The Humoral Theory Of Transplantation: Epitope Analysis And The Pathogenicity Of Hla Antibodies., Edward J Filippone, John Farber

Central to the humoral theory of transplantation is production of antibodies by the recipient against mismatched HLA antigens in the donor organ. Not all mismatches result in antibody production, however, and not all antibodies are pathogenic. Serologic HLA matching has been the standard for solid organ allocation algorithms in current use. Antibodies do not recognize whole HLA molecules but rather polymorphic residues on the surface, called epitopes, which may be shared by multiple serologic HLA antigens. Data are accumulating that epitope analysis may be a better way to determine organ compatibility as well as the potential immunogenicity of given HLA …

Chronic tachycardia is a well-known cause of nonischemic cardiomyopathy. We hypothesized that nebivolol, a β-blocker with nitric oxide activity, would be superior to a pure β-blocker in preventing tachycardia-induced cardiomyopathy in a porcine model.
Fifteen healthy Yucatan pigs were randomly assigned to receive nebivolol, metoprolol, or placebo once a day. All pigs underwent dual-chamber pacemaker implantation. The medication was started the day after the pacemaker implantation. On day 7 after implantation, each pacemaker was set at atrioventricular pace (rate, 170 beats/min), and the pigs were observed for another 7 weeks. Sickle cell disease is a risk factor for cerebrovascular accidents in the pediatric population. This risk is compounded by hypoxemia. Cyanotic congenital heart disease can expose patients to prolonged hypoxemia. To our knowledge, the long-term outcome of patients who have combined sickle cell and cyanotic congenital heart disease has not been reported. We retrospectively reviewed patient records at our institution and identified 5 patients (3 girls and 2 boys) who had both conditions. Their outcomes were uniformly poor: 4 died (age range, 12 mo-17 yr); 3 had documented cerebrovascular accidents; and 3 developed ventricular dysfunction. The surviving patient had developmental …

Superior vena cava syndrome is a well-known disease entity that carries substantial rates of morbidity and mortality. Although most cases of superior vena cava syndrome are secondary to a malignant process, additional causes (such as mediastinal fibrosis, pacemaker lead implantation, or central venous catheter placement) have been reported. Multiple treatment options include percutaneous transluminal angioplasty, stent implantation, thrombolysis, mechanical thrombectomy, and venous grafting. We present a case of superior vena cava syndrome in a symptomatic 30-year-old woman who obtained complete relief of obstruction and marked symptomatic improvement through venoplasty and stenting, aided by our use of a balloon-in-balloon catheter system.

Antiplatelet therapy reduces the risk of myocardial infarction, stroke, and vascular death in patients who have symptomatic peripheral artery disease. However, a subset of patients who take aspirin continues to have recurrent cardiovascular events. There are few data on cardiovascular outcomes in patients with peripheral artery disease who manifest aspirin resistance.
Patients with peripheral artery disease on long-term aspirin therapy (≥4 wk) were tested for aspirin responsiveness by means of the VerifyNow Aspirin Assay. The mean follow-up duration was 22.6 ± 8.3 months. The primary endpoint was a composite of death, myocardial infarction, or ischemic stroke. Secondary endpoints were the …

Background: Viruses are incredibly diverse organisms and impact all forms of life on Earth; however, individual virions are challenging to study due to their small size and mass, precluding almost all direct imaging or molecular analysis. Moreover, like microbes, the overwhelming majority of viruses cannot be cultured, impeding isolation, replication, and study of interesting new species. Here, we introduce PCR-activated virus sorting, a method to isolate specific viruses from a heterogeneous population. Specific sorting opens new avenues in the study of uncultivable viruses, including recovering the full genomes of viruses based on genetic fragments in metagenomes, or identifying the …

Accounts of privacy law have focused on legislation, federal agencies, and the self-regulation of privacy professionals. Crucial agents of regulatory change, however, have been ignored: the state attorneys general. This article is the first in-depth study of the privacy norm entrepreneurship of state attorneys general. Because so little has been written about this phenomenon, I engaged with primary sources — first interviewing state attorneys general and current and former career staff, and then examining documentary evidence received through FOIA requests submitted to AG offices around the country.

