Porcine cysticercosis is a neglected and underestimated disease caused by metacestode stage of the tapeworm, Taenia solium(T. solium). Pigs are the intermediate hosts of T. solium while human are the only known definitive host. The disease has an economicconsequence because the affected farmers lose 50−100 percent of the value of pigs if they are infected. Lack of affordable, easy touse, sensitive, and specific molecular diagnostic tools for detection of infections at the farm level hinders the control of porcinecysticercosis in endemic areas. -ermophilic Campylobacter species are clinically important aetiologies of gastroenteritis in humans throughout the world. -e colonization of different animal reservoirs by Campylobacter poses an important risk for humans through shedding of the pathogen in livestock waste and contamination of water sources, environment, and food. A review of published articles was conducted to obtain information on the prevalence and antimicrobial resistance (AMR) profiles of thermophilic Campylobacter species in humans and animals in sub-Saharan Africa (SSA). Glucosinolates (GSs) are common anionic plant secondary metabolites in the order Brassicales. Together with glucosinolate hydrolysis products (GSHPs), they have recently gained much attention due to their biological activities and mechanisms of action. We review herein the health benefits of GSs/GSHPs, approaches to improve the plant contents, their bioavailability and bioactivity. In this review, only literature published between 2010 and March 2020 was retrieved from various scientific databases. Findings indicate that these compounds (natural, pure, synthetic, and derivatives) play an important role in human/animal health (disease therapy and prevention), plant health (defense chemicals, biofumigants/biocides), and food industries (preservatives). © 2020, The Author(s). Background: Safe, highly curative, short course, direct acting antiviral (DAA) therapies are now available to treat chronic hepatitis C. DAA therapy is freely available to all adults chronically infected with the hepatitis C virus (HCV) in Australia. If left untreated, hepatitis C may lead to progressive hepatic fibrosis, cirrhosis and hepatocellular carcinoma. Australia is committed to eliminating hepatitis as a public health threat by 2030 set by the World Health Organization. However, since the introduction of funded DAA treatment, uptake has been suboptimal. Australia needs improved strategies for testing, treatment uptake and treatment completion to address …

