Managed Pressure Drilling (MPD) is an emerging drilling technology that utilizes mud weight, surface backpressure and annular frictional pressure loss (AFP) to precisely control the wellbore pressure. The goal of this project is to identify the most appropriate initial response and kick circulation method for the kicks that result from complications specific to MPD. These complications that can cause a reduction in bottomhole pressure were classified as surface equipment failures and unintended equivalent circulating density (ECD) reductions. Well Leg Compartment Syndrome: The Debit Side Of Hemilithotomy Position, Shahryar Noordin, Salim Allana, Muhammad Abdul Wajid

We describe a case of well leg compartment syndrome following unilateral lithotomy position in a patient undergoing contralateral antegrade intramedullary femoral nailing. Following two-incision four compartment fasciotomy, the patient recovered with no residual defects. We dissuade use of the hemilithotomy position for antegrade femoral nailing procedures in favour of another previously described position, because this predisposes patients to position-associated complications without offering any substantial advantage.

Inadequate access to healthy food sources may contribute to less nutritious diets and an increased risk for chronic disease. Numerous studies on nutrition environments and food access have found disparities between low income neighborhoods and higher income neighborhoods in regards to access to supermarkets and healthier foods. The purpose of this study was to examine the retail food environment, access to food, and food security in Missoula, Montana in relation to the socioeconomic and health status of its residents.

Arbuscular mycorrhizal fungi (AMF) are soil fungi forming symbiotic associations with majority of land plants. AMF alter soil organic matter (SOM) directly through stabilization of soil aggregates and indirectly providing a path in which plant fixed C02 is transferred below-ground. Understanding contributions of AMF to SOM via protein production and stabilization of soil aggregates will greatly aid our understanding of soil carbon sequestration, nutrient cycling and mitigation of soil erosion. The work presented in chapter 2 challenges the glomalin extraction process and assesses the accuracy of the Bradford and monoclonal-antibody ELISA detection methods. OBJECTIVE: Presence of autonomic neuropathy in diabetic patients can alter the haemodynamic response to induction and tracheal intubation. This trial was conducted to compare this response in 30 non-diabetic (control group) and 30 diabetic patients including both insulin and non-insulin dependent.

METHODS: A prospective, age matched case controlled study was conducted at the Aga Khan University Hospital, for one year. After pre-medication with tablet midazolam 7.5 mgs orally, patients received pethidine 0.8 mg kg(-1), thiopentone sodium 4 to 5 mg kg(-1) for induction and vecuronium 0.1 mg kg(-1) to facilitate tracheal intubation. The question of Taiwan’s status has faced legal challenges from the one- China policy under both domestic law and international law. The article argues that the state status of the Republic of China (ROC) on Taiwan has never ceased to exist as a result of either the loss of diplomatic recognition or the United Nations Resolution 2758, which transferred the UN seat from the ROC to the People’s Republic of China (PRC). Burrell V. Armijo: The Role Of Comity In Federal Court Review Of Tribal Court Judgments, Erin Garcia

This article discusses the on-going issue of tribal versus federal judicial power. While tribal courts have been recognized as an important aspect of tribal sovereignty, federal precedent has diluted tribal authority. This dilution of authority is evident by the restriction on jurisdiction given to the tribal courts by the federal government. The author explores this issue using the case of Burrell v. Armijo. She contends that this Tenth Circuit case demonstrates the continued undermining of tribal jurisdiction and that the general rule for federal courts to give great deference to tribal courts was not properly applied, leading to continued weakening …

In the United States similar arguments are made concerning the historic maltreatment meted out to African Americans. The study focused on how narratives told among social network participants can create a “culture of encouragement” surrounding risky sexual short-term relationships. The study adopted a social network approach to examine the “hookup” culture and risky sexual behavior. Specifically, this study examines who students are talking to, how frequently they discuss sexual experiences, and the specific accounts they discuss in connection with the hookup culture. Hematologic Effects Of Placental Pathology On Very Low Birthweight Infants Born To Mothers With Preeclampsia., Kelly J. Zook, Md, Amy B. Mackley, Rnc, Jennifer Kern, David A. Paul. Md

OBJECTIVE: To investigate the effect of placental pathology on neonatal neutrophils, platelets, hematocrit and nucleated red blood cells in very low birthweight (VLBW) infants born to mothers with preeclampsia. STUDY DESIGN: Retrospective cohort study of infants with birthweight < 1500 g born to mothers with preeclampsia from july, 2002 to july, 2006 at a single level III neonatal intensive care unit. Placental pathology was reviewed for the presence of placental infarction and vasculopathy. Hematologic parameters from day of life 0, 1 and 2 were obtained. Statistical analysis included repeated-measures analysis of variance and multivariable analysis using logistic regression. RESULT: The study sample included 203 infants with estimated gestational age of 28+/-3 weeks; 45% had placental infarctions and 26% placental vasculopathy. Infants with neutropenia and thrombocytopenia did not have an increased occurrence of placental infarction or maternal vasculopathy but were more likely to be of small gestational age (SGA) and of lower gestational age compared with infants without neutropenia or thrombocytopenia. After multivariable analysis, gestational age and SGA remained associated with both neutropenia and thrombocytopenia whereas placental infarction and vasculopathy did not remain in the models. CONCLUSION: In our population of VLBW infants born to mothers with preeclampsia, placental pathology was common. There was no association of placental infarction or vasculopathy with neonatal neutropenia and thrombocytopenia. The data suggest that neonatal hematologic effects of maternal preeclampsia, if related to the placenta, are associated with factors other than placental histology.

