Covid-19, Human Rights And Public Health In Prisons: A Case Study Of Nova Scotia’S Experience During The First Wave Of The Pandemic, Adelina Iftene
Dalhousie Law Journal
The importance of preventing outbreaks in prisons during a pandemic, such as COVID-19, cannot be overstated. The risk of the infection spreading rapidly once inside these institutions is much higher than in the community, due to the underlying vulnerabilities of prison populations and the congregated living nature of prisons. This article documents the Nova Scotia provincial prison system’s experience in dealing with COVID-19 during the first wave, including its uniquely swift decarceration efforts. One goal of this investigation is to identify a set of best practices that can help Canadian prisons systems with their short-term responses to crisis in a …

Modulation Of Effector Gene Expression In The Blood-Feeding Arthropods Cimex Lectularius Linnaeus, (Hemiptera: Cimicidae) And Aedes Albopictus Skuse, (Diptera: Culicidae), Travis C. Van Warmerdam
Theses and Dissertations
Hematophagy, or blood-feeding behavior, has independently evolved multiple times in the class Insecta and results in the transmission of vector-borne parasites, bacteria and viruses causing 17% of all infectious diseases and over 700,000 human deaths annually. One of the major challenges in studying the discrete mechanisms involved in host-response to hematophagy or the host seeking behavior of hematophagous insects, is the vast amount of signaling processes and genes that are inextricably tied to these interactions. These experiments used the clustered regularly interspersed short palindromic repeat (CRISPR)/CRISPR-associated (Cas) and dsRNA knock-down technologies to examine mechanisms of immunological reactivity and host-seeking behavior. …
Urban-Rural Differences In Lifespan Variation In The United States, Benjamin H. Walker
Urban-Rural Differences In Lifespan Variation In The United States, Benjamin H. Walker
Theses and Dissertations
Since the mid-1980s, mortality rate improvements in urban areas have outpaced those in rural areas, leading to substantial urban-rural disparities in mortality. Research on urban-rural mortality disparities has focused exclusively on differences in mortality between urban and rural areas and has not examined differences in the amount of inequality in the length of life within these areas. Lifespan variation is an important dimension of health inequality that complements traditional metrics of mortality (i.e., mortality rates and life expectancy) by indicating the amount of inequality of lifespans within a population. This dissertation provides several contributions to our understanding of urban-rural differences …

Case Report: Neurologic Presentation Of West Nile Virus: Difficult Diagnosis, Eron G. Manusov, Amalia Mora Campuzano, Omar Ahmed, Samantha Macias, Carolina Gomez De Ziegler, Gerardo Munoz Monaco
School of Medicine Publications
West Nile virus infections have surged across the globe. South Texas, located on the path of bird migration, with Culex quinquefasciatus and other Culex species, and biotic primers that predispose the area to epidemics (floods, amplifying hosts, and lack of mosquito control and prevention) remains a highly endemic area for arbovirus spread. West Nile virus infection ranges from mild febrile illness to severe central nervous system involvement. The purpose of this report is to highlight complex presentations of WNV and how confounding presenting symptoms delay diagnosis. The secondary goal is to describe how pandemics, such as SARS-CoV-2, can overwhelm the …
Implementation Of Teach-Back For Discharge Teaching In A Critical Access Hospital: A Quality Improvement Project, Katie Beth Notch
Implementation Of Teach-Back For Discharge Teaching In A Critical Access Hospital: A Quality Improvement Project, Katie Beth Notch
Theses and Dissertations
A lack of comprehension of discharge instructions may cause high readmission and emergency room revisit rates for organizations. At the project site, there was no current evidence-based practice to ensure patient comprehension of discharge instructions. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the implementation of the Institute for Healthcare Improvement (IHI) Always Use Teach-Back Toolkit would impact emergency room revisit rates among adult medical-surgical patients in a critical access hospital in rural Minnesota over four weeks. Afaf Meleis’ transitions theory and the Iowa model for evidence-based practice were the scientific underpinnings of the project. …