In this study, we evaluated the predictive value of circulating tumour DNA (ctDNA) to inform therapeutic outcomes in metastatic melanoma patients receiving systemic therapies. We analysed 142 plasma samples from metastatic melanoma patients prior to commencement of systemic therapy: 70 were treated with BRAF/MEK inhibitors and 72 with immunotherapies. Patient-specific droplet digital polymerase chain reaction assays were designed for ctDNA detection. Plasma ctDNA was detected in 56% of patients prior to first-line anti-PD1 and/or anti-CTLA-4 treatment. การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อศึกษากระบวนการดำเนินการ และปัญหา อุปสรรคของการนำนโยบายการป้องกันการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 ไปปฏิบัติ โดยใช้รูปแบบการวิจัยเชิงคุณภาพ และการสัมภาษณ์แบบเชิงลึก (In-depth Interview) ผู้บริหาร ผู้ปฏิบัติงาน และประชาชนในพื้นที่จังหวัด ทั้งหมด 26 คน พบว่า นโยบายและมาตรการของรัฐบาลในการป้องกันการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 ถูกกำหนดในรูปแบบกฎหมาย นำนโยบายมาปฏิบัติผ่านคณะกรรมการ ด้วยการประชุม สั่งการ/มอบหมายงานที่มีความชัดเจน ปัจจัยด้านการสื่อสารของหน่วยงาน มีการใช้ช่องทางการสื่อสาร ทั้งในรูปแบบการสั่งการในที่ประชุม เอกสารทางราชการ การใช้เทคโนโลยีดิจิทัลตอบสนองต่อสถานการณ์ได้รวดเร็ว ปัจจัยด้านศักยภาพของหน่วยงาน ในส่วนทรัพยากรด้านงบประมาณ อุปกรณ์ เครื่องมือ ไม่เพียงพอต่อการปฏิบัติงาน ปัจจัยด้านคุณลักษณะของผู้นำนโยบายไปปฏิบัติ ที่เป็นคนในพื้นที่ ส่งผลให้สามารถขอความร่วมมือการปฏิบัติตามมาตรการได้ และปัจจัยด้านภาวะผู้นำ ผู้ว่าราชการจังหวัด มีการวางแผนเตรียมการรับมือกับสถานการณ์ และบัญชาสั่งการที่ชัดเจน โดยการติดตามประเมินผลการปฏิบัติตามมาตรการอย่างเข้มข้น ส่งผลให้สามารถนำนโยบายไปปฏิบัติได้ตามวัตถุประสงค์ ผู้วิจัยมีข้อเสนอแนะเชิงนโยบาย คือ ควรมีการบริหารจัดการให้มีศูนย์ข้อมูลกลางของจังหวัด เพื่อใช้ในการวางแผน ประกอบการตัดสินใจ และควรมีการจัดโครงสร้างหลักในการบริหารจัดการ ที่บูรณาการหน่วยงานต่างๆ เพื่อลดความซ้ำซ้อนกันของภารกิจ

การวิจัยนี้มีวัตถุประสงค์เพื่อ ศึกษานโยบายขององค์การในช่วงประสบปัญหาโรคติดเชื้อไวรัสโคโรนา 2019 ทราบถึงความพึงพอใจของพนักงานที่มีต่อนโยบายขององค์การในช่วงประสบปัญหาโควิด-19 และศึกษาแนวทางการพัฒนานโยบายขององค์การสายการบิน โดยเก็บรวบรวมข้อมูลจากพนักงานภายใน บริษัท ไทยสมายล์แอร์เวย์ จำกัด จำนวน 300 คน ผลการวิจัยพบว่า พนักงานมีความพึงพอใจต่อนโยบายขององค์การในช่วงประสบปัญหาโควิด-19 โดยรวมอยู่ในระดับมาก พบว่าด้านลักษณะงาน มีความพึงพอใจต่อการมีโอกาสร่วมแสดงความคิดเพื่อแก้ปัญหาในการปฏิบัติงานในช่วงประสบปัญหาโควิด-19 ด้านนโยบาย มีความพึงพอใจต่อการบริหารงานตามนโยบายที่ได้กำหนดไว้ของผู้บังคับบัญชาอยู่ในระดับมาก