Left Atrial Ablation For Atrial Fibrillation: Creating The “Box Lesion” With A Bipolar Radiofrequency Device, Leonid Sternik, Hartzel V Schaff, David Luria, Michael Glikson, Alexander Kogan, Ateret Malachy, Maya First, Ehud Raanani Jan 2011

Left Atrial Ablation For Atrial Fibrillation: Creating The “Box Lesion” With A Bipolar Radiofrequency Device, Leonid Sternik, Hartzel V Schaff, David Luria, Michael Glikson, Alexander Kogan, Ateret Malachy, Maya First, Ehud Raanani

The Texas Heart Institute Journal

The maze procedure is the gold standard for the ablation of atrial fibrillation, and the “box lesion” around the pulmonary veins is the most important part of this procedure. We have created this lesion with a bipolar radiofrequency ablator, abandoning the usual use of this device (to achieve bilateral epicardial isolation of the pulmonary veins).

From March 2004 through the end of May 2010, we performed surgical ablation of atrial fibrillation in 240 patients. Of this number, 205 underwent operation by a hybrid maze technique and the remaining 35 (our study cohort) underwent the creation of a box lesion around …

Prehypertension: A Meta-Analysis Of The Epidemiology, Risk Factors, And Predictors Of Progression, Xiaofan Guo, Liling Zou, Xingang Zhang, Jue Li, Liqiang Zheng, Zhaoqing Sun, Jian Hu, Nathan D Wong, Yingxian Sun

The Texas Heart Institute Journal

We investigated the prevalence and risk factors of prehypertension, as well as the predictors of progression from prehypertension to hypertension. To do this, we performed a systematic review and meta-analysis of cross-sectional and longitudinal studies, after unrestricted searches of PubMed and The Cochrane Library through September 2010. In addition, we reviewed references, major textbooks, and review articles. Pooled prevalence, standardized mean differences, and odds ratios were estimated by using a random-effects model.

Twenty-six articles met our inclusion criteria; these included 20 cross-sectional and 6 longitudinal studies, with a total sample of 250,741 individuals. The overall pooled prevalence of prehypertension was …

Cardiac Dysfunction After Cancer Treatment, Michael S Ewer, Sandra M Swain, Daniela Cardinale, Anecita Fadol, Thomas M Suter Jan 2011

Cardiac Dysfunction After Cancer Treatment, Michael S Ewer, Sandra M Swain, Daniela Cardinale, Anecita Fadol, Thomas M Suter

The Texas Heart Institute Journal

Onco-cardiology is an evolving discipline that requires the consideration of cardiotoxicity in preclinical, clinical, and therapeutic aspects of protocol development, treatment, and surveillance of patients who have undergone interventions using cardiotoxic agents. Only then can we foster new ways to maximize survival while keeping cardiac damage within acceptable limits. It is to this end that our working together has brought us to our present understanding. In the future, we may expect onco-cardiology to play an even greater role in the care of cancer patients.

The Evaluation And Management Of Electrical Storm, Michael Eifling, Mehdi Razavi, Ali Massumi Jan 2011

The Evaluation And Management Of Electrical Storm, Michael Eifling, Mehdi Razavi, Ali Massumi

The Texas Heart Institute Journal

Electrical storm is an increasingly common and life-threatening syndrome that is defined by 3 or more sustained episodes of ventricular tachycardia, ventricular fibrillation, or appropriate shocks from an implantable cardioverter-defibrillator within 24 hours. The clinical presentation can be dramatic. Electrical storm can manifest itself during acute myocardial infarction and in patients who have structural heart disease, an implantable cardioverter-defibrillator, or an inherited arrhythmic syndrome. The presence or absence of structural heart disease and the electrocardiographic morphology of the presenting arrhythmia can provide important diagnostic clues into the mechanism of electrical storm. Electrical storm typically has a poor outcome.

