Patients Evaluated For Liver Transplant: Transplant List Denial And Subsequent Outcomes, Kara Elam May 2009

Patients Evaluated For Liver Transplant: Transplant List Denial And Subsequent Outcomes, Kara Elam

Theses and Dissertations

Background: The evaluation process for listing a patient on the liver transplant list is complicated and involves multiple consultations from various specialists, as well as extensive imaging and physiological studies. Although there are data on the outcomes of those listed, we know little about those that are denied listing. This research project will identify the reasons for liver transplant listing denial and predictors of death following denial for this challenging group of patients. Methods: Data from all patients (n=1,500) evaluated for a liver transplant from 1997 to 2007 by the Department of Gastroenterology, Hepatology, and Nutrition located at Virginia Commonwealth …

Active Site Chemistry Of The Nadph-Dependent:7-Cyano-7-Deazaguanine (Preq0) Nitrile Oxidoreductase, An Enzyme Involved In Queuosine Biosynthesis, Bobby Wai Keung Lee

Dissertations and Theses

Queuosine (Q) is a modified nucleoside found at the wobble position of bacterial and eukaryotic transfer RNAs (tRNAs) that are specific for the amino acids tyrosine, histidine, aspartate and asparagine. A recently discovered enzyme in the biosynthetic pathway of Q, the NADPH-dependent 7-cyano-7-deazaguanine oxidoreductase (QueF), carries out the two-fold, four-electron reduction of Q precursor preQ0 to preQ1 and represents the first example of the enzymatic conversion of the nitrile functional group to an amine. Presented herein are kinetic, spectroscopic, mutational, biophysical, and isotope labeling studies directed at the elucidation of the chemical and kinetic mechanisms of this new class of …

Stress, Serotonin Transporter Genotype, And Emotion Processing In Children, Amy Meadows May 2009

Stress, Serotonin Transporter Genotype, And Emotion Processing In Children, Amy Meadows

Yale Medicine Thesis Digital Library

Child maltreatment is a significant contributor to psychopathology, including depressive disorders and Post-Traumatic Stress Disorder (PTSD). Multiple studies have demonstrated important changes in emotion processing and regulation in children exposed to early life stress, but the underlying neural mechanism is unknown. There is significant variability in outcome in children exposed to early life stress which may be moderated by genetic polymorphisms in the serotonin transporter. This study was designed as a functional Magnetic Resonance Imaging project using dichotic listening to study emotion-processing pathways and interhemispheric transfer. Eighteen children were recruited in a two-by-two factorial design with maltreatment and serotonin genotype …

Feminism, Globalism, And Leadership: How Women Make A "Worldly" World, Scott C. Hammond, Susan R. Madsen, Jory Jensen May 2009

Feminism, Globalism, And Leadership: How Women Make A "Worldly" World, Scott C. Hammond, Susan R. Madsen, Jory Jensen

Marketing and Strategy Faculty Publications

The influx of women into leadership, management, and idea leader roles is clearly one of the significant aspects of globalization over the last 50 years. While women are still underrepresented in global business and politics, their influence on reconceptualizing leadership models is undeniable. While it would be difficult to precisely determine a “feminist” perspective on leadership, idea leaders such as Follett, Kanter, and Wheatley have influenced the concept and practice of leadership beyond any gender construct. The unique themes within their work are present in the global dialogue that is reframing how we think about leadership. This paper will discuss …

Radiation Is An Important Component Of Multimodality Therapy For Pediatric Supratentorial Non-Pineal Neuroectodermal Tumors, Sean Mcbride May 2009

Radiation Is An Important Component Of Multimodality Therapy For Pediatric Supratentorial Non-Pineal Neuroectodermal Tumors, Sean Mcbride

Yale Medicine Thesis Digital Library

Purpose: We reviewed a historical cohort of pediatric patients with supratentorial primitive neuroectodermal tumors (sPNET) in order to clarify the role of radiation in the treatment of these tumors. Patients and Methods: Fifteen children <18 years old with non-pineal sPNETs diagnosed between 1992 and 2006 were identified. Initial therapy consisted of surgical resection and chemotherapy (CT) in all patients and up-front radiotherapy (RT) in 5 patients. Five patients had RT at the time of progression and five received no RT whatsoever. Kaplan-Meier estimates of overall-survival (OS) were then calculated. Results: The median follow-up from diagnosis for all patients was 31 months (range 0.5-165) and for surviving patients was 49 months (range 10-165). Of the 5 patients who received up-front RT, all were alive without evidence of disease at a median follow-up of 50 months (range 25-165). Only 5 of the 10 patients who did not receive up-front RT were alive at last follow-up. There was a statistically significant difference in overall survival between the group of patients that received up-front RT and the group that did not (P=0.048). Additionally, we found a trend toward a statistically significant improvement in overall-survival for those patients that received gross total resections (P=0.10). Conclusions: Up-front radiotherapy and gross total resection may confer a survival benefit in patients with sPNET. Local failure was the dominant pattern of recurrence. Efforts should be made to determine patients most likely to have local failure exclusively or as a first recurrence in order to delay or eliminate cranio-spinal irradiation (CSI).