แต่ความพึงพอใจต่อนโยบายการลดค่าตอบแทน/ค่าเบี้ยขยันรายเดือนอยู่ในระดับน้อย ด้านค่าจ้างและสวัสดิการ มีความพึงพอใจโดยรวมอยู่ในระดับปานกลาง ด้านสภาพแวดล้อม มีความพึงพอใจโดยรวมอยู่ในระดับมากที่สุด ในการจัดสรรสิ่งอำนวยความสะดวกและเครื่องมือเครื่องใช้ในการปฏิบัติงาน การใช้เจลแอลกอฮอล์ หน้ากากอนามัยและถุงมือ กับการจัดสภาพแวดล้อมของหน่วยงานที่เอื้อต่อการทํางาน การส่งเสริมอาชีวอนามัยสุขภาพและความปลอดภัย อาทิ การเช็ดทำความสะอาดจุดสัมผัส การอบฆ่าเชื้อห้องโดยสาร ด้านความมั่นคงและความก้าวหน้าในตำแหน่งงาน มีความพึงพอใจต่อความมั่นคงในหน้าที่การงานอยู่ในระดับมาก รวมถึงความพึงพอใจที่มีต่อการสนับสนุนให้บุคลากรได้นำความรู้มาใช้ในการพัฒนาคุณภาพงานที่รับผิดชอบ การทำกิจกรรมกลุ่มประจำสัปดาห์ การใช้ทักษะในการทำงานใหม่ๆอยู่เสมอ และด้านพฤติกรรมในการทำงาน มีความพึงพอใจทุกข้ออยู่ในระดับมาก

Using old or uncommon drugs as alternative choices are interesting option to avoid drug resistance problems. Fosfomycin is an old antibacterial drug with broad spectrum and excreted via urine in active form. Fosfomycin tromethamine (FOS) has been approved to treat uncomplicated urinary tract infection (UTI) in women by FDA. However, in dogs, FOS data is inadequate and precise dosages have not been established yet. Bovine mastitis is a common disease in dairy cows. The majority causes of bovine mastitis disease is an intramammary bacterial infection via the teat orifice. In general, antibiotics are used for bovine mastitis treatment, its inappropriate usage leads to antibiotic-resistance problem and residue in meat and dairy products. One of the alternative ways against pathogen to reduce antibiotic usage is antimicrobial peptides which are innate immune of life. Three cationic peptides as Pm11, Pep64, and L10 peptides were evaluated for an antimicrobial activity to gram-positive and gram-negative bacteria. This study develops a prototype of a ready-to-cook Cambodian Korko soup base by surveying the perception of 400 Cambodian consumers and found that 44.5% of replies prefer ready-to-cook Cambodian Korko soup base in powder form. After that, the effect of different drying temperatures on the properties of Cambodian Korko soup base ingredients (herb, spice, and fermented fish) was investigated on physicochemical, antioxidants, and microbiological properties. At higher temperature 80 oC, water activity, moisture content was decreased faster and DPPH and FRAP were decreased compared to lower temperatures 70 oC and 60 oC. In this study, we aim to explore the response and the role of immune-related gene/protein and