Baylisascaris Procyonis Exposure Case Study, Andrea M. Lauffer M.D., Patricia Lutz M.D., Susan L. Flesher M.D.
Pediatrics
We report a case of exposure to raccoon feces found to be contaminated with baylisascaris procyonis. The exposure was recognized early enough by the family to allow prophylaxis with albendazole. Because of the potential fatal or neurologically catastrophic effects of this disease immediate treatment is indicated. This is started in advance of environmental studies that are done to determine if the feces is indeed contaminated.

In The Postoperative Cardiothoracic Surgical Patient Being Mechanically Ventilated, Is There A Difference In Outcomes When Comparing Sedation With Dexmedetomidine Versus Propofol?, Benjamin Heinrich Riebesel
Doctoral Projects
Patients undergoing a cardiothoracic operation typically require mechanical ventilation in the postoperative phase. Each year approximately 395,000 of these operations are performed in the United States alone. As many as 10% of these patients require reoperation within the first few hours of recovery due to complications (Barash & Cullen, 2013). This comprehensive review of the literature was performed to determine whether postoperative sedation with dexmedetomidine leads to better patient outcomes than sedation with propofol. Inclusion criteria included publications written in the English language, articles available in full text, articles written within the last 10 years, and publications with a focus …

White-Nosed Coatis (Nasua Narica) As Hosts For The Endoparasite, Giardia Lamblia, December 2016, Megan Sha
Monteverde Institute: Tropical Ecology and Conservation
Giardiasis, the disease caused by the protozoan endoparasite, Giardia lamblia, has become an increasingly concerning threat to both animals and humans, especially for travelers in the form of “travelers’ diarrhea” and in developing countries such as Costa Rica. Parasitic infections are an omnipresent problem that are often a result of zoonotic diseases, or diseases that are transmitted from animal to human. This study strove to determine if white-nosed coatis (Nasua narica) could potentially be hosts to Giardia lamblia in Monteverde, similar to how they are capable of acting as hosts for other zoonoses. I collected 28 coati fecal samples at …

The Xpert MTB/RIF assay is both sensitive and specific as a diagnostic test. Xpert also reports quantitative output in cycle threshold (CT) values, which may provide a dynamic measure of sputum bacillary burden when used longitudinally. We evaluated the relationship between Xpert CT trajectory and drug exposure during tuberculosis (TB) treatment to assess the potential utility of Xpert CT for treatment monitoring. We obtained serial sputum samples from patients with smear-positive pulmonary TB who were consecutively enrolled at 10 international clinical trial sites participating in study 29X, a CDC-sponsored Tuberculosis Trials Consortium study evaluating the tolerability, safety, and antimicrobial activity …

I noticed that maintenance workers had the same itinerary when emptying trashcans, meanwhile some trashcans needed to be emptied urgently. Traditionally, ETSU maintenance operate on daily routes to pick trash on designated time, regardless the level of the containers. The time, resources and labor invested in collecting the trash could be saved. Therefore, I decided to use the Internet of Things (IoT) to create a device that will optimize trash collection, to reduce costs and pollution.

Vaginal prolapse is a common condition that affects a large number (approximately 3.3 million in the US) of women, mainly above the age of 50 years. The condition is characterized by the weakening and breakdown of the pelvic support structures such as muscles and connective tissues, leading to the dislodging and prolapse of pelvic organs. Current examinations of the pelvic tissue are manually performed by the physician who assesses its rigidity. These exams are subjective and offer qualitative rather than quantitative results of the state of the pelvic tissue. In this research, a complete system was conceptualized and designed to …

Compliance With Hand Hygiene Policy In A Nursing Home: A Pilot Study, Nina Ling
Compliance With Hand Hygiene Policy In A Nursing Home: A Pilot Study, Nina Ling
2016 Fall Honors Capstone Projects - Archive
This project investigated the unanswered question of nursing staff’s practice and attitudes toward hand hygiene policy in a nursing home. Hand hygiene is the most essential measure known to limit nosocomial infection (Eveillard et al., 2011). The gerontologic population is one of the biggest population segments in the healthcare system due to the period of the baby boomers (Eveillard et al., 2011). This population is known to have a weaker immune system in comparison to younger generations (Travers et al., 2015). Infection prevention is especially important to prevent the geriatric cascade, and the decline in quality of life (Travers et …

