High-throughput sequencing (HTS) of immunoglobulin (B-cell receptor, antibody) and T-cell receptor repertoires has increased dramatically since the technique was introduced in 2009 (1-3). This experimental approach explores the maturation of the adaptive immune system and its response to antigens, pathogens, and disease conditions in exquisite detail. It holds significant promise for diagnostic and therapy-guiding applications. New technology often spreads rapidly, sometimes more rapidly than the understanding of how to make the products of that technology reliable, reproducible, or usable by others. Personalized medicine (PM) aims to harness a wave of 'omics' discoveries to facilitate research and discovery of targeted diagnostics and therapies and increase the efficiency of healthcare systems by predicting and treating individual predispositions to diseases or conditions. Despite significant investment, limited progress has been made bringing PM to market. We describe the major perceived regulatory, intellectual property, and reimbursement challenges to the development, translation, adoption, and implementation of PM products into clinical care. KEYNOTE-030 (ClinicalTrials.gov ID, NCT02083484) was a global expanded access program that allowed access to pembrolizumab, an antiprogrammed death 1 antibody, for patients with advanced melanoma before its regulatory approval. Patients with unresectable stage III/IV melanoma that progressed after standard-of-care therapy, including ipilimumab and, if BRAF mutant, a BRAF inhibitor, were eligible to receive pembrolizumab 2 mg/kg every 3 weeks. Response was assessed by immune-related response criteria by investigator review. Adverse events (AEs) were graded according to the National Cancer Institute Common Terminology Criteria for Adverse Events, version 4.0. Crossflow Ultrafiltration For Removing Direct-15 Dye From Wastewater Of Textile Industry, Ahmad A.L., Puasa S.W., Abiding S.

Ultrafiltration membrane was used to treat the effluent from textile industries. Crossflow ultrafiltration using GN polymeric membrane was used to remove the dye from textile effluent. A synthetic textile effluent of Direct-15 dye was used. The study focused through the effect of feed concentration, transmembrane pressure and solution’s pH on the permeate flux and percentage of dye removal were investigated. Dye concentration had significant effects on flux values. Under the fixed pressures and pH, the flux decreased while the dye rejection increased with increasing feed concentration. Transmembrane pressure also had significant effect on flux values. Under the fixed feed concentration …

The dataset is representative of 20 research cruises spanning from July 2010 to August 2013 in the North and West Florida Shelf regions of the Gulf of Mexico. This dataset reports microtoxicity data from each site and is an extension and revision of C-IMAGE I dataset R1.x135.119:0005. The North Florida Self data also contains PAH measurements for three cruises (12/6/2010, 12/15/2011, 8/6/2013).

T-cell acute lymphoblastic leukemia (T-ALL) is an aggressive malignancy of thymocytes. Using a transgenic screen in zebrafish, thymocyte selection–associated high mobility group box protein (TOX) was uncovered as a collaborating oncogenic driver that accelerated T-ALL onset by expanding the initiating pool of transformed clones and elevating genomic instability. TOX is highly expressed in a majority of human T-ALL and is required for proliferation and continued xenograft growth in mice. Using a wide array of functional analyses, we uncovered that TOX binds directly to KU70/80 and suppresses recruitment of this complex to DNA breaks to inhibit nonhomologous end joining (NHEJ) repair. …

OBJECTIVE: To investigate the relationship of vitamin D deficiency and risk of AMI in a Pakistani population, and to find out any associationbetween vitamin D binding protein (VDBP) genotypes and risk of AMI in this population.
METHODS: In a comparative cross-sectional study, 246 patients (age: 20-70 years; 171 males and 75 females) with first AMI were enrolled with informed consent. Similarly, 345 healthy adults (230 males and 115 females) were enrolled as controls. Growing Up Fast: Implications For Foster Youth When Independence And Early Adulthood Collide, Brenda M. Morton

Foster care alumni face overwhelming challenges as they transition from care to independence. Torn between their desire to be independent, yet acknowledging they need support, they struggle to find their footing. Adopting a survivor self-reliance mind-set, they set out to earn a bachelor's degree on their own. As they struggle, they compare themselves to non-foster peers who, by enlarge, have a support system enabling them a prolonged entrance to adulthood, which provides a safety net. Without a safety net, and with a focus on independence, decisions youth from foster care make, result in few alumni earning a bachelor's degree.

