To define ourselves as Americans, we often like to invoke the still-potent idea, inherited from the Puritans, of a "city upon a hill." Steeped in a mythic discourse on our earliest conception of America as an elect nation, this phrase envisions the community at large as "the pilot society for the world" engaged in a noble experiment of innocence, consensus, justice, and freedom for all, while driving to achieve myriad forms of greatness. Cyclooxygenase-2 Expression In Post-Mastectomy Chest Wall Relapse, Janet Kim

The purpose of this study was to assess the prognostic significance and clinical correlations of cyclooxgenase-2 expression (COX) in a cohort of patients treated with radiation (RT) for post-mastectomy chest wall relapse (PMCWR). Between 1975 and 1999, 113 patients were treated for isolated PMCWR. All patients were treated with biopsy and/or excision of the CWR followed by RT. Median follow-up was 10 years. All clinical data including demographics, pathology, staging, receptor status, HER-2/neu status, and adjuvant therapy were entered into a computerized database. Updating Medicare's Physician Fees: The Sustainable Growth Rate Methodology, Laura A. Dummit

National Health Policy Forum

Medicare’s method to annually update the fees it pays physicians has been under fire for some time—specifically, since the method determined that physician fees should be reduced rather than increased. The update method, called the sustainable growth rate (SGR), was implemented to control the growth in Medicare physician spending. Yet Congress, in response to physician concerns about beneficiary access to care, has acted to avert physician fee cuts since 2003. Although this signals dissatisfaction with the SGR methodology, there is yet to be a widely accepted physician fee update proposal that balances federal budgetary realities with the need to ensure …

An Anatomy Based Health Education Curriculum Taught By Medical Students May Improve High School Students Health Knowledge, Jason Knight

To date, few high school based interventions have been shown to have lasting effects on adolescents' health behaviors. The need for health interventions targeting adolescents is underscored by data showing that several health behaviors with significant short and long term adverse effects begin in early adolescence and become progressively more prevalent toward late adolescence. This project tested the efficacy of a novel anatomy based health education curriculum at increasing health knowledge. The course was taught by first year Yale medical students. The curriculum placed emphasis on nutrition, physical activity and infectious disease. Forty Juniors from Career High School visited Yale's …

High-Resolution Imaging Of Proteins In Human Teeth By Scanning Probe Microscopy, Alexei Gruverman, D. Wu, B. J. Rodriguez, Sergei V. Kalinin, S. Habelitz

High-resolution studies of dental tissues are of considerable interest for biomedical engineering and clinical applications. In this paper, we demonstrate the application of piezoresponse force microscopy (PFM) to nanoscale imaging of internal structure of human teeth by monitoring the local mechanical response to an electrical bias applied via a conductive tip. It is shown that PFM is capable of detecting dissimilar components of dental tissues, namely, proteins and calcified matrix, which have resembling morphology but different piezoelectric properties. It is demonstrated that collagen fibrils revealed in chemically treated intertubular dentin exhibit high piezoelectric activity and can be visualized in PFM …

We present Follow-Up Network for Gamma-Ray Bursts (FUN GRB) Collaboration observations of the optical afterglow of GRB 021211 made between 143 s and 102 days after the burst. Our unique data set includes the earliest filtered detections and color information for an afterglow in the pre-Swift era. We find that the afterglow is best described by (1) a propagation through a wind-swept medium, (2) a cooling break that is blueward of the observed optical frequencies, and (3) a hard electron energy distribution. However, superimposed on this ‘‘standard model’’ behavior we find, one and possibly two, significant chromatic variations during the …

BACKGROUND: Increased focus surrounds identifying patients with advanced non-small cell lung cancer (NSCLC) who will benefit from treatment with epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKI). EGFR mutation, gene copy number, coexpression of ErbB proteins and ligands, and epithelial to mesenchymal transition markers all correlate with EGFR TKI sensitivity, and while prediction of sensitivity using any one of the markers does identify responders, individual markers do not encompass all potential responders due to high levels of inter-patient and inter-tumor variability. Coral-Based Climate Variability In The Western Pacific Warm Pool Since 1867, Terrence M. Quinn, Frederick W. Taylor, Thomas J. Crowley

We have generated monthly resolved, stable isotope (δ18O and δ13C) and Sr/Ca time series from a massive Porites coral from Rabaul (4°S, 152°E): a site located in the warmest sector of the Western Pacific Warm Pool (WPWP). The coral δ18O and Sr/Ca time series are well correlated to each other and positive excursions in both records coincide with times of ENSO warm phase events. These time series contain abundant interannual variability that exhibits the well‐recognized pattern of low amplitude ENSO variation between ∼1920–1960 and high amplitude ENSO variation between 1880–1920 and 1960–1997.
