Periprosthetic Joint Infection Increases The Risk Of One-Year Mortality., Benjamin Zmistowski, Joseph A. Karam, M.D., Joel B Durinka, David S Casper, Md, Javad Parvizi Md

Rothman Institute Papers

BACKGROUND: Periprosthetic joint infection continues to potentially complicate an otherwise successful joint replacement. The treatment of this infection often requires multiple surgical procedures associated with increased complications and morbidity. This study examined the relationship between periprosthetic joint infection and mortality and aimed to determine the effect of periprosthetic joint infection on mortality and any predictors of mortality in patients with periprosthetic joint infection.

METHODS: Four hundred and thirty-six patients with at least one surgical intervention secondary to confirmed periprosthetic joint infection were compared with 2342 patients undergoing revision arthroplasty for aseptic failure. The incidence of mortality at thirty days, ninety …

Battery Energy Storage Systems To Mitigate The Variability Of Photovoltaic Power Generation, Heath Alan Gurganus Dec 2013

Battery Energy Storage Systems To Mitigate The Variability Of Photovoltaic Power Generation, Heath Alan Gurganus

Dissertations and Theses

Methods of generating renewable energy such as through solar photovoltaic (PV) cells and wind turbines offer great promise in terms of a reduced carbon footprint and overall impact on the environment. However, these methods also share the attribute of being highly stochastic, meaning they are variable in such a way that is difficult to forecast with sufficient accuracy. While solar power currently constitutes a small amount of generating potential in most regions, the cost of photovoltaics continues to decline and a trend has emerged to build larger PV plants than was once feasible. This has brought the matter of increased …

Outcomes Of An Emotion Word Intervention For Children With Social Communication Impairments, Rebecca Cloward Mansfield

Theses and Dissertations

Children with social communication disorders have been found to exhibit deficits in emotional intelligence, including the ability to identify emotions attributed to facial expressions. The purpose of this thesis was to examine the efficacy of a social communication intervention program designed to increase the accuracy of emotion based word use in three elementary school-aged participants with social communication disorders. The participants took part in a multiple-baseline, 20-session treatment including story enactment, journaling procedures, and supplementary activities. The story enactment portion of the intervention centered on Mercer Mayer's A Boy, A Dog, and A Frog (1967) wordless picture book series. Participants' …

A Dynamic Optimization Framework With Model Predictive Control Elements For Long Term Planning Of Capacity Investments In A District Energy System, Jose Luis Mojica Velazquez Dec 2013

A Dynamic Optimization Framework With Model Predictive Control Elements For Long Term Planning Of Capacity Investments In A District Energy System, Jose Luis Mojica Velazquez

Theses and Dissertations

The capacity expansion of a district heating system is studied with the objective of evaluating the investment decision timing and type of capacity expansion. District energy is an energy generation system that provides energy, such as heat and electricity, generated at central locations and distributed to the surrounding area. The study develops an optimization framework to find the optimal investment schedule over a 30 year horizon with the options of investing in traditional heating sources (boilers) or a next-generation combined heat and power (CHP) plant that can provide heat and electricity. In district energy systems, the investment decision on the …

Mechanisms Of Dietary Protein-Induced Changes In Calcium Absorption Efficiency, Jessica D. Bihuniak Dec 2013

Mechanisms Of Dietary Protein-Induced Changes In Calcium Absorption Efficiency, Jessica D. Bihuniak

Doctoral Dissertations

We undertook a randomized, double-blind, crossover study in 14 healthy women who were required to ingest a 6-day low protein diet supplemented with CaSR-AAA (calcium-sensing receptor-activating amino acids), DAA (dibasic amino acids) or methylcellulose capsules (control). All subjects ingested all three diets in random order. Prior to each experimental diet subjects consumed an 11 day adjustment diet. Calcium absorption was measured on day 5 of each dietary intervention using dual-stable calcium isotopes. The clinical trial tested the hypothesis that dietary protein affects the efficiency of intestinal calcium absorption by activating the calcium sensing receptor (CaSR). Supplementation with CaSR-AAA did not …