Autism spectrum disorders have been associated with maternally derived duplications that involve the imprinted region on the proximal long arm of chromosome 15. Here we describe a boy with a chromosome 15 duplication arising from a 3:1 segregation error of a paternally derived translocation between chromosome 15q13.2 and chromosome 9q34.12, which led to trisomy of chromosome 15pter-q13.2 and 9q34.12-qter. Using array comparative genome hybridization, we localized the breakpoints on both chromosomes and sequence homology suggests that the translocation arose from non-allelic homologous recombination involving the low copy repeats on chromosome 15. Objective: To determine the appropriate duration of prolonged monitoring following decelerations noted on outpatient non-stress test (NST).

Conclusion: Prolonged monitoring beyond 120 min is not associated with increased rate of delivery secondary to NRFS or worse neonatal outcome measured by the 5 minute Apgar score. Monitoring after a deceleration on outpatient NST may be limited to less than 120 minutes. Larger studies are needed to confirm this finding.

Objective: To estimate whether the use of 17 alpha-hydroxyprogesterone caproate (17P) in women with an ultrasound-indicated cerclage (UIC) reduces the rate of preterm birth (PTB).

Conclusions: Among women with a prior spontaneous PTB and current UIC for CL<25mm, 17P did not reduce the rate of PTB <35 weeks.

Objective: To estimate if cervical biopsy at the time of colposcopy changes the management or outcome of abnormal cervical cytology during pregnancy.

Conclusion: Biopsy of suspected cervical lesions can be safely deferred until after pregnancy because there were no significant changes between antepartum and postpartum CC and no missed cases of cervical cancer were detected.

The purpose of this study was to examine the effect of anxiety on the intellectual functioning of children. Specifically, the current researchers sought to replicate previous findings that children with higher levels of anxiety have significantly lower scores on tests of intelligence. A second goal was to examine possible reasons for these deficits, including possible deficits in working memory and/or attention. Participants were divided into two groups with high and low anxiety, based on a self-report measure, though none of the children reported clinically problematic anxiety. The rapid pace of U.S. wind energy development is generating a growing number of conflicts over competing wind rights. The “wake” of a commercial wind turbine creates turbulence and unsteady wind flow that can reduce the productivity of other wind turbines situated downwind. Existing law is unclear as to whether a landowner who installs a wind turbine on its property is liable for the lost productivity of a downwind neighbor’s turbine resulting from such wake effects. In this thesis, I examine Elizabeth Gaskell’s development as a middle-class author, which is a position that most scholars take for granted. Moving away from traditional Marxist readings and drawing on revisionist class studies, I reconsider Gaskell as the typical bourgeois woman of her era by looking at her relationship with the middle class and its ideals over the course of her career. Overall, her large body of work reveals an increasing awareness of, and willingness to engage with the divisions within the middle class. In the African American experience of the twentieth century, cultural modes of expression became the primary outlet for a politics of resistance, a politics of fulfillment, and a politics of transfiguration within the context of a modem Western nation-state that marginalized and discriminated against the African American subject through ideology and cultural hegemony. The prevalence of psychiatric symptoms in next-of-kin (NOK) of intensive care unit (ICU) patients has been reported at higher than 70% when screening is performed using the Hospital Anxiety and Depression Scale (HADS). The primary purpose of this study was to assess the ability of the HADS to predict psychiatric illness, diagnosed with the aide of a validated tool, the Structured Clinical Interview for DSM-IV (SCID). In addition, we asked NOK to rate aspects of the ICU experience to determine possible associations with psychiatric diagnosis. The role of the egg is 2-fold - protection of the contents and conditioning the incubation. We used a variety of search engines to collate literature pertinent to the ostrich egg from PubMed, Medline, and Agricola. An ostrich egg weighs ca. 1.5 kg, and the mean length and width are 156 and 129 mm, respectively. The vertical and horizontal circumferences reach a maximum of 45 and 40 cm, respectively. The possible role of Brucella spp. in 65 abortion cases, 55 from sheep and 10 from goats, occurring in the birth seasons of 2004 and 2005 in northwestern Turkey was investigated. Colony morphology, agglutination by acriflavin, H2S production, CO2 requirement, dye sensitivity in thionin, basic fuchsin, growth characteristics in streptomycin, lysis with Tbilisi phage, and agglutination with monospecific A- and M antisera were examined for identification and biotyping. The isolates from 21 of 55 sheep and 1 of 10 goat abortion cases were identified as Brucella melitensis. Biotyping revealed that 14, 6, and 1 of 22 Brucella spp. were B. …
