BACKGROUND: Carbapenemase-producing (CP) Escherichia coli (CP-Ec) are a global public health threat. We aimed to describe the clinical and molecular epidemiology and outcomes of patients from several countries with CP-Ec isolates obtained from a prospective cohort.
METHODS: Patients with CP-Ec were enrolled from 26 hospitals in 6 countries. Clinical data were collected, and isolates underwent whole-genome sequencing. Clinical and molecular features and outcomes associated with isolates with or without metallo-β-lactamases (MBLs) were compared. The primary outcome was desirability of outcome ranking (DOOR) at 30 days after the index culture.

Perinatal opioid use and neonatal withdrawal continue to rise rapidly in the face of the growing epidemic of opioid addiction in the United States, with rural areas more severely impacted. Despite several decades of research and development of practice guidelines, maternal and neonatal outcomes have not improved substantially. This focused ethnography aimed to address that gap by exploring rural women's experiences and perceptions of care to inform development of efficacious, holistic models of care to improve outcomes for these women and their children. Background
Fetal exposure to maternal mood dysregulation influences child cognitive and emotional development, which may have long-lasting implications for mental health. However, the neurobiological alterations associated with this dimension of adversity have yet to be explored. Here, we tested the hypothesis that fetal exposure to entropy, a novel index of dysregulated maternal mood, would predict the integrity of the salience network, which is involved in emotional processing.Methods

The TP53 tumor suppressor gene is mutated early in the majority of patients with triple-negative breast cancer (TNBC). The most frequent TP53 alterations are missense mutations that contribute to tumor aggressiveness. We developed an autochthonous somatic K14-Cre driven TNBC mouse model with p53R172H and p53R245W mutations in which mutant p53 can be toggled on and off genetically while leaving the tumor microenvironment intact and wild-type for p53. These mice develop TNBCs with a median latency of 1 y. Deletion of mutant p53R172H or p53R245W in vivo in these tumors blunts their tumor growth and significantly extends survival of mice. The severity of infectious disease outbreaks is governed by patterns of human contact, which vary by geography, social organization, mobility, access to technology and healthcare, economic development, and culture. Whereas globalized societies and urban centers exhibit characteristics that can heighten vulnerability to pandemics, small-scale subsistence societies occupying remote, rural areas may be buffered. Accordingly, voluntary collective isolation has been proposed as one strategy to mitigate the impacts of COVID-19 and other pandemics on small-scale Indigenous populations with minimal access to healthcare infrastructure. The locust borer, Magacyllene robiniae, belongs to the longhorned beetle family, referring to the long antennae of most of the species in this group. In Utah, locust borer has damaged black locust trees in most northern Utah counties. The only host is black locust; honeylocust and other trees are not affected by this pest. This fact sheet describes locust borer and its life cycle, symptoms, and control.

Modern cosmology has revealed a Universe that is wholly unexpected; a defining feature is that its largest constituent parts are either dark or hidden behind the cosmological horizon, making them in principle unobservable.
These discoveries raise the important question of what constitutes an aesthetic appreciation of the Universe? By considering various modes of appreciation, I argue that a fuller aesthetic appreciation of the Universe would need to augment its visual forms of appreciation with conceptual and imaginative modes of appreciation informed by scientific knowledge.
This argument can have a direct bearing on the long-standing debate concerning the role of cognitive …

We recently reported that children with multiple birth defects have a significantly higher risk of childhood cancer. We performed whole-genome sequencing on a cohort of probands from this study with birth defects and cancer and their parents. Structural variant analysis identified a novel 5 kb de novo heterozygous inframe deletion overlapping the catalytic domain of USP9X in a female proband with multiple birth defects, developmental delay, and B-cell acute lymphoblastic leukemia (B-ALL). Her phenotype was consistent with female-restricted X-linked syndromic intellectual developmental disorder-99 (MRXS99F). Supranucleosomal chromatin structure, including chromatin domain conformation, is involved in the regulation of gene expression and its dysregulation has been associated with carcinogenesis. Prior studies have shown that cells in the buccal mucosa carry a molecular signature of lung cancer among the cigarette-smoking population, the phenomenon known as field carcinogenesis or field of injury. Thus, we hypothesized that chromatin structural changes in buccal mucosa can be predictive of lung cancer. BACKGROUND: Postoperative respiratory failure is a major complication that affects up to 10% of patients who undergo cardiac surgery and has a high in-hospital mortality rate. Few studies have investigated whether patients who require tracheostomy for postoperative respiratory failure after continuous-flow left ventricular assist device (CF-LVAD) implantation have worse survival outcomes than patients who do not.
OBJECTIVE: To identify risk factors for respiratory failure necessitating tracheostomy in CF-LVAD recipients and to compare survival outcomes between those who did and did not require tracheostomy.

