Gillick Competence: An Unnecessary Burden, Nigel Zimmermann

This study of the implications of Gillick competence argues it is an unnecessary burden with an unethical foundation. The ethics of adolescent medical decision-making is a fraught area for medical ethics because it deals with the threshold boundaries between childhood and adulthood and Gillick adds a burden upon children and adolescent patients that is unwarranted and through which damage is done to integral human relationships. In light of Gillick, it can be seen that the context of adolescent decision-making and childhood, is a neglected topic of ethical reflection.

While the United States continues to recover from the 2008 Great Recession, the country still faces unprecedented inequality as increasing numbers of poor families struggle to get by with little assistance from the government. Holes in the Safety Net: Federalism and Poverty offers a grounded look at how states and the federal government provide assistance to poor people. With chapters covering everything from welfare reform to recent efforts by states to impose work requirements on Medicaid recipients, the book avoids unnecessary jargon and instead focuses on how programs operate in practice. Objective: To describe urinary tract infection (UTI) treatment among Veterans’ Affairs (VA) Community Living Centers (CLCs) nationally and to assess related trends in antibiotic use.

Design: Descriptive study.

Setting and participants: All UTI episodes treated from 2013 through 2017 among residents in 110 VA CLCs. UTI episodes required collection of a urine culture, antibiotic treatment, and a UTI diagnosis code. UTI episodes were stratified into culture-positive and culture-negative episodes.

Methods: Frequency and rate of antibiotic use were assessed for all UTI episodes overall and were stratified by culture-positive and culture-negative episodes. Joinpoint software was used for regression analyses of trends …

Access and utilization of health care is essential for the maintenance of overall health and prevention of chronic diseases. Several factors contribute to healthcare access disparities, including race, ethnicity, poverty, and rurality. Mobile healthcare clinics can be used to travel to medically underserved areas and reduce healthcare disparities. This rapid review examined the scope and impact of mobile healthcare units, and their use in reducing healthcare disparities. The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles (“MISEV”) guidelines for the field in 2014. Decoding health and disease pathways drives healthcare evolution. Historically, therapeutic paradigms have relied on interventions that mitigate symptoms of established diseases. Increasingly, molecular insights into pathophysiology now provide unprecedented opportunities to offer curative solutions or even prevent disease and thereby secure longitudinal wellness. These opportunities extend past individual patients to entire populations and geographies. Moreover, they optimize prospective healthspan across lifespan. Linking discovery science and its translatable innovations beyond reactive disease intervention to proactive prevention will maximize society’s returns creating the greatest benefit for the greatest number of people globally.

The JSU Yearbook Collection spans the years from 1926 to the present. The publication began in 1926 as the Teacola (some volumes spelled "Teacoala") and continued under this title through 1933. The yearbook appears to have ceased printing after that year; the student newspaper took the name Teacola (later changed to the Collegian and finally the Chanticleer) and began publication in 1934. The yearbook started up again in 1947 under the name Mimosa. The Library holds all volume years that were printed: 1926-1933, 1947-1950, 1952-present.

Introduction: Neonatal Abstinence Syndrome (NAS) is a growing problem and poses significant burden on the healthcare system.
Evidence summary: Three emerging themes were recognized and further guided the development and execution of the proposed DNP project: 1) A need to improve NAS assessment practices that will decrease frustration, discrepancies, and inconsistent NAS scoring. Despite specific training, inaccuracy and inconsistency still exists among nurses using traditional scoring methods, leading to a longer hospital stay. 2) Family-centered care is vital for infants with NAS. Latinx adolescents may begin middle school with lower levels of life satisfaction and hope. In the current study, we implemented a small-series (N = 4) single-case research design to evaluate the impact of a group counseling wellness experience to increase Latinx adolescents' life satisfaction and hope. Analysis of participants' scores on outcome measures yielded treatment effects indicating that the group counseling wellness experience may be effective for increasing hope and life satisfaction. Implications for school counselors are provided.

Carotenoids serve various ecological roles in animals including coloration, immune responses, and vision. Carotenoid-derived coloration is greatly emphasized in the literature, particularly relating to mate choice and aposematic warning. However, the trade-offs between the color and non-color functions of carotenoids are not thoroughly explored. In the redheaded pine sawfly (Neodiprion lecontei), some larval populations have yellow pigmentation, using carotenoids derived from their diets for aposematic warning coloration. Other larval populations are white in color, having genetically lost the ability to produce the yellow pigment. Background. Misuse of antibiotics for upper respiratory tract infections (URTIs) in pediatric primary care settings persists despite growing concern over antibiotic resistance. Interventions aimed at improving prescription use, coined Antibiotic Stewardship Programs (ASPs) often target clinicians for improved clinical decision-making. More recently, ASPs have begun educating patients to antibiotic needs, and for pediatric cases, parents who might pressure clinicians into prescribing unneeded antibiotics. While many reviews have targeted adult patients or emergency settings, few evaluate practices for pediatric populations. A commercial feedlot study utilizing 1,737 crossbred heifers (initial BW 690 lb) compared the effect of two implant strategies [Synovex ONE Feedlot (day 0) or Synovex Choice (day 0) followed by Synovex Plus (day 95)] on performance and carcass characteristics. No differences were observed in carcass weight, final body weight, or gain, but heifers implanted with Synovex ONE Feedlot had slightly greater feed conversion and greater intake than heifers implanted using Synovex Choice/Synovex Plus. Heifers implanted with Synovex Choice/Synovex Plus had lower marbling score and yield grade, higher dressing percentage, and greater loin muscle area compared to heifers implanted with …

