Background: It is often felt that developing countries need to improve their quality of healthcare provision. This study hopes to generate data that can help managers and doctors to improve the standard of care they provide in line with the wishes of the patients.Methods: It was a cross sectional study carried out at a major tertiary care hospital of Karachi. Patients between the ages of 18 and 80 years admitted to the hospital for at least one day were included. Direct acoustic stimulation of the cochlea by the sound-pressure difference between the oval and round windows (called the "acoustic route") has been thought to contribute to hearing in some pathological conditions, along with the normally dominant "ossicular route." To determine the efficacy of this acoustic route and its constituent mechanisms in human ears, sound pressures were measured at three locations in cadaveric temporal bones [with intact and perforated tympanic membranes (TMs)]: (1) in the external ear canal lateral to the TM, PTM; (2) in the tympanic cavity lateral to the oval window, POW; and (3) near the round window, PRW. …

A Modified Delphi study identified patterns of success for urban Extension 4-H programming in the next five years. The Delphi panel of 20 experts represented all regions, program areas, organizational levels as well as two external friends of Extension. The Delphi process provided both quantitative and qualitative data on the topics of collaboration, target populations, and programming. Fifteen patterns of success were identified for urban 4-H youth development. These patterns should serve as best practices for Extension youth development programming in urban counties for “making the best better!” in the near future.

Using Art In Occupational Therapy With People Who Have Cancer: A Qualitative Case Study, Jennifer R. Dochod

OBJECTIVE. To explore the role of art, with a small group of people who have cancer, as a means provide distraction from pain, to help express feelings, and to assist in making connections with others.
METHOD. Using a randomized sample from Gilda’s Club, a support center for those with cancer, in-house social workers recruited two participants to take part in a small group case study conducted by a master’s level occupational therapy student.
RESULTS. Two participants enrolled in the art class conducted by the researcher. Predetermined questions gathered through participant interviews and class discussions provided the data. Although the participant …

Interactions Across Spatial Scales Among Forest Dieback, Fire, And Erosion In Northern New Mexico Landscapes, Craig D. Allen

Ecosystem patterns and disturbance processes at one spatial scale often interact with processes at another scale, and the result of such cross-scale interactions can be nonlinear dynamics with thresholds. Examples of cross-scale pattern-process relationships and interactions among forest dieback, fire, and erosion are illustrated from northern New Mexico (USA) landscapes, where long-term studies have recently documented all of these disturbance processes. For example, environmental stress, operating on individual trees, can cause tree death that is amplified by insect mortality agents to propagate to patch and then landscape or even regional-scale forest dieback. Microarray time course (MTC) gene expression data are commonly collected to study the dynamic nature of biological processes. One important problem is to identify genes that show different expression profiles over time and pathways that are perturbed during a given biological process. While methods are available to identify the genes with differential expression levels over time, there is a lack of methods that can incorporate the pathway information in identifying the pathways being modified/activated during a biological process. Introduction: In order to prepare third year medical students in the Rural Physician Associate Program for a nine-month community-based continuity care experience in rural Minnesota, USA, a clinical skills day that featured human patient simulators and standardized patients was developed. Patients presenting with common urgent and routine primary-care problems were developed and presented using the objective structured clinical examination for teaching. The goals of the day were to: (1) distinguish urgent from non-urgent clinical presentation; (2) use clinical guidelines for making decisions; (3) communicate effectively in stressful situations; and (4) uncover a significant clinical issue with a different presenting complaint. …

Urgency Is A Non-Monotonic Function Of Pulse Rate, Frank A. Russo, Jeffery A. Jones

Magnitude estimation was used to assess the experience of urgency in pulse-train stimuli (pulsed white noise) ranging from 3.13 to 200 Hz. At low pulse rates, pulses were easily resolved. At high pulse rates, pulses fused together leading to a tonal sensation with a clear pitch level. Urgency ratings followed a nonmonotonic (polynomial) function with local maxima at 17.68 and 200 Hz. The same stimuli were also used in response time and pitch scaling experiments. Response times were negatively correlated with urgency ratings. Pitch scaling results indicated that urgency of pulse trains is mediated by the perceptual constructs of speed …
