Racecadotril For The Treatment Of Severe Acute Watery Diarrhoea In Children Admitted To The Kenyatta National Hospital - A Randomised Double Blinded Placebo Controlled Trial, Jaspreet Singh Gharial
Master of Medicine in Paediatrics and Child Health
Background: Diarrhoeal disease is the second most common cause of death in children under 5 years of age in Kenya. Treatment with Oral Rehydration Solution (ORS), elemental zinc and continued feeding (as recommended by the World Health Organisation) led to a significant reduction in mortality in the 1970s and 1980s, which has slowed since then. Adjunctive treatments have however, not gained much popularity either due to modest improvements in morbidity and mortality or unacceptable adverse effect profiles. Racecadotril, an enkephalinase inhibitor, has been used for over 2 decades in parts of Europe but is only recently gaining recognition in other …
Hospital-Acquired Malnutrition In Children In A Tertiary Care Hospital, Del-Rossi Sean Quadros
Hospital-Acquired Malnutrition In Children In A Tertiary Care Hospital, Del-Rossi Sean Quadros
Master of Medicine in Paediatrics and Child Health
Introduction: Hospital-acquired Malnutrition occurs as a result of reduction in food intake, increased dietary loss and/or increased calorie requirements as a result of disease-induced high catabolic state. A child's nutritional status often deteriorates after admission to the hospital resulting in longer duration of hospital stay and increased risk of complications, which also increases treatment cost. Hospital-acquired Malnutrition is usually assessed using anthropometric measurements and/or serum pre-albumin. Objectives: This study sought to investigate the incidence of Hospital-acquired Malnutrition using anthropometric measurements and to determine diagnostic utility of serum pre-albumin in predicting weight change. A secondary objective was to identify factors associated …

Cura Personalis: A Healthcare Delivery Quandary At The End Of Life, George P. Smith Ii
Cura Personalis: A Healthcare Delivery Quandary At The End Of Life, George P. Smith Ii
Scholarly Articles
Holistic Medicine traces its provenance to the foundational value or chrism of the Society of Jesus of cura personalis which directs respect be given to all individuals and to their souls — especially whenever medical healing is required. Today, the notion of best patient care should include not merely attention to somatic issues of refractory pain management but, equally, to non-somatic or existential suffering. It is at the end-stage of life that palliative — as opposed to curative — care must be provided. When a condition is seen as medically futile, this Article advocates palliative or deep sedation — when …
How Law Made Silicon Valley, Anupam Chander
How Law Made Silicon Valley, Anupam Chander
Emory Law Journal
Explanations for the success of Silicon Valley focus on the confluence of capital and education. In this Article, I put forward a new explanation, one that better elucidates the rise of Silicon Valley as a global trader. Just as nineteenth-century American judges altered the common law in order to subsidize industrial development, American judges and legislators altered the law at the turn of the Millennium to promote the development of Internet enterprise. Europe and Asia, by contrast, imposed strict intermediary liability regimes, inflexible intellectual property rules, and strong privacy constraints, impeding local Internet entrepreneurs. This study challenges the conventional wisdom …

Background: The ability to predict the survival time of breast cancer patients is important because of the potential high morbidity and mortality associated with the disease. To develop a predictive inference for determining the survival of breast cancer patients, we applied a novel Bayesian method. In this paper, we propose the development of a databased statistical probability model and application of the Bayesian method to predict future survival times for White Hispanic female breast cancer patients, diagnosed in the US during 1973-2009. Materials and Methods: A stratified random sample of White Hispanic female patient survival data was selected from the …