This is an exploration of the perceptions and perspectives of early childhood leaders and practitioners with regards to preschool learners with autism engaged in inclusive educational settings. At a time when inclusive education is acknowledged as best practice, there is still a concern that many preschool learners with disabilities are receiving most of their supports in a segregated setting. Additional concerns noted in the current research relate directly to the beliefs, values, and attitudes towards inclusive education for learners with disabilities. Background—The CEGS N-GRID 2016 Shared Task in Clinical Natural Language Processing (NLP) provided a set of 1000 neuropsychiatric notes to participants as part of a competition to predict psychiatric symptom severity scores. This paper summarizes our methods, results, and experiences based on our participation in the second track of the shared task.
Objective—Classical methods of text classification usually fall into one of three problem types: binary, multi-class, and multi-label classification. Cardiovascular diseases possess a major cause for fatality and disability the world over. Since last several decades, the rates of cardiovascular diseases-related deaths have decreased in a number of high-income countries but increased in low- and middle-income countries with around 80% of the burden. Objective: In this study we implemented the “flipped classroom” model to enhance active learning in medical students taking neurosciences module at Aga Khan University, Karachi.

Importance: Although the increased incidence of second primary malignant neoplasms (SPMs) is a well-known late effect after cancer, few studies have compared survival after an SPM to survival of the same cancer occurring as first primary malignant neoplasm (PM) by age.
Objective: To assess the survival impact of SPMs in adolescents and young adults (AYAs) (15-39 years) compared with that of pediatric (<15 >years) and older adult (≥40 years) patients with the same SPMs.

Until the December 2016 passage of the Cures Act, the FDA had regulatory power over clinical decision support (CDS) software; however, the Act removed a large group of CDS software from the FDA’s statutory authority. Congressional intent was to increase innovation by removing regulatory blockades—such as device testing and certification—from the FDA’s purview. This note argues that the enactment of this specific provision of the Act will instead stymie innovation and overlook the unfortunate safety consequences inherent in its deregulation. This article argues that the current regulation of the minicipal securities market is inadequate, and that regulatory reform is not only necessary but also permissible as the Securities and Exchange Commission has the legal authority under the current statutory framework to substantially remedy such inadequacy. In making this argument, this article focuses on the legislative history of the Securities Reform Act of 1975, analyses of statutory text, the current regulatory framework surrounding the municipal securities market, prior attempts to effect regulatory reform, and one of the principal issues with the current regulatory framework - the lack of uniform accounting principles …

While the disciplines of oceanography and limnology often operate in isolate, freshwater, estuarine, and marine ecosystems are intricately linked. The emphasis of this special issue for Limnology and Oceanography is on the aquatic continuum and the connectivity between aquatic ecosystems from headwater streams and inland waters, to coastal and marine systems. Changes in the transport and transformation of elements as well as ecological functions occur along this aquatic continuum. Assemblages of organisms change in a way that reflects the ecological and biogeochemical conditions of the aquatic gradient. Alectryon Vitiensis: A New Species Of Sapindaceae Endemic To Fiji, Sven Buerki, Porter P. Lowry Ii, Jérôme Munzinger, Marika Tuiwawa, Alivereti Naikatini, Martin W. Callmander

A new species of Alectryon Gaertn. (Sapindaceae) endemic to the Fijian archipelago is described as A. vitiensis Buerki, Lowry, Munzinger & Callm. based on morphological and molecular evidence. It can easily be distinguished from the two congeners currently known from Fiji by its smaller leaves, subsessile leaflets, apetalous flowers, and crested fruits. A phylogenetic analysis using ITS sequence data shows that the new species is closely related to two Australian endemics, A. diversifolius (F. Muell.) S. T. Reynolds and A. oleifolius (Desf.) S. T. Purpose
Although organic solvents are often used in agricultural operations, neurotoxic effects of solvent exposure have not been extensively studied among farmers. The current analysis examined associations between questionnaire-based metrics of organic solvent exposure and depressive symptoms among farmers.
Methods
Results from 692 male Agricultural Health Study participants were analyzed. Solvent type and exposure duration were assessed by questionnaire. An “ever-use” variable and years of use categories were constructed for exposure to gasoline, paint/lacquer thinner, petroleum distillates, and any solvent. Depressive symptoms were ascertained with the Center for Epidemiologic Studies Depression Scale (CES-D); scores were analyzed separately as continuous (0–60) …
