OBJECTIVE: To assess the effects of green and black tea on carbon tetrachloride (CCl4)-induced oxidative stress in rats. METHODS: This study was completed in the period of January to April 2005. Male wistar albino rats (6 weeks old) were randomly assigned to one of 3 diets (12 rats/group), control diet, black tea diet, or green tea diet. At the end of dietary treatment, a single dose of CCl4 (2.5 ml/kg intraperitoneally) in paraffin (1:1 v/v) was given to 6 rats in each group after overnight fasting. OBJECTIVE: The human epidermal growth factor receptor-2 (HER-2)/neu is a proto-oncogene that is amplified in 10-30% of breast cancers. It is known to be associated with a poor overall survival. We studied the relationship between its amplification and different histological gradings of breast cancer. METHODS: We studied 196 patients diagnosed with breast cancer in 2005 at the Omid and Ghaem Training Hospital, Mashhad Medical University, Iran. The HER-2/neu oncoprotein was measured by immunohistochemistry and the histological gradings were carried out according to the Bloom-Richardson Grading system. RESULTS: Sixty-seven (34.2%) cases were HER-2/neu positive and 129 (65.8%) cases were HER-2/neu negative. …

OBJECTIVE: To evaluate the frequency of 5 mutations and their relationship with the Tel Hashomer criteria in 85 FMF patients. METHODS: We looked for mutations in the Mediterranean fever (MEFV) gene in 84 consecutive patients who admitted to the Department of Medical Genetics of Afyon Kocatepe University, with a variable (from high to low) clinical suspicion of FMF. OBJECTIVE: To evaluate whether the use of intravenous immunoglobulin in newborn infants with isoimmune hemolytic jaundice due to Rh and ABO incompatibility is an effective treatment in reducing the need for exchange transfusion. METHODS: We conducted a retrospective chart review of 342 children who presented for elective minor surgical procedures in the Division of Pediatric Surgery, King Khalid University Hospital, Riyadh, Saudi Arabia from January 2004 to December 2004. OBJECTIVE: To review short term results of uncemented cup implantation and the fate of bulk femoral head autografts in patients with acetabular bone deficiency due to dysplasia of the hip. We used bone scintigraphy to assess the viability of the grafts. METHODS: We treated 19 hip joints of 17 patients with osteoarthritis due to developmental dysplasia of the hip with uncemented total hip arthroplasty between 1997-2003 in the Department of Orthopedics and Traumatology, Gazi University, Ankara, Turkey. OBJECTIVE: To investigate the effect of low doses of subcutaneous testosterone in hypogonadal men since the intramuscular route, which is the most widely used form of testosterone replacement therapy, is inconvenient to many patients. METHODS: All men with primary and secondary hypogonadism attending the reproductive endocrine clinic at Royal Victoria Hospital, Monteral, Quebec, Canada, were invited to participate in the study. OBJECTIVE: To identify the causes that affect the time from the onset of symptoms to admission to the hospital, in patients with a diagnosis of acute myocardial infarction (MI). METHODS: The study was carried out between January 2004 and January 2005 in the Emergency Room of the Uludag University Faculty of Medicine (UUFM) Hospital, Turkey. A total of 180 patients were included. Residents of the UUFM Department of Cardiology distributed a questionnaire to all patients. OBJECTIVE: To analyze the factors affecting survival of patients with chronic obstructive pulmonary disease (COPD) during the follow-up period using a 4-year prospective study. METHODS: The study included 276 out-patients with COPD. The study took place in Ankara University, Cebeci Hospital, Ankara, Turkey between September 2000 and January 2005. We used cox proportional hazards model in investigating the effects of clinical variables on survival. Risk factors related with mortality were analyzed. OBJECTIVE: To estimate the prevalence and clinical spectrum of gastroesophageal reflux disease (GERD) in a young population and evaluate the risk factors. METHODS: This cross-sectional study using a reliable and valid questionnaire was performed during June and July 2005, on randomized selection of 620 students form Azad University of Tabriz. Gastroesophageal reflux disease was defined as at least weekly heartburn and/or acid regurgitation. RESULTS: Mean age (± SD) of the responders was 22.48 ± 1.98 years. OBJECTIVE: To show the practice of a pain clinic in Saudi Arabia, to estimate the prevalence of various types of chronic pain managed in there and to find the relationship of patient9s age and gender to type of pain. METHODS: A retrospective study was carried out over a period of 5 years (January 2000 - December 2004) at a teaching hospital in Jeddah. The knowledge of vascular variations like other anatomical variations, is important during the operative, diagnostic, and endovascular procedures in abdomen. This report describes multiple variations in the upper abdominal vessels as found during the routine dissection in a 60-year-old male cadaver. The variations found were; presence of a celiaco-mesenterico-phrenic trunk, a common inferior phrenic trunk, 2 right renal arteries originating from abdominal aorta, 2 suprarenal arteries originating from the lower right renal artery, 3 right renal veins opening separately into inferior vena cava, and termination of right testicular vein into the lowest vein among the 3 right renal veins. We described a Saudi patient infected with Diphyllobothrium latum D. latum. A 38-year-old male presented, complaining of passing worms. He had a history of recent travel to Europe and South East Asia. Stools examination revealed typical D. latum eggs. He was treated with praziquantel followed by saline purge, after which he discharged an intact tapeworm. Macroscopic and microscopic examinations of the worm confirmed the diagnosis of D. latum. This is the first case of diphyllobothriasis to be reported in Saudi Arabia. The epidemiology and methods of prevention of diphyllobothriasis are discussed.

