Climatic conditions of environmental zone Nemoral 2 of Europe are favorable for agricultural development. However, more frequent events of hot air-waves and prolonged droughts occurring as a consequence of climate change lead to soil moisture content reduction down to the plant wilting point. Although regular screening mammography has been suggested to be associated with improvements in the relative survival of breast cancer in recent years, the appropriate age to start screening mammography remains controversial. In November 2009, the United States Preventive Service Task Force published updated guidelines for breast cancer, which no longer support routine screening mammography for women aged 40–49 years, but instead, defer the choice of screening in that age group to the patient and physician. The age to begin screening differs between guidelines, including those from the Task Force, the American Cancer Society and the World Health Organization. It remains …

Section 305 of the FDA Food Safety Modernization Act specifically calls for FDA to develop recommendations on whether and how to harmonize requirements under the Codex Alimentarius Commission (“Codex”), an international organization charged with developing food standards, guidelines, codes of practice and “other recommendations to ensure fair practices in food trade and protect[ion of] the health of consumers.” FDA’s International Food Safety Capacity-Building Plan is largely supportive and deferential to Codex, concluding that “the use of Codex standards helps assure a safe global food supply.” To be sure, Codex’s stated mission and policies should create and facilitate adoption of universal …

The inherent tendency of patented seeds to self-replicate has led to fears that farmers might face liability for inadvertent patent infringement. To address the perceived problem, some have proposed severely limiting the availability of effective patent protection for self-replicating technologies. Typical examples include denying patent rights to "second generation" self-replicating products, and even broadly declaring such technologies ineligible for patent protection. The fact is, lawsuits against inadvertently infringing farmers remain of largely hypothetical concern. However, changes in the market could soon render such lawsuits a reality. In addressing the resulting policy concerns, Congress and the courts have at their disposal …

Patient Populations, Clinical Associations, And System Efficiency In Healthcare Delivery System, Yazhuo Liu

The efforts to improve health care delivery usually involve studies and analysis of patient populations and healthcare systems. In this dissertation, I present the research conducted in the following areas: identifying patient groups, improving treatments for specific conditions by using statistical as well as data mining techniques, and developing new operation research models to increase system efficiency from the health institutes’ perspective. The results provide better understanding of high risk patient groups, more accuracy in detecting disease’ correlations and practical scheduling tools that consider uncertain operation durations and real-life constraints.

The aim of this research was to understand mutualistic relationships between plants and endophytic microbes under the influence of an altered gravity vector. Understanding the outcome of the plant-microbe symbiosis in altered gravity is vital to developing life support systems for long distance space travel and colonization of other planets. This research utilized the model tripartite relationship among Medicago truncatula–Sinorhizobium meliloti–Rhizophagus irregularis. Plants were inoculated with rhizobial bacteria (S. meliloti), arbuscular mycorrhizal fungi (R. irregularis), both microbes, or none (microbial control), and placed on a rotating clinostat for 28 d. Obesity is associated with increased risk for chronic migraine and migraine progression, but associations with episodic migraine (em) and episodic tension-type headache (tth) are unclear. Most studies have relied on bmi as an indicator of adiposity. More accurate anthropometric measures that distinguish adipose tissue from other body tissue are critical to examine adiposity-headache associations, including validated measures of abdominal adiposity and established measurement formulas such as body adiposity index (bai) and body composition equations developed by Peterson et al. Alfalfa is the most widely produced perennial forage legume in North America. However, its use in the semiarid northern Great Plains is limited due to poor stand establishment and persistence under drought condition. The development of drought-tolerant alfalfa cultivars is of great need. Some Medicago sativa subsp. falcata populations have demonstrated promising drought resistance when compared to M. sativa. Morphological and physiological mechanisms play a critical role in drought tolerance by influencing seedling survival, stand establishment and drought recovery. Background: Cardiac arrest, or downtime, can result in ischemic damage to myocardial tissue, which prompts caution in accepting hearts with such a history for transplant. Our aim is to provide guidance about whether these hearts are suitable and which among them confer optimal outcome.
Methods: We analyzed all first-time adult cardiac transplantations in the United States between 1988 and 2010 as reported in the United Network of Organ Sharing (UNOS) database. Stratification was between donors with downtime history prior to brain death versus those without such a history for univariate and multivariate analyses.

