Decreasing Pre-Procedural Fasting Times: A Benchmark Study, Sheila Gasparek
Decreasing Pre-Procedural Fasting Times: A Benchmark Study, Sheila Gasparek
MSN Capstone Projects
On a busy Surgical Trauma unit, scheduled procedures and operations are often delayed for priority emergent cases. Patients admitted to the unit with delayed procedures often endure extended fasting times beyond the six-hour intention of standard nothing by mouth (NPO) after midnight fasting order. With prolonged fasting times, patients inquire about pending surgery times and dietary options. Some providers may allow ice chips, but most doctors prefer patients to remain NPO in case Operating Room availability opens for surgery, leaving already anxious and uncomfortable patients more frustrated as prolonged fasting affects patients’ physical and psychological well-being (Anderson & Comrie, 2009). …

Evidence-Based Individualized Discharge Educational Intervention For Coronary Artery Disease (Cad) Patients; A Benchmark Project, Sumita Thapa
MSN Capstone Projects
Coronary artery disease (CAD) is the leading cause of death worldwide, whereas, in the United States, 16,800,000 people are affected, resulting in 450,000 deaths annually. Patients’ perceptions of self-management related to treatment adherence, lifestyle modifications, and risk-factor management are significant indicators of outcomes following a cardiac event. To promote self-management in CAD patients, this benchmark project aims to determine the impact of individualized education interventions by establishing a standardized nurse-led educational intervention in the cardiac unit that emphasizes adherence to self-management and treatment regimen to improve quality of life and prevent rehospitalization due to worsening cardiac events. The evidence-based findings …

A Need For Change: Patient Education And Diabetic Control, Meredith Sexton
A Need For Change: Patient Education And Diabetic Control, Meredith Sexton
MSN Capstone Projects
Type II diabetes is a difficult diagnosis for patients to receive. Patients must work through the stigma attached to the diagnosis and understand how it affects their daily lives. It is important for our patients to understand the disease, how to control their blood sugars and how to reduce the risk of preventable complications. This project is based on the following question: “In patients diagnosed with Type II diabetes, how does going to a formal support and education session at least once per week compared to not going to a formal support group affect diabetic control, as measured by patients’ …
Reducing 30-Day Hospital Readmissions Through Interprofessional Collaboration/Multidisciplinary Team-Based Care Approach: A Benchmark Project, Julius Solis
MSN Capstone Projects
Hospital readmissions are a costly burden that hospitals often must sustain because of current reimbursement policies. The Hospital Readmission Reduction Program (HRRP), a value-based purchasing program, was established to reduce or penalize payment to hospitals for excessive readmissions (Centers for Medicare & Medicaid Services, 2020). Lower financial payments by payors often have a negative impact on a hospital’s overall performance through the inability to purchase new and up to date equipment and materials, budget cuts, downsizing of personnel and departments, amongst all other things. Auerbach et al. (2016) estimate that approximately one-quarter or 26.9% of hospital readmissions were preventable. The …

Kinesiology and Nutrition Sciences Faculty Research
Although it has been generally acknowledged that participating in rehabilitation programs is better for chronic diseases or post-surgery, the adherence rates of these programs remain lower than expected. According to the World Health Organization (WHO), adherence has been defined as follows: "the extent to which a person's behavior corresponds with agreed recommendations from a healthcare provider." In general, rehabilitation is well investigated, and in chronic diseases like chronic obstructive pulmonary disease (COPD), cardiovascular disease, neuromuscular disease, cancer, and even psychiatric diseases like depression, it has been shown that exercise therapy, in particular, has beneficial effects on morbidity, mortality, and convalescence …

Understanding The Effect Of Dietary Palmitic Acid On Glycolysis During Innate Immune Memory In Macrophages, Khaleda A. Aqaei
Understanding The Effect Of Dietary Palmitic Acid On Glycolysis During Innate Immune Memory In Macrophages, Khaleda A. Aqaei
PSU McNair Scholars Online Journal
Trained immunity is long-term innate immune memory induced by a primary stimulus, which leads to hyper-inflammation upon secondary stimulation with a homologous or heterologous ligand. Trained immunity is mediated by epigenetic and metabolic reprogramming of the target cell and leads to modification of gene expression and cellular function. Classically, trained immunity is initiated by b-glucans, an inflammatory molecule found on the exterior of fungal species. Interestingly, our lab has recently described that dietary fatty acids can initiate trained immunity, working through similar pathways as b-glucans. Specifically, our data show that a pre-treatment with a specific dietary saturated fatty acid (SFA), …

Rural communities are disproportionally affected by food insecurity, making them vulnerable to the consequences of supply disruptions caused by the COVID-19 pandemic. While access to food was initially diminished due to food supply disruptions, little is known about the mechanisms through which federal emergency assistance programs impacted food access in rural populations. Through a series of five focus groups in spring 2021, we examined the impact of the COVID-19 pandemic on food access in a rural Appalachian community in Kentucky. Data were analyzed using a Grounded Theory Approach. Findings revealed the following four primary themes: food scarcity in grocery stores; …

