Obesity is a significant global health concern. Non-alcoholic fatty liver disease and non-alcoholic steatohepatitis (NASH) are common risk factors for hepatocellular carcinoma (HCC) and are closely associated with metabolic comorbidities, including obesity and diabetes. The TP53 tumor suppressor is the most frequently mutated gene in liver cancers, with half of these alterations being missense mutations. These mutations produce highly abundant proteins in cancer cells which have both inhibitory effects on wildtype (WT) p53, and gain-of-function (GOF) activities that contribute to tumor progression. This study evaluates the perceptions of 122 pre-service and in-service Nigerian teachers on adopting an online training module during the COVID-19 pandemic, a period marked by significant educational challenges and shifts. Employing a qualitative approach through open-ended online questions, it delves into the factors critical to enhancing online training effectiveness. Key findings highlight the importance of training in imparting theoretical knowledge and practical skills, the quality and relevance of content, interactivity, practical examples, and appropriate video length. These align with existing literature, underscoring the effectiveness of online training in challenging times. Significantly, nearly all participants expressed satisfaction with the training. …

In order to improve conservation outcomes translocation or reintroduction of individuals may be necessary. When song learning birds are translocated, changes in the cultural diversity of song repertoires, or abnormal vocalizations, in the new population can be a problem. We monitored song production over 8 years in a reintroduced population of the cirl bunting (Emberiza cirlus). Chicks were removed from nests in Devon, UK, between 2006 and 2011, translocated at 6 days old to be hand-reared and released in Cornwall, UK. Idiopathic pulmonary fibrosis (IPF) is a fatal and devastating lung disease of unknown etiology, described as the result of multiple cycles of epithelial cell injury and fibroblast activation. Despite this impressive increase in understanding, a therapy that reverses this form of fibrosis remains elusive. In our previous study, we found that miR-29b has a therapeutic effect on pulmonary fibrosis. However, its anti-fibrotic mechanism is not yet clear. Recently, our study identified that F-Actin Binding Protein (TRIOBP) is one of the target genes of miR-29b and found that deficiency of TRIOBP increases resistance to lung fibrosis in vivo. Renal and perinephric abscesses are rare purulent infections within or around renal parenchyma, typically treated with antibiotics or various procedural approaches depending on abscess size. In this case report, we describe the novel use of a transgastric endoscopic ultrasound (EUS)-guided technique with placement of a stent for drainage between a renal abscess and the stomach in a patient who had failed attempted percutaneous drainage twice and where an open surgical approach was deemed inappropriate. This article explores the relationship between contract and status in the context of contemporary social policy. Using examples of contract in the areas of unemployment policy (what is here called the workfare contract) and what has become known as the financialisation of the welfare state (Social Impact Bonds), the article identifies the types of bonds and obligations involved in those contracts and their sources. Purpose: To compare the effects of weight-based, reduced, and standard IV insulin doses on potassium reduction and hypoglycemia incidence in the treatment of acute hyperkalemia.

Methods: The hyperkalemia treatment order-set was updated to include reduced and weight-based insulin dose options. Patients ≥ 18 years old with acute hyperkalemia treated with insulin using the order-set were included. Patients with missing data or who started dialysis before follow-up potassium monitoring were excluded. The primary efficacy outcome was reduction in serum potassium. The primary safety outcome was incidence of hypoglycemia and severe hypoglycemia. Secondary outcomes included insulin dose administered, completion of …

The Hoffmann sign or reflex is commonly taught as part of the physical examination to detect abstract upper motor neuron pathology. We present a case of a patient with congenital spinal stenosis and a fall who had a positive Hoffmann sign as the result of a central cord syndrome. The materials contain a video of this physical exam finding and briefly review its history, reliability and clinical utility.

Purpose: To investigate how microgreens educational programming impacts the relationship between nutrition attitudes and healthy eating habits. Methods: A three-part workshop in Philadelphia, PA that consisted of a pre-survey, presentation, and microgreens kit assembly session was held. A post-survey was distributed electronically 1 week after the event. Results: 9 participants voluntarily attended the workshop and completed the pre-survey, and one-third of the participants submitted post-survey feedback. Participants were middle-aged, Black women from the Allegheny West neighborhood of Philadelphia, a predominantly (97.5%) Black neighborhood. The survey showed microgreens were well-received and suited for urban communities with little space and cold weather. …

Objective: In 2019 the American Academy of Pediatrics (AAP) called for increased access to Metabolic and Bariatric Surgery (MBS). To understand adolescent weight loss programs offering MBS in Pennsylvania, a survey was completed.

Methods: Adolescent weight loss centers in Pennsylvania offering MBS were identified using the American Society for Metabolic and Bariatric Surgery (ASMBS) on-line directory and Google search. Phone interviews were conducted with the director of each program.

Results: Eight programs were identified. Operations were performed in seven adult hospitals. Six had Comprehensive Adolescent Accreditation through the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP). A total …

The study of modern cave deposits forming under near isotopic equilibrium conditions can potentially help disentangle the processes influencing the oxygen isotope system and suitability of stalagmites as archives of past hydrological or thermal changes. We used cave monitoring to evaluate the impact of kinetic isotope fractionation and assess the conditions under which modern cave carbonates form in the Caumont cave and quarry system, located in Normandy, northwest France. Over 20 months, we collected climatological data, dripwater, and modern carbonate samples at 2–4-week intervals at three different stations inside the Caumont cave and quarry system. Introduction. Thromboangiitis obliterans, often referred to as Buerger's disease, is a vascular disease that causes chronic inflammatory conditions, edema, and vascular obstruction due to blood clots. In Cipto Mangunkusumo General Hospital (CMGH), sympathectomy is a rarely done procedure. This evidence–based case report aimed to prove the effectiveness of sympathectomy for Buerger's Disease to reduce pain.