The use of national inpatient databases for orthopaedic surgery research has been increasing. However, large databases that rely on administrative data, such as International Classification of Diseases Ninth Revision (ICD-9) codes, may misrepresent patient information, thus affecting the results of studies using this data.
The present study uses easily quantified and objective variables of obesity and anemia as example comorbidities to assess the accuracy of ICD-9 codes in the setting of their continued use in orthopaedic surgery database studies.

It has been shown that elderly patients have decreased anesthetic requirements due to age-related changes in pharmacokinetics, pharmacodynamics and drug sensitivity. Previous work has also shown that anesthetic dosing is often not being rigorously corrected for age.
Purpose:
To determine whether the dosing of intravenous (IV) induction anesthetics in patients undergoing gastrointestinal (GI) surgeries are corrected appropriately for age.

Melanoma therapy has changed rapidly due to the emergence of new therapies: MAPK-pathway targeted drugs and immunomodulatory agents. Given the relative success of these new individual drugs, this work set out to evaluate and develop effective melanoma treatments using combination therapies in a preclinical mouse melanoma models. Therapies tested include BRAF kinase inhibition in combination with: immune checkpoint inhibitors anti-CTLA4, anti-PDL1, and with the topical TLR7/8 agonist imiquimod. Background: Cocaine is the third most commonly used illicit substance in pregnancy. Few studies exist to specifically look for correlates of cocaine use in pregnancy.
Objective: To identify correlates of frequency of cocaine use in pregnancy
Methods: We analyzed retrospective data from 50 postpartum women with a history of cocaine use or dependence who participated in a treatment trial for postpartum cocaine use. Study variables included severity of baseline cocaine use, age of onset of use, comorbid mental illness, poly-substance use, trauma and social support history. Data were analyzed using GLMM Negative Binomial Regression.

Basic palliative and end-of-life care skills are necessary for all physicians regardless of their field of specialty. Education should begin during the early stages of medical training, as early as medical school, to ensure that all physicians acquire a basic understanding of these aspects of medical care. At the Yale School of Medicine, the End-of-Life and Palliative Care Curriculum was formalized in 2008 to address this need and was expanded in 2012 to include an original educational online module. Research is necessary to uncover ways to improve adolescent nutrition and reduce obesity rates, particularly in the Appalachian region, which has high rates of food insecurity and adolescent obesity. The current study examines rural cultural norms about food, memorable messages adolescents received about nutrition, and the sources of identified memorable messages. Adolescents shared memorable messages during comprehensive, semi-structured, small group interviews in which participants revealed their individual experiences. Thematic framework analysis is used to present the range and nature of memorable messages about nutrition and to develop strategies for future health campaigns and interventions. Environmental toxicants can alter epigenetic regulatory features such as DNA methylation and microRNA expression. As the sensitivity of epigenomic regulatory features may be greatest during the in utero period, when critical windows are narrow, and when epigenomic profiles are being set, this review will highlight research focused on that period. I will focus on work in human populations, where the impact of environmental toxicants in utero, including cigarette smoke and toxic trace metals such as arsenic, mercury and manganese, on genome-wide, gene-specific DNA methylation has been assessed. DL-Methionine (Met) has been conventionally used in swine diets with assumption of similar bioefficacy with L-Met. However, because L-Met is the form that is utilized by animals for protein synthesis, L-Met could, theoretically, be more available. Four experiments were conducted to evaluate L-Met bioavailability in nursery pigs with 21-day growth trials. A total of 105,105,112 and 84 crossbred pigs were used in Exp. 1, 2, 3 and 4, respectively. Each experiment had a low Met basal diet and 3 levels of the Met sources (DL-Met and L-Met). In addition to the basal diet, supplementation levels were 0.053%, 0.107% and 0.160% …