Information on criminal cases on torture is provided. The author …
Dicer Represses Antiviral Innate Immunity Pathways In Mouse Embryonic Stem Cells, Chandan Gurung
Dicer Represses Antiviral Innate Immunity Pathways In Mouse Embryonic Stem Cells, Chandan Gurung
Dissertations
Recent studies have demonstrated that embryonic stem cells (ESCs) are deficient in expressing type I interferons (IFN), the cytokines that play key roles in antiviral responses. However, the underlying molecular mechanisms and biological implications of this finding are poorly understood. In this study, I used a synthetic RNA-based assay that can simultaneously assess multiple forms of antiviral responses in ESCs. Dicer is an enzyme essential for RNA interference (RNAi), which is used as a major antiviral mechanism in invertebrates but not clear in vertebrates. RNAi activity is detected in wild-type ESCs but is abolished in Dicer knockout ESCs (D−/−ESCs) as …

Defining Disparities In Cochlear Implantation Through The Social Determinants Of Health, Marissa Schuh, Matthew L. Bush
Defining Disparities In Cochlear Implantation Through The Social Determinants Of Health, Marissa Schuh, Matthew L. Bush
Otolaryngology--Head & Neck Surgery Faculty Publications
Hearing loss is a global public health problem with high prevalence and profound impacts on health. Cochlear implantation (CI) is a well-established evidence-based treatment for hearing loss; however, there are significant disparities in utilization, access, and clinical outcomes among different populations. While variations in CI outcomes are influenced by innate biological differences, a wide array of social, environmental, and economic factors significantly impact optimal outcomes. These differences in hearing health are rooted in inequities of health-related socioeconomic resources. To define disparities and advance equity in CI, there is a pressing need to understand and target these social factors that influence …

Optimizing Empiric Vancomycin Use In Febrile Neutropenia Patients, Nicole Tadros, Erika Dittmar, Radhan B. Gopalani
Nursing & Health Sciences Research Journal
Introduction: National guidelines do not support routine empiric vancomycin use in the initial management of febrile neutropenia (FN) and only recommend it in patients with specific clinical indications. This bi-phasic quality improvement project aimed to evaluate current vancomycin use in FN and improve compliance with a guideline-driven algorithm (GDA) to ensure appropriate prescribing and therapy duration in a community hospital.
Methods: Phase I was a retrospective review of charts of adults receiving empiric vancomycin therapy for FN at a community hospital. Phase II was a prospective review of charts of patients with FN, who received pharmacist-led interventions, to improve de-escalation …

Along with inadequate access to high-quality care, competing health priorities, fragile health systems, and conflicts, there is an associated delay in evidence generation and research from LMICs. Lack of basic epidemiologic understanding of the disease burden in these regions poses a significant knowledge gap as solutions can only be developed and sustained if the scope of the problem is accurately defined. Congenital heart disease (CHD), for example, is the most common birth defect in children. The prevalence of CHD from 1990 to 2017 has progressively increased by 18.7% and more than 90% of children with CHD are born in Low …

COVID-19, for many across the globe, has been a pandemic the likes of which has never been seen before. It has caused many aspects of life to change in order to adapt to the new limitations and cautions brought about by the global spread of a novel virus. However, almost exactly 100 years prior, a strain of the H1N1 influenza virus also known as the Spanish Flu of 1918, similarly uprooted life for millions of people around the globe. These two pandemics, while similarly leading to great disruption of global health, had many differences in the demographics impacted as well …

Like humans, outbred Sprague-Dawley CD rats exhibit a polygenic pattern of inheritance of the obese phenotype and not all individuals exposed to a high calorie intake develop obesity. We hypothesized that differences in gut microbiota composition account for phenotype differences between obese prone (OP) and obese resistant (OR) rats. We studied the gut microbiota composition of OPand OR rats after a high fat (HF) diet and how they respond to fermentation of resistant starch (RS). In phase 1 of the study 28 OP and 28 OR rats were fed a HF diet. In order to determine causal role of microbiota …