The effective …

The Texas Heart Institute Journal

Takayasu arteritis is an inflammatory condition that involves the large cardiac vessels, predominantly the aorta and its main branches. It typically affects young women (age, ≤40 yr), most often Asians and Latin Americans. Herein, we describe a rare manifestation of Takayasu arteritis in a 19-year-old black Tunisian man who presented with acute inferior myocardial infarction and complete atrioventricular block after occlusion from a giant aneurysm in the right coronary artery. The coronary artery disease was associated with aneurysmal dilations in the carotid, vertebral, and right renal arteries. Medical therapy improved Thrombolysis in Myocardial Infarction flow in the area of the …

Endovascular Repair Of Aortoesophageal And Aortobronchial Fistulae, Roberto Chiesa, Andrea Kahlberg, Yamume Tshomba, Enrico M Marone, Massimiliano M Marrocco-Trischitta, Germano Melissano Jan 2011

Endovascular Repair Of Aortoesophageal And Aortobronchial Fistulae, Roberto Chiesa, Andrea Kahlberg, Yamume Tshomba, Enrico M Marone, Massimiliano M Marrocco-Trischitta, Germano Melissano

The Texas Heart Institute Journal

Aortoesophageal fistulae (AEF) and aortobronchial fistulae (ABF) are uncommon, but are in general fatal if left untreated. Conventional treatment requires surgical replacement of the thoracic aorta, along with several adjunctive procedures to repair the esophageal or tracheobronchial lesion. The mortality rates associated with open surgery reach 61% in cases of primary fistula and 78% in secondary fistula.

Thoracic endovascular aortic repair (TEVAR) has been proposed as an alternative strategy to surgical management.Although less invasive, this technique has important limitations in treating AEF and ABF, the chief of which is the high risk of graft contamination. Moreover, we still lack sufficient data …

Reflections On A Heart Surgery Career With Insights For Western-Trained Medical Specialists In Developing Countries, Sami S Kabbani Jan 2011

Reflections On A Heart Surgery Career With Insights For Western-Trained Medical Specialists In Developing Countries, Sami S Kabbani

The Texas Heart Institute Journal

Herein, I describe my experience (spanning 40 years) in helping to develop the specialty of cardiovascular surgery in Syria. Especially in the early years, the challenges were daunting. We initially performed thoracic, vascular, and closed-heart operations while dealing with inadequate facilities, bureaucratic delays, and poorly qualified personnel. After our independent surgical center was established in early 1976, we performed 1 open-heart and 1 closed-heart procedure per day. Open-heart procedures evolved from the few and simple to the multiple and complex, and we solved difficulties as they arose. Today, our cardiac surgical center occupies an entire 6-floor building. We have 12 …

Heterogeneity Of Left Ventricular Signal Characteristics In Response To Acute Vagal Stimulation During Ventricular Fibrillation In Dogs, Alireza Nazeri, Macarthur A Elayda, Lubomir Dragnev, Christopher M Frank, Jihong Qu, Valtino X Afonso, Abdi Rasekh, Mohammad Saeed, Jie Cheng, Mossaab Shuraih, Ali Massumi, Mehdi Razavi

The Texas Heart Institute Journal

Studies have shown that long-term vagal stimulation is protective against ventricular fibrillation; however, the effects of acute vagal stimulation during ventricular fibrillation in the normal heart have not been investigated. We examined the effects of acute vagal stimulation on ventricular fibrillation in a canine model. In 4 dogs, we induced 30-second periods of ventricular fibrillation by means of intraventricular pacing. During 2 of the 4 periods of fibrillation that we analyzed, vagal stimulation was delivered through electrodes in the caudal ends of the vagus nerves. Noncontact unipolar electrograms were recorded from 3 ventricular regions: the basal septum, apical septum, and …

Central Venous Pressure And Pulmonary Capillary Wedge Pressure, Tony S Ma, Biykem Bozkurt, David Paniagua, Biswajit Kar, Kumudha Ramasubbu, Carl F Rothe Jan 2011

Central Venous Pressure And Pulmonary Capillary Wedge Pressure, Tony S Ma, Biykem Bozkurt, David Paniagua, Biswajit Kar, Kumudha Ramasubbu, Carl F Rothe

The Texas Heart Institute Journal

Heart-failure phenotypes include pulmonary and systemic venous congestion. Traditional heart-failure classification systems include the Forrester hemodynamic subsets, which use 2 indices: pulmonary capillary wedge pressure (PCWP) and cardiac index. We hypothesized that changes in PCWP and central venous pressure (CVP), and in the phenotypes of heart failure, might be better evaluated by cardiovascular modeling. Therefore, we developed a lumped-parameter cardiovascular model and analyzed forms of heart failure in which the right and left ventricles failed disproportionately (discordant ventricular failure) versus equally (concordant failure). At least 10 modeling analyses were carried out to the equilibrium state. Acute discordant pump failure was …