Evaluation Of A Memory Book Intervention With Orphaned Children In South Africa, Barbara J. Braband, Tamara Faris, Kaye Wilson-Anderson
Faculty Publications - College of Nursing
The purpose of this collaborative research study was to evaluate the use of the Memory Book intervention for orphaned children's grief and loss recovery. A qualitative phenomenological approach was implemented to evaluate the Memory Book intervention with orphaned children at two children's homes in South Africa. Study findings support the ability of children to work through loss and grief when they are assisted in preserving and telling their story. The Memory Book intervention assists children to chronicle their lives and demonstrates the potential to guide future interventions by care providers and nurses in this context.
The P2y2 Receptor Interacts With Ve-Cadherin And Vegf Receptor-2 To Regulate Rac1 Activity In Endothelial Cells, Zhongji Liao, Chen Cao, Jianjie Wang, Virginia H. Huxley, Olga Baker, Gary A. Weisman, Laurie Erb
The P2y2 Receptor Interacts With Ve-Cadherin And Vegf Receptor-2 To Regulate Rac1 Activity In Endothelial Cells, Zhongji Liao, Chen Cao, Jianjie Wang, Virginia H. Huxley, Olga Baker, Gary A. Weisman, Laurie Erb
College of Health and Human Services
Vascular endothelial cadherin (VE-cadherin) mediates homophylic adhesion between endothelial cells and is an important regulator of angiogenesis, blood vessel permeability and leukocyte trafficking. Rac1, a member of the Rho family of GTPases, controls VE-cadherin adhesion by acting downstream of several growth factors, including angiopoietin-1 and vascular endothelial growth factor (VEGF). Here we show that UTP-induced activation of the Gq protein-coupled P2Y2 nucleotide receptor (P2Y2R) in human coronary artery endothelial cells (HCAECs) activated Rac1 and caused a transient complex to form between P2Y2R, VE-cadherin and VEGF receptor-2 (VEGFR-2). Knockdown of VE-cadherin expression with siRNA did not affect UTP-induced activation of extracellular …

Deflation Troughs, Water, And Prehistoric Occupation On The Margins Of The South Texas Sand Sheet, Juan L. Gonzalez, Russell K. Skowronek, Bobbie L. Lovett
History Faculty Publications
Within the South Texas Plains, the area broadly defined by the Rio Grande to the south and the Nueces River to the north, a distance of ca. 175 km, evidence of open human occupation is remarkably abundant. Because it is predominantly a region of loose, sandy soils and active and relict sand dunes where wind processes dominate, the area is known as the South Texas Sand Sheet (STSS). There is no running water within the STSS and all streams are ephemeral. Existing drainage systems are small, localized, and not integrated, carrying water for a few days and up to two …

Combating Trademark Squatting In China: New Developments In Chinese Trademark Law And Suggestions For The Future, Sunny Chang
Combating Trademark Squatting In China: New Developments In Chinese Trademark Law And Suggestions For The Future, Sunny Chang
Northwestern Journal of International Law & Business
Abstract: This Note explores the phenomenon of trademark squatting in China. Several characteristics relating to the development of Chinese Trademark Law, such as the first-to-file and multi-class application system, as well as the inherent complexities of the Chinese language, contributed to creating an environment amenable to trademark squatting. New developments in China, including a December 2011 opinion from the Supreme Court and a recent amendment to Chinese Trademark Law, signal that the country is moving forward towards stronger protection of intellectual property. However, these changes will likely not be enough to prevent trademark squatters from targeting well-known foreign trademarks, since …
An International Organization’S Identity Crisis, Sungjoon Cho
An International Organization’S Identity Crisis, Sungjoon Cho
Northwestern Journal of International Law & Business
The ontological status of international organizations remains largely nebulous despite their contemporary prominence. Traditional international relations theories regard international organizations (IOs) as instruments primarily created to serve powerful states’ interests (neorealism) or to facilitate interstate cooperation on certain regulatory areas (neoliberal institutionalism). These theories hardly offer a satisfactory explanation of a distinctive mode of IOs’ identity forming process, in which a particular IO, as a separate and autonomous organic entity, grows, evolves and eventually makes sense of its own existence. This Article offers a novel perspective that attempts to overcome the aforementioned theoretical deficiency. Drawing on the identity theory in …