Retinoic Acid Signaling Regulates Krt5 Independently Of Stem Cell Markers In Submandibular Salivary Gland Epithelium., Timur Maratovich Abashev
Electronic Theses and Dissertations
Vitamin A metabolism, which produces the signaling molecule Retinoic Acid (RA), has been demonstrated to be important for growth and branching morphogenesis of mammalian embryonic salivary gland epithelium. However, it is not known whether RA functions directly within epithelial cells or in associated tissues that influence morphogenesis of salivary epithelium. Moreover, downstream targets of RA transcriptional regulation have not been identified. Here we show that canonical RA signaling occurs in multiple tissues of embryonic mouse salivary glands, including epithelium, associated parasympathetic ganglion neurons, and non‑neuronal mesenchyme. By culturing epithelium explants in isolation from other tissues we demonstrate that RA influences …
Mechanism Of Interaction Of Peptide Modified Nanoparticles With Porphyromonas Gingivalis., Ankita Jain
Mechanism Of Interaction Of Peptide Modified Nanoparticles With Porphyromonas Gingivalis., Ankita Jain
Electronic Theses and Dissertations
Studies suggest that P. gingivalis functions as a keystone pathogen and interacts with primary colonizers in the supragingival biofilm such as S. gordonii. This interaction contributes to the initial colonization of the oral cavity by P. gingivalis and thus represents a potential target for therapeutic intervention. We have identified a peptide (BAR) derived from the streptococcal SspB protein that functions to inhibit P. gingivalis adherence to S. gordonii. In addition, we showed that nanoparticles (NPs) functionalized with BAR inhibit this interaction more potently than free soluble peptide, possibly by promoting interaction with P. gingivalis at higher valency than …

Detecting Evidence Of Systemic Inflammation From Osteological Markers In The Indian Knoll Population Of Ohio County, Kentucky., Krysta Wilham
Electronic Theses and Dissertations
Indian Knoll is an Archaic shell midden site located in the Green River region of west-central Kentucky, and was excavated twice in the first half of the 20th century. While Indian Knoll has been the subject of frequent bioarchaeological studies, the present study presents an osteological analysis of the relationships between skeletal markers commonly associated with local inflammatory processes, and that have the potential to be used as proxies for systemic inflammation. The analysis revealed a significant positive association in the presence of periodontal disease (PD) and periosteal lesions (PL), suggesting a potential underlying hyper-inflammatory status or phenotype. These …

Few data are available measuring the cost burden of mycobacterial diseases on the national level. The purpose of this study was to evaluate the cost burden and measure emerging trends in hospitalization of pulmonary TB and NTM in the US from 2001 through 2012. Methods: This study was a retrospective community based cost analysis of hospitalized patients with a principal diagnosis of pulmonary mycobacterial diseases from 2001 through 2012. Data for pulmonary TB and NTM were retrieved from the Healthcare Cost and Utilization Project (HCUP), US Department of …
Concurrent Biological, Electromagnetic Pulse, And Cyber Attacks - A Challenge To The Interagency Response, Patricia Rohrbeck
Concurrent Biological, Electromagnetic Pulse, And Cyber Attacks - A Challenge To The Interagency Response, Patricia Rohrbeck
Graduate Theses/Dissertations
The U.S. including its military depends on an electrical grid and electricity-based critical infrastructure. An electromagnetic pulse (EMP) and cyber attack can disable not just a significant portion of the electrical grid and critical infrastructure, but also the networkcentric military response to such an attack. There is a large range of actors that might attempt EMP attacks against the U.S.. Health surveillance systems are network-centric, and if mass destruction is the goal of an adversary, launching a biological attack concurrently with EMP and cyber attacks may achieve this goal. Current agency response plans focus on one WMD attack at a …
Interventions To Increase Vaccination Rates In Homeless Adults Aged 50 Years And Older In A Shelter-Based Clinic, Rubeena Smith
Interventions To Increase Vaccination Rates In Homeless Adults Aged 50 Years And Older In A Shelter-Based Clinic, Rubeena Smith
Doctor of Nursing Practice
Pronounced disparities in adult immunizations exist across the country. In homeless adults over 50 years of age with chronic diseases, coverage rates for influenza and pneumococcal vaccinations falls at 30% as compared to the 60% coverage rate found in the general public. Hepatitis B immunization rates are also significantly lower in the elderly homeless population. Increased prevalence of chronic diseases in the elderly homeless shelter residents’ increase their risk for severe complications following influenza, hepatitis B and meningococcal infections. To prevent disease epidemics and further disability in the homeless population, the need for appropriate and timely vaccinations is critical. Attitudes …

