Purpose: The CDC Core Elements of Hospital Antibiotic Stewardship Programs recommends implementing policies to support optimal antibiotic prescribing including documentation of dose, duration and indication. On January 31, 2017, the study institution implemented new physician order entry screens in the electronic health record requiring the input of indication and duration on all antibiotic orders. The objective of this study is to determine if implementation of mandatory indication and duration for antibiotic orders decreases antibiotic duration of therapy in hospitalized patients with pneumonia.

Purpose: Recent literature has highlighted MRSA nasal screening as a possible antimicrobial stewardship program tool for avoiding unnecessary empiric anti-MRSA therapy for pneumonia. MRSA is an important cause of pneumonia and clinicians must determine when empiric antimicrobial therapy directed toward this pathogen is needed. Negative MRSA nasal swab has been shown to have a high negative predictive value ( >95%) across different types of pneumonia. The objective of this study is to assess the impact of negative MRSA nasal swabs on the duration of anti-MRSA therapy in a community hospital setting.

Cybersecurity has become an increasingly important concern for both military and civilian infrastructure globally. Because of the complexity that comes with wireless networks, adversaries have many means of infiltration and disruption of wireless networks. While there is much research done in defending these networks, understanding the robustness of these networks is tantamount for both designing new networks and examining possible security deficiencies in preexisting networks. This dissertation proposes to examine the robustness of wireless networks on three major fronts: the physical layer, the data-link layer, and the network layer. Although the prevalence of obesity has rapidly increased in the low- and middle-income countries of the Middle East and North Africa (MENA) and Latin America and the Caribbean (LAC) regions, child undernutrition remains a public-health challenge. We examined region-specific sociodemographic determinants of this double burden of malnutrition, specifically, the co-occurrence of child stunting and overweight, using Demographic and Health Survey and Multiple Indicator Cluster Survey data (2003–2016) from 11 countries in the MENA (n = 118,585) and 13 countries in the LAC (n = 77,824) regions. In this dissertation, a new family of skew distributions is introduced and developed, the Epsilon Skew Rayleigh. The members of this family are bimodal skewed distributions with location, scale and skewness parameters. There exist two unimodal parameter cases. The distribution can be skewed or symmetric. This distribution family has many applications including population demographics, signal dynamics, ocean wave heights and hardware failure rates. The effects of the parameters are described and developed. We derive the moment generating and maximum likelihood functions, as well as the expected value, median, modes, variance, skewness and kurtosis. The properties of a random variable with …

This work fits the measured in-pixel source-follower noise in a CMOS Quanta Image Sensor (QIS) prototype chip using physics-based 1/f noise models, rather than the widely-used fitting model for analog designers. This paper discusses the different origins of 1/f noise in QIS devices and includes correlated double sampling (CDS). The modelling results based on the Hooge mobility fluctuation, which uses one adjustable parameter, match the experimental measurements, including the variation in noise from room temperature to –70 °C. This work provides useful information for the implementation of QIS in scientific applications and suggests that even lower read noise is attainable …

This paper aims to explore and expand on Wittgenstein’s remarks on the nature of mental imagery. Despite some rather cryptic passages and obvious objections, his notion of mental imagery as possessing a constitutive (and not merely added) element of expressive thought and conceptuality offers critical insights linking perceptual capacities with our shared practices. In particular I seek to further develop Wittgenstein’s claim that perceptual impressions presuppose a “mastery of a technique.” I argue that this sense of technique, understood as acquired conceptual capacities, can explain and capture the rich and varied spectrum of expressive visual content that can be accessed …