small RNA in the VPAHPND-infected shrimp. This research was divided into 2 parts. The objective of the thesis entitled is to survey and genetic characterize zoonotic viruses in dogs and humans who have high-risk occupations and in close contact with dogs. Coronavirus (CoV) is an enveloped single positive-stranded RNA virus CoV can cause enteric and respiratory diseases in mammals, avian and human. Bats are the natural reservoir of CoVs and distribute all over Thailand. Sample collection was performed in two bat colonies in Ayutthaya and Saraburi provinces during March 2018 - February 2019. Total 1,487 samples were collected from 730 bats. The bat species identification via morphological measurement and cytochrome B detection showed that bats in both colonies are Lyle's flying fox (Pteropus lylei). เบต้ากลูแคนเป็นสารเสริมในอาหารสัตว์ที่มีการใช้กันอย่างแพร่หลายในอุตสาหกรรมการเลี้ยงสุกร ถึงแม้ว่าการเสริมสารเบต้ากลูแคนในสูตรอาหารสุกร จะเชื่อว่าสามารถช่วยพัฒนาการเจริญเติบโตและปรับการทำงานของภูมิคุ้มกันในสุกรได้ แต่อย่างไรก็ตาม ในปัจจุบันยังไม่มีข้อมูลการศึกษาที่ชัดเจนเกี่ยวกับผลของการเสริมเบต้ากลูแคนที่สกัดมาจากสาหร่ายต่ออัตราการเจริญเติบโตและการตอบสนองทางภูมิคุ้มกันโดยเฉพาะระบบภูมิคุ้มกันแบบจำเพาะในสุกร ดังนั้นการศึกษานี้จึงมีวัตถุประสงค์เพื่อประเมินผลของการใช้สารเสริมเบต้ากลูแคนจากสาหร่ายในอาหารสุกรต่อประสิทธิภาพการเจริญเติบโตและการตอบสนองทางภูมิคุ้มกันแบบจำเพาะในสุกรอนุบาลและสุกรขุนเล็ก โดยแบ่งสุกรหย่านมสามสายพันธุ์เป็น 3 กลุ่มการทดลอง ได้แก่ กลุ่มควบคุมที่ได้รับอาหารสูตรมาตรฐาน กลุ่มทดลองที่ 2 และ 3 ได้รับอาหารชนิดเดียวกันที่เสริมด้วยสารเสริมเบต้ากลูแคนจากสาหร่ายในปริมาณ 100 และ 200 กรัมต่อตันตามลำดับ จากการทดลอง ไม่พบความแตกต่างอย่างมีนัยสำคัญของประสิทธิภาพการผลิตและอัตราการเจริญเติบโตของสุกรสุกร การทดสอบการตอบสนองของระบบภูมิคุ้มกันที่จำเพาะหลังการฉีดวัคซีน CSFV และ PRRSV พบว่าสุกรที่ได้รับสารเสริมเบต้ากลูแคนจากสาหร่ายในอาหาร มีระดับ neutralizing antibody ต่อ CSFV สูงขึ้นอย่างมีนัยยะสำคัญ นอกจากนี้ พบการเพิ่มขึ้นของจำนวนเซลล์ CD4+(T lymphocytes) และ CD4+CD8+ (putative memory T lymphocytes) ซึ่งสร้างไซโตคายน์ IFN-g+ ที่มีความจำเพาะต่อ CSFV และ PRRSV ในกลุ่มสุกรที่ได้รับสารเสริมเบต้ากลูแคนจากสาหร่ายอีกด้วย จากการศึกษาสรุปได้ว่า ถึงแม้ว่าการให้สารเสริมเบต้ากลูแคนจะไม่สามารถเสริมสมรรถภาพการผลิตของสุกรได้ แต่มีคุณสมบัติทางชีวภาพในการเป็นสารกระตุ้นภูมิคุ้มกันซึ่งอาจเป็นการเพิ่มความต้านทานโรคติดเชื้อไวรัสในสุกรอนุบาลและสุกรขุนเล็กได้