We report a 36-year-old woman, presented with cervical lymphadenopathy and low-grade fever. Two fine needle aspiration cytology and one excisional biopsy were performed in the referral hospital, all showed granulomatous lesions without necrosis. A tentative diagnosis of tuberculosis was made, and she started on antituberculous treatment. However, there was no clinical improvement. She presented to our institution one year after the initial diagnosis, and a new biopsy from the cervical lymph node revealed effacement of the whole node by marked non-necrotizing granulomatous reaction. However, there were scattered large cells with few classic Reed-Sternberg cells between the granulomas. Immunohistochemistry reveals strong …

An 18-year-old single female patient, presented with non specific gastrointestinal symptoms of anorexia, abdominal pain, and change in bowel habit. Clinically she was anemic, cachectic, and depressed. Abdominal examination revealed mobile epigastric mass. The scalp alopecia and endoscopy coupled by computed tomography scan, confirmed the diagnoses of trichobezoar, but it was not diagnosed as Rapunzel syndrome except after laparotomy, gastrotomy, and enterotomy. There are less than 16 cases of Rapunzel syndrome described worldwide, and this is the first case to be described in the middle east.

強調多元開放的後現代主義正蔓延於全球社會现象中,後现代主義思潮對台灣的課程改革也產生影響。台灣過去十年來的教育改革理念引用不少後现代課程理論的歐觀點,如多元價值的尊重、教育结構的鬆绑、及教育主體性的尊重。本文旨在闡述後現代課程理讑的觀點及對當今課程改革產生的影響,借鏡九年一貫的課程改革經駙,對護理教師在面對後現代課程政策時提出因應策略上的建義。護理教師在後現代課程理論中是課程的運用者、設計者、評值者及改善者,應體訒課程改革爲時勢所趨,唯有理解以後現代課程的理念,充實相關的課程專業能力,輿志趣相投的老師合作投身課程研究,並採取合宜的步調進行課程輿教學品質的改善,才能落實課程的改革。

The astronomical growth in technology and medical information of the last century brought with it innovations in medical education, such as the problem-based learning (PBL) which emphasizes the integration of basic and clinical sciences. The clinical skills unit (CSU) is a major facility for promoting these principles in the PBL-based curriculum of the International Medical University (IMU) which exposes students to clinical scenarios early in their training. The teaching of clinical skills is systems-based, but also holistic. 美國住院醫師訓練的目標依Accreditation Council for Graduate Medical Education的建議是培訓住院醫師六項核心能力,即病人照顧、醫學知識、臨床工作中的學習與改善、人際關係與溝通技巧、專業素養,以及制度下的臨床工作。多數醫學院內科提供的住院醫師訓練分為三大類:一為傳統路徑;二為primary care internist路徑;三為preliminary路徑。在美國,為便於住院醫師訓練,內科病房大多是一般內科病房。住院醫師的門診及急診訓練占訓練總時數三分之一以上。內科住院醫師訓練期間為三年,包括至少30個月的一般內科、內科次專科、加護醫學、急診醫學訓練。近幾年來,美國內科住院醫師訓練完畢者每年約7千名,內科專科執照通過率約90%。目前美國內科執業模式可分為三類﹕一般內科醫師、住院病患專責醫師、次專科醫師。為了拉近住院醫師訓練與執業之差距,住院醫師訓練時間要合理分配到各項執業模式並執行高品質、安全、有效、全人醫療之訓練。

Although deficits in visual attention are often postulated as an important component of many declines in cognitive processing and functional outcomes in older adults, surprisingly little emphasis has been placed on evaluating psychometric instruments with which individual differences in visual attention ability can be assessed. This article reports the development and beginning psychometric evaluation of DriverScan, a change detection measure of attentional search for older adults. A constrained graded response model is used to approximate response speed and accuracy with categories of immediate, delayed, or no response. This study investigated the link between daily health symptoms and spousal emotional well-being in a sample of 96 older dyads. Higher negative mood and lower positive mood were associated with spousal symptoms in couples wherein husbands or wives reported higher average levels of symptoms. For wives, partner effects were moderated by husbands’ marital satisfaction and illness severity. Specifically, higher husband marital satisfaction and illness severity were associated with higher negative mood and lower positive mood for wives on days where husbands reported higher symptom levels.
