advanced thyroid cancers has led to significant progress in the management of these patients. Background: This study investigated the influence of oral microbial features on the trajectory of oral mucositis (OM) in patients with squamous cell carcinoma of the head and neck.

Methods: OM severity was assessed and buccal swabs were collected at baseline, at the initiation of cancer treatment, weekly during cancer treatment, at the termination of cancer treatment, and after cancer treatment termination. Sample multiplexing significantly enhanced the depth of single-cell proteomic analysis in CyTOF (Cytometry by Time-Of-Flight). New polymer-based chelators have broadened the utility of metal isotopes, enabling improved tagging and simultaneous analysis of multiple samples. These approaches minimize batch effects, streamline experiments, conserve valuable samples, reduce costs, enhance throughput, and increase the accuracy of biological data, thereby facilitating novel discoveries.

The mediastinal vasculature can be affected by various etiologies in cancer patients. Both direct and indirect sequela of cancer may result in life-threatening clinical presentations. Tumor growth may cause vessel narrowing and decreased blood flow from either extrinsic mass effect, invasion into the vascular wall, or tumor thrombus within the lumen. In addition, cancer patients are predisposed to indirect sequela to the mediastinal vasculature from an increased risk of benign thromboembolic events, tumor thrombus, or iatrogenic complications during cancer treatments. Fungal infection (FI) after allogeneic hematopoietic cell transplantation (HCT) is associated with increased morbidity and mortality. Neutropenia, HLA mismatch, graft-versus-host disease (GVHD), and viral infections are risk factors for FI. The objectives of this Center for International Blood and Marrow Transplant Research registry study were to compare the incidence and density of FI occurring within 180 days after HCT in matched sibling (Sib) transplants with either calcineurin inhibitor (CNI)-based or post-transplantation cyclophosphamide (PTCy)-based GVHD prophylaxis and related haploidentical transplants receiving PTCy, and to examine the impact of FI by day 180 on transplantation outcomes.

Rift Valley fever (RVF), a mosquito-borne transboundary zoonosis, was first confirmed in Rwanda’s livestock in 2012 and since then sporadic cases have been reported almost every year. In 2018, the country experienced its first large outbreak, which was followed by a second one in 2022. To determine the circulating virus lineages and their ancestral origin, two genome sequences from the 2018 outbreak, and thirty-six, forty-one, and thirty-eight sequences of small (S), medium (M), and large (L) genome segments, respectively, from the 2022 outbreak were generated. Newcastle disease (ND) is a viral disease affecting a wide range of bird species and has a considerable financial impact on the world's poultry market. The ND virus (NDV) strains currently circulating in poultry throughout Africa, and especially in East Africa, exhibit significant genetic variation.

