Identifying patients with depression is essential, and early detection of depression leads to early intervention. Routine screening of patients for depression is the responsibility of the primary care provider, but many patients have not had a primary care visit in the recent past. Those patients without a primary care provider are likely not being screened for depression. Urgent care (UC) may be the best opportunity to screen patients who do not have a medical home. Standardized Medication Reconciliation (Med Rec) policies can impact healthcare organizations' patient safety by reducing these medication errors, decreasing readmission rates, enhance the management of chronic disease while also offering fiscal stewardship for healthcare systems. Medication management standardized policies are an expectation of systems aiming to gain Joint Commission (JC) accreditation. Abstract
Background: A north-central Virginia university and a hospital in the British West Indies (BWI) entered a collaborative agreement in 2020. Growing competitive environments heightened awareness and urgency in creating expansively competent, influential nurse leaders in the BWI. Without formalized data to quantify or validate nurse manager strengths, leadership style, or competencies, an assessment of these items and an evidence-based coaching intervention were performed.
Methods: The population for this evidence-based (EB) practice project was 20 inpatient nurse leaders. The Multifactor Leadership Questionnaire (MLQ) and the Clifton Strength Finders Assessment (CSF) for Managers were deployed as pre-intervention assessment tools. …

The COVID-19 pandemic disrupted HIV care and prevention services. COVID-19 guidelines required physical and social distancing, resulting in movement restrictions and limited healthcare services. People living with HIV (PLWH) may be more vulnerable to the pandemic’s impact due to immune suppression and socioeconomic status than people without HIV. Consistent use of pre-exposure prophylaxis (PrEP), condoms, and regular sexually transmitted infection tests are critical strategies for HIV prevention. The pandemic’s impact on PLWH have not been thoroughly studied, and whether PLWH may be more impacted than PrEP users is inconclusive.

Advancements in next-generation sequencing technologies have made omics technologies accessible to more researchers and large omics datasets are becoming more common in biological research. Omics technologies, including genomics, transcriptomics, proteomics, and metabolomics, have been pivotal in our current understanding of development, how developmental defects can lead to cancer, and importantly, our approach to chemotherapeutic treatment for cancer patients. This dissertation describes two projects using both experimental and omics technologies to investigate molecular mechanisms in Pancreatic Ductal Adenocarcinoma (PDAC) and mammary gland development.

Formal caregivers are the direct care workforce in nursing homes that aid residents. Providing care to residents is hazardous and demanding, as highlighted during the pandemic. Caregiver burden affects formal caregivers; however, is interchanged with caregiver stress and caregiver burnout. The overall purpose of this dissertation was to describe formal caregiver burden among nursing assistants who provide direct care to residents in nursing homes. Cancer therapies often lead to premature ovarian insufficiency by ovarian damage and follicle depletion. However, the oocyte death pathway induced by chemotherapies has been debated. Here we clarified the precise mechanism of ovarian depletion induced by cyclophosphamide and doxorubicin. Cyclophosphamide induced direct apoptosis in oocytes rather than overactivation of primordial follicles. Dormant oocytes instead of activated oocytes with high PI3K activity were more sensitive to cyclophosphamide. Although the ABL inhibitor, GNF2, has been proposed to ameliorate the apoptosis of primordial follicles against cyclophosphamide, GNF2 did not prevent the loss of primordial follicles induced by cyclophosphamide in the systems tested. Cholesterol is essential for life. It is particularly important in the brain as it relies on de novo synthesis of cholesterol following the formation of the blood brain barrier (BBB). As such, disrupting sterol biosynthesis during neurodevelopment can have devastating outcomes. The most common post-lanosterol sterol biosynthesis disorder, Smith-Lemli-Opitz Syndrome, arises from a faulty DHCR7 enzyme. DHCR7 has also been shown to be inhibited by several psychotropic medications. Here we assess six beta-blockers and their effects on sterol biosynthesis in vitro. Two beta-blockers, metoprolol and nebivolol strongly inhibit DHCR7 in four separate in vitro models of both mouse and …

