Reactive oxygen species may play a role in the mediation of acetaminophen (APAP) hepatotoxicity. N-acetylcysteine (NAC) is the standard therapy for treatment of the APAP overdose patient. The suspected mechanism of hepatic injury appears to be similar to that of APAP and the empiric use of NAC may be justified, especially in significant endosulfan poisoning. An 18-year-old male presented to the emergency department because of unconsciousness and seizure that developed after acute intentional ingestion of endosulfan. Severe diaphoresis was noted and he had a Glasgow Coma Score of 7 (E1M4V2). Within 8 hours of his arrival in the emergency department, …

Purtscher´S Retinopathy: An Unusual Association With A Complicated Mandibular Fracture, Tongabay Cumurcu, Hüseyi̇n Yardim, Mehmet Güven, Raşi̇t Kiliç, Ni̇lüfer Özkan

We present a case of unilateral traumatic retinal angiopathy following a mandibular fracture. An 11-year-old child involved in a bicycle accident developed Purtscher´s retinopathy in the left eye with retinal hemorrhages, cotton wool spots, and optic disc swelling on funduscopy. The visual acuity in this eye was counting fingers at one meter.

Generalized Choreoathetosis In An Acquired Immune Deficiency Syndrome Patient With Cerebral Toxoplasmosis, İpek Mi̇di̇, Hari̇ka G. Çalişkan, Zafer Toktaş, Aydin Sav, Di̇lek İ. Günal

Many hypokinetic and hyperkinetic movement disorders (e.g. parkinsonism, chorea, myoclonus, dystonia) are associated with acquired immune deficiency syndrome (AIDS) and may sometimes represent the initial manifestation of the illness. We described a 37-year-old human immunodeficiency virus (HIV)-infected patient with generalized choreoathetoid movements associated with AIDS. Cranial MRI revealed multiple cerebral and cerebellar abscesses. The diagnosis of toxoplasmosis was established by brain biopsy. Hemichorea and hemiballismus in AIDS patients are pathognomonic findings of cerebral toxoplasmosis. Generalized choreiform movement is a rare condition and related to the bilateral cerebral involvement in toxoplasma disease affecting the subcortical structures.

The tenth anniversary of Michael Denning's The Cultural Front: The Laboring of American Culture in the Twentieth Century is a fitting occasion to revisit a work posing questions that remain fresh today and probably will as long as there is a need to analyze the relationship between dissident cultural movements and a non-egalitarian social order. Encyclopedic in scope, the book is a study of the "cultural production" of the Popular Front of the 1930s and 40s that moves eruditely through a wide range of objects, including proletarian literature, popular music, theater, film, cartoons, and cultural theory. Although contemporary representations of interracial romance may appear, on the surface, progressive, they share elements of what Patricia Hill Collins has called "past in present" racism; that is, although the films no longer portray interracial romance as taboo, their presentations obscure deeper messages that perpetuate traditional views of interracial romance, race, and gender. This article analyzes a classic interracial romance film, Guess Who's Coming to Dinner, and two contemporary films, Guess Who and Something New, in order to explore traditional ideologies inherent within these seemingly progressive representations. Country music, in its lyrics, its image and its themes, attempts to reflect the interests and experiences of the US white working class. Centred on the figure of the cowboy, this musical genre frequently draws upon influences from music originating in other communities, yet this process is "whitewashed" from the finished product. This encapsulates two broader trends: an attempt to construct a genuinely "American" form of popular music, shaped through often dubious ethnographic techniques, and forms of marketing which reflect official preoccupations expressed in the 1924 National Origins Act and its attempt to freeze immigration in proportion to the 1890 …

Having identified the strands that would later make up country music, we now consider the further development of the search for a national music. It coincided with the growing romantic attachment of certain members of the elite to a racialized Anglo-Saxon American "folk" whose culture they re-imagined, shorn of any uncouth or labor-related themes, in contrast to more urban, black or immigrant cultures. The English proselytiser for Morris dancing, Cecil Sharp, exceeded the efforts of others in highlighting the music of the Appalachians as particularly Anglo-Saxon and thus worthy of attention. Information Technology (IT) has experienced a worrisome decline in female participation over two decades, much of which can be attributed to fewer women choosing IT careers. However, women IT professionals also demonstrate mid-career turnover for reasons such as work-life balance, work exhaustion, role ambiguity, role conflict, and growth needs. This study explores alienation among women IT professionals and examines factors that lead to work alienation and abandonment of IT careers. Such alienation appears to be less prevalent in Asian countries where women perceive IT careers to be more conducive to female participation. A comparison among women from American and Asian …

Several factors are known to influence the absorption of dietary iron. The type of dietary iron consumed, what foods or supplements are consumed with dietary iron can all influence iron absorption. Understanding these factors can lead to better knowledge of what may or may not influence iron absorption. Obtaining accurate dietary data is important in nutrition research. Food frequency questionnaires can be used to estimate intake over a year time period, however, validation of a food frequency questionnaire against the 24 hour recall is essential for assessing the accuracy of the food frequency questionnaire. This article explores the use of a new governance approach in the context of American acute care hospital regulation, specifically focusing on the core regulatory process of licensure. This article calls for the alteration of current command and control regulations through the adoption of a four-part revisionist licensing model. The model seeks to reinvigorate the licensing process by making it not only more relevant to efficient operations, but also adaptable to current industry challenges. Based generally on alternative regulatory models such as responsive regulation, meta-regulation, and management-based regulation, the revisionist licensing proposal is driven by the broad goals of bureaucratic …

Suppression Of Aromatase In Human Breast Cells By A Cyclooxygenase-2 Inhibitor And Its Analog Involves Multiple Mechanisms Independent Of Cyclooxygenase-2 Inhibition, Bin Su, Edgar S. Diaz-Cruz, Serena Landini, Robert W. Brueggemeier

Previous studies have demonstrated that cyclooxygenase-2 (COX-2) inhibitor NS-398 decrease aromatase activity at the transcript level in breast cancer cells. However, N-Methyl NS-398, which does not have COX-2 inhibitory activity but has very similar structure to NS-398, decreases aromatase activity and transcription in MCF-7 and MDA-MB-231 breast cells to the same extent as NS-398. This suggests that NS-398 decrease aromatase expression in breast cancer cells via other mechanism(s). Further investigations find that both compounds only decrease aromatase activity stimulated by forskolin/phorbol ester at the transcript level in both breast cancer cell lines and in breast stromal cells from patients. Model predictive control is a strategy well-suited to handle the highly complex, nonlinear, uncertain, and constrained dynamics involved in aircraft engine control problems. However, it has thus far been infeasible to implement model predictive control in engine control applications, because of the combination of model complexity and the time allotted for the control update calculation. In this paper, a multiplexed implementation is proposed that dramatically reduces the computational burden of the quadratic programming optimization that must be solved online as part of the model-predictive-control algorithm.
