The International Health Regulations 10 Years On: The Governing Framework For Global Health Security, Lawrence O. Gostin, Mary C. Debartolo, Eric A. Friedman
Georgetown Law Faculty Publications and Other Works
The World Health Organization (WHO) and its global health security treaty, the International Health Regulations (2005) (IHR) have lost the world's confidence after the West African Ebola epidemic. The epidemic led to several high-level reviews of the IHR and global health security more broadly. Here, we propose a series of recommendations for operational and legal reforms to enhance the functioning of the FCGH. It is critical that WHO act on them quickly, before the window of opportunity for fundamental reform closes.
WHO should ensure that all states fulfill their obligations to develop national core surveillance and response capacities, including through …

An innate front-line defense mechanism termed mucociliary clearance (MCC) occurs in the airway epithelium and is critical for respiratory health. MCC is made possible by the coordinated efforts of goblet and multiciliated cells, two cell types that line the airway epithelium. Maintaining the relative populations of goblet and multiciliated cells is necessary for optimum MCC, however, decreases in the multiciliated cell population and dysfunctional MCC are seen in a variety of respiratory diseases. In this essay, I analyze discourses circulating during the 2004 re-election campaign of George W. Bush and Dick Cheney as a means to explore the interactions of three tropes of “evil” as identified by James P. McDaniel (2003). In the months between September 11, 2001 and November 2, 2004, the tropes of “Evil-in-itself,” “Evil-for-itself,” and “Evil-for-others” converged, combined, and competed in the culmination of criticism leveled at the Bush-Cheney campaign regarding the screening of entrants into events and rallies. Integral to this interaction is the articulation of American democracy with capitalism, as theorized by Kenneth Burke (1969). Ultimately, I argue …

Use of all-terrain vehicles (ATVs) has become standard practice on the modern ranch. The unique operating conditions present on a ranch, subject the occupational ATV user to hazards requiring awareness and specialized training. The purpose of this study was to apply social marketing methods to address a specific environmental health and safety issue present in the agricultural industry. A series of four ATV tip sheets were created in topic areas specific to the challenges that ATV operators encounter on a ranch. The steroid alkaloid cyclopamine has shown much promise as a treatment for cancers in which aberrant hedgehog signaling plays a role. The compound, originally discovered due to its teratogenic effects in sheep, binds to the hedgehog signaling receptor Smoothend and prevents downstream activation. As this pathway is primarily active during embryonic development, overactivation later in life can lead to tumor formation and proliferation. Several studies have shown that cyclopamine can inhibit and even reverse tumor growth, but limited supply will prevent widespread use upon FDA approval of it or any of its semi-synthetic analogs. This is a theory paper that advocates experimental evolution as a novel approach to study economic preferences. Economics could benefit because preferences are exogenous, axiomatic, and contentious. Experimental evolution allows the empirical study of preferences by placing organisms in designed environments and studying their genotype and phenotype over multiple generations. We describe a number of empirical studies on different aspects of preferences. We argue that experimental evolution has the potential to improve economics.

This study investigated the relationship between host efflux system of the non-vertebrate nematode Caenorhabditis elegans and Burkholderia cepacia complex (Bcc) strain virulence. This is the first comprehensive effort to profile host-transporters within the context of Bcc infection. With this aim, two different toxicity tests were performed: a slow killing assay that monitors mortality of the host by intestinal colonization and a fast killing assay that assesses production of toxins. A Virulence Ranking scheme was defined, that expressed the toxicity of the Bcc panel members, based on the percentage of surviving worms. According to this ranking the 18 Bcc strains were …

Publication of accurate and detailed descriptions of methods in research articles involving animals is essential for health scientists to accurately interpret published data, evaluate results and replicate findings. Inadequate reporting of key aspects of experimental design may reduce the impact of studies and could act as a barrier to translation of research findings. Reporting of animal use must be as comprehensive as possible in order to take advantage of every study and every animal used. Animal models are essential to understanding and assessing new chemotherapy candidates for Chagas disease pathology, a widespread parasitic disease with few treatment options currently available. …

In recent years, Native, Indigenous, First Nations, and Aboriginal scholars and writers have forged alliances to initiate and support decolonization efforts and the reassertion of native survivance. Native and non-Native scholars have responded to modern challenges by reconceptualizing notions of peoplehood, identity, and nationalism. Following these intellectual contours, rather than conceiving of native culture as totalizing, static, and/or incommensurable—as always already foreign—responsive readings informed by the critical work of Gerald Vizenor can support more sophisticated understandings of native literary production while revealing sites of native transmotion. Mice are widely used for human tumor xenograft studies of cancer development and drug efficacy and toxicity. Stable isotope tracing coupled with metabolomic analysis is an emerging approach for assaying metabolic network activity. In mouse models there are several routes of tracer introduction, which have particular advantages and disadvantages that depend on the model and the questions addressed. This protocol describes the bolus i.v. route via repeated tail vein injections of solutions of stable isotope enriched tracers including 13C6-glucose and 13C5,15N2-glutamine. We report the genome of a Staphylococcus aureus strain (ILRI_Eymole1/1) isolated from a nasal swab of a dromedary camel (Camelus dromedarius) in North Kenya. The complete genome sequence of this strain consists of a circular chromosome of 2,874,302 bp with a GC-content of 32.88 %. In silico annotation predicted 2755 protein-encoding genes and 76 non-coding genes. This isolate belongs to MLST sequence type 30 (ST30). Phylogenetic analysis based on a subset of 283 core genes revealed that it falls within the human clonal complex 30 (CC30) S. aureus isolate cluster but is genetically distinct. Fuel (wood, bone, dung and coal) and the selection of it has always been an integral part in the lifeways of nomadic pastoralists. Certainly, the gathering of fuel, the storage of it, and its eventual use represents a large part of daily culture. Yet, it remains an understudied aspect of ancient household economies. We study the correlation properties of word lengths in large texts from 30 ebooks in the English language from the Gutenberg Project (www.gutenberg.org) using the natural visibility graph method (NVG). NVG converts a time series into a graph and then analyzes its graph properties. First, the original sequence of words is transformed into a sequence of values containing the length of each word, and then, it is integrated. Next, we apply the NVG to the integrated word-length series and construct the network. We show that the degree distribution of that network follows a power law, P(k)∼k−γP(k)∼k-γ, with two regimes, which …

As the number of individuals, particularly the elderly, using herbal products with prescription drugs continues to grow, the risk for adverse interactions increases but remains poorly recognized. The true incidence and nature of adverse herb reactions or herb-drug interactions remains unknown since no postmarketing surveillance mechanism exists. Adverse events are greatly underreported, and information regarding safety mainly comes from case reports and suboptimally conducted studies in a limited number of healthy young volunteers or patients with limited comorbidities. Alcohol consumption and its abuse is a major health problem resulting in significant healthcare cost in the United States. Infection with intestinal helminths results in immunological changes that influence the odds of comorbid infections, and might also affect fecundity by inducing immunological states supportive of conception and pregnancy. Here we investigate associations between intestinal helminths and fertility in human females, utilizing nine years of longitudinal data from 986 Bolivian forger-horticulturalists, experiencing natural fertility and a 70% helminth prevalence. We find that different species of helminth are associated with opposing effects on fecundity. Infection with roundworm (Ascaris lumbricoides) is associated with earlier first births and shortened interbirth intervals, while infection with hookworm is associated with delayed first pregnancy and extended …