How regular physical activity is able to improve health remains poorly understood. The release of factors from skeletal muscle following exercise has been proposed as a possible mechanism mediating such systemic benefits. We describe a mechanism wherein skeletal muscle, in response to a hypertrophic stimulus induced by mechanical overload (MOV), released extracellular vesicles (EVs) containing muscle-specific miR-1 that were preferentially taken up by epidydimal white adipose tissue (eWAT). In eWAT, miR-1 promoted adrenergic signaling and lipolysis by targeting Tfap2α, a known repressor of Adrβ3 expression. Inhibiting EV release prevented the MOV-induced increase in eWAT miR-1 abundance and expression of …

As a relatively new topic, previous literature fails to address the relation of social determinants of health to COVID-19. By combining existing knowledge on previous epidemics, information on the role of social determinants of health, and data on COVID-19, conclusions can be made about the path going forward. Historically, nonpharmacologic interventions such as quarantine periods, face coverings, and social distancing have been used in epidemics such as the Spanish Influenza of 1918-19. In terms of social determinants of health, geographical location, educational level, and income level all play a part in the health of an individual. Rather than emphasizing personal …

Novel Approaches For Enhancing Cell Survival And Function In Vivo, Ou Wang
Department of Chemical and Biomolecular Engineering: Dissertations, Theses, and Student Research
FDA has approved several cell-based therapeutics and hundreds of cell therapy clinical trials are ongoing. Cells will be a significant type of medicine after small molecule and protein drugs. However, several obstacles need to be addressed to achieve the widespread use of cellular therapeutics. The first challenge is the low efficacy of cell transplantation due to low retention, survival, integration, and function of cells in vivo. The second challenge is producing a massive number of cells for clinical treatment with cost-effectively and reproducibly technologies.
In this thesis, we proposed and investigated two approaches to address these challenges. To begin …

Zoom In Closer explores ideas of sensuality through glass and narratives. I am interested in the material qualities of glass, including translucency, fluidity, and warmth; and how these qualities can be translated into languages of intimacy. This thesis is based on personal experience and scholarly research on the enigma of female sexual desires. Zoom In Closer is a body of work that utilizes glass objects, glass processes, films, illustrations, and photography that question what does it means to live authentically despite the influences of rape culture.
Exploring Non-Invasive Features For Continuous Glucose Monitoring, Brian Allen Bogue Jimenez
Exploring Non-Invasive Features For Continuous Glucose Monitoring, Brian Allen Bogue Jimenez
Electronic Theses and Dissertations Archive
Glucose monitoring technologies allow users to monitor glycemic fluctuations (e.g., current glucose levels in their blood, also known as glycemia). This is particularly important for individuals who suffer from diabetes mellitus (DM), commonly referred to as diabetes. Traditional self-monitoring blood glucose (SMBG) devices require the user to prick their finger and extract a blood drop to measure the blood glucose based on chemical reactions with the blood. Unlike traditional glucometer devices, non-invasive continuous glucose monitoring (NICGM) devices aim to solve these issues by consistently monitoring users’ blood glucose levels (BGL) and without invasively acquiring a sample. This Master Thesis aims …

Background: Intestinal inflammation and malabsorption in environmental enteric dysfunction (EED) are associated with early childhood growth faltering in impoverished settings worldwide.
Objectives: The goal of this study was to identify candidate biomarkers associated with inflammation, EED histology, and as predictors of later growth outcomes by focusing on the liver-gut axis by investigating the bile acid metabolome.
Methods: Undernourished rural Pakistani infants (n = 365) with weight-for-height Z score (WHZ) < -2 were followed up to the age of 24 mo and monitored for growth, infections, and EED. Well-nourished local children (n = 51) were controls, based on consistent WHZ > 0 and height-for-age Z score (HAZ) > -1 on 2 consecutive visits at 3 and 6 mo. Serum bile acid (sBA) profiles were measured by tandem MS at the ages of 3-6 …
The Molecular Basis For Natural Competence In Acinetobacter, Yafan Yu
The Molecular Basis For Natural Competence In Acinetobacter, Yafan Yu
Department of Food Science and Technology: Dissertations, Theses, and Student Research
Bacteria use Horizontal Gene Transfer (HGT) to acquire genetic material, leading to the development of novel traits, such as the spread and development of antibiotic resistance. Natural competence is one type of HGT accomplished by DNA uptake from the environment and incorporation into the genome. Bacteria from Acinetobacter are wildly distributed in the environment and are naturally competent. This propensity is a key factor to the steady increase in drug resistance in Acinetobacter, which is a cause for concern in human health as Acinetobacter is a major source of nosocomial infections. During natural competence, type IV pili (T4P) and related …

The Application Of Mathematical Optimization And Flavor-Detection Technologies For Modeling Aroma Of Hops, Yutong Liu
Department of Food Science and Technology: Dissertations, Theses, and Student Research
In recent years, proprietary hops (Citra, Simcoe, and Mosaic) become the most sought-after hops among brewers due to their excellent aroma. However, they are restricted to the owners unless other growers purchase the costly licensing agreements. Many public hops are available to the growers without any additional costs, but their aroma is difficult to match to the proprietary hops. Although proprietary and public hop varieties are unique in their aroma profiles, all hops varieties contain similar volatile compounds, merely differ in the quantity of different individual compounds. The main objective of this thesis was to investigate the feasibility of matching …
