Patient-Centered Medical Homes: An Analysis Of The Effects Of Practice Capabilities On Costs, Mary Temm

The purpose of this research was to examine the relationship between Patient-Centered Medical Homes (PCMH) practice capabilities and costs. Complexity science theory was used to develop a theoretical framework to address three primary and twelve secondary hypotheses. Secondary data from a consortium of physician organizations participating in Blue Cross Blue Shield of Michigan's Physician Group Incentive Program and Blue Cross Blue Shield of Michigan's administrative claims data were used to test hypothesized relationships using fixed effects panel regression models. It was found that higher levels of aggregated PCMH practice capabilities were negatively associated with certain costs, specifically average readmission costs. …

Although Tuberculosis (TB) is often regarded as a disease of the past, it is still one of the leading killers of people worldwide. TB is caused by Mycobacterium tuberculosis, a bacterium that mostly infects the lungs but can also infect other organs throughout the body. One goal for TB treatment is to create a biocompatible, biodegradable drug delivery system that employs the use of multiple drugs while simultaneously relieving the patient from the burden of self-medicating. Capturing carbon dioxide (CO2) from stationary sources and injecting it into deep underground geologic formations has been identified as a viable method for reducing carbon emissions to the atmosphere. Sedimentary rocks, such as sandstones overlain by shales or evaporites, are the preferred formations because their morphology and structure provide pore space, and containment for the long term storage of CO2. Sandstone formations have also served as repositories to migrating hydrocarbons, and are the sites of many oil recovery operations. For many depleted oil reservoirs, secondary waterflooding recovery methods are no longer efficient or economically viable, hence the application of tertiary …

Multiple Sclerosis (MS) is an autoimmune disease resulting from aberrant immune cells causing demyelination. We utilize the well-studied mouse model, experimental autoimmune encephalomyelitis (EAE), to decipher the underlying pathogenesis of MS. Effector CD4 T-cells play a critical role during EAE because the disease is induced by adoptively transferring myelin-specific CD4 T-cells into naïve mice. Classically, effector CD4 T-cells have been divided into distinct subsets based on their functional properties: Th1 cells produce IFNγ, Th2 cells secrete IL-4, IL-5, and IL-13, Th17 cells make IL-17A, IL-17F, IL-21, and IL-22, and regulatory T cells produce IL-10. In this dissertation, we approach quantum computing from a communication perspective that is to model quantum computing with a generalized noisy communication channel approach. We proposed a generalized noisy communication channel approach (GNCom) for quantum computing. The proposed GNCom approach is applied to quantum computing as a universal quantum register. This dissertation examined the relationship between high-technology medical services and hospital performance by focusing on financial and quality performance dimensions. An initial systematic review disclosed the paucity of research and mixed findings about the technology-performance relationship. The review indicated the need for further analyses by emphasizing some limitations of existing studies. The resource-based view (RBV) of a firm was used as a conceptual framework in examining the relationship between high-tech medical services and hospital financial performance. In non-excitable cells, STIM1 and Orai1 are the primary mediators of store-operated Ca2+ entry (SOCE); however little is known about their roles in cardiomyocyte Ca2+ handling. We have previously shown that acute increases in the levels of O-GlcNAcylation (the attachment of O-linked N- acetylglucosamine (O-GlcNAc) to target proteins) are cardioprotective through attenuating Ca2+-mediated injury and increased O-GlcNAcylation blunts SOCE in cardiomyocytes. Therefore, the goals here were to determine the regulation of STIM1 and STIM1-mediated SOCE by O-GlcNAcylation in cardiomyocytes. Insulin resistance plays a central role in the pathogenesis of Type 2 Diabetes (T2DM) and involves diminished stimulation of glucose transport in skeletal muscle. Improvements in insulin sensitivity are essential for both prevention and treatment of T2DM. However, the limitation of current medications highlights the need for alternative molecular regulators of insulin action and developing new insulin sensitizers. Our laboratory has initially identified roles for the orphan nuclear receptor NR4A3 in insulin-sensitization and stimulation of glucose transport. However, the cellular effect and downstream mechanisms of NR4A3 and its agonists (e.g. 6-mercaptopurine/6-MP) on insulin effector system in skeletal muscle remain unknown. …

