Introduction Atrial fibrillation is the most common cardiac arrhythmia and is associated with significant morbidity and mortality. The classic cut-and-sew maze procedure is successful in 85–95% of patients. However, it is technically challenging and requires prolonged cardiopulmonary bypass. The left modified maze procedure is successful in 65–85% of patients. The aim of this study was to compare the success of two different ablation sources in the left modified MAZE. Introduction Retrograde transcatheter closure of perimembranous aneurysmal ventricular septal defects (pmVSD) can be a better option for transcatheter closure of ventricular septal defect with device placement into ventricular septal aneurysm with fewer complications. Also, a retrograde approach might decrease procedure time and radiation exposure time. Aim The purpose of this study was to report off-label experience using Amplatzer Vascular Plug II (AVPII) for the trans catheter closure of perimembranous aneurysmal VSD. The left internal mammary artery (LIMA) has been used as a conduit for coronary artery revascularization since 1968. Here we are reporting A total of 48 consecutive patients were operated upon using double mammary arteries. All of them received a mammary graft to the left anterior descending artery (LAD). All 48 of them were controlled angiographically at 1 year postoperative to ensure satisfactory well-functioning grafts.

Among visceral artery aneurysms, inferior mesenteric artery aneurysm is the rarest. Although most cases are of degenerative etiology, a small fraction of these cases are caused by autoimmune diseases. We report a case of spontaneous retroperitoneal hemorrhage from a ruptured inferior mesenteric artery aneurysm in a 30-year-old patient with systemic lupus erythematosus, treated by ligation.

American Indigenous populations are underrepresented in American political science discourse. There is a lack of knowledge on public perception of political trust within Indigenous communities. I argue that contemporary discourses on data and political participation of American Indigenous people are incomplete without framing that data within the context of ongoing settler colonialism. National data shows that nearly 71% of all American Indigenous people live in urban settings. Framing American Indigenous political participation requires an in depth examination of the role of American Settler colonialism. Highly conserved throughout evolution, lipins are dual functioning proteins found from yeast to humans. Functioning in the cytoplasm as phosphatidate phosphatase enzymes (PAP), lipins produce diacylglycerol that serves as a precursor for neutral fats and membrane phospholipids. Alternatively, nuclear lipins are responsible for the regulation of metabolic genes. Interestingly, both the mammalian lipin 1 paralog and the single Drosophila Lipin ortholog are highly phosphorylated proteins. Target of rapamycin (TOR) has previously been identified as one of the kinases that controls the subcellular localization of both lipin 1 and Drosophila Lipin. Pay secrecy is a burgeoning debate in compensation research. On one side of the debate, it is argued that pay secrecy is a useful and beneficial practice because it can prevent potential dissatisfaction and destructive competition possible when people make unfavorable pay comparisons. On the other side, it is argued that pay secrecy is undesirable because it obscures the motivational properties of pay and does not prevent people from making pay comparisons, nor safeguard them from the resulting dissatisfaction. Mercenaries are more powerful than experts realize, a grave oversight. Those who assume they are cheap imitations of national armed forces invite disaster because for-profit warriors are a wholly different genus and species of fighter. Private military companies such as the Wagner Group are more like heavily armed multinational corporations than the Marine Corps. Their employees are recruited from different countries, and profitability is everything. Patriotism is unimportant, and sometimes a liability. Unsurprisingly, mercenaries do not fight conventionally, and traditional war strategies used against them may backfire.

Fentanyl is a major topic in the news these days because of its significant contribution to the ongoing opioid epidemic in the United States. It clearly is a major counternarcotic challenge. But there also has been some reporting, including about congressional interest, as to whether fentanyl additionally should be considered a weapon of mass destruction (WMD) and whether U.S. Government chemical defense efforts should place greater emphasis on it. This paper provides some perspective on fentanyl as a chemical weapon.

Connected and autonomous vehicles (‘‘CAVs”) can collect, store, process and transmit vast amounts of data. Understanding the use (and potential misuse) of this data, particularly when that data is about an identifiable individual within the meaning of data protection law, is regarded critical to the success of this new mode of transportation. However, what constitutes personal information in relation to coneccted and automated vehicle data on a case-by-case basic. This presents a policy challenge for the government and creates uncertainty for businesses wishing to make use of this data.

