An inconsistent application of neuroprotective care provided by clinical staff, based on my observations, to minimize the negative effects of painful procedures that can negatively impact the neurodevelopment of neonates in the neonatal intensive care unit (NICU), was the identified practice problem for my Doctor of Nursing Practice (DNP) staff education project. Implementation of evidence-based neuroprotective and neuropromotive strategies aim to optimize outcomes for premature infants and minimize risk of potential negative effects to the neonate’s short and long term neurodevelopment. The purpose of this qualitative study was to explore the perspectives of employees at a Mid-Atlantic nonprofit regarding their satisfaction and to develop a strategic plan to address the organization's challenges. This study utilized a qualitative case study design with data collected through semistructured interviews. The research questions focused on understanding employee satisfaction, perceptions of improving quality of life, and strategies for attracting new talent. Data for this study was from 12 participants who provided in-depth insights through one-on-one interviews which were analyzed using thematic analysis. Introduction: There is a growing body of evidence regarding the contribution to the management of chronic pain by occupational therapists. However, there is limited research available regarding contemporary practice in the United Kingdom (UK). Method: An on-line questionnaire was circulated via social media, profession specific publications and specialist interest groups. Responses were requested from UK-based Health and Care Professions Council registered occupational therapists who identified working with those living with chronic pain and their friends, family and supporters. Objective: The purpose of this study was to explore the experiences of patients and healthcare professionals participating in a randomized feasibility trial exploring the management of acute post-traumatic benign paroxysmal positional vertigo (BPPV), and to establish the acceptability and tolerability of BPPV assessment and treatment procedures. Methods: This was a multi-center qualitative study nested within a wider randomized feasibility trial. Purposive sampling was used to gather a cohort of healthcare professionals and patients from three major trauma centres in London, UK. IntroductionHemiplegic shoulder pain (HSP) is a common medical complication following stroke, that impacts the overall functional outcomes of the person with stroke (PWS). While prevention is key, clinical guidelines offer little guidance on how to effectively achieve this. This study aimed to investigate neuro-therapists (Occupational Therapists and Physiotherapists) current clinical practice to prevent HSP.MethodsA cross-sectional, online survey was designed, piloted and distributed through specialist interest groups, social media and encouraging sharing of the link. Protein post-translational modifications, such as phosphorylation, are important regulatory signals for diverse cellular functions. In particular, intrinsically disordered protein regions (IDRs) are subject to phosphorylation as a means to modulate their interactions and functions. Toward understanding the relationship between phosphorylation in IDRs and specific functional outcomes, we must consider how phosphorylation affects the IDR conformational ensemble. Various experimental techniques are suited to interrogate the features of IDR ensembles; molecular simulations can provide complementary insights and even illuminate ensemble features that may be experimentally inaccessible. Histone methyltransferases (HMTs) and histone demethylases (HDMs) are critical enzymes that regulate chromatin dynamics and gene expression through the addition and removal of methyl groups on histone proteins. HMTs, such as PRC2 and SETD2, are involved in the trimethylation of histone H3 at lysine 27 and lysine 36, influencing gene silencing and activation. Dysregulation of these enzymes often leads to abnormal gene expression and contributes to tumorigenesis. In contrast, HDMs including KDM7A and KDM2A reverse these methylation marks, and their dysfunction can drive disease progression. Arousal states are thought to influence many aspects of cognition and behavior by broadly modulating neural activity. Many studies have observed arousal-related modulations of alpha (~8 to 15 Hz) and gamma (~30 to 50 Hz) power and coherence in local field potentials across relatively small groups of brain regions. However, the global pattern of arousal-related oscillatory modulation in local field potentials is yet to be fully elucidated. Intrinsically disordered protein regions (IDRs) are well established as contributors to intermolecular interactions and the formation of biomolecular condensates. In particular, RNA-binding proteins (RBPs) often harbor IDRs in addition to folded RNA-binding domains that contribute to RBP function. To understand the dynamic interactions of an IDR-RNA complex, we characterized the RNA-binding features of a small (68 residues), positively charged IDR-containing protein, Small ERDK-Rich Factor (SERF). At high concentrations, SERF and RNA undergo charge-driven associative phase separation to form a protein- and RNA-rich dense phase. Orthogonal polynomials have had vast historical applications in applied mathematics, particularly in differential equations and numerical analysis. In recent years, they have become instrumental as a natural basis for constructing kernel systems in machine learning (ML). In this thesis, we orthogonalize a set of polynomials, the Logistic Polynomials, first developed by George and Rousseau, using the Gram-Schmidt process. We show that this new Logistic Orthogonal Polynomial System (LOPS) satisfies the hypergeometric differential equation and we relate the system to the Legendre polynomials. Purpose: Attention-deficit/hyperactivity disorder (ADHD) is a multifactorial disorder that has been linked to increased risk for behavioral, academic, and interpersonal problems. Developmental language disorder (DLD) has also been linked to poor outcomes in these areas. The presence of co-occurring disorders in children’s profiles is generally associated with poorer outcomes relative to profiles consisting of only one disorder. Using The Lehmer Mean To Assess Business Data Protection: Statistical Disclosure Control And The Truncated Moment Problem, Mark Stander, Julian Stander

