To improve the quality and safety of health care in the United States, the Institute of Medicine identified the need for strong organizational leadership and an organizational culture that supports learning. The paper will review the application of the adaptive leadership model to support organizational culture change in health care. A review of the literature on health care organizational culture and the Adaptive Leadership model will be presented along with a case study of 10 health care organizations that have been working on culture change through the Institute of Clinical Systems Improvement. The intent of this field project was to study the disconnection that is experienced by homeless persons within the healthcare system. Homelessness is a growing concern that affects all age groups throughout the United States. It is not a secret that the increase in homelessness is stressing the health care system and limiting choices of becoming a healthy individual. The state of the health care system today and the nomadic habits of homeless persons make it impossible for those struggling with homelessness to stay connected to health care services. The beneficial effects of religion and spiritual factors on cardiovascular functioning have become an area of increasing research. Similarly, considerable research has also investigated the negative effects of marital distress on cardiovascular functioning. Little is known, however, about potential protective factors against the deleterious effects of marital distress on cardiovascular functioning. Social support has been identified as a potential buffer against the negative effects of marital distress on ambulatory blood pressure, and the current study hypothesized that spirituality, religiosity, and religious support would also buffer the negative effects of marital distress on ambulatory blood pressure. Phytoestrogens are chemicals produced by plants that act like estrogens and have the ability to bind to the mammalian estrogen receptor system. The purpose of this study is to evaluate a new phytoestrogen analog called 4-acetoxy Resveratrol. Resveratrol is a phytoestrogen that has been found in the skin of grapes. Resveratrol has been shown to be able to bind to the estrogen receptors and has a similar molecular structure as estradiol. Resveratrol has been shown to have many positive health benefits such as improving cardiovascular health, serving as a neuroprotective agent, acting as an anti-inflammatory agent, working as an anti-cancer …

We reported previously that low concentrations of sodium citrate strongly promote biofilm formation by Staphylococcus aureus laboratory strains and clinical isolates. Here, we show that citrate promotes biofilm formation via stimulating both cell-to-surface and cell-to-cell interactions. Citrate-stimulated biofilm formation is independent of the ica locus, and in fact, citrate represses polysaccharide adhesin production. We show that fibronectin binding proteins FnbA and FnbB and the global regulator SarA, which positively regulates fnbA and fnbB gene expression, are required for citrate's positive effects on biofilm formation, and citrate also stimulates fnbA and fnbB gene expression. Objectives: To create an evidence‐based research tool to inform and guide policy and program managers as they develop and deploy new service delivery models for oral disease prevention and intervention. Methods: A village‐level discrete event simulation was developed to project outcomes associated with different service delivery patterns. Evidence‐ based outcomes were associated with dental health aide activities, and projected indicators (DMFT, F+ST, T‐health, SiC, CPI, ECC) were proxy for oral health outcomes. Model runs representing the planned program implementation, a more intensive staffing scenario, and a more robust prevention scenario, generated 20‐year projections of clinical indicators; graphs and tallies were …

Background: Worldwide, the prevalence of obesity among children has increased dramatically. Although the etiology of childhood obesity is multifactorial, to date, most preventive interventions have focused on school-aged children in school settings and have met with limited success. In this review, we focus on another set of influences that impact the development of children's eating and weight status: parenting and feeding styles and practices. Equol is known to be a selective androgen modulator and has the ability to bind and inhibit 5-alpha dihydrotestosterone (5α-DHT). Equol is also a selective estrogen receptor modulator and is able to bind beta estrogen receptors with high affinity. As estrogen receptors are found in the hypothalamus, pituitary, and gonads, prenatally administered equol could affect the morphological and reproductive development of offspring. To test this hypothesis, during gestational days 14 to 20, forty-two pregnant Long-Evans rats were given one of six treatments: 1) no treatment, 2) injection with dimethyl sulfoxide (DMSO), 3) injection with 10 mg/kg equol, 4) injection with …

This paper offers a broad overview of the issues surrounding the social and environmental determinants of children’s health. These issues were explored during a discussion convened by the National Health Policy Forum on June 28, 2007, among a group of individuals concerned about the influences beyond medical care on the health of children. The paper considers the policy and financing tensions that exist across programs and populations that make addressing the full range of influences challenging. It also highlights some of the community-based initiatives that have been successful in providing services to children and families, as described during the workshop. …

This report presents findings from a telephone survey of children currently enrolled in or recently disenrolled from MaineCare, the State‘s Medicaid and State Children‘s Health Insurance Program (SCHIP). The sample was randomly selected, and stratified to include children enrolled in MaineCare through the Medicaid eligibility category, and through two SCHIP eligibility categories, Medicaid Expansion and the Separate Child Health Program (CHP). 1 These three eligibility categories include children ages 18 or under living in households with income up to 200% of the Federal Poverty Level. Income eligibility limits are lowest for the Medicaid eligibility category, followed by the Medicaid Expansion …