Which Biomarkers Reveal Neonatal Sepsis?, Kun Wang, Vineet Bhandari, Sofya Chepustanova, Greg Huber, Stephen O′Hara, Corey S. O′Hern, Mark D. Shattuck, Michael Kirby

Publications and Research

We address the identification of optimal biomarkers for the rapid diagnosis of neonatal sepsis. We employ both canonical correlation analysis (CCA) and sparse support vector machine (SSVM) classifiers to select the best subset of biomarkers from a large hematological data set collected from infants with suspected sepsis from Yale-New Haven Hospital's Neonatal Intensive Care Unit (NICU). CCA is used to select sets of biomarkers of increasing size that are most highly correlated with infection. The effectiveness of these biomarkers is then validated by constructing a sparse support vector machine diagnostic classifier. We find that the following set of five biomarkers …

A Content Analysis Of Advertisements In The Fra And Craftsman Magazines, 1908-1916, Kathryn Runyan

Theses

The present study used content analysis to examine advertisements appearing in Elbert Hubbard's The Fra and Gustav Stickley's The Craftsman magazines from 1908 through 1916. A total of 599 ads were analyzed to determine what type of advertisements dominated this period and to discover differences in the persuasive and informative appeals used by two creative forces in American business at the beginning of the 20th century, Elbert Hubbard and Gustav Stickley. The purpose of this study is to understand the advertising strategies appearing in The Craftsman and The Fra magazines.

Those who finish either an Associates of Occupational Science or Associates of Applied Science degree programs tend to go to work in the chemical, biological, biotechnology, pharmaceutical, environmental, forensic, industrial, and food analysis fields. At first glance, the LST …

An Inverse Relationship Between Resistance To 2-Deoxy-D-Glucose And Glucose Starvation In Tumor Cells Under Normoxia, Katherine Beth Philips Dec 2013

An Inverse Relationship Between Resistance To 2-Deoxy-D-Glucose And Glucose Starvation In Tumor Cells Under Normoxia, Katherine Beth Philips

Open Access Dissertations

A distinguishing feature of tumor cells is their upregulation of glucose metabolism. This metabolic change from oxidative phosphorylation to aerobic glycolysis creates a selective window that can be exploited for anticancer therapy. Here we investigate mechanisms by which pancreatic cancer cells respond to therapeutic (2-deoxy-D-glucose, 2-DG) and physiologic (glucose starvation, GS) forms of glucose restriction. 2-Deoxy-d-Glucose (2-DG) is a sugar analog of both glucose and mannose and thus has established activities inhibiting glycolysis and interfering with glycosylation, respectively. Due to 2-DG’s dual functions, it has classically been thought of as a glucose deprivation mimetic. Additionally, these roles render 2-DG an …

The Escherichia coli sequence type 131 (ST131) clone is notorious for extraintestinal infections, fluoroquinolone resistance, and extended-spectrum beta-lactamase (ESBL) production, attributable to a CTX-M-15-encoding mobile element. Here, we applied pulsed-field gel electrophoresis (PFGE) and whole-genome sequencing to reconstruct the evolutionary history of the ST131 clone. PFGE-based cluster analyses suggested that both fluoroquinolone resistance and ESBL production had been acquired by multiple ST131 sublineages through independent genetic events. In contrast, the more robust whole-genome-sequence-based phylogenomic analysis revealed that fluoroquinolone resistance was confined almost entirely to a single, rapidly expanding ST131 subclone, designated H30-R. Strikingly, 91% of the CTX-M-15-producing isolates also …

Although a greater degree of personal obesity is associated with weaker negativity toward overweight people on both explicit (i.e., self-report) and implicit (i.e., indirect behavioral) measures, overweight people still prefer thin people on average. We investigated whether the national and cultural context – particularly the national prevalence of obesity – predicts attitudes toward overweight people independent of personal identity and weight status. Data were collected from a total sample of 338,121 citizens from 71 nations in 22 different languages on the Project Implicit website (https://implicit.harvard.edu/) between May 2006 and October 2010. We investigated the relationship of the explicit and implicit …