HIV disproportionately affects Black men who have sex with men (MSM), yet there are few evidence-based programs that respond to the diverse realities of Black MSM communities. This article examines the development of Real Talk, a new harm reduction-based, sexual health intervention for Black MSM. We first analyze the key themes from our formative research: (1) stigma, discrimination, and intersectionalities in the lives of Black MSM, (2) the importance of safe spaces and community provided by health promotion programs, and (3) moving beyond condoms in sexual health messaging. An increase in targeted violence incidents (TVIs), primarily active shooter events, at institutions of higher education (IHEs) has exposed gaps in campus security plan preparation and exercises. The purpose of this qualitative collective case study was to discover barriers to and best practices of universities and colleges conducting security preparedness activities for TVIs. The theory that guided this study was vested interest theory which predicts how attitudes will influence behavior in a commitment to preparedness fundamentals. Although there are several educational benefits resulting from participation in high school sports, African American students do not always experience the positive outcomes that other populations share. The purpose of this causal comparative study was to investigate the effect of athletic participation on the academic achievement of African American high school female students. The sample consisted of 100 female African American 12th-grade students. Descriptive statistics were collected and analyzed. The purpose of this transcendental phenomenological study was to discover the lived experiences of teaching students with ADHD for art, music, and physical education teachers at elementary schools. The theories guiding this study are Zentall’s (1975) Optimal Stimulation Theory and Ajzen’s (1991) Theory of Planned Behavior as they explain how teachers’ attitudes and use of stimulation in the classroom affects the central phenomenon. Community colleges have begun to rely more heavily on private funding sources like alumni donations to support their overall fiscal health. This correlational study tested the theory of planned behavior as it relates to level of engagement through social media for community college alumni and alumni giving behavior for Virginia’s community colleges. More specifically, it attempted to determine whether a relationship exists between alumni use of their alma mater’s Facebook page and engagement behaviors. The purpose of this multiple case study was to explore and capture how Pennsylvania homeschooled graduates transitioned from their homeschooled academic environment to a collegiate one. This research was founded on the need for further research addressing homeschoolers and their transition into college. Existing literature falls short which not only documents the first-hand, personal experiences taken directly from homeschooled graduates, but from their mothers, as well. Interest is peaking among educators in North America, and around the world, on issues relating to school climate. A primary reason for this strong interest is research confirmation that school climate may have a positive or negative effect on educational processes. For many years I have been collecting leaves and discovered a phenomena that I find truly fascinating on specific leaves called "leaf miner's leaves". The leaves are the dwellings of animals, various insects' architectural habitat. Leaf miner's leaves are a phenomena in nature in the plant kingdom where the larvae of various insects tunnel their way through a leaf eating its living tissue. The leaf miners create distinctive patterns on each leaf. The snake-like serpentine tunnels protect them from predators and plant defenses.

To be successful in a STEM career, not only STEM knowledge and skills but also creativity is required. Therefore, the arts have been integrated into STEM disciplines and subsequently designated as STEAM education (Sousa & Pilecki, 2013). One example of informal learning environments that STEAM education provided is a summer camp. In this study, middle and highs school students’ use of their creativity in the Project-based Learning (PBL) courses was examined to determine students’ belief about the use of the arts in STEM activities. This dissertation determined the effect of known and theoretical risk factors for LBP in the Olympic class sailors utilizing a cohort design. A secondary goal was to determine if any factors related to sailors or sailing would provide a protective effect, lowering the risk of LBP. Sailors exposed to hiking (a sailing-specific activity) and non-hiking sailing classes were analyzed for the effect of hiking activity as a risk factor for LBP. Hiking exposure increases risk of LBP. Exposure to more than two hours of unloading the sailors’ boats and equipment prior to a regatta was a second unique risk factor …