Coprological examination using centrifugal sedimentation technique is the gold standard method for detection of Platynosomum spp.'s eggs. However, microscopic examination method has a limited sensitivity, particularly in cases of complete biliary obstruction and inconsistent egg shedding. งานวิจัยนี้มีวัตถุประสงค์สังเคราะห์ท่อนาโนไททาเนีย บนชิ้นส่วนโลหะผสมไทเทเทียมผสมที่สร้างด้วยกระบวนการพิมพ์สามมิติ และวิเคราะห์พฤติกรรมอัตราการปลดปล่อยยาแวนโคมัยซิน ความเข้มข้น 200 พีพีเอ็ม ภายในเวลา 24 ชั่วโมง โดยประสิทธิภาพกลไกการปลดปล่อยยาจากโครงสร้าง ในระดับนาโน ตรวจสอบโดย การตรวจคุณสมบัติทางเคมีบนพื้นผิว และ การวิเคราะห์ทางแบบจำลองทางจลนศาสตร์ Korsmeyer-Peppas ศึกษาสัณฐานวิทยาของท่อนาโนไททาเนีย ด้วยกล้องจุลทรรศน์อิเล็กตรอนแบบส่องกราดที่มีสมรรถนะสูง และลักษณะทางเคมีบนพื้นผิวของชิ้นส่วนโลหะผสมไทเทเทียมผสมและท่อนาโนไททาเนีย ด้วยเครื่องวัดมุมสัมผัส เครื่องฟูเรียร์ทรานฟอร์มอินฟราเรดสเปกโตรมิเตอร์ และเทคนิคสเปกโตรสโกปีโฟโตอิเล็กตรอน ด้วยรังสีเอ๊กซ์ โดยที่พฤติกรรมการปล่อยยาแวนโคมัยซิน ความเข้มข้น 200 พีพีเอ็ม จากท่อนาโนไททาเนีย ภายใต้สภาวะการควบคุม ในระยะเวลา 24 ชั่วโมง ตรวจวัดโดยใช้เครื่องโครมาโทกราฟีของเหลวสมรรถนะสูง แสดงตำแหน่งรีเทนไทม์อยู่ที่ 2.5 นาที จากการตรวจสอบสัณฐานวิทยาลักษณะพื้นผิว ในระดับนาโน พบว่า ท่อนาโนไททาเนียมีลักษณะคล้ายรังผึ้ง เรียงตัวกันอย่างหนาแน่น และมีการปลดปล่อยสะสมสูงสุด ภายใน 24 ชั่วโมงของยาแวนโคมัยซินอยู่ที่ 34.7% (69.5 พีพีเอ็ม) ตลอดจนคุณสมบัติทางเคมีบนพื้นผิวของชิ้นส่วนไทเทเนียมผสมจากการพิมพ์สามมิติ (68 ± 1 องศา) และท่อนาโนไททาเนีย (0 องศา) แสดงค่ามุมการสัมผัสต่ำกว่า 90 องศา ซึ่งบ่งบอกถึงพื้นผิว มีสมบัติการเปียกผิวที่ดี จากการศึกษาความเข้ากันได้ทางชีวภาพ ด้วยการทดสอบความเป็นพิษต่อเซลล์กระดูกหนู สายพันธ์ุ C57BL/6 ชนิด MC3T3-E1 พบว่า บนพื้นผิวท่อนาโนไททาเนียที่ผ่านกระบวนการแอโนไดเซซัน เป็นเวลา 1 และ 4 ชั่วโมง และ ผ่านการบรรจุยาแวนโคมัยซิน แสดงค่าเปอร์เซ็นต์ความมีชีวิตของเซลล์ที่น้อย อาจเนื่องมาจากวาเนเดียมออกไซด์ฟิล์มชนิด V2O4 และ V2O5 บนพื้นผิวของท่อนาโนไททาเนีย

การวิจัยนี้มีวัตถุประสงค์ 1) เพื่อศึกษาและกำหนดกรอบแนวคิดหลักสูตรศิลปศึกษาและบริบทร่วมสมัย 2) เพื่อศึกษาและกำหนดลักษณะหลักสูตรศิลปศึกษาบนฐานบริบทร่วมสมัยที่ผู้ใช้หลักสูตรต้องการ 3) เพื่อพัฒนานวัตกรรมหลักสูตรศิลปศึกษาบนฐานบริบทร่วมสมัย การดำเนินการวิจัยใช้รูปแบบวิจัยแบบผสมวิธีพหุระยะ แบ่งการวิจัยออกเป็น 3 ระยะ คือ 1) การวิจัยเชิงคุณภาพด้วยการศึกษาค้นคว้าตำรา เอกสาร งานวิจัยที่เกี่ยวข้อง และการสัมภาษณ์ผู้เชี่ยวชาญด้านศิลปศึกษาและด้านบริบทร่วมสมัย 19 คน 2) การวิจัยเชิงปริมาณ เก็บรวบรวมข้อมูลด้วยแบบสอบถามกับกลุ่มผู้มีส่วนได้ส่วนเสียกับหลักสูตร 803 คน 3) การวิจัยเชิงคุณภาพ พัฒนานวัตกรรมหลักสูตรศิลปศึกษาบนฐานบริบทร่วมสมัย และจัดประชุมกลุ่มผู้เชี่ยวชาญด้านศิลปศึกษาและด้านบริบทร่วมสมัย 12 คน เพื่อประเมินร่างนวัตกรรมหลักสูตร วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการกำหนดกรอบประเด็น