Examining physiologic responses of cerebral blood flow (CBF) for subjects in challenging situations requires development of new technology. Challenging circumstances include strenuous physical activity, intensive care, and surgical procedures involving the head, neck and thorax. Headset technology to employ transcranial Doppler (TCD), which is commercially available, fails to meet several requirements for monitoring in these situations. These requirements include stable signal acquisition, non-interference with other physiological monitors, ability to fit all head sizes including infants and children. The design and development of a novel TCD headset was performed to meet these requirements. The new device theoretically allows the user to …

Direct service workers play a central role in the quality of the long-term services and supports provided to older people and people with disabilities in home and community settings. Nationally, and at the state level, there is a critical need for more information about this workforce in order to inform workforce policy and measure improvements in workforce quality and stability over time. Maine was one of seven states to administer baseline surveys under a grant funded by the U.S. Centers for Medicare and Medicaid Service. The Effects Of River Sediment, Endosulfan, And Moderate Hypoxia On Blue Crabs (Callinectes Sapidus) From The Tidal, Freshwater James River, Laura Williams

Juvenile male blue crabs move into the tidal, freshwater James River during warmer months to feed and grow by undergoing molting. In crustaceans, growth and molting are hormonally controlled. The physiological effects of a multiple-stressor environment are determined by comparing the blue crab’s oxygen uptake after exposure to pure sand, James River sediment, or endosulfan-spiked sand. The effect of multiple stressors on molting is measured by the activity level of N-acetyl-ß-glucosaminidase (NAG), an enzyme in epidermal tissue important to molting. The oxygen uptake was decreased by exposure to James River sediment but not for exposure to endosulfan for seven days. …

Any discussion of flaws in the United States patent system inevitably turns to the system’s modern villain: non-practicing entities, known more colorfully as patent trolls. For many years, however, discussions about non-practicing entities have been long on speculation and short on data. In 2011 Congress directed the nonpartisan Government Accountability Office to study the effects of non-practicing entities on patent litigation. At the request of the GAO, we collected and coded a set of patent lawsuits filed over the past five years. This article presents our analysis of the data and its implications. Septins are a class of GTP-binding proteins conserved throughout many eukaryotes. Individual septin subunits associate with one another and assemble into heteromeric complexes that form filaments and higher-order structures in vivo. The mechanisms underlying the assembly and maintenance of higher-order structures in cells remain poorly understood. Septins in several organisms have been shown to be phosphorylated, although precisely how septin phosphorylation may be contributing to the formation of high-order septin structures is unknown. Four of the five septins expressed in the filamentous fungus, Ashbya gossypii, are phosphorylated, and we demonstrate here the diverse roles of these phosphorylation sites …

Do Damages Caps Reduce Medical Malpractice Insurance Premiums?: A Systematic Review Of Estimates And The Methods Used To Produce Them, Kathryn Zeiler, Lorian E. Hardcastle

Despite common claims made in policy debates, the theoretical connection between tort reform and medical malpractice insurance premiums is ambiguous. Simple models suggest reforms such as statutory damages caps reduce premiums. More elaborate models that account for changes in physician behavior suggest caps might increase or have no impact on premiums. A number of empirical studies have been conducted to estimate the impacts of caps on premiums, and several qualitative literature reviews have attempted to draw general conclusions from the literature. No review, however, has offered a comprehensive and systematic analysis of the full set of empirical studies. This chapter …

Normal wound healing is a complex process that occurs in overlapping phases and depends upon interactions of the patient, environment and a large number of cells, growth factors, cytokines, chemokines, and other biochemical mediators. Psychological stress has been shown to adversely affect the normal wound healing process through its impact on cellular immunity. Cellular immunity impacts wound healing through the production and regulation of many of the above biochemical mediators of wound healing. The purpose of this study is to analyze the experiences of military youth as they go through the process of parental deployment, relocation, and interacting with the stressors of consistently adjusting to new places (including making new friends, etc.). The future research recommended for this study includes sampling from a larger population, as well as from a greater number of families involved with other armed forces' experiences.

On divided roadways, cross-median fatalities are disproportionately overrepresented in fatal and serious injury crashes. Many state DOTs sought to mitigate cross-median crash risk by installing cable median barriers. Despite increased crash rates, studies evaluating crash rates before and after cable median barriers were installed indicated excellent improvement in overall safety.

Although cable median barriers are intended to prevent serious cross-median crashes, cable barriers can also contribute to serious injury and fatality crashes which would not have otherwise occurred, including penetration and rollover crashes. A window of one hour or less from start of signs and symptoms of infection to antibiotic administration (AWT) is considered good clinical practice in developed countries, but less feasible in resource poor countries. This study aimed to identify predictors of AWT and studied its association with hospital length of stay (LOS) and intensive care unit (ICU) admission. A survey of healthcare providers assessed quality of care associated with the institutional capacity and response. AWT averaged four days with patient status being the only predictor upon multivariate analysis. For predictors associated with outcomes, parental education and patient status were associated …

Political discourse is an observable, measurable, and testable manifestation of political worldviews. However, when worldviews collide, notions of truth and of lies are put to the test. The challenge for researchers is how to establish confidence in their analysis. Transgressing Sexuality: An Interdisciplinary Study Of Economic History, Anthropology, And Queer Theory, Jason Gary Damron

This interdisciplinary thesis examines the concept of sexuality through lenses provided by economic history, anthropology, and queer theory. A close reading reveals historical parallels from the late 1800s between concepts of a desiring, utility-maximizing economic subject on the one hand, and a desiring, carnally decisive sexological subject on the other. Social constructionists have persuasively argued that social and economic elites deploy the discourse of sexuality as a technique of discipline and social control in class- and gender-based struggles. Although prior scholarship discusses how contemporary ideas of sexuality reflect this origin, many anthropologists and queer theorists continue to use "sexuality" uncritically …