Changes In Cerebral White Matter, Vascular Risk And Cognition Across The Adult Lifespan, Andrew Robert Bender
Changes In Cerebral White Matter, Vascular Risk And Cognition Across The Adult Lifespan, Andrew Robert Bender
Wayne State University Dissertations
Numerous studies over the past decade have used diffusion tensor imaging (DTI) to examine associations between age, diffusion and anisotropy measures of cerebral white matter (WM), and cognitive performance. However, few have examined relationships between intra-individual change in DTI measures of WM and cognitive function. It is possible that the extant cross-sectional findings are a poor representation of age-related change in WM and cognition. The present study used latent difference-score modeling (LDM) to assess change over two years in DTI indices fractional anisotropy (FA), radial diffusivity (DR), axial diffusivity (DA) and mean diffusivity (MD). In addition, we examined the effects …
Regulation Of Nkcc2 Trafficking By Vesicle Fusion Proteins Vamp2 And Vamp3 In The Thick Ascending Limb, Paulo Sebastian Caceres Puzzella
Regulation Of Nkcc2 Trafficking By Vesicle Fusion Proteins Vamp2 And Vamp3 In The Thick Ascending Limb, Paulo Sebastian Caceres Puzzella
Wayne State University Dissertations
The thick ascending limb (TAL) in the kidney regulates extracellular fluid volume and blood pressure. The Na/K/2Cl cotransporter NKCC2 plays a central role in NaCl absorption by the TAL and blood pressure. NKCC2 trafficking to the apical membrane is a major mechanism to control NKCC2 activity. However, little is known about the proteins that mediate NKCC2 trafficking. Inhibition of the vesicle fusion proteins VAMP2 and VAMP3 blunts the increase in surface NKCC2 expression and NaCl absorption in response to stimulation by cAMP. In other cells, VAMPs mediate fusion of exocytic vesicles with the plasma membrane. Whether VAMP2 and VAMP3 mediate …

The Mechanism Of Regulation Of Autosomal Heterochromatic Genes In Drosophila Melanogaster Males By Rox Rna And Msl Proteins, Satya Kiran Koya
Wayne State University Dissertations
In humans and flies, males and females have different set of sex chromosomes contributing to different levels of X-linked gene expression. To equalize X-linked gene dosage between sexes, both humans and flies developed independent strategies which are called dosage compensation. Human females randomly inactivate one of their X chromosome into barr body and Drosophila males up regulate their single X chromosome two fold. Both strategies equalize of X linked gene dose between sexes.
In Drosophila, dosage compensation is brought about by the ribonucleoprotein Male Specific Lethal (MSL) complex that binds hundreds of sites along the X chromosome and modifies …