การให้รหัส/ทำดัชนี ตีความและสรุปข้อมูล วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าความต้องการจำเป็น ผลการวิจัยระยะแรกพบว่า กรอบแนวคิดหลักสูตรศิลปศึกษาควรประกอบด้วย 7 แนวคิดคือ 1) ทฤษฎีศิลปศึกษาเป็นฐาน 2) การจัดการเรียนรู้ศิลปศึกษา 3) เทคโนโลยีและนวัตกรรมศิลปศึกษา 4) ศิลปศึกษาเพื่อภูมิปัญญาและท้องถิ่น 5) การพัฒนาผู้ประกอบการศิลปศึกษา 6) ศิลปศึกษาตามศักยภาพบุคคล และ7) สหศาสตร์ศิลปศึกษา ส่วนแนวคิดบริบทร่วมสมัยประกอบด้วย 5 แนวคิดคือ 1) ลักษณะความเป็นส่วนบุคคล 2) ความเป็นสหวิทยาการ 3) ความเป็นท้องถิ่น 4) การสร้างนวัตกรรม 5) การเงินและการประกอบการ ผลการวิจัยระยะที่สองพบว่า กลุ่มอาจารย์มีความต้องการแนวคิดหลักสูตรด้านทฤษฎีศิลปศึกษาเป็นฐานสูงสุด กลุ่มบัณฑิตมีความต้องการแนวคิดหลักสูตรด้านการจัดการเรียนรู้ศิลปศึกษาสูงสุด กลุ่มผู้ใช้บัณฑิตมีความต้องการแนวคิดหลักสูตรด้านทฤษฎีศิลปศึกษาเป็นฐานสูงสุด ผลการวิจัยระยะที่สามพบว่า นวัตกรรมหลักสูตรศิลปศึกษาบนฐานบริบทร่วมสมัยคือ หลักสูตรที่มีการจัดการเรียนรู้แบบรายบุคคลที่กำหนดหน่วยกิตวิชาเลือกมากกว่าหน่วยกิตวิชาบังคับในกลุ่มวิชาเอก เปิดโอกาสให้ผู้เรียนเลือกพัฒนาตนเองได้ตามศักยภาพและความสนใจ และมีเนื้อหาหลักสูตรในลักษณะของการบูรณาการข้ามศาสตร์ทั้งการบูรณาการร่วมกับกลุ่มสาระการเรียนรู้ศิลปะอันได้แก่ สาขาวิชาดนตรีศึกษา และนาฏศิลป์ศึกษา และการบูรณาการกับสาขาวิชาด้านมนุษยศาสตร์ สังคมศาสตร์ วิทยาศาสตร์

ที่มา: การล้างมือด้วยแอลกอฮอล์เป็นสิ่งสำคัญในการลดการแพร่กระจายเชื้อในโรงพยาบาล ในปัจจุบันการล้างมือมีทั้งหมด 3 วิธี คือ 3 ขั้นตอน, 6 ขึ้นตอนและ 7 ขั้นตอน โดยพบว่าการล้างมือ 6 ขั้นตอนลดจำนวนแบคทีเรียได้ดีกว่า 3 ขั้นตอน แต่ยังไม่มีการศึกษาเปรียบเทียบการล้างมือระหว่าง 6 ขั้นตอนและ 7 ขั้นตอน วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบอัตราส่วนของจำนวนแบคทีเรียหลังล้างมือหารด้วยจำนวนแบคทีเรียก่อนล้างมือของวิธีการล้างมือด้วยแอลกอฮอล์ทั้ง 6 และ 7 ขั้นตอน วิธีการศึกษา: การวิจัยเชิงทดลองแบบสุ่มและข้ามกลุ่มในบุคคลากรของโรงพยาบาลจุฬาลงกรณ์ ตั้งแต่เดือนกุมภาพันธ์ พ.ศ. 2563 ถึงพฤษภาคม พ.ศ.2563 โดยแบ่งเป็น 3 กลุ่ม คือ แพทย์, พยาบาล และนักศึกษาแพทย์ ผลการศึกษา: ไม่พบความแตกต่างอย่างมีนัยสำคัญของอัตราส่วนจำนวนแบคทีเรียหลังล้างมือเทียบก่อนล้างมือด้วยแอลกอฮอล์ระหว่างหกและเจ็ดขั้นตอน (0.35 และ 0.42 ตามลำดับ, โดย p = 0.44) พื้นที่บนมือที่ไม่โดนแอลกอฮอล์ทั้งหกและเจ็ดขั้นไม่แตกต่างอย่างมีนัยสำคัญ (5% เท่ากันทั้งสองวิธี, โดย p = 0.55) และพบว่าบริเวณหลังมือของนิ้วโป้งและนิ้วก้อยเป็นตำแหน่งที่ล้างมือไม่โดนมากที่สุด เวลาในการล้างมือด้วยแอลกอฮอล์โดยวิธีเจ็ดขั้นตอน (ค่ามัธยฐาน 34 วินาที) จะใช้เวลามากกว่าการล้างมือหกขั้นตอน (ค่ามัธยฐาน 30 วินาที) สรุปผล: จากการวิจัยเชิงทดลองแบบสุ่มและข้ามกลุ่มเพื่อศึกษาเปรียบเทียบวิธีการล้างมือด้วยแอลกอฮอล์ระหว่างหกและเจ็ดขั้นตอน พบว่าการล้างมือด้วยวิธีหกขั้นตอนไม่ด้อยไปกว่าการล้างมือเจ็ดขั้นตอนทั้งในส่วนของจำนวนแบคทีเรียหลังล้างมือ พื้นที่มือที่ไม่สัมผัสแอลกอฮอล์ และระยะเวลาสำหรับล้างมือของหกขั้นตอนน้อยกว่าเจ็ดขั้นตอน

