QUESTION: Should patients with newly-diagnosed metastatic brain tumors undergo open surgical resection versus whole brain radiation therapy (WBRT) and/or other treatment modalities such as radiosurgery, and in what clinical settings? Target population These recommendations apply to adults with a newly diagnosed single brain metastasis amenable to surgical resection. Recommendations Surgical resection plus WBRT versus surgical resection alone Level 1 Surgical resection followed by WBRT represents a superior treatment modality, in terms of improving tumor control at the original site of the metastasis and in the brain overall, when compared to surgical resection alone. QUESTION: Should patients with newly-diagnosed metastatic brain tumors undergo stereotactic radiosurgery (SRS) compared with other treatment modalities? Target population These recommendations apply to adults with newly diagnosed solid brain metastases amenable to SRS; lesions amenable to SRS are typically defined as measuring less than 3 cm in maximum diameter and producing minimal (less than 1 cm of midline shift) mass effect. Recommendations SRS plus WBRT vs. WBRT alone Level 1 Single-dose SRS along with WBRT leads to significantly longer patient survival compared with WBRT alone for patients with single metastatic brain tumors who have a KPS > or = 70.Level 1 …

Obesity is a common problem that can lead to numerous comorbid conditions, including Type 2 diabetes. Currently, there are few pharmacologic options available to help obese patients lose weight. Pramlintide is an injectable, amylin analogue that is indicated in patients with Type 1 and Type 2 diabetes for use in conjunction with insulin to improve glycemic control. In addition to helping patients decrease hemoglobin A1c levels, pramlintide has also been shown to minimize weight gain, especially in patients with Type 2 diabetes. Obesity will increase the risk of developing serious medical conditions that may cause harm to human health and wellbeing. One condition that can be associated with obesity is known as Obstructive Sleep Apnea. Offence Definitions, Conclusive Presumptions, And Slot Machines, Michael Plaxton

Canadian evidence scholars frequently claim that conclusive presumptions are nothing more than substantive offence definitions. This position reflects a persistent confusion, not about the function of legal presumptions in the law of evidence, but about the function of offence definitions beyond the law of evidence. Offence definitions, unlike conclusive presumptions, serve the normative function of defining wrongful conduct for citizens. This commentary argues that the language of conclusive presumptions allows us to distinguish the gravamen of a criminal offence from a means of facilitating proof of that wrong. In Latin America, like many countries in Europe, hate speech is prohibited. Yet Latin America is rarely included in the transnational discussion regarding the regulation of hate speech. Instead, the discourse focuses on a comparison of the advisability of Europe's hate speech regulations and free speech acceptance of hate speech in the United States. As a result, the ability to fundamentally examine the connections between hate speech and inequality, in addition to the most effective legal mechanisms for addressing it, is undermined. This Commentary Article aims to illustrate the value of comparative law to the jurisprudence of Critical Race Theory (CRT), particularly with reference to the CRT project of deconstructing the mystique of "postracialism. " The central thesis of the Article is that the dangerous seductions of a U.S. ideology of "post-racialism" are more clearly identified when subject to the comparative law lens. In particular, a comparison to the Brazilian racial democracy version of "post-racialism"is an instructive platform from which to assess the advisability of promoting post-racial analyses of U.S. racial inequality. The Readiness for Hospital Discharge Scale, Quality of Discharge Teaching Scale, and Post-Discharge Coping Difficulty Scale were developed by Dr. Marianne Weiss to measure nurse-sensitive discharge care processes and patient outcomes. Three versions of the scales were developed concurrently for different patient populations – Adult medical-surgical patients, Parents of hospitalized children, and Postpartum Patient mothers (see Weiss & Piacentine, 2006, Weiss et al., 2007, and Bobay et al., 2018 for description of the development and initial testing). We argue that a spending tax, as opposed to an income or wage tax, is the “last best hope” for a return to significantly more progressive marginal tax rates than obtain today. The simple explanation for this central claim looks to incentive effects, especially for “rich people,” as both economists and commentators are inclined to focus. High marginal tax rates under an income tax fall on and hence deter the socially productive activities of work and savings. In their article “The Uneasy Case for Product Liability,” Professors Polinsky and Shavell assert the extraordinary claim that there should be no tort liability - none at all - for injuries caused by widely-sold products. In particular, they claim to have found convincing evidence that the threat of tort liability creates no additional incentives to safety beyond those already provided by regulatory agencies and market forces, and that tort compensation adds little or no benefit to injury victims beyond the compensation already provided by various forms of insurance. The article explores the last 10 years of literature relating to Davidoff and Florance’s informationist concept. An informationist provides many of the same services as a medical librarian, is a permanent member of the clinical team, and resides on the clinical floors. The author explores the job functions of the informationist and examines whether this service has come to fruition. The author argues that the informationist service can only be fully realized in large academic health sciences libraries, large teaching hospitals, and medical research organizations such as the National Institutes of Health. Why has the NLRA been so resistant to legislative change for more than 60 years? How was Congress able to enact two major labor relations laws within a 12-year period (1935 and 1947) but then unable to approve proposed reforms in the years since 1947? In an effort to answer these questions, the article closely examines contemporaneous newspaper accounts from the 1935 and 1947 legislative “successes” as well as from two more recent congressional “failures” in 1978 and 1992. Biodiversity surveys were conducted on Saba Bank, Netherlands Antilles, to assess ichthyofaunal richness and to compare with published surveys of other Caribbean localities. The primary objective was to estimate the total species richness of the Saba Bank ichthyofauna. A variety of sampling techniques was utilized to survey the fish species of both the visually accessible megafauna and the camouflaged and small-sized species comprising the cryptic ichthyofauna. Based on results presented herein, the number of species known on Saba Bank is increased from 42 previously known species to 270 species. This qualitative study examines the experiences of 18 new child protection workers in Southern Ontario. The workers, who had 5 to 18 months experience in child welfare, were interviewed regarding their experiences of joining a child welfare agency. A follow up focus group was conducted with child protection supervisors. The study examined what motivated workers to join, their training experience, the rewards and supports and the overwhelming nature of the experience. The study illuminated the struggles that new workers experience in child welfare with respect to value and belief challenges. The study reflects on the implications of new worker experiences …

Constructed wetlands (CWs) are complex treatment environments, requiring an integrative research approach to improve our understanding of them. The goal of this thesis was to establish an understanding of the functional and structural characteristics of microbial communities within bench-, field- and industrial-scale environmental treatment systems. The impact of pathogenic and/or antibiotic contaminants on these communities based on their functional and structural profiles using community-level physiological profiling (CLPP) and denaturing-gradient gel electrophoresis (DGGE), respectively, was investigated. Under normal operation, bench-, field- and industrial-scale treatment systems were able to produce similarly behaving structural and functional profiles. This qualitative study aimed to understand what the body stories of girls who exhibit signs of disordered eating reveal about the concepts of autonomy and connection during early adolescence. The study was guided by the research question “Are the symptoms of disordered eating one of the ways the female body “talks” about the experience of disconnection during adolescence?” Informed by Relational Cultural Theory, data was collected from two focus groups of 16 adolescent girls aged 11 to 14 years. This study considers the industrial development and subsequent decline of the town of Indiana, Ontario, during the years 1830–1900, a period of intense socioeconomic change and population mobility. This dissertation applies interdisciplinary frameworks, especially those derived from archaeological inquiry, in order to assess the documentary evidence and also the material culture of nineteenth-century Indiana, in the interests of understanding both the historic process of rural industrialization by means of a case study and also the elusive processes of social and familial interaction in the Ontario towns caught up by the swirl of socioeconomic change during this period.