Sirolimus-Eluting Stents Versus Bare-Metal Stents In Routine Clinical Use, Muhammad Munir, Jonathan Aliota, Amany Ahmed, Anwarullah Mohammed, Vei Vei Lee, Macarthur A Elayda, James M Wilson

The Texas Heart Institute Journal

Conflicting patient outcomes have been reported after the use of sirolimus-eluting stents or bare-metal stents. In this nonrandomized study, we examine the outcomes after placement of sirolimus-eluting versus bare-metal stents in an unselected population of patients who underwent percutaneous coronary revascularization.

We used THIRD-base, a longitudinal data registry of patients who underwent revascularization at our institution, to compare demographics and outcomes in patients treated with a sirolimus-eluting or bare-metal stent from January 2001 through June 2006. Outcome measures included major acute coronary and cerebral events at 30 days, target-vessel failure at 9 months and at 3 years, and stent thrombosis. …

Reshaping To Serve Evolving Communities: Addressing Immigrant Needs In The Library, Jen Hoyer

Publications and Research

Libraries face evolving communities and must constantly reorient themselves to serve new needs. This article examines the impact of emerging immigrant communities on library services. It discusses the emergence of library services to immigrants and the diverse faces of these services, which may include foreign language collections, access to technology and connections with local services, as appropriately determined through community analysis. The Atwater Library and Computer Centre in Montréal, Québec is examined as a case study of community analysis informing service reorientation. This article will be useful for organizations working with immigrants or community service providers confronted with changing demographics.

A New Youth Discipling Model For The Los Altos Chinese Seventh-Day Adventist Church, John Kangho Ang Jan 2011

A New Youth Discipling Model For The Los Altos Chinese Seventh-Day Adventist Church, John Kangho Ang

Doctor of Ministry Projects

Problem. The Los Altos Chinese Seventh-day Adventist (LACSDA) church has not been able to effectively disciple and retain its American-born Chinese (ABC) members. Over the years, the LACSDA church has seen a gradual withdrawal of the ABC members from the church; most have either left the area to attend other Seventh-day Adventist churches, or have stopped attending church altogether. This attrition of membership was a serious problem for the LACSDA church and proper measures were needed if the trend was to be reversed.

Method. This research project led to the development and implementation of an innovative model of ministry that …

Implicit Leadership Theory And Bass And Riggio's (2006) Transformational Leadership Model As A Means Of Leader-Renewal At The Napa Community Seventh-Day Adventist Church, John Walter Grys Jan 2011

Implicit Leadership Theory And Bass And Riggio's (2006) Transformational Leadership Model As A Means Of Leader-Renewal At The Napa Community Seventh-Day Adventist Church, John Walter Grys

Doctor of Ministry Projects

Problem

The Napa Community Seventh-day Adventist Church did not have an active, engaged, and participating leadership core throughout the previous ten years of church life. Current leaders, while demonstrating faithfulness and desire, were not actively being engaged and challenged to grow in their own leadership understanding and practice. The absence of such an ongoing process contributed to the lack of leader and leadership renewal within our faith community.

Methodology

The combined introduction of Implicit Leadership Theory (ILT) and Transformational Leadership (Bass & Riggio 2006) through an eight-week process would provide a means to inject into our community an external theory …

A Strategy For The Formation Of A Shared Vision Among The Churches Of North Dallas To Address The Challenge Of Secondary Education, Paul A. Hunt Jan 2011

A Strategy For The Formation Of A Shared Vision Among The Churches Of North Dallas To Address The Challenge Of Secondary Education, Paul A. Hunt

Doctor of Ministry Projects

Problem. For over forty years, the North Dallas area of the Texas Conference has been unable to create a viable, long-term solution for providing high school education. Without a shared vision for Christian education amongst the North Dallas churches, further decline is expected, as the present model of education is unsustainable.