Testis-Expressed Profilins 3 And 4 Show Distinct Functional Characteristics And Localize In The Acroplaxome-Manchette Complex In Spermatids, Martina Behnen, Kai Murk, Petri Kursula, Heike Cappallo-Obermann, Martin Rothkegel, Abraham L. Kierszenbaum, Christiane Kirchhoff

Publications and Research

Background Multiple profilin isoforms exist in mammals; at least four are expressed in the mammalian testis. The testis-specific isoforms profilin-3 (PFN3) and profilin-4 (PFN4) may have specialized roles in spermatogenic cells which are distinct from known functions fulfilled by the "somatic" profilins, profilin-1 (PFN1) and profilin-2 (PFN2). Results Ligand interactions and spatial distributions of PFN3 and PFN4 were compared by biochemical, molecular and immunological methods; PFN1 and PFN2 were employed as controls. β-actin, phosphoinositides, poly-L-proline and mDia3, but not VASP, were confirmed as in vitro interaction partners of PFN3. In parallel experiments, PFN4 bound to selected phosphoinositides but not to …

Anxiety And Birth Order: Does Birth Order Play A Role In A Child's Anxiety Level?, Tarah R. Pearson

Theses and Dissertations

The purpose of this study was to see if there is an interaction between anxiety levels and birth order. It was hypothesized that the first-born child would have a higher level of total anxiety than a non-first born child. It was also hypothesized that females would have an overall higher level of anxiety than males. The Revised Children's Manifest Anxiety Scale 2nd Edition, which was used to measure the different subtests of anxiety, tested for specific types of anxiety: total anxiety, worrying, social anxiety, physiological anxiety, and defensiveness. It was hypothesized that females would feel a higher level of …

Diminished Circulating Monocytes After Peripheral Bypass Surgery For Critical Limb Ischemia, Dania Magri May 2009

Diminished Circulating Monocytes After Peripheral Bypass Surgery For Critical Limb Ischemia, Dania Magri

Yale Medicine Thesis Digital Library

Introduction: Mononuclear cells (MNCs) have been shown to increase neovascularization and ulcer healing after direct injection into the ischemic limbs of patients with unreconstructable peripheral vascular disease (PVD). Circulating MNCs are composed of lymphocytes (85%), monocytes (15%) and endothelial progenitor cells (EPCs; 0.03%). It is thought that MNCs may be effective in ameliorating ischemia since EPCs are a component of the monocyte fraction, and EPCs have been shown to participate in vascular healing. We hypothesized that ischemic areas secrete paracrine signals such as cytokines and growth factors that recruit bone marrow-derived monocytes into the circulation in order to augment vascular …

Childhood Temperament And Its Effects On Adjustment To College, Ashley L. Prim

Theses and Dissertations

The purpose of this study was to determine whether temperaments seen in early childhood would have an effect on adjustment throughout life, especially during adjustment to college. Specifically, the researcher hypothesized that children with easy temperaments would have an easier time adjusting to college; whereas, children with slow-to-warm-up or difficult temperaments would have a more difficult time adjusting to college. A self-created survey was distributed to 30 undergraduate psychology students at Rowan University. The survey consisted of temperament and adjustment measures. Correlations were run using SPSS in order to determine the effects of temperament on adjustment to college. While the …

Development And Investigation Of Intensity-Modulated Radiation Therapy Treatment Planning For Four-Dimensional Anatomy, Yelin Suh May 2009

Development And Investigation Of Intensity-Modulated Radiation Therapy Treatment Planning For Four-Dimensional Anatomy, Yelin Suh

Theses and Dissertations

Lung cancer is the leading cause of cancer-related deaths worldwide. Radiotherapy is one of the main treatment modalities of lung cancer. However, the achievable accuracy of radiotherapy treatment is limited for lung-based tumors due to respiratory motion. Four-dimensional radiotherapy explicitly accounts for anatomic motion by characterizing the motion, creating a treatment plan that accounts for this motion, and delivering this plan to the moving anatomy. This thesis focuses on the current problems and solutions throughout the course of four-dimensional radiotherapy. For characterization of respiratory-induced motion, patient tumor motion data were analyzed. It is shown that tumor motion can be significant …

Tracer Studies Of Sheet Flow In The Florida Everglades, David T. Ho, Victor C. Engel, Evan A. Variano, Paul J. Schmieder, Madison Condon

Faculty Scholarship

Information on sheet flow patterns in the marsh ridge and slough habitat of the Florida Everglades is scarce, primarily because of difficulty in taking measurements across large enough spatial scales in such a heterogeneous environment. As part of the Everglades Tracer Release Experiment (EverTREx), two SF6 tracer releases were conducted to measure sheet flow in relatively intact ridge and slough habitats. The first was a pilot experiment that allowed the analytical equipment to be tested in the Everglades, and yielded some preliminary, coarse-scale measurements of advection and dispersion in multiple sloughs. In the second experiment, higher-resolution measurements of tracer distributions …