A m-opioid receptor (OPRM1) single-nucleotide-polymorphism, found in both humans and rhesus macaques mediates the mother-infant attachment bond. Because mothers treat their sons and daughters differently, it is somewhat surprising that the role of infant sex has not been assessed in the context of a maternal-OPRM1-genotype-by-infant-sex interaction. The present study investigates the effect of maternal-OPRM1-genotype and infant sex on mother-infant behaviors. Over the first six months of offspring life, mother-infant behavioral data assessing attachment quality was collected twice weekly from a large number of rhesus monkey mother-infant pairs (N=161 dyads; n=64 female infants, n=97 male infants). The study examines use of library information resources by students of Sokoine University of Agriculture in Tanzania. The study employed cross-sectional research. Systematic sampling was to select 120 respondents. Data was collected using structured questionnaire. Quantitative data was coded and analyzed using the statistical package for social science (SPSS). Frequency and percentages was analyzed. The results found that most of the respondents mainly used special collection, general collection and special reserve. Despite increasing incidence, male breast cancer (MBC) remains understudied in almost all aspects of disease management. MBC patients are more likely to be diagnosed with larger tumors, at an advanced stage, and to experience poorer survival than female patients. There is a need to identify modifiable factors that can improve MBC patients’ survival.
This dissertation aimed to investigate whether there was a difference in the refusal of recommended treatments and related survival, by gender, in breast cancer patients enrolled in the National Cancer Database. Hydroxychloroquine (HCQ) is an established drug that has been used for the treatment of multiple diseases, including malaria, rheumatoid arthritis, and lupus erythematosus. Several recently published studies have highlighted its beneficial use in various types of cancers, and a plethora of clinical trials are currently evaluating its use in cancer therapies, wherein some of the results look promising. Frailty, the state of being vulnerable with susceptibility to decreased age-related physical reserve, is found in 56-79% of adults with end-stage HF receiving durable left ventricular assist device (dLVAD) therapy. Frailty increases risk of adverse events, including increased mortality in the dLVAD population, but less is known about depression, quality of life (QOL), cognition and frailty responsiveness. Protein kinase Cα (PKCα) is a member of the PKC family of serine/threonine kinases, which have been implicated in regulation of many cellular processes, including cell proliferation, differentiation, survival, and transformation. A large body of evidence from the Black laboratory and others support an anti-proliferative function of PKCα in normal epithelial tissues, including the intestinal mucosa and endometrial epithelium. PKCα is also tumor suppressive in epithelial cancers, such as colorectal cancer (CRC) and endometrial cancer (EC). Aortic aneurysm is a widespread condition, characterized by dilation and weakening of the aortic vessel wall. Dissection and rupture of the aorta due to uncontrolled aneurysm progression carries a high risk of death. Although both thoracic aortic aneurysms (TAA) and abdominal aortic aneurysms (AAA) share similar risks of rupture and mortality, the pathophysiology of each disease is unique. TAA is a hallmark feature of Marfan syndrome (MFS), a connective tissue disorder characterized by malformation of functional elastic fibers in the aortic extracellular matrix (ECM). Patients who are immunosuppressed, such as solid organ transplant recipients (SOTRs), are known to have a higher risk of developing cutaneous squamous cell carcinoma (cSCC). This population is at a higher risk of metastasis and worse disease-specific survival, with over 1 million cases and an estimated 3,932 to 8,791 deaths each year in the United States. Large cancer genomic databases have reported on genomic drivers for many cancer subtypes, yet cSCC has not been included. Hematopoietic stem cells (HSCs) allow for the formation of all cell types in the blood and maintain these populations throughout a person’s life. The process of hematopoiesis is regulated through a variety of molecular mechanisms that are either signaled from the cell’s environment (extrinsic) or from within the cell itself (intrinsic). One intrinsic mechanism that regulates hematopoietic cell fate decisions is the ubiquitin proteasome system (UPS). Prostate cancer (PCa) is the second leading cause of cancer-related deaths in the U.S. in men, mainly due to the development of castration-resistant PCa (CRPC). MnTnBuOE-2-PyP5+ (BuOE), a superoxide dismutase (SOD) mimic, inhibits human PCa cell proliferation through hydrogen peroxide (H2O2). p66Shc protein level is upregulated in CRPC and is associated with cell proliferation in PCa cells.We propose that the different sensitivity of those PCa cells to BuOE could be due to the expression levels of p66Shc protein.

Mitochondria are at the center of biological phenomena such as aging and diseases, especially neurodegenerative diseases. While the discovery of mitochondria only came approximately 200 years after the cell was discovered, a lot of progress has been made since. The mitochondrial genome encodes proteins vital for mitochondrial function. These proteins are only a subset of the proteins present in mitochondria; the rest are nuclear encoded. The nucleus also encodes cytosolic proteins vital for mitochondrial maintenance. To comprehensively capture the risk and protective factors of ACE exposure and ACE-related outcomes, we propose to consider a social-ecological model framework. The concept of the Social-Ecological Model was originally introduced in the 1970s by psychologist Urie Bronfenbrenner and designed to theoretically capture the multifaceted influence different personal, social, and environmental factors have on health behaviors and outcomes. Additionally, this would help to determine what prevention and intervention methods would be more appropriate to implement. According to the Centers for Disease Control and Prevention, in 2015-2016 45.8% of the U.S. population used prescription drugs in the past 30 days. (CDC, 2019) Although drug therapy can be effective in treating disease, full benefits are often not realized because many patients do not take their medications as prescribed. INTRODUCTION: To promote their personal and professional growth, medical educators need practical, actionable feedback on their scholarship, as well as guidance for documenting their scholarship in educator portfolios. We offer a framework and resources to provide formative and summative feedback to faculty, administrators, and/or learners delivering an oral presentation at a face-to-face or virtual health professions education meeting.