Brucellosis poses serious public health implications and substantial economic losses in pastoral rural settings in South Sudan. In humans, brucellosis is almost always originating from animals. Maize lethal necrosis (MLN) is a maize disease caused by the maize chlorotic mottle virus (MCMV), a potyvirus which causes yield losses of 30–100%. The present study aimed to isolate nanobodies against the MCMV coat protein (CP) for the diagnosis of MLN. MCMV CP expressed in Escherichia coli was used for llama immunization. VHH (i.e. variable heavy domain of heavy chain) gene fragments were prepared from blood drawn from the immunized llama and used to generate a library in E. coli TG1 cells. The aim of this study was to evaluate the antimicrobial potential and perform the phytochemical screening of ethanolic (70%) leaf extracts of Bauhinia rufescens and Ocimum basilicum and root extracts of Salvadora persica, three plants used for the treatment of numerous diseases in Chad. The extraction of the plants was made in a ratio of 70% volume of ethanol against 30% volume of distilled water. The yields obtained in the extraction of the leaves of Bauhinia rufescens, Ocimum basilicum, and Salvadora persica roots were respectively 20, 17, and 8.8%. Tilapia, a significant aquaculture species globally, relies heavily on feed for its production. While numerous studies have investigated the impact of soybean and seaweed-based diets on tilapia, a comprehensive understanding remains elusive. This review aimed at evaluating and synthesizing the existing literature on these diets’ effects, focusing on growth performance, feed utilization, and gut microbiota. A systematic search of databases was conducted using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and a total of 57 studies were included in the qualitative analysis and 24 in the meta-analysis. Juvenile Asian seabass (Lates calcarifer) with an average body weight of 10 ± 0.7 g were used to investigate the efficacy of an inactivated Streptococcus iniae vaccine and the probiotic Bacillus subtilis. Fish were intraperitoneally vaccinated with 1012 CFU fish−1 of formalin-killed S. iniae (V) or sham vaccinated (S) with sterile normal saline solution (NSS). Both groups were fed with Bacillus probiotic (5 × 109 CFU kg-1 diet) for 6 weeks. ภาวะโรคไม่ติดต่อเรื้อรัง หรือ Non-Communicable Disease (NCDs) เป็นภาวะโรคที่ไม่ได้ติดต่อจากคนสู่คนได้รวมถึงไม่ได้เกิดจากการติดเชื้อโรค โดยสาเหตุหนึ่งของภาวะกลุ่มโรคนี้คือการเกิดภาวะ oxidative stress ที่ทำให้ร่างกายมีปริมาณของสารอนุมูลอิสระไม่สมดุล โดยที่ผ่านมาจำนวนผู้ป่วยในโรคนี้มีจำนวนเพิ่มขึ้นอย่างต่อเนื่อง ซึ่งแนวทางการรักษาส่วนใหญ่มักจะมาจากการรักษาตามอาการ ดังนั้นการแก้ปัญหาโรคนี้ให้ได้ผลมากที่สุดคือการป้องกันการเกิดโรคมากกว่าการรักษา ผู้วิจัยจึงสนใจที่จะนำมะกรูดที่มีสรรพคุณต้านอนุมูลอิสระมาทดสอบ โดยมีวัตถุประสงค์เพื่อศึกษาฤทธิ์ของสารสกัดจากใบมะกรูดและผิวมะกรูด ในการช่วยลดการเกิดภาวะ oxidative stress สำหรับการช่วยยืดอายุของ C. elegans ที่ถูกชักนำให้เกิดภาวะ oxidative stress โดยแบ่งการศึกษาออกเป็น 4 ขั้นตอน ได้แก่ ตอนที่ 1 ศึกษาฤทธิ์ของสารสกัดจากมะกรูดต่อ E.coli OP50 ที่เป็นอาหารของ C. elegans พบว่าสารสกัดจากใบมะกรูดและผิวมะกรูดไม่มีผลในการยับยั้งการเจริญของ E.coli OP50 เมื่อเทียบกับการยับยั้งการเจริญเติบโตของยา Penicillin-Streptomycin ที่ใช้เป็นกลุ่มควบคุม จึงสามารถใช้สารสกัดจากใบมะกรูดและผิวมะกรูดในการทดสอบได้ โดยที่ไม่มีผลต่อ E.coli OP50 ที่ใช้เป็นอาหารของ C. elegans ตอนที่ 2 การศึกษาฤทธิ์ของสารสกัดจากใบมะกรูดและผิวมะกรูดที่มีผลต่อการรอดชีวิตของ C. elegans ที่ถูกชักนำให้เกิดภาวะ oxidative stress พบว่าสารสกัดจากใบมะกรูดและผิวมะกรูดที่ความเข้มข้น 50 , 100 และ 200 ug/ml มีอัตราการรอดชีวิตของ C. elegans เมื่อเทียบกับ C. elegans ที่ไม่ได้ให้สารสกัดจากใบมะกรูดและผิวมะกรูด แตกต่างกันอย่างมีนัยสำคัญทางสถิติ 0.05 และตอนที่ 3 การศึกษาฤทธิ์ของสารสกัดจากใบมะกรูดและผิวมะกรูดต่อการยับยั้งปริมาณ ROS ของ C. elegans ที่ถูกชักนำให้เกิดภาวะ oxidative stress โดยพบว่าใบมะกรูดและผิวมะกรูดที่ความเข้มข้น 50 , 100 และ 200 ug/ml มีค่าปริมาณสาร ROS ของ C. elegans ที่ให้สารสกัดจากใบมะกรูดและผิวมะกรูด เมื่อเทียบกับ C. elegans ที่ไม่ให้สารสกัดจากใบมะกรูดและผิวมะกรูด …

การเสียสมดุลของจุลินทรีย์ในลำไส้อาจทำให้เกิดภาวะลำไส้รั่ว เกิดการติดเชื้อในกระแสเลือดตามมาได้ ซึ่งสภาวะดังกล่าวมักพบมากในผู้สูงอายุ การศึกษาก่อนหน้านี้พบว่าโพรไบโอติกสามารถลดการเสียสมดุลของจุลินทรีย์ในลำไส้ แต่การศึกษาเรื่องผลของการรับประทานเชื้อโพรไบโอติดต่อของการลดความรุนแรงจากการติดเชื้อในกระแสเลือด รวมทั้งภาวะ Sepsis-associated encephalopathy (SAE) ในผู้สูงอายุนั้นยังไม่มีการศึกษาที่ชัดเจน วัตถุประสงค์ของการศึกษา เพื่อทดสอบความสามารถของเชื้อโพรไบโอติกสายพันธุ์ Lacticaseibacillus rhamnosus GG (LGG) ในการช่วยลดความรุนแรงของการติดเชื้อในกระแสเลือดในหนูที่ถูกเหนี่ยวนำให้เกิดสภาวะชราโดยการใช้น้ำตาลกาแลคโตส ผู้วิจัยทำการให้เชื้อ LGG ทางปากกับหนูทดลอง เปรียบเทียบกับกลุ่มควบคุม และทดสอบความแตกต่างของจุลินทรีย์ในลำไส้ในอุจจาระของหนูด้วยวิธี 16S RNA sequencing พร้อมวัดสภาวะความชราของหนูด้วยเทคนิค Histochemistry ในอวัยวะภายใน จากนั้นหนูทั้ง 2 กลุ่มจะถูกกระตุ้นให้เกิดการติดเชื้อในกระแสเลือดโดยวิธีการผูกและเจาะลำไส้ใหญ่ส่วนต้น (CLP) และวัดความรุนแรงของการติดเชื้อในกระแสเลือดโดยการวัดอัตราการตาย ระดับ creatinine (Scr), Alanine transaminase (ALT), cytokines (IL-6 TNF-α IL-10 และ IL-1ß) ในซีรัมโดยเทคนิค Enzyme-linked immunosorbent assay (ELISA) การตรวจวัด Sepsis encephalopathy ผู้วิจัยใช้ค่ามาตรฐาน SHIRPA เพื่อช่วยในการวัด นอกจากนี้เพื่อหาสาเหตุของการลดความรุนแรงจากการติดเชื้อในกระแสเลือดด้วยเชื้อโพรไบโอติก ผู้วิจัยนำ Lactobacillus condition media (LCM) ร่วมกับ Lipopolysaccharide (LPS) มาใช้ในการกระตุ้นเซลล์ RAW 264.7 , เซลล์ BV-2 และเซลล์ HT-22 จากนั้นทำการวัดปริมาณ Inflammatory cytokines ด้วยเทคนิค ELISA และวัดการแสดงออกของยีน IL-6 TNF-α IL-10 IL-1ß Fizz Arg-1 NF-kB TLR-4 และยีน iNOS ด้วยเทคนิค Quantitative real-time PCR (qPCR) จากผลการวิเคราะห์จุลินทรีย์ในอุจจาระ แสดงให้เห็นว่าหนูชรามีการสูญเสียสมดุลของจุลินทรีย์ในลำไส้ และพบการเกิดพังผืดในปอด ไต และตับ ซึ่งเชื้อ …

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อเพื่อบรรยายประสบการณ์ของพยาบาลเด็กในการจัดการการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพ ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่มีประสบการณ์ในการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 คัดเลือกผู้ให้ข้อมูลแบบเจาะจงจำนวน 9 ราย เก็บรวบรวมข้อมูลด้วยวิธีการสัมภาษณ์เชิงลึก การบันทึกเทป การสังเกต และการบันทึกภาคสนาม นำข้อมูลทั้งหมดมาวิเคราะห์เนื้อหาตามวิธีการของ van Manen ผลการศึกษาประสบการณ์ของพยาบาลเด็กในการจัดการการดูแลผู้ป่วยเด็กติดเชื้อไวรัสโคโรนา 2019 ประกอบด้วยประเด็นหลักและประเด็นย่อยดังต่อไปนี้ 1. การเตรียมความพร้อมในการปฏิบัติงานดูแลผู้ป่วยเด็กโควิด-19 ประกอบด้วย 3 ประเด็นย่อย 1.1) การจัดเตรียมอุปกรณ์ทางการแพทย์ให้เพียงพอ 1.2) การปรับปรุงพื้นที่เพื่อรับผู้ป่วยติดเชื้อ 1.3) การเตรียมความรู้และทักษะในการป้องกันการติดเชื้อ 2. กระบวนการแรกรับผู้ป่วยเด็กติดเชื้อโควิด 19 เข้ารับการรักษา ประกอบด้วย 3 ประเด็นย่อย 2.1) การประเมินอาการเบื้องต้นเพื่อเตรียมห้องและอุปกรณ์การแพทย์ 2.2) การเตรียมผู้ปกครองให้พร้อมสำหรับการดูแลเด็ก 2.3) การทำหัตถการตามแผนการรักษา 3. การปรับรูปแบบการดูแลผู้ป่วยเด็กติดเชื้อโควิด 19 ประกอบด้วย 4 ประเด็นย่อย 3.1) การเฝ้าระวังอาการผู้ป่วยเด็กผ่านจอมอนิเตอร์ 3.2) การรวบกิจกรรมการพยาบาลเป็นรอบเวลา 3.3) การให้ผู้ปกครองมีส่วนร่วมเป็นผู้ดูแลหลัก 3.4) การประสานการส่งต่อผู้ป่วย 4. การดูแลผู้ปกครองที่ติดเชื้อโควิด 19 ประกอบด้วย 2 ประเด็นย่อย 4.1) การเบิกจ่ายยาและส่งต่อไปรักษาหากมีอาการหนัก 4.2) การดูแลด้านจิตใจให้คลายความเครียด 5. มาตรการป้องกันการแพร่กระจายเชื้อโควิด 19 ประกอบด้วย 4 ประเด็นย่อย 5.1) การใช้อุปกรณ์การแพทย์แบบครั้งเดียวแล้วทิ้ง 5.2) ของเยี่ยมจากภายนอกได้รับการฆ่าเชื้อก่อนส่งต่อให้ผู้ป่วย 5.3) การจัดเวลาและพื้นที่ในการรับประทานอาหารของพยาบาล 5.4) การอบห้องด้วยรังสียูวีทุกวัน 6. ความท้าทายและปัญหาที่พบในการปฏิบัติงาน ประกอบด้วย 4 ประเด็นย่อย 6.1) ความยากลำบากในการทำหัตถการ 6.2) อุปกรณ์ทางการแพทย์ไม่เพียงพอต่อการใช้งาน 6.3) การสื่อสารกับผู้รับบริการต่างชาติ 6.4) ความไม่พึงพอใจของผู้ปกครองและญาติ 7. ผลลัพธ์ทางบวกที่ได้รับจากการปฏิบัติงาน ประกอบด้วย 3 ประเด็นย่อย …
