Consumer direction, a model of long-term care service delivery where service recipients and their families/advocates have enhanced control and choice over the services that they receive, is a growing phenomenon in United States. As it becomes greater utilized, it is important to understand the model and study its impacts. This dissertation details the history of the consumer direction movement, describes the current landscape of consumer direction in the United States, and presents a comparison study of users of consumer-directed (CD) services in three of Virginia Home and Community Based Services (HCBS) Medicaid Waivers. Adaptive Management Report For The Clark County, Nevada Multiple Species Habitat Conservation Plan, Desert Conservation Program

The Desert Conservation Program administers an incidental take permit for desert tortoise and 77 other species. The permit was issued in 2001 by the United States Fish and Wildlife Service in accordance with the Endangered Species Act. Compliance with the permit requires implementation of the Clark County Multiple Species Habitat Conservation Plan. This plan requires an adaptive management program and a biennial report by an independent science advisor to assess the status of the plan and make recommendations for future funding. Chemistry Faculty Publications and Presentations

Linear polymers of PARACEST agents were prepared by using classical free radical chain polymerization conditions. The Eu3+-polymers exhibited similar intermediate-to-slow water exchange and CEST characteristics as the Eu3+-monomers. This provided an avenue to lower the detection limit of these imaging agents substantially and makes them potentially useful as MRI sensors for molecular imaging.

Impact Of Seated And Standing Bicycle Riding Position On Subsequent Running Performance, Randall L. Jensen‡, Brendon J. Ewers†, Brian Cunniffe*, Brian Phelan*, Andrew J. Harrison‡, Amir Shafat‡

International Journal of Exercise Science

This study examined the effects of cycling posture on subsequent running performance similar to the transition phase of a triathlon. Experienced, non-elite triathletes completed two trials of a cycle-run transition. During the last three minutes of a 30 minute cycling bout, at power output equal to lactate threshold, subjects either remained seated (SEAT), or alternated seated and standing cycling (30 s at a time; ALT). Latent class analysis (LCA) and latent class regression (LCR) are widely used for modeling multivariate categorical outcomes in social sciences and biomedical studies. Standard analyses assume data of different respondents to be mutually independent, excluding application of the methods to familial and other designs in which participants are clustered. In this paper, we develop multilevel latent class model, in which subpopulation mixing probabilities are treated as random effects that vary among clusters according to a common Dirichlet distribution. We apply the Expectation-Maximization (EM) algorithm for model fitting by maximum likelihood (ML). This approach works well, but is computationally intensive when …

Medicare Advantage's Private Fee-For-Service Plans: Paying For Coordinated Care Without The Coordination, Brian Biles, Emily Adrion, Stuart Guterman

Like the private managed care plans offered under Medicare Advantage, private fee-for-service (PFFS) plans are paid more per beneficiary than those individuals would be expected to cost if they were enrolled in traditional fee-for-service Medicare. However, PFFS plans are not required to provide the same type of coordinated care required of Medicare Advantage plans. Payments to PFFS plans in 2008 average 16.6 percent more than costs in traditional Medicare, or $1,248 for each of the 2 million enrollees in PFFS plans—a total of nearly $2.5 billion in extra payments.
