Dementia Care Excellence: Promoting Autonomy Through Person-Centered Care, Alexa Adams
OTD DEx Reports - College of Pharmacy and Health Sciences
"Dementia care excellence is dementia care practice recommendations that promote quality care practices based on a comprehensive review of current evidence, best practices recommendations, and expert opinion, to better define quality care across all care settings, and throughout the disease course. Dementia is a general term used to describe a progressive decline in memory and cognition that results in impaired independence of daily functioning. Dementia care is comprised of medical and social needs, which consistently change through various stages of the disease."

Reducing Behavioral And Psychological Symptoms Of Dementia And Supporting Memory Care: A Multi-Sensory Approach, Olivia Bernas
OTD DEx Reports - College of Pharmacy and Health Sciences
"Dementia is a progressive disease that encompasses a multitude of changes in the body and mind. Individuals with Alzheimer’s disease and related dementias (ADRD) may face changes affecting their ability to engage in familiar roles, routines, activities of daily living, leisure, and community engagement. As individuals with ADRD progress throughout each of the stages of dementia, communication may become increasingly difficult, which can subsequently lead to frustration, agitation, depression and other forms of behavioral and psychological symptoms of dementia (BPSD)."

Feeling Good: Happiness And Health In Dutch Older Adults, Joshua Ray Tanzer
Feeling Good: Happiness And Health In Dutch Older Adults, Joshua Ray Tanzer
Open Access Dissertations
Statement of the problem: Positive psychology research has sought to identify cost efficient and intuitive ways to increase subjective happiness, with broad implications to personal health and overall quality of life (Lee Duckworth, Steen, & Seligman, 2005; Rashid, 2009; Seligman, 2008; Seligman, Ernst, Gillham, Reivich, & Linkins, 2009; Seligman, Rashid, & Parks, 2006). A complication in this research has been conflicting findings in the longitudinal stability of happiness (Easterlin, 2006). This analysis examines age related changes in happiness and physiological immune responsiveness to clarify the implications of positive health interventions.
Methods: Pretest-posttest structural equation modeling is employed to examine longitudinal …

Is There A Role For Ct Pan-Scans In The Initial Work-Up Of Fragility Fracture Patients?, Eric Lepkowsky, Trevor Simcox, Hunter Rogoff, Shahidul Islam, Omid Barzideh
Stony Brook Orthopaedic and Musculoskeletal Journal
Background: CT pan-scans have become increasingly commonplace as part of the initial diagnostic work-up for patients sustaining traumatic injuries. They have proven effective in improving diagnostic accuracy in those with high energy mechanisms of injury. However, the utility of pan-scans in the geriatric population sustaining low energy traumatic injuries remains unproven. There is no study that assesses the usefulness of CT pan-scans in patients who sustain injuries related to osteoporosis, also known as fragility fractures.
Methods: A retrospective review was conducted of patients who sustained a fragility fracture at a Level 1 trauma center over a 15-month period. Patients underwent …

Although exercise modulates appetite regulation and food intake, it remains poorly understood how exercise impacts decision-making about food. The purpose of the present study was to assess the impact of an acute exercise bout on hypothetical choices related to the amount and timing of food intake. Forty-one healthy participants (22.0 ± 2.6 years; 23.7 ± 2.5 kg/m 2 , 56% female) completed 45 min of aerobic exercise and a resting control condition in randomized order. Food amount preferences and intertemporal food preferences (preference for immediate vs. delayed consumption) were assessed using electronic questionnaires with visual food cues. Compared to rest, …

Intimate partner violence includes psychological aggression, physical violence, sexual violence, and stalking from a current or former intimate partner. Past research suggests that exposure to intimate partner violence can impact cognitive and psychological functioning, as well as neurological outcomes. These seem to be compounded in those who suffer a brain injury as a result of trauma to the head, neck or body due to physical and/or sexual violence. However, our understanding of the neurobehavioral and neurobiological effects of head trauma in this population is limited due to factors including difficulty in accessing/recruiting participants, heterogeneity of samples, and premorbid and comorbid …
From Bedside To Mountainside: Preparing New Nurse Practitioners For Advanced Clinical Skills In Remote Settings Using A Novel Training Program, Benjamin Woodard
From Bedside To Mountainside: Preparing New Nurse Practitioners For Advanced Clinical Skills In Remote Settings Using A Novel Training Program, Benjamin Woodard
Yale School of Nursing Digital Theses
The purpose of this doctoral project was to develop and pilot an expert-panel validated training program specifically for nurse practitioners (NPs) in remote settings, and to evaluate how new remote NPs rate their self-efficacy (a measure of confidence) in performing specific advanced clinical skills and procedures (ACSPs) before and after completing this program. The practice of primary care in remote regions requires expanded clinical skills for high acuity scenarios given the isolation. With more new primary care NPs practicing in more remote locales, their preparation for performing ACSPs, (including in complex and high-acuity cases) is critical for safe and effective …