Inconsistency was found in forage quality (crude protein and energy) when esophageally fistulated diets were compared to Nutrition Balance Analyzer (NUTBAL) analysis of fecal samples. On upland range sites, hand-clipping of samples (not a recommended practice to measure forage quality), was closer to fistulated diets than NUTBAL analysis. If cattle managers are solely utilizing NUTBAL for estimates of forage value, incorrect supplemental energy and protein decisions will likely be made resulting in the purchase of unnecessary supplements, thereby reducing the profitability of the operation.

Selection by the animal, sample handling, lab analysis, and relating the lab analysis to the animal are issues that have been researched the last 20 years. Fall cover crops have been shown to be an effective way to background calves into the winter. An oat monoculture was planted in late summer at 100 lb/ac and compared to oats planted at 50 lb/ac with rapeseed included at 3 lb/ac. Initial forage yield was not affected by the inclusion of rapeseed with fall oat. Calf gain tended to be greater and cost of gain was decreased when rapeseed was included. Including rapeseed in late summer planted oats may be beneficial for producers who want to graze growing calves in the fall.

A modeling study evaluated the effects of milk production level on nutrient balance in March-and May-calving cows grazing Sandhills upland range during the breeding season. Forage quality of upland range peaks in June and steadily declines in July until November. In March-calving cows, metabolizable protein (MP) and energy were deficient by July 1 in all milking potential cows, which is exacerbated in greater milking potential cows. This study retrospectively evaluated the effect of cow size on cow-calf performance and post-weaning steer feedlot performance of cows at the Gudmundsen Sandhills Laboratory, Whitman. Cows were categorized at small, medium, or moderate within cow age from 13 years of data. Small cows had decreased reproductive performance, weaned smaller calves, and produced steer progeny with smaller carcass weights. In this dataset and under the environmental and management conditions at Gudmundsen Sandhills Laboratory, overall productivity of the cowherd decreased as cow size decreased with 1,150 to 1,200 lb cow being the most productive cow size.

May-calving dams were assigned to graze either sub-irrigated meadow or upland range with or without supplementation (1 lb/d 33% CP). Treatment began at approximately 160 d of gestation. Supplementation continued for 85 ± 2 d, while grazing system treatment continued for 116 ± 2 d. Steer progeny were backgrounded after weaning and then assigned to either a short or long yearling feedlot system. Dam supplementation tended to decrease marbling score within both feedlot systems. Short yearling steers had an increased percentage of carcasses grade USDA low Choice or greater if their dams grazed meadow in late gestation. Long yearling steers …

May-calving dams in late gestation grazed either sub-irrigated meadow with or without supplement or upland range with or without supplement. Supplementation was 1 lb/d of a 33% CP (DM) supplement. Heifer progeny from these dams were followed through their first and second breeding seasons. Both dam grazing and supplement treatment affected heifer progeny BW and BCS. Heifers born to dams who grazed meadow in late gestation attained a greater percentage of mature BW at the start of their first breeding season and increased pregnancy rates as primiparous cows. Purpose: Nonadherence to indicated therapy reduces treatment effectiveness and may increase cost of care. HUMIRA Complete, a Patient Support Program (PSP), aims to reduce nonadherence in patients prescribed adalimumab (ADA). The objective of this study was to assess the relationship between participation in the PSP and prescription abandonment rates among ADA-treated patients.

Freeze-casting is a popular method to produce biomaterial scaffolds with highly porous structures. Three approaches are developed to predict the scaffold pore structure as function of experimental conditions including mold geometry, material and thermal boundary conditions. First, a mathematical model integrating Computational Fluid Dynamics (CFD) with Population Balance Model is developed to predict average pore size (APS) of 3D porous chitosan alginate scaffolds and to assess the influence of the geometrical parameters of mold on scaffold pore structure. Purpose: The purpose of this study was to evaluate muscular adaptations between low-, moderate-, and high-volume resistance training protocols in resistance-trained men.

Methods: Thirty-four healthy resistance-trained men were randomly assigned to one of three experimental groups: a low-volume group performing one set per exercise per training session (n = 11), a moderate-volume group performing three sets per exercise per training session (n = 12), or a high-volume group performing five sets per exercise per training session (n = 11). Non-lethal, trait-mediated effects of predation impact prey behavior and life-history traits. Studying how these effects in turn influence prey demography is crucial to understand prey life-history evolution. Mosquitoes are important vectors that claim several million lives every year worldwide by transmitting a range of pathogens. Several ecological factors affect life-history traits of both larval and adult mosquitoes, creating effects that cascade to population-level consequences.