Confidential business data needs protection against disclosure. Often this data is protected by releasing sample means, variances and higher power moments. Motivated by statistical disclosure control obligations and the need to publish business data safely, we explain how calculating the Lehmer mean from released power moments can lead to the unwanted disclosure of the largest data value. We explain how similar disclosure can apply to smaller data values and provide an approximate solution to the Truncated Moment Problem. We briefly discuss the Gini mean and the relationship between sample central and raw moments.

OBJECTIVES To evaluate the clinical effectiveness and safety of a perioperative algorithm for cardiac output-guided haemodynamic therapy in patients undergoing major gastrointestinal surgery. DESIGN Multicentre randomised controlled trial. SETTING Surgical services of 55 hospitals worldwide. PARTICIPANTS 2498 adults aged ≥65 years with an American Society of Anesthesiologists physical status classification of II or greater and undergoing major elective gastrointestinal surgery, recruited between January 2017 and September 2022. INTERVENTIONS Participants were assigned to minimally invasive cardiac output-guided intravenous fluid therapy with low dose inotrope infusion during and four hours after surgery, or to usual care without cardiac output monitoring. Soil erosion is a world-wide issue driven by land management and climate change. Research has focussed on soil loss rates from agricultural land. However, the loss of trace elements essential for soil and plant health, or potentially toxic elements that occur as impurities in fertilisers and manures, is poorly understood. This study reports on the loads and forms of copper, cadmium, manganese, nickel, selenium and zinc lost from three types of agricultural systems at Rothamsted Research’s North Wyke Farm Platform over five individual storm events. The wild yam Dioscorea hirtiflora subsp. orientalis, native to the Mtwara rural district in Tanzania, remains underutilized despite its considerable nutritional and health benefits. This study aimed to assess the nutritional composition and antioxidant properties of its tubers. Standard AOAC procedures were employed to analyze dried tuber samples, while antioxidants and antioxidant capacity were evaluated. The tuber was found to have carbohydrates content ranging from 21.02% to 23.57%, crude protein from 1.0% to 1.5%, crude lipids between 0.46% and 0.68%, and crude fiber from 11.26% to 13.52%, with an energy value of 97.26 to 101.1 Kcal/100 g. BACKGROUND: Multiplexed Assays of Variant Effects (MAVEs) can test all possible single variants in a gene of interest. The resulting saturation-style functional data may help resolve variant classification disparities between populations, especially for Variants of Uncertain Significance (VUS).

METHODS: We analyzed clinical significance classifications in 213,663 individuals of European-like genetic ancestry versus 206,975 individuals of non-European-like genetic ancestry from All of Us and the Genome Aggregation Database. Then, we incorporated clinically calibrated MAVE data into the Clinical Genome Resource's Variant Curation Expert Panel rules to automate VUS reclassification for BRCA1, TP53, and PTEN.

Brain malformations represent a heterogeneous group of abnormalities of neural morphogenesis, often associated with aberrations of neuronal connectivity and brain volume. Prenatal detection of brain malformations requires a clear understanding of embryology and developmental morphology through the various stages of gestation. This expert panel review is written with the central aim of providing an easy-to-understand road map to improve prenatal detection and characterization of structural malformations based on the current understanding of normal and aberrant brain development. Background: Higher circulating concentrations of NT-proBNP (N-terminal pro-B-type natriuretic peptide) and high-sensitivity cardiac troponin T (hs-cTnT) and I (hs-cTnI) are associated with left ventricular remodeling and with incident heart failure. The associations of these cardiac biomarkers with changes in cardiac structure and function over time are uncharacterized.
