A Randomized Control Trial Comparing Weight Adjusted Dose Versus Fixed Dose Prophylactic Phenylephrine Infusion On Maintaining Systolic Blood Pressure During Caeserean Section Under Spinal Anaesthesia, Lucy Wambui Mwaura

Master of Medicine in Anaesthesiology

Background: Spinal anaesthesia is the standard of care for elective caesarean delivery. Its advantages over general anaesthesia include: decreased blood loss, reduced postoperative pain and faster recovery time. Despite these advantages, the sympathetic blockade induced by spinal anaesthesia results in 80 percent incidence of hypotension without prophylactic management. This hypotension can cause: nausea, vomiting, cardiovascular collapse or even loss of consciousness in the mother, and fetal acidosis. Current evidence supports co-loading with intravenous fluids in conjunction with the use of vasopressors as the most effective way to prevent and treat the hypotension. In Citizens United v. FEC, a 5-4 majority of the Supreme Court ruled that “the Government cannot restrict political speech based on the speaker's corporate identity.” The decision remains controversial, with many arguing that the Court effectively overturned more than 100 years of precedent. I have previously argued that this decision turned on competing conceptions of the corporation, with the majority adopting a contractarian view while the dissent advanced a state concession view. However, the majority opinion was silent on the issue of corporate theory, and the dissent went so far as to expressly disavow any role for corporate …

Aim: To determine the incidence and risk factors for acute myocardial injury after non-cardiac surgery at Aga Khan University Hospital Nairobi. Design: Prospective cohort study Background: Myocardial injury is the most common major cardiovascular complication in patients undergoing non-cardiac surgery. Large numbers of intermediate-risk and high-risk cardiac patients will be undergoing surgery as the population grows older, and thus myocardial injury after non-cardiac surgery could be a rising problem. This thesis examines the complex, interdisciplinary nature of counterinsurgency in the 21st century by assessing the whole-of-government, population-centric approach that the Dutch Armed Forces and Government took to combat the Taliban in Uruzagn, Afghanistan from 1 August 2006 to 1 August 2010. The Dutch approach resulted in increased security, as well as increased political and economic capacity in the three districts of Uruzgan where they focused their efforts. By the end of Task Force Uruzgan, the Government of Afghanistan had increased its majority control from 0 percent of the population to approximately 60 percent, and the Afghan National Security Forces …

Psychoanalysis has long been a breakthrough in the field of literary criticism. This book celebrates the intermarriage between two fields: psychology and literature. It provides a thorough analysis of selected dramatic texts authored by the Afro-American playwright Lorraine Hansberry. Such analysis is achieved through employing the humanistic psychology theories of the leading humanistic psychologists: Abraham Maslow, Carl Rogers and Rollo May to emphasize that features of humanistic psychology are detected in Hansberry's plays. Nebulous combinations of face-to-face and online learning are increasingly common across Australian higher education contexts. This paper reports on part of a redesign project of an undergraduate education subject at a regional university. The aim of the redesign was to enhance e-learning and blended learning environments. An approach that maps the evaluation research activities to the design and development cycle of e-learning tools and learning tasks was adopted (Phillips et al., 2012). The research took a participatory format involving ongoing reflective exchange with pre-service teachers with the aim of transforming practice. Since the early spring of 2014, the world has watched Russia utilize military forces to invade and annex territory belonging to Ukraine. These actions are, unsurprisingly, raising concerns in Eastern Europe over the prospect of armed conflict in the region, the political consequences of Russian annexation of Ukrainian territory, and the effect of this conflict on ordinary civilians. But there is another potential cost associated with Russia's actions that should not be overlooked - the loss of Ukrainian cultural heritage. Communication is essential to a successful patient-provider interaction. Within health communication literature, a substantial body of research has focused on verbal communication; however, few studies have fully dedicated to nonverbal communication research. The study examined the relationship between perceptions of client nonverbal immediacy and ratings of the therapeutic alliance. Additionally, the study provided an analysis comparing counselor and client ratings of both client nonverbal immediacy and the therapeutic alliance. Results indicated a significant relationship between counselor ratings of client nonverbal immediacy and counselor ratings of the therapeutic alliance. Counselors and clients also rated client nonverbal immediacy similarly, indicating that the …

The Visible #Family: Using The Hashtag To Study Family Construction, Joshua M. Parcha

The purpose of this study is to see, through a raw collection of data, how the family is currently being portrayed on social networking sites. Twitter is used to examine the social construction of family based upon observing tweets that contain #family. This paper argues that the family is best understood as being constructed through interactions with others, using symbolic interaction theory for theoretical grounding. Through examining over 1,000 tweets using a grounded theory methodological approach, three main themes emerged: family types, statements of family, and the esoteric family. The emergent themes suggest several implications, including that the family is …