Purpose: Practice guidelines recommend patients with non-variceal upper gastrointestinal (GI) bleeds who have a high risk of active bleeding, nonbleeding visible vessels, and adherent clots should be given proton pump inhibitors (PPIs) intravenous (IV) bolus followed by continuous infusion after esophagogastroduodenoscopy (EGD). The use of PPIs before EGD has shown to reduce the requirement for EGD, however, there are no studies evaluating the difference in outcomes of continuous infusion PPI versus intermittent IV bolus PPI prior to EGD. Penelitian ini bertujuan untuk mengetahui pengaruh bahan organik gulma paitan dan pupuk NPK terhadap sifat kimia tanah (C organik, N, dan P total), serta pertumbuhan dan hasil tomat pada Ultisols. Penelitian dilaksanakan di screen house Fakultas Pertanian, Universitas Jenderal Soedirman, Purwokerto, pada bulan Juni sampai September 2018. Kentang (Solanum tuberosum L.) adalah salah satu komoditas yang mendapat prioritas pengembangan, karena produk tanaman ini dipakai sebagai sumber karbohidrat serta memiliki potensi dalam diversifikasi pangan. Salah satu permasalahan dalam produksi kentang adalah terbatasnya persediaan benih kentang, karena adanya fase dormansi. Percobaan ini bertujuan untuk mengetahui interaksi antara suhu penyimpanan dan konsentrasi sitokinin terhadap pematahan dormansi benih kentang. Rancangan percobaan yang digunakan adalah Rancangan Petak Terbagi (Split Plot Design) dengan tiga ulangan. Purpose: The purpose of this study is to evaluate push dose vasopressor (PDV) usage patterns, efficacy, and safety in a single community hospital emergency department when used for rapid sequence intubation (RSI). Pre-intubation hypotension is a positive correlator to incidences of cardiac arrest. Precautions should be in place to prevent and treat hypotension during emergent intubation. The emerging strategy of utilizing PDV in emergency department (ED) patients is an underexplored and potentially efficacious option.

Purpose: Parents can be effective and consistent sex educators of their children, but research suggests that most parents only engage in a one-time talk about sex with their children. That being said, we know little about the potential variability in trajectories of parent-child sex communication over time. Thus, the present study took a person-centered approach to parent-child sex communication about sexual risk and explored predictors and outcomes of varying trajectories.
Methods: Participants included 468 adolescents and their parents who took part in a longitudinal study every year from ages 14e18 years (52% female, 67% white, and 33% single parents). …

Retinitis Pigmentosa is a disease which can cause blindness and affects around 2.5 million people worldwide. It is a disorder that has a multitude of ways of being inherited, such as autosomal dominant, autosomal recessive as well as X-linked and mitochondrial linked disorder. While some versions of RP may be syndromic (20%-30%), a majority of cases are in fact non-syndromic. The majority of non-syndromic cases inherited via autosomal dominant RP having RHO gene mutations, and X-linked means of inheritance having majority of patients with RPGR gene mutations. Metastatic Melanoma Of The Gallbladder Presenting As Polyp In Acute Cholecystitis., Nirvana B Saraswat, William B Devoe

Malignant melanoma is an aggressive neural crest cell-derived neoplasm with a propensity for metastasis to almost any organ. Gastrointestinal metastasis may manifest as gallbladder polyps. We report a case of metastatic malignant melanoma diagnosed in an 81-year-old male after cholecystectomy performed for acute cholecystitis. Cholecystectomy remains the standard of care for treatment of isolated gallbladder metastasis, especially in the setting of symptomatic disease. Mutation-directed chemotherapeutic and immunotherapeutic modalities serve as efficacious adjunctive therapy in addition to primary surgical resection for this rare condition.

Livestock production systems have faced social pressures related to animal welfare; the laying hen industry also has faced changes in governing regulations/policies. This study employs experimental data to evaluate 3 confinement-housing systems: standard cages with no perches, cages with standard perches, and cages with cooled perches. Based on the cost-benefit analysis conducted of revenue less feed costs, operators are expected to have revenue, after covering feed costs, which could contribute to installation and maintenance of standard and/or cooled perches. Thus, egg producers may be able to use caged housing that incorporates perches profitably, depending on the final costs of such …

Mountainous snowpack represents up to 70% of the total water resources for areas in the western United States. Temperature and precipitation control the accumulation and ablation of the snowpack throughout the cold season, both of which are subject to a changing climate. This study will use a network of snow telemetry (SNOTEL) stations to investigate changes in the snowpack season, snow water equivalent (SWE), and temperature in the southern Rocky Mountain region. Depression is the leading cause of disability worldwide, affecting more than 300 million people. As more millennials take on leadership positions, it is important to understand how companies are currently addressing mental health in the workplace and compare this to how millennials approach the subject. The quantitative analysis completed through this study helped to determine what a mentally healthy workplace looks and feels like for millennials. Companies are increasingly investing in mental health initiatives for several reasons: to increase brand recognition, to improve company culture, to mitigate potential lawsuits, and to increase employee engagement and retention. The purpose of this research is to detail a case study of U.S. tourism to Cuba in a politicized context; specifically, to compare and contrast the Obama and Trump administrations. This study seeks to examine how the Cuban “brand” and the island’s overall tourism strategy is formulated, circulated, shaped and reshaped by various actors and the public in the changing context of the newly antagonistic bilateral U.S.-Cuba political relationship.
