BURKHART EXECUTIVE DOCTOR OF SCIENCE IN ADMINISTRATION-HEALTH SERVICES ABSTRACT A study of the relationship between primary care physicians (PCPs) and hospitalists was conducted by surveying the Florida Academy of Family Physicians (FAFP) membership to determine whether they had an established relationship with a hospitalist or hospitalist group. The survey responses were then matched with Blue Cross and Blue Shield of Florida (BCBSF) administrative claims data to provide two groupings of study respondents with data regarding patients' average length-of-stay, average 30 day readmission rate, and average hospital claims payment. The facultative pathogen Streptococcus pneumoniae is capable of producing a polysaccharide (PS) capsule that prevents bacterial recognition and clearance by the host immune system. Over 90 antigenically distinct capsule serotypes have been identified. Because anti-capsule antibodies can mediate bacterial clearance, capsule diversity is critical to S. pneumoniae survival. However, the mechanisms of capsule evolution remain unclear. Due to the complex interaction of capsule synthesis (cps) genes, it is commonly thought that pneumococcal serotypes evolve once and that all clinically relevant serotypes are genetically derived from a single founding clone. In her two recent articles on pregnancy support, “Preglimony” in the Stanford Law Review and “The Price of Pleasure” in the Northwestern Law Review, Professor Shari Motro argues that American laws should no longer treat “lovers as strangers.” To minimize “the fundamental gender imbalance,” she urges that the relationship between an unwed heterosexual couple whose sex prompts pregnancy “demands it own legal category.” She calls for a new paradigm that generally holds men financially responsible to women for the burdens of pregnancy, employing both contract and tort principles. Interleukin-7 (IL-7) is an essential cytokine for lymphocyte growth that has the potential for promoting proliferation and survival. While the survival and proliferative functions of IL-7 are well established, the identities of IL-7 signaling components in pathways other than JAK/STAT, that accomplish these tasks remain poorly defined. To this end, we used IL-7 dependent T-cells to examine those components necessary for cell growth and survival. Our studies revealed two novel signal transducers of the IL-7 growth signal: BimL and JunD. IL-7 promoted the activity of JNK (Jun N-terminal Kinase), and that JNK, in turn, drove the expression of JunD, a …

Phosphatidylserine (PS) is an endogenously occurring phospholipid that has been shown to have cognition and mood enhancing properties in humans, possibly through its role as an enzyme co-factor in cellular signal transduction. Specifically, PS has been identified as activator of classical isoforms of protein kinase C, an enzyme known to be involved in the growth and differentiation of neural cells, and is therefore thought to play a role in the protection of neurons. Infectious disease remains one of the leading causes of morbidity and mortality worldwide. Many bacteria that cause disease have the capacity to enter into eukaryotic cells such as epithelial cells and tissue macrophages. Gaining access into the intracellular environment is one of the most critical steps in their survival and/or in pathogenesis. The entry mechanisms employed by these organisms vary considerably, but most mechanisms involve sabotaging and manipulating host cell functions. Invasion of epithelial cells involves triggering host signal transduction mechanisms to induce cytoskeleton rearrangement, thereby facilitating bacterial uptake. Fuelwood Collection And Consumption: A Case Study In Lupeta Tanzania, Katie M. Preston

Fuelwood is a major resource in rural areas. Fuelwood collection and consumption habits were monitored in Lupeta, Tanzania through household interviews and fuelwood collection walks. Social dimensions, economic aspects of fuelwood, and alternative fuel sources were also examined. The study found that for all wealth classes, fuelwood is the primary source of fuel used within the village, with the middle and upper classes occasionally supplementing fuelwood with charcoal. Women collect and consume fuelwood for cooking. The majority of women (69%) prefer to use charcoal because fuelwood collection is labor intensive and time consuming. While advocates of Community-Based Natural Resource Management (CBNRM) contend that this decentralized approach to natural resource management has higher potential for the distribution of conservation and social benefits throughout civil society than conventional protected area approaches, there is limited empirical research to confirm it, especially regarding goals and claims of gender empowerment. The cellular form of prion protein (PrPC) is a cell-surface glycoprotein attached to lipid rafts via its glycosylphosphatidylinositol anchor. Conversion of PrPC to its "scrapie" conformer (PrPSc, the fibrillar form) constitutes the key event of the etiology of prion diseases. Fibril dissociation is necessary for efficient conversion and continued propagation of the disease state. Recent studies have revealed that conversion occurs along the endocytic pathway. Despite widespread undernutrition among the homeless, there has been little anthropological research on the experience of food insecurity in this population. Between 20 and 40 percent of the homeless population is undernourished and one third regularly miss meals (Gelberg 1995). This thesis addresses the significant problem of food insecurity in the homeless from a political economic perspective, analyzing how larger social structures influence the individual person. Fifteen residents at a shelter in Missoula, MT were interviewed about their dietary practices and experience of social service programs. Among the main mechanisms of evolution we find adaptation, genetic drift, gene flow, mutation, natural selection and speciation. The following thesis explores bacterial mutation rate, mutants, mutators, and how competition acts as a driving force of adaptation.