Basal-like breast cancers (BLBC) display aggressive clinical behavior attributed to the presence of cancer stem cells (CSCs). TRA-8, a monoclonal antibody that binds specifically to TRAIL death receptor 5, preferentially induces apoptosis in BLBC subtype cells; however it is unknown if TRA-8 is also cytotoxic to the CSC population. Moreover, some BLBC cell lines are resistant to TRA-8, and overcoming resistance is critical for clinical translation of this therapy. One potential approach to overcome TRA- 8 resistance is to target the Wnt/ß-catenin pathway. High blood pressure is a prevalent precursor to cardiovascular disease. An estimated 3% of US adolescents have hypertension. In addition, prehypertension is predictive of hypertension in adolescents, with progression between these stages at approximately 7% per year. Tobacco use/exposure is strongly linked to cardiovascular risk and disease in adults. Further, rural communities have higher tobacco use prevalence and fewer community policies restricting tobacco use. Due to the growing trend in interracial marriages in the United States, the issue of birth outcomes to infants born to such couples deserves more attention and examination. The purpose of this population-based, cross-sectional, observational study is to examine the association between interracial couple status and birth outcomes (i.e. low birth weight, very low birth weight, preterm birth, very preterm birth, small for gestational age) among singleton infants delivered in the United States. This article is a revised update to a previously released study of the U.S. News academic reputation scores (“peer assessment scores”). The revised study incorporates the results from the 2013 U.S. News rankings and encompasses the period between 1998 and 2013. The study reveals that there has been a significant downward trend in the academic reputation scores of law schools since 1998. While the academic reputation scores tended to be relatively stable throughout the sixteen year period, over 60% of the law schools in the 172 school data set finished the sixteen year period with academic reputation scores that were …

Adult learners of Chinese learned new characters through writing, visual chunking or reading-only. Following training, ERPs were recorded during character recognition tasks, first shortly after the training and then three months later. We hypothesized that the character training effects would be seen in ERP components associated with word recognition and episodic memory. Results confirmed a larger N170 for visual chunking training than other training and a larger P600 for learned characters than novel characters. Another result was a training effect on the amplitude of the P100, which was greater following writing training than other training, suggesting that writing training temporarily …

The connections among language, writing system, and reading are part of what confronts a child in learning to read. We examine these connections in addressing how reading processes adapt to the variety of written language and how writing adapts to language. The first adaptation (reading to writing), as evidenced in behavioral and neuroscience data, is achieved through a universal constraint that language places on writing and through the tuning of reading procedures imposed by specific features of writing systems. Children acquire skill in reading through increasing specialization of procedures tuned to their writing system, while also acquiring more general (universal) …

This paper advances the notion of civil non-state actors in peacekeeping and peacebuilding. Using Sierra Leone, Liberia, and Côte d’Ivoire as cases studies, the paper identifies three kinds of civil non-state actors in war-torn countries: international non-governmental organizations (NGOs), community-based NGOs, and ad hoc community organizations. In addition, it argues that civil non-state actors play a critical problem-solving role in peacekeeping and peacebuilding and complement the role of state actors. The paper examines the role of civil non-state actors through their dialectical affinity with state actors in the peacekeeping and peacebuilding processes. It further expands the notion of non-state actors …

Even without a majority rationale, the opinions in Troxel v. Granville, 530 U.S. 57 (2000), support the notion that there are federal constitutional “liberty interests of parents in the care, custody, and control of their children” which generally foreclose state – compelled nonparent childcare over parental objections. They also recognize there can be exceptions, including perhaps when parental wishes are accorded “at least some special weight;” when “harm or potential harm” to children may ensue; when there are already “substantial relationships” between children and nonparents; or when states legitimately seek to preserve “established familial or family-like bonds.” As well, the …