Method. The development of a strategy and process for the formation of a shared, biblical vision of Christian education will be explored to address the challenge of providing a sustainable system of education.

Results. The development and execution of a strategy for the formation of a shared, biblical vision …

A Dosing Study Of Bone Marrow Mononuclear Cells For Transendocardial Injection In A Pig Model Of Chronic Ischemic Heart Disease, Guilherme V Silva, Marlos R Fernandes, Cristiano O Cardoso, Ricardo R Sanz, Edie M Oliveira, Pilar Jimenez-Quevedo, Javier Lopez, Franca S Angeli, Yi Zheng, James T Willerson, Emerson C Perin

The Texas Heart Institute Journal

We studied the effect of the dose of bone marrow mononuclear cells, delivered via transendocardial injection, upon capillary density and fibrosis in pigs with chronic ischemic heart disease.

Pigs (n = 16) that had undergone ameroid constrictor placement (left circumflex coronary artery) to induce chronic ischemia were divided equally into 4 groups on the basis of bone marrow mononuclear cell dose: control (saline injection) and 50, 100, or 200 × 106 bone marrow mononuclear cells. Thirty days after ameroid placement, each pig received 13 transendocardial NOGA-guided injections. An implantable loop recorder monitored possible arrhythmias caused by cell transplantation. Thirty days …

Behavioral Spillovers In Coordination Games, Timothy N. Cason, Anya C. Savikhin, Roman M. Sheremeta Jan 2011

Behavioral Spillovers In Coordination Games, Timothy N. Cason, Anya C. Savikhin, Roman M. Sheremeta

ESI Working Papers

Motivated by problems of coordination failure observed in weak-link games, we experimentally investigate behavioral spillovers for minimum- and median-effort coordination games. Subjects play these coordination games simultaneously and sequentially. The results show that successful coordination on the Pareto optimal equilibrium in the median game influences behavior in the minimum game when the games are played sequentially. Moreover, this positive, Pareto-improving spillover is present even when group composition changes across games, although the effect is not as strong. We also find that the precedent for uncooperative behavior in the minimum game does not influence play in the median game. These findings …

Asset Market Reactions To News: An Experimental Study, Gunduz Caginalp, David Porter, Li Hao Jan 2011

Asset Market Reactions To News: An Experimental Study, Gunduz Caginalp, David Porter, Li Hao

ESI Working Papers

An experimental asset market is used to test the effect of news concerning the underlying value of an asset on its trading price. Participants were divided into two groups and received different expected earnings values. Statistical support is found for the hypothesis that investors underreact to news on asset valuation. The results are consistent with the viewpoint that price and valuation history have a significant effect on trader behavior. Two sets of experiments involve a single asset with the same final earnings at the end of the experiment. Expected earnings are updated at the midpoint of the market trading. The …

Combinatorial Clock Auctions: Price Direction And Performance, David R. Munro, Stephen J. Rassenti Jan 2011

Combinatorial Clock Auctions: Price Direction And Performance, David R. Munro, Stephen J. Rassenti

ESI Working Papers

This paper addresses three concerns with ascending price Combinatorial Clock Auc- tions (APCA); price guidance toward e ciency relevant packages, computational bur- den, and susceptibility to collusive bidding. We propose a descending price Combi- natorial Clock Auction (DPCA) with a newly devised pricing strategy to alleviate all of these concerns. Mimicking bidding behavior of human subjects found in previous laboratory experiments, agent-based simulations of a DPCA show improvements in ef- ciency resulting from better price guidance and a reduction in computational burden when compared to an APCA.

The Effect Of Bidding Information In Ascending Auctions, Mun Chui, David Porter, Stephen Rassenti, Vernon Smith Jan 2011

The Effect Of Bidding Information In Ascending Auctions, Mun Chui, David Porter, Stephen Rassenti, Vernon Smith

ESI Working Papers

We study the effect of the drop out and reenter information in an environment where bidders‟ values involve both private and common value components. We find that (1) providing bidding information does not have a significant effect on expected revenue and expected efficiency. (2) The effect of information on winner‟s expected profit depends on the range of uncertainty of the common value component and the level of Nash profit prediction, which the auctioneer has no a priori knowledge. In our environment, where bidders have a private component to their value and the auction takes place in ascending clock format, (3) …
