Acute Paraplegia After Spinal Manipulation: A Casereport, Kao-Chih Chen, Yao-Jen Chen

This is a casereport about a 53-year-old female patient with a history of chronic back pain, suffered from complete paraplegia shortly after spinal manipulation. CT and MRI images consistently showed ossification of the ligamenta flava in conjunction with bilateral facet joints hypertrophy at the T10-11, T11-12 and L1-2 levels. Spinal cord injury with T12 paraplegia ASIA-A was thus diagnosed. Posterior laminectomy at the T10, T11, L1 and L2 levels and interbody fusion from T10 to L2 spines for fixation were performed. After rehabilitation, the patient regained some activities of daily living despite complete paraplegia. To raise public awareness, our report …

A 37-year-old female had an occlusion of right middle cerebral artery (MCA) due to traumatic internal carotid artery (ICA) dissection without significant neck injury after a traffic accident. Although this phenomenon has been documented in recent studies, it remains a relatively rare event. She presented with a left shoulder abrasion injury and mild neck pain initially, although there was no direct head injury or loss of consciousness after the traffic accident. After 12 hours, she had developed confusion, weakness over left limbs and slurred speech. Cerebral venous thrombosis (CVT) usually occurs in young adult or middle aged adult women. Although not of high incidence, it is an important cause of brain hemorrhage or infarction in pregnant women. According to previous studies, the most common symptom is headache. However, because of only headache mostly, correct diagnosis is a challenge before progression to stroke. The patient was a 30-year-old female. Except for hyperthyroidism history during last pregnancy, she did not have any special associated past history. She suffered sudden onset of left side weakness; when her term was 32 weeks. As human life expectancy has increased, osteoporosis has become a major epidemic disorder. The most common consequences of this are fractures in different parts of the body. In addition to regular follow-up tests of bone-mineral density, treatment methods include: 1. increasing the dietary intake of calcium and vitamin D, 2. maintenance of body weight, avoiding alcohol and tobacco, regular exercise, and fall prevention, 3. medications, the most widely used being bisphosphonates. Bisphosphonates can limit bone loss via the inhibition of osteoclasts. Imbalance in automatic cardiovascular function has been shown to increase the risk of ventricular arrhythmias and sudden death in patients with coronary artery disease and after myocardial infarction. The study evaluated preoperative heart rate variability (HRV) as an independent prognostic factor for hospitalized patients undergoing coronary artery bypass surgery (CABG).In this single-blind study, we enrolled 25 patients scheduled to undergo CABG. Preoperative Holter electrocardiography was used to collect linear analysis and non-linear analysis (Poincare plot analysis) data. Four patients died after the operation. The study was to evaluate the effect of pre-operative education for diaphragmatic breathing and coughing techniques in elective open heart surgery patients on the post-operative pulmonary complications, ventilator use duration and post-operative hospital days.The retrospective chart reviewarticle of the 27 elective open heart surgery patients was from March 2008 to April 2009. For decades, ultrasonography has shown a high degree of accuracy in the evaluation of soft tissue tumors. In this study, we report the condition of a 80-year-old male patient who had been suffering from left thumb pain for 2 years. The patient had initially been diagnosed with de Quervain's disease, but responded poorly to conservative treatment. An ultrasonographic examination unexpectedly identified a well-margined solid tumor with marked vascularity. Suspicion of a neurogenic tumor led to an excision surgery and finally pathology revealed a Schwannoma. Following excision, the symptoms were entirely ameliorated. People with cognitive disabilities often face difficulties when performing various activities in their daily lives. Although today's compensatory assistive technology (AT) can reduce the impact of such difficulties and enhance the quality of life in this population, universal guidelines for the appropriate application of cognitive AT have not been developed. The present study reviewarticleed and integrated information regarding models, assessments, selections, needs, and other relevant evidence. Stimulated Creation Of Quanta During Inflation And The Observable Universe, Ivan Agullo, Leonard Parker

Inflation provides a natural mechanism to account for the origin of cosmic structures. The generation of primordial inhomogeneities during inflation can be understood via the spontaneous creation of quanta from the vacuum. We show that when the corresponding stimulated creation of quanta is considered, the characteristics of the state of the universe at the onset of inflation are not diluted by the inflationary expansion and can be imprinted in the spectrum of primordial inhomogeneities. The non-gaussianities (particularly in the so-called squeezed configuration) in the cosmic microwave background and galaxy distribution can then tell us about the state of the universe …

Local Negotiation With Heterogeneous Groundwater Users, Gordon C. Rausser, Susan Stratton Sayre, Leo K. Simon

This paper assesses the political implications of intra-aquifer heterogeneity in the benefits and costs of optimal groundwater management. We use simulation modeling to predict groundwater extraction regimes under two alternative local decision-making structures and compare these structures to optimal management. Local collective action performs poorly when the intra-aquifer disparity in the potential gains is large. Moreover, large intra-aquifer disparity is generally associated with large potential gains. As a result, local collective action is unlikely to be successful in capturing the largest welfare gains. Individual subregions within a groundwater basin almost always benefit most from political structures whose outcomes diverge from …