Impact Of A Novel Application In Electrocardiogram Interpretation Confidence And Performance In Nurse Practitioner Students, Hailey Bobin Cole
Doctor of Nursing Practice Projects
For the last hundred years and to this day, one of the most important diagnostic tools in medicine has been the electrocardiogram (EKG or ECG). The decision to pursue a life-saving intervention, or not, may be heavily based on the interpretation of the results of a diagnostic test. With the weight of impacts to outcomes, one would think there would be ample, if not excessive, time and effort given to teaching practitioners how to interpret an ECG. Unfortunately, with each graduating class of physicians, nurse practitioners, and physician assistants, all of whom have within their scope of practice to interpret …

Honors Program Theses
The need for quality end-of-life care has increased drastically around the world in the last few centuries due to a surge in life expectancy. As people are living much longer, they are developing and succumbing to chronic progressive diseases which can significantly decrease quality of life. But what exactly does it mean to care for the aging population who are nearing death? Defining quality care at the end-of-life is important but also differs for individuals based on a variety of factors including age, culture, and life experiences. End-of-life care does not come as “one size fits all” and it challenges …
Health Care Utilization And Costs For Dementia Patients In China, Fang Ba
Health Care Utilization And Costs For Dementia Patients In China, Fang Ba
Public Health Theses
The number of people in the world living with Alzheimer’s Disease and related dementias (ADRD) continues to rise rapidly, which places a substantial burden on people with dementia (PWD), their families, and the society. Such a burden has been rising faster in low-income and middle-income countries (LMIC) where around two-thirds of PWD live. While prior research documents dementia prevalence in China and indicates its large geographic disparities, no study to date has documented the national pattern of health care utilization, quality and costs in China. The absence of knowledge would largely impede the effective allocation of resources for combating dementia …

Considering that the aging population in the Southeast Asian region is rapidly growing, there is an increasing need to promote the subjective well-being of older adults. However, few studies have investigated mechanisms of life satisfaction in developing countries in the Southeast Asian region. This research gap needs to be addressed, since aging and life satisfaction are complex processes that are experienced differently in various sociocultural contexts. The purpose of this study is to understand meanings of life satisfaction and experiences of aging through the perspectives of older adults who live in Jumla, Nepal. We conducted semi-structured, in-depth interviews with twenty …

Exploring Therapeutic Nurse-Patient Communication: Techniques And Barriers, Pierina Rossini
Senior Honors Theses
Effective nurse-patient communication, also described as therapeutic communication, is essential for providing high quality nursing care. It increases patient satisfaction and health, decreases patient anxiety and disease symptoms, and increases patient cooperation and compliance to treatment. Some therapeutic communication techniques include attentive listening, empathy, silence, focusing, open-ended questions, clarification, exploring, clarifying, and summarizing. Unfortunately, there are barriers to therapeutic communication that have been identified. Research regarding nurse-patient therapeutic communication, consisting of secondary, qualitative, descriptive data, points to several barriers to therapeutic communication. Some of the barriers to therapeutic communication include nurse-patient gender difference; patient physical discomfort; nurse-patient language, culture, and …

Purpose: Although ample research has been conducted on resilience to HIV/AIDS, most studies have utilized quantitative methods and focused almost exclusively on people living with HIV/AIDS. A relatively untapped source of knowledge is the perspectives of HIV-negative, middle-aged and older men who have sex with men (MSM) who have been navigating risks and building resilience to HIV/AIDS since the 1980s. Our qualitative, community-based participatory research study examined the perspectives of HIV-negative, middle-aged and older MSM on factors that helped mitigate the risks of and build resilience to HIV/AIDS. Methods: In collaboration with community-based organizations, fourteen participants were recruited for in-depth …

Graduate Student Theses, Dissertations, & Professional Papers
With increasing age, older individuals face a higher likelihood of developing dementia. The rate of cognitive decline resulting from dementia is not equivalent for all, as some patients with dementia are able to function independently longer than others, despite having similar disease burden. The cognitive reserve (CR) theory provides one explanation for the differing rate of decline. CR suggests that there are factors — most notably, educational and occupational attainment — that can protect against the cognitive decline that is a hallmark of dementia. Although the beneficial effects of these notable CR factors are clear, they may not be modifiable. …