The relatively new phenomenon of binge watching presents a dramatic shift from the norm of traditional television viewing. However, is this ubiquitously popular media marathon that contains negative connotations really bad for you? A survey administered to 157 television binge-watchers identified the role of stress and self-control in influencing both the gratifications sought and the consequences of binge watching through the Structural Equation Modeling (SEM) analysis. The current study found that binge watching to avoid problems is a commonly employed strategy for individuals to cope with stress. Gross' emotion regulation model, Porges' polyvagal theory, and other existing research suggest that regulation of emotions, tactics used to handle conflict, and certain physiological processes that occur within the autonomic nervous system (ANS) in response to stress are significantly related, especially in relational contexts. However, despite their pervasiveness and negative impacts, there is a noticeable lack of research on predictors of violent, aggressive, or abusive conflict tactics in couples. This study explores the impact of cultural competence on success in completing key missionary tasks. Qualitative survey results are supported by data from an intercultural effectiveness assessment and a Mandarin Chinese listening proficiency test to describe themes related to missionary communicative success and to explore correlations between intercultural effectiveness and listening proficiency. Missionary communicative tasks are clarified into themes:"obtaining referrals,"obtaining teaching opportunities," and"helping people make and keep commitments." Factors perceived as associating with communicative success include"feeling and communicating love" and receiving"spiritual guidance." Married women in the banking and management consulting firms in Nigeria encounter challenges that affect their commitment to their families while working long hours in demanding jobs. This study explores the challenges married women encounter and the impacts they have on women’s family lives, social lives, and health. I analyze primary and secondary sources to understand how organizational work culture such as long working hours, work competitiveness, and Nigeria’s unstable economy negatively affect the work-life balance of married women in banking and management consulting firms. Melanocytoma is a deeply pigmented variant of melanocytic nevus that classically occurs in the optic disk, sometimes with contiguous involvement of the adjacent retina or choroid. Historically, this tumor was often confused with malignant melanoma both clinically and histopathologically. Today, however, it is generally recognized by its typical clinical features that differ from most melanomas and erroneous enucleation is rarely done. Histopathologically, melanocytoma is composed of intensely pigmented round to oval nevus cells with benign features. Although traditionally believed to be a relatively stationary lesion, it is now known to exhibit minor enlargement in 10--15% of cases and can cause …

A 60-year-old Caucasian female was referred for biopsy-proven amelanotic orbito-conjunctival melanoma. Map biopsies revealed residual invasive melanoma on the deep tarsal margin at the site of previous surgery. Repeat excisions were required after recurrence was detected following 3 months and 7 months. Positron emission tomography scan detected liver metastasis and additional orbito-conjunctival melanoma recurrence. Biomarker testing showed NRAS mutation without BRAF or c-KIT mutations and without PD-L1 expression. Systemic checkpoint inhibitor therapy was initiated with regression of both the orbito-conjunctival melanoma and liver metastasis. Invasive, non resectable orbito-conjunctival melanoma with liver metastasis can demonstrate a response to systemic checkpoint inhibitor …

Purpose: Intra-arterial chemotherapy (IAC) has emerged as an effective treatment for retinoblastoma (RB) however, little information exists regarding its use in older patients (>5 years). In the present study, we evaluate the use of IAC (2008-2018) for RB in older patients and compare the outcomes to those in the prechemotherapy (<1994) and intravenous chemotherapy (IVC) (1994-2007) eras.
Methods: A retrospective analysis of all patients older than 5 years treated with IAC for RB from 2008-2018. Comparisons were made to 26 active RB cases in older children treated in the prechemotherapy era and to 12 active RB cases treated in the IVC era.

Introduction: Alopecia areata (AA) is an autoimmune disease characterised by non-scarring hair loss in single or multiple areas of the scalp. The disease affects hair on the head or other parts of the body. AA occurs in people of all ages and affects 1-2% of human population.