Paragonimiasis is a zoonotic, food-borne trematode infection that affects 21 million people globally. Trematodes interact with their hosts via extracellular vesicles (EV) that carry protein and RNA cargo. We analyzed EV in excretory-secretory products (ESP) released by Paragonimus kellicotti adult worms cultured in vitro (EV ESP) and EV isolated from lung cyst fluid (EV CFP) recovered from infected gerbils. The majority of EV were approximately 30-50 nm in diameter. We identified 548 P. kellicotti-derived proteins in EV ESP by mass spectrometry and 8 proteins in EV CFP of which 7 were also present in EV ESP. No parasite-derived proteins were …

Synergistic drug combinations provide huge potentials to enhance therapeutic efficacy and to reduce adverse reactions. However, effective and synergistic drug combination prediction remains an open question because of the unknown causal disease signaling pathways. Though various deep learning (AI) models have been proposed to quantitatively predict the synergism of drug combinations, the major limitation of existing deep learning methods is that they are inherently not interpretable, which makes the conclusions of AI models untransparent to human experts, henceforth limiting the robustness of the model conclusion and the implementation ability of these models in real-world human-AI healthcare. The Church appears to lack a concrete framework for analyzing and responding to violations of women’s dignity. This oversight can be addressed by applying the principles espoused in Catholic Social Teaching to the present-day situation of women worldwide to articulate an affirmative Catholic vision for the protection and promotion of women. This paper examines the tradition of the Catholic Church and her scriptures in order to find positive affirmations of the dignity of women and call to mind places where such affirmations are still lacking. This article ties together reflections on the Eucharist and reflections on the pandemic by appealing to the author’s experiences with celiac disease. Both celiac and the pandemic force Christians to ask the question “What does it mean to approach Christ’s body when it threatens our life?” Acknowledging the complexity of this question is key to understanding and communicating with Christians across the world, especially in high- stakes situations like a pandemic, where misrepresentation runs rampant. As embodied people who benefit spiritually from the concrete aspects of the sacraments for the understanding of spiritual realities, breastfeeding provides a concrete experience that reflects the bodily self-gift of Christ’s Passion. The analogy of desiring the Eucharist as an infant desiring to latch provides an opportunity to consider the transformative power of the Eucharist to unite one to Christ, nourish one’s spiritual growth, and strengthen the bond of the Body of Christ.

Introduction: While the effects of long-term measures of inflammation and stress are well studied, less is known about the effects of an acute exercise challenge on exercise in young healthy individuals. Methods: This was a randomized crossover design (mean age = 19.25, SD = 1.45)that measured biomarkers of stress (cortisoland salivary alpha amylase, sAA)and inflammation (IL-1β, CRP)in an exercise and control condition. In the exercise condition, participants walked or ran on a treadmill at 75-85% of their maximum heart rate for 40minutes. Under the control condition, participants stood for 40 minutes to control for orthostatic effects. Biomarkers were quantified from …

Skin Extracellular Matrix Breakdown Following Paclitaxel Therapy In Patients With Chemotherapy-Induced Peripheral Neuropathy., Nathan P Staff, Sybil C Hrstka, Surendra Dasari, Enrico Capobianco, Sandra Rieger

The chemotherapeutic agent paclitaxel causes peripheral neuropathy, a dose-limiting side effect, in up to 68% of cancer patients. In this study, we investigated the impact of paclitaxel therapy on the skin of breast cancer patients with chemotherapy-induced peripheral neuropathy (CIPN), building upon previous findings in zebrafish and rodents. Comprehensive assessments, including neurological examinations and quality of life questionnaires, were conducted, followed by intraepidermal nerve fiber (IENF) density evaluations using skin punch biopsies. Additionally, RNA sequencing, immunostaining for Matrix-Metalloproteinase 13 (MMP-13), and transmission electron microscopy provided insights into molecular and ultrastructural changes in this skin. Biotic perchlorate reduction, a metabolic process that potentially occurs on Mars, has long been the subject of speculation in astrobiology. To determine whether it could be a viable metabolic strategy on the Red Planet, we sought to determine whether biotic perchlorate reduction could evolve and persist in Mars-analog environments on Earth. In this study, we introduce Gene Dive, a novel computational methodology that leverages profile hidden Markov models, sequence alignment, phylogenetic tree construction, and Faith's phylogenetic diversity to probe the prevalence and diversity of perchlorate reducing genes in metagenomes. A lattice-based version of the aggregation-volume-bias Monte Carlo method that was introduced recently has allowed for the extension of the calculation of the nucleation free energies from liquid clusters to solid clusters. Here, it was used to calculate the nucleation free energies of both bcc and fcc clusters formed by Lennard-Jones particles. Under the simulation conditions considered in this study, a cross-over of the thermodynamic stability from the bcc to the fcc structure was observed directly from the free energy results.