In chapter 1, the system used in reference for mutation is the devolopment of Pseudomonas aeruginosa antibacterial resistance in the Cystic Fibrosis patients. Current methods for determining mutation rate are quite involved mathematically or complicated experimentally. A new method for estimating mutation rate is discussed and compared with two other widely used methods.

Fungal pathogens cause various diseases for plant and animal hosts. Despite the extensive impact of fungi on human health and life, the threats posed by emerging fungal pathogens are poorly understood. Specifically, there exist few fungal virulence gene databases, which prevent effective bioinformatics studies on fungal pathogens. Therefore, we constructed a comprehensive online database of known fungal virulence factors, which collected 2058 pathogenic genes produced by 228 fungal strains from 85 genera. This database creates a pivotal platform capable of stimulating and facilitating further bench studies on fungal pathogens.

T. pallidum can survive a primary immune response and continue growing in the host for an extended period of time. T. pallidum is thought to bind serum fibronectin (FN) through Tp0483 on the surface to obscure antigens. A Tp0483 fragment (rTp0483) was adsorbed onto functionalized self-assembled monolayers (SAMs) with FN. FN capture by adsorbed rTp0483 depended greatly on surface chemistry with COO- groups being best for FN binding. Hemocompatibility was determined by analysis of plasma protein adsorption, intrinsic pathway activation, and platelet activation. rTp0483+FN bound an equal or lesser amount of fibrinogen (Fg), human serum albumin (HSA), and factor XII …

The goal of this research was to determine the extent of resistance that turkey roundworms, Ascaridia dissimilis, have developed to anti-parasitic chemicals used in commercial poultry operations. Roundworm infections in turkeys have resulted in monetary losses for the poultry industry for years, generally due to poor feed conversion. The infection itself is subclinical and many turkeys have a light to moderate worm burden. Since parasitisms are light, this leads to the infections being noticed only during processing. Background

Although cervical cancer is the leading cancer in Cambodia, most women receive no routine screening for cervical cancer and few treatment options exist. Moreover, nothing is known regarding the prevalence of cervical HPV or the genotypes present among women in the country. Young sexually active women, especially those with multiple sex partners are at highest risk of HPV infection. We examine the prevalence and genotypes of cervical HPV, as well as the associated risk factors among young women engaged in sex work in Phnom Penh, Cambodia.

Endemic diseases of cattle, such as bovine viral diarrhea, have significant impact on production efficiency of food of animal origin with consequences for animal welfare and climate change reduction targets. Many modeling studies focus on the local scale, examining the on-farm dynamics of this infectious disease. However, insight into prevalence and control across a network of farms ultimately requires a network level approach. Here, we implement understanding of infection dynamics, gained through these detailed on-farm modeling studies, to produce a national scale model of bovine viral diarrhea virus transmission. Opportunistic fungal infections have become problematic in recent years. There has been an emergence of new fungal pathogens, development of antifungal resistance, and increasing prevalence. In addition, toxicity, resistance, and lack of efficacy as a single agent limit the effectiveness of current antifungal drugs. Therefore, new drugs targeting new pathways are greatly needed. An analog derived from the marine natural product phloeodictine, analog PHL5-34A, was identified as having strong in vitro antifungal activity. A portable, low-power water purification system was developed for use by aid workers in underdeveloped world regions. The design included activated carbon and ceramic candle filtration to 0.2 microns for bacteria and turbidity reduction followed by UV irradiation for virus destruction. Flexibility and modularity were incorporated into the system with AC, DC, and environmental power supply options as well as the facility to purify water from either surface or local utility sources. Initial prototype testing indicated the successful filtration of methylene blue dye to well above the 75% UV transmittance level required for virus destruction by the UV reactor.