ที่มา การศึกษาครั้งนี้มีวัตถุประสงค์ในการเปรียบเทียบประสิทธิภาพระหว่างโซเดียมไบคาร์บอเนตและเฮปพาริน ในการลดอัตราการสูญเสียสายฟอกเลือดจากการอุดตันของสายหรือการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายฟอกเลือดของผู้ป่วยไตเรื้อรังที่ฟอกเลือดด้วยเครื่องไตเทียม กระบวนการและระเบียบวิธีวิจัย เป็นการศึกษาทดลองแบบสุ่มชนิดมีกลุ่มควบคุมในศูนย์ไตเทียม 7 แห่ง ในจังหวัด กรุงเทพมหานคร ประเทศไทย โดยการแบ่งกลุ่มผู้ป่วยที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมผ่านสายฟอกเลือด 2 หรือ 3 ครั้งต่อสัปดาห์แบบสุ่มทั้งหมด 118 ราย เป็น 2 กลุ่ม ได้แก่ กลุ่มที่ได้รับ 7.5% โซเดียมไบคาร์บอเนต (57 ราย) และกลุ่มที่ได้รับเฮปพาริน (61 ราย) ในการล็อคสายหลังฟอกเลือดทุกครั้ง และติดตามผลลัพธ์ต่อเนื่องเป็นระยะเวลา 6 สัปดาห์ วัตถุประสงค์หลัก คือ อัตราการสูญเสียสายฟอกเลือดจากสายอุดตันและการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายฟอกเลือด วัตถุประสงค์รอง คือ ผลลัพธ์รวมของอัตราการเกิดสายอุดตัน การติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายฟอกเลือด และ การติดเชื้อบริเวณทางออกหรืออุโมงค์ของสายฟอกเลือด ผลการศึกษา พบว่าไม่มีการสูญเสียสายฟอกเลือดจากสายอุดตันและการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายฟอกเลือดในทั้ง 2 กลุ่มการศึกษา แต่ในกลุ่มโซเดียมไบคาร์บอเนตมีผลลัพธ์รวมของอัตราการเกิดสายอุดตัน การติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายฟอกเลือด และ การติดเชื้อบริเวณทางออกหรืออุโมงค์ของสายฟอกเลือดมากกว่ากลุ่มเฮปพารินอย่างมีนัยสำคัญทางสถิติ (13.8 เทียบกับ 0.8 ต่อ 1,000 วันของการใช้สายฟอกเลือด) (P=0.004) โดยมีระยะเวลาเฉลี่ยของการเกิดสายอุดตัน 23.6 วัน อย่างไรก็ตามทุกการเกิดสายอุดตันสามารถแก้ไขได้ด้วยการล็อคสายด้วยยา recombinant tissue plasminogen activator (rt-PA) เพียง 1 โดสและไม่พบความแตกต่างในด้านค่าใช้จ่ายรวมของทั้ง 2 กลุ่ม นอกจากนี้ยังพบว่ากลุ่มโซเดียมไบคาร์บอเนตมีแนวโน้มที่จะเกิดอัตราการติดเชื้อในกระแสเลือดและการติดเชื้อบริเวณทางออกและอุโมงค์ของสายสวนที่น้อยกว่ากลุ่มเฮปพาริน (0 เทียบกับ 0.8 ต่อ 1,000 วันของการใช้สายฟอกเลือด) (P=0.56) สรุปผล การใช้เฮปพารินซึ่งเป็นสารล็อคสายฟอกเลือดมาตรฐานสามารถป้องกันสายอุดตันได้ดีกว่าโซเดียมไบคาร์บอเนตในผู้ป่วยไตเรื้อรังที่ฟอกเลือดด้วยเครื่องไตเทียม อย่างไรก็ตามการใช้มาตรการการล็อคสายวิธีใหม่ กล่าวคือ การใช้โซเดียมไบคาร์บอเนตร่วมกับ rt-PA ทุก 3 สัปดาห์ในการล็อคสายฟอกเลือด อาจนำมาใช้เป็นวิธีทางเลือก โดยเฉพาะกรณีที่ผู้ป่วยมีข้อห้ามต่อการใช้เฮปพาริน