Investigation Into The Binding Interactions Of Klenow Fragment To Dna Modified With Carcinogens Af And Aaf Using Surface Plasmon Resonance, Ashley M. Floyd
Wayne State University Dissertations
The two major forms of DNA adducts from the carcinogen N-acetoxyacetyl-2-aminofluorene, N-(deoxygunanonsin-8-yl)-2-acetylaminofluorene (dG-C8-AAF) and N-(deoxyguanosin-8-yl)-2-aminofluorene (dG-C8-AF), are both known to impede replication, though in different ways. AAF is a strong block to replication leading to frameshift mutations, while the AF adduct is more easily bypassed, causing base substitutions. Surface plasmon resonance (SPR) was used to study the binding of exonuclease deficient E. coli polymerase I, Klenow fragment (KF), to DNA modified with AF or AAF at two locations: as a templating base or in the last formed base pair. KF binding to the modified DNA bases was also monitored to …
Trauma Exposure, Posttraumatic Stress, And Depression In A Community Sample Of First-Time Mothers, Mickey Sperlich
Trauma Exposure, Posttraumatic Stress, And Depression In A Community Sample Of First-Time Mothers, Mickey Sperlich
Wayne State University Dissertations
The adverse effects of posttraumatic stress and depression have separately been well-documented in the perinatal mental health literature. However, few studies have considered the comorbidity between trauma, posttraumatic stress and depression. This dissertation study brings attention to this comorbidity and explores implications of recent changes to diagnostic criteria for posttraumatic stress disorder related to the ability to predict postpartum depression and impairments in mother/infant bonding. Following a conceptual framework which outlines the effects of violence and trauma on adverse childbearing outcomes, hypotheses were that many women with depression in pregnancy would endorse trauma and would be at risk for subthreshold …
New Carbohydrate-Based Anti-Cancer And Anti-Bacterial Vaccines, Zhifang Zhou
New Carbohydrate-Based Anti-Cancer And Anti-Bacterial Vaccines, Zhifang Zhou
Wayne State University Dissertations
The unique carbohydrates expressed on the surface of cancer, bacterial, viral and fungal cells are excellent target antigens for the design of therapeutic or preventive vaccines. However, as antigens carbohydrates have problems. First, carbohydrates usually have low immunogenicity. Second, even if immunogenic, carbohydrates typically elicit T cell-independent immune responses. To overcome these problems and design useful vaccines based on carbohydrate antigens, they are usually coupled with carrier proteins to form conjugates to enhance the immunogenicity of the antigens. However, there are still some issues existing in glycoprotein vaccines, such as poor reproducibility of the conjugates, difficulties in quality control and …
Lanthanide-Based Precatalysts For Carbon-Carbon Bond-Forming Reactions In Aqueous Media, Derek James Averill
Lanthanide-Based Precatalysts For Carbon-Carbon Bond-Forming Reactions In Aqueous Media, Derek James Averill
Wayne State University Dissertations
The formation of carbon-carbon bonds is of great interest to synthetic chemists because these bonds make up the majority of biologically active compounds. The Mukaiyama aldol reaction is a Lewis-acid-catalyzed carbon-carbon bond-forming reaction that has the ability to produce optically active β-hydroxy carbonyls which can be found in many pharmaceuticals and natural products. Because of precatalyst instability towards hydrolysis, anhydrous solvents are commonly used. Recent efforts have focused on water-tolerant versions of enantioselective Mukaiyama aldol reactions because of the financial and environmental benefits of using aqueous media. Consequently, the Lewis-acidic and water-tolerant features of Ln3+ ions have aroused great …

Results: …
Migratory Connectivity And Carry-Over Effects In Northwest Atlantic Loggerhead Turtles (Caretta Caretta, L.), Simona Ceriani
Migratory Connectivity And Carry-Over Effects In Northwest Atlantic Loggerhead Turtles (Caretta Caretta, L.), Simona Ceriani
Electronic Theses and Dissertations
Migration is a widespread and complex phenomenon in nature that has fascinated humans for centuries. Connectivity among populations influences their demographics, genetic structure and response to environmental change. Here, I used the loggerhead turtle (Caretta caretta, L.) as a study organism to address questions related to migratory connectivity and carry-over effects using satellite telemetry, stable isotope analysis and GIS interpolation methods. Telemetry identified foraging areas previously overlooked for loggerheads nesting in Florida. Next, I validated and evaluated the efficacy of intrinsic markers as a complementary and low cost tool to assign loggerhead foraging regions in the Northwest Atlantic Ocean (NWA), …

Electronic Theses and Dissertations
This research examines the history of a small Florida agricultural community over the course of the twentieth century from a rhetorical perspective in order to understand the technological and communicative transitions that governed the development of American agricultural production. By examining archival and oral histories, this research will add to our understandings of how written and oral communications temper the relationships and social situations of an agricultural community, including the knowledge-making and technological adaptation resulting from communications within the community and with outside institutions and entities. Agricultural villages are not isolated entities, but rather sites of multiple rhetorical situations, and …

Monitoring Of Livestock Grazing Effects On Bureau Of Land Management Land, Kari E. Veblen
Wildland Resources Faculty Publications
Public land management agencies, such as the Bureau of Land Management (BLM), are charged with managing rangelands throughout the Western United States for multiple uses such as livestock grazing and conservation of sensitive species and their habitats. Monitoring of condition and trends of these rangelands, particularly with respect to effects of livestock grazing, provide critical information for effective management of these multi-use landscapes. We therefore investigated the availability of livestock grazing-related quantitative monitoring data and qualitative region-specific Land Health Standards (LHS) data across BLM grazing allotments in the Western United States. We then queried university and federal rangeland science experts …