An Introduction To Principal Surrogate Evaluation With The Pseval Package, Michael C. Sachs, Erin E. Gabriel
The R Journal
We describe a new package called pseval that implements the core methods for the evaluation of principal surrogates in a single clinical trial. It provides a flexible interface for defining models for the risk given treatment and the surrogate, the models for integration over the missing counterfactual surrogate responses, and the estimation methods. Estimated maximum likelihood and pseudo-score can be used for estimation, and the bootstrap for inference. A variety of post-estimation methods are provided, including print, summary, plot, and testing. We summarize the main statistical methods that are implemented in the package and illustrate its use from the perspective …

Food safety is a major concern in the shellfish industry, as severe illness can result from consuming shellfish that have accumulated waterborne pathogens. Shellfish harvesting areas are typically monitored for indicator bacteria such as fecal coliforms that serve as proxies for enteric pathogens introduced from surrounding waterways, but these indicators have shown little relation to some naturally occurring pathogenic bacteria such as Vibrio parahaemolyticus (Vp). To examine the dynamics and ecology of pathogenic and non-pathogenic strains of Vp and address the relevance of indicator bacteria in predicting Vp concentrations, field surveys and experiments were carried out in western Long Island …

In 2014, an estimated 480,000 people developed MDR-TB and 190,000 people died as a result of it. Approximately 9.7 % of people with MDR-TB are estimated to have XDR-TB. With the increase in the number of MDR-TB and XDR-TB cases, there is a dire need for the development of new anti-TB agents with novel mechanism of action. FtsZ, a crucial bacterial cytokinesis protein is a potential target for the development of new anti-TB agents. Previously a library of 2,5,6- and 2,5,7-trisubstituted benzimidazoles were synthesized and tested for their anti-TB activity. The lead molecules inhibited the assembly of Mycobacterium tuberculosis FtsZ …

Simian immunodeficiency virus (SIV) is a robust pathogen used in non-human primates to model HIV vaccines. SIV encodes a number of potential vaccine targets. By far the largest and most conserved protein target in SIV is its gag-pol protein that bears many epitopes to drive multivalent immune T cell responses. While gag-pol is an attractive antigen, it is only translated after a frame shift between gag and pol with the effect that gag and pol are expressed at an approximate 10/1 ratio. The codon bias of native lentiviral genes are also mismatched with the abundance of tRNAs in mammalian cells …

North American bats are becoming increasingly threatened due to stressors such as wind energy development and the emerging infectious disease white-nose syndrome. A better understanding of the seasonal behavior and habitat associations of temperate bat species can inform conservation strategies in the face of current threats, aiding effective management of remaining populations. To this end, we used acoustic bat detectors to examine summer occupancy and winter activity of bats in Big South Fork National River and Recreation Area on the Cumberland Plateau. Our specific objectives were 1) investigate the effects of post-fire landscape conditions on the summer occurrence of commuting …

In nature, most bacteria live in surface attached communities called bacterial biofilms. Community living is beneficial to the bacteria, as it offers heightened resistance to environmental stresses due to the production of a protective exopolymeric matrix. When the growth of a bacterial community reaches a certain threshold, some bacteria disperse to find new frontiers to occupy and repopulate. Bacterial biofilms can be beneficial as is the case with those found in our gastrointestinal tracks that help us digest food particles. However, biofilms can pose a major threat to human health when they are composed of pathogenic bacteria that cause chronic …

© 2016 John Wiley & Sons Ltd Amidst the rapid advancement in next-generation sequencing (NGS) technology over the last few years, salamanders have been left behind. Salamanders have enormous genomes—up to 40 times the size of the human genome—and this poses challenges to generating NGS data sets of quality and quantity similar to those of other vertebrates. However, optimization of laboratory protocols is time-consuming and often cost prohibitive, and continued omission of salamanders from novel phylogeographic research is detrimental to species facing decline. Here, we use a salamander endemic to the southeastern United States, Plethodon serratus, to test the utility …

West Nile virus (WNV) is a neurotropic ssRNA flavivirus that can cause encephalitis, meningitis, and death in humans and mice. Human TLR7 and TLR8 and mouse TLR7 recognize viral ssRNA motifs and induce antiviral immunity. However, the role of mouse TLR8 in antiviral immunity is poorly understood. In this article, we report that TLR8-deficient (Tlr8−/−) mice were resistant to WNV infection compared with wild-type controls. Efficient WNV clearance and moderate susceptibility to WNV-mediated neuronal death in Tlr8−/− mice were attributed to overexpression of Tlr7and IFN-stimulated gene-56 expression, whereas reduced expression of the proapoptotic gene coding Bcl2-associated X protein was observed. …