จุดประสงค์ : เพื่อเปรียบเทียบระหว่างการดูดเลือดผ่าน 1 รู และ 2 รู จากสายสวนหลอดเลือดดำส่วนกลาง ชนิดหลายรูในการวินิจฉัยการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายสวนหลอดเลือดส่วนกลาง วิธีการศึกษา : การศึกษาแบบไปข้างหน้า เก็บข้อมูลตั้งแต่วันที่ 1 สิงหาคม พ.ศ. 2563 ถึงวันที่ 20 กุมภาพันธ์ พ.ศ. 2564 ในผู้ป่วยที่มีอาการทางคลินิกที่สงสัยมีการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายสวนหลอดเลือดส่วนกลาง ในผู้ป่วยที่ทำการใส่สายสวนหลอดเลือดชนิดหลายรูมานานมากกว่า 48 ชั่วโมงและอยู่ในหอผู้ป่วยแผนกอายุรกรรมและศัลยกรรม โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โดยทำการดูดเลือด 2 ครั้ง ครั้งแรกผ่านรูที่มีรูเปิดปลายสุด และครั้งที่สองจากรูที่มีรูเปิดใกล้สุดหรือรูที่เปิดตรงกลางของสายสวนหลอดเลือดดำส่วนกลางชนิด 3 รู (triple-lumen CVC) และสายสวนหลอดเลือดดำใหญ่ผ่านทางหลอดเลือดดำส่วนปลายที่ผิวหนังชนิด 2 รู (PICC) ร่วมกับทำการเจาะเลือดดำส่วนปลาย โดยใช้การวินิจฉัยตามที่สมาคมโรคติดเชื้อประเทศสหรัฐอเมริกากำหนด ผลการศึกษา : ช่วงที่ทำการศึกษามีผู้ป่วยจำนวน 115 คน ที่มีอาการทางคลินิกที่สงสัยมีการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายสวนหลอดเลือดส่วนกลาง ประกอบด้วย 86 คน (ร้อยละ 74.8) และ 29 คน (ร้อยละ 25.2) ที่ทำการใส่สายสวนหลอดเลือดดำใหญ่ผ่านทางหลอดเลือดดำส่วนปลายที่ผิวหนังชนิด 2 รู และสายสวนหลอดเลือดดำส่วนกลางชนิด 3 รู ตามลำดับ 96 คน (ร้อยละ 83.5) และ 19 คน (ร้อยละ 16.5) อยู่ในหอผู้ป่วยแผนกอายุรกรรมและศัลยกรรมตามลำดับ ในการศึกษานี้พบผู้ป่วยที่ได้รับการวินิจฉัยการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายสวนหลอดเลือดส่วนกลางร้อยละ 4.34 (5 คน จากผู้ป่วย 115 คน) โดยมีอัตราการเกิดการติดเชื้อในกระแสเลือดที่สัมพันธ์กับสายสวนหลอดเลือดส่วนกลางเพิ่มขึ้นจากร้อยละ 13.33 (2 คนที่มีเชื้อขึ้นจากการดูดเลือดผ่านรูที่เปิดปลายสุด เป็นร้อยละ 33.33 (จำนวนเพิ่มขึ้น 3 คนที่มีเชื้อขึ้นจากการดูดเลือดผ่านรูที่เปิดใกล้สุดและเปิดตรงกลาง) จากผู้ป่วย 15 คนที่มีเชื้อขึ้นจากการดูดเลือดผ่านสายสวนหลอดเลือดอย่างน้อย 1 รู โดยมีการปนเปื้อนจากการเก็บสิ่งตรวจร้อยละ 1.74 …

Drug repositioning, discovering new indications for existing drugs, is a competent strategy to reduce time, costs, and risk in drug discovery and development. Many computational methods have been developed to identify new drug-disease associations for further validation and drug development. A recent approach showing superior performance with less required data is a meta-path based approach, which derives network-based information using path patterns from drug to disease nodes. However, existing meta-path based methods discard information of intermediate nodes along paths, which are important indicators for describing relationships between drugs and diseases.