Pharmacy Faculty Articles and Research

Antibiotic-resistant bacterial pathogens threaten public health. Because many antibiotics target specific bacterial enzymes or reactions, corresponding genes may mutate under selection and lead to antibiotic resistance. Accordingly, antimicrobials that selectively target overall microbial cell integrity may offer alternative approaches to therapeutic design. Naturally occurring mammalian α- and θ-defensins are potent, non-toxic microbicides that may be useful for treating infections by antibiotic-resistant pathogens because certain defensin peptides disrupt bacterial, but not mammalian, cell membranes. To test this concept, clinical isolates of methicillin-resistant Staphylococcus aureus (MRSA), including vancomycin heteroresistant strains, and ciprofloxacin-resistant Pseudomonas aeruginosa (CipR-PA) were tested for sensitivity to …

In lower or simple species, such as worms and flies, disruption of the insulin-like growth factor (IGF)-1 and the insulin signaling pathways has been shown to increase lifespan. In rodents, however, growth hormone (GH) regulates IGF-1 levels in serum and tissues and can modulate lifespan via/or independent of IGF- 1. Rodent models, where the GH/IGF-1 axis was ablated congenitally, show increased lifespan. However, in contrast to rodents where serum IGF-1 levels are high throughout life, in humans, serum IGF-1 peaks during puberty and declines thereafter during aging. Thus, animal models with congenital disruption of the GH/ IGF-1 axis are unable …

Variation Found In Rates Of Restraint And Seclusion Among Students With A Disability, Douglas J. Gagnon, Marybeth J. Mattingly, Vincent J. Connelly

Carsey Publications

The restraint and seclusion of individuals—practices usually associated with highly restrictive environments—are extreme responses to student behavior used in some public schools. In this brief, authors Douglas Gagnon, Marybeth Mattingly, and Vincent Connelly report that restraint and seclusion are used much more frequently on students with a disability than on students without a disability. In addition, the majority of U.S. school districts does not restrain or seclude students with a disability; 59.3 percent of districts report no instances of restraint, while 82.5 percent do not report a single instance of seclusion. However, a small proportion of districts report exceedingly high …

Brief Monocular Deprivation As An Assay Of Short-Term Visual Sensory Plasticity In Schizophrenia - "The Binocular Effect", John J. Foxe, Sheralyn Yeap, Victoria M. Leavitt

Publications and Research

Background: Visual sensory processing deficits are consistently observed in schizophrenia, with clear amplitude reduction of the visual evoked potential (VEP) during the initial 50-150 of processing. Similar deficits are seen in unaffected first-degree relatives and drug-naïve first-episode patients, pointing to these deficits as potential endophenotypic markers. Schizophrenia is also associated with deficits in neural plasticity, implicating dysfunction of both glutamatergic and GABAergic systems. Here, we sought to understand the intersection of these two domains, asking whether short-term plasticity during early visual processing is specifically affected in schizophrenia.

Methods: Brief periods of monocular deprivation (MD) induce relatively rapid changes …

Online Social Media As A Social-Ecological Systems Research Tool: Facebook And Two Rural Alaskan Communities, Richard E. Hum Dec 2013

Online Social Media As A Social-Ecological Systems Research Tool: Facebook And Two Rural Alaskan Communities, Richard E. Hum

Rural Development

The earth has transitioned into the anthropocene, which is defined by complex environmental change linked to human behavior and requires new tools of analysis in order to understand shifting social-ecological system (SES) dynamics. In this work, I explore taking advantage of widespread online social media participation to develop the tools for doing so. Spatially grounded public exchanges on Facebook are examined with three goals in mind: 1) examine the types of SES content being passed through this communication medium, 2) compare community observations to relevant scientific observations, and 3) define a flexible and reproducible research method for integrating these communications …