American state parentage laws have evolved significantly in the past half century in response to changes in both reproductive technologies and human conduct. Yet further evolution, if not a revolution, seems inevitable. In particular, American state laws increasingly recognize parenthood by consent for a nonbiological and nonadoptive parent-like figure in an intimate or familial relationship with a child’s biological or adoptive parent. Lawmakers, especially state legislators, will need to consider who may be granted standing as parents to seek court-ordered childcare, including former stepparents, former boyfriends and girlfriends, and present and former grandparents. This Article addresses the ambiguity regarding the legal protectability of tattoos and the negative impact such ambiguity has on the rights of tattoo artists and their clients/customers. Notwithstanding the lack of federal court decisions in this area of the law, this Article determines that tattoos meet the requirements of copyrightability in the United States—originality and fixation in a tangible medium of expression. This Article examines the challenges presented by providing copyright protection for tattoos. In this article we review the literature on financial literacy, financial education, and consumer financial outcomes. We consider how financial literacy is measured in the current literature, and examine how well the existing literature addresses whether financial education improves financial literacy or personal financial outcomes. We discuss the extent to which a competitive market provides incentives for firms to educate consumers or offer products that facilitate informed choice. We review the literature on alternative policies to improve financial outcomes, and compare the evidence to evidence on the efficacy and cost of financial education. Finally, we discuss directions for future research.

What Goes Up Must Come Down? Experimental Evidence On Intuitive Forecasting, John Beshears, James J. Choi, Andreas Fuster, David Laibson, Brigitte C. Madrian

Do laboratory subjects correctly perceive the dynamics of a mean-reverting time series? In our experiment, subjects receive historical data and make forecasts at different horizons. The time series process that we use features short-run momentum and long-run partial mean reversion. Half of the subjects see a version of this process in which the momentum and partial mean reversion unfold over 10 periods (‘fast’), while the other subjects see a version with dynamics that unfold over 50 periods (‘slow’). Typical subjects recognize most of the mean reversion of the fast process and none of the mean reversion of the slow process.

Parental involvement is an often proposed, but rarely researched, key element of youth programs. Questions remain regarding the impact of parental involvement on program processes and outcomes. Qualitative data were collected over a one-year period with youth participants (n=46), parents (n=26), and teachers (n=5) associated with an international immersion/service learning program for adolescents. Three main research questions guided the data analysis: (1) what role does parental involvement play in the youths’ experience in the program; (2) how does parental involvement in the program influence the parent/child relationship; and (3) what role does parental involvement play in terms of the program’s …

STEM Librarians at Syracuse University actively participate throughout the scientific publication process. This chapter will outline points in the process where librarian involvement is applicable and beneficial. Librarians Anne Rauh and Linda Galloway will explain how and why it is important to teach tools that organize scientific literature to faculty and students before they begin the writing process. The use of assessment techniques to help inform targeted publication venues will be described. Post-publication, this chapter will describe how to help authors gauge the impact of their work, promote their publications and increase their visibility.

Pulmonary inhalation chemotherapeutic drug delivery offers many advantages for lung cancer patients in comparison to conventional systemic chemotherapy. Inhalable particles are advantageous in their ability to deliver drug deep in the lung by utilizing optimally sized particles and higher local drug dose delivery. In this work, spray-dried and co-spray dried inhalable lung surfactant-mimic PEGylated lipopolymers as microparticulate/nanoparticulate dry powders containing paclitaxel were rationally designed via organic solution advanced spray drying (no water) in closed-mode from dilute concentration feed solution. Experimental and numerical studies were conducted to understand the effect of plate curvature on blast response of carbon/epoxy composite panels. A shock-tube system was utilized to impart controlled shock loading to quasi-isotropic composite panels with differing range of radii of curvatures. A 3D Digital Image Correlation (DIC) technique coupled with high-speed photography was used to obtain out-of-plane deflection and velocity, as well as in-plane strain on the back face of the panels. Macroscopic post-mortem analysis was performed to compare yielding and deformation in these panels.
