This article surveys the history of bankruptcy data and identifies the BAPCPA provisions that bear directly on research. It concludes by examining how such studies will and should proceed. BAPCPA provides both opportunities and hazards to advance our understanding of bankruptcy. The development of comprehensive federal data offers the potential to dramatically increase the scope of knowledge about the bankruptcy system. The peril lies in the government conducting its research without the transparency and accountability necessary to convince private industry, academic scholars, and the general public of the integrity and usefulness of these data. The Westboro Baptist Church, led by Fred Phelps and based in Topeka, Kansas, has received national attention since the early 1990s, when members of this vehemently anti-homosexual group began actively protesting events involving prominent homosexual people. Eventually, these protests grew to include people who were even marginally supportive of homosexuality. While these protests incited outrage among various groups of people, no widespread effort was made to limit the group's ability to protest at such events. Computer technology and the Internet have become inseparable parts of our daily lives. However, as people grow older and their physiological and cognitive functions deteriorate, many stages in the interaction with computers can be compromised. Taking into account the importance of interactive (web based) computer systems in our rapidly evolving “knowledge society “, and their inherent benefits for senior citizens and for the entire community, we initiated an ongoing research activity to study the compatibility of interactive computer displays, including health-care interfaces, for the elderly. Civic environmentalism is a holistic perspective that sees community health, ecosystem health, social health, economic health, present conditions and future conditions, all as interdependent. The aim of the paper is to propose values for making and re-making the city in a way consistent with promoting physically and socially-sustainable environments. This article reviews the position of the New York courts on the obligation of local governments to zone for affordable housing and concludes that it is time for legislative action at the state level. Although municipalities are beginning to adopt inclusionary zoning ordinances, most are doing little to eliminate barriers to housing or stimulate needed production. Additional encouragement, guidance, and resources are needed to create an adequate supply of affordable housing. Patient: 65-year-old female status-post liver transplant secondary to cirrhosis.

Case Description: Prior to the rehabilitation consult on post-operative day 42, she had an episode of acute rejection requiring rapid escalation of cyclosporine dosage, later changed to high dose tacrolimus for immunosuppression, resulting in high blood levels of both calcineurin inhibitors. She then complained of paroxysms of 10/10 pain over her entire body not relieved by opioids despite escalation in medication by the acute pain service. She was not participating in a rehabilitation program because of pain. Examination revealed an anxious woman for whom any tactile stimulation …

The position effect variegation in Drosophila and Schizosaccharomyces pombe, and higher-order chromatin structure regulation in yeast, is orchestrated by modifier genes of the Su(var) group, (e.g., histone deacetylases ([HDACs]), protein phosphatases) and enhancer E(Var) group (e.g., ATP [adenosine 5'-triphosphate]-dependent nucleosome remodeling proteins). Higher-order chromatin structure is regulated in part by covalent modification of the N-terminal histone tails of chromatin, and histone tails in turn serve as platforms for recruitment of signaling modules that include nonhistone proteins such as heterochromatin protein (HP1) and NuRD. ABSTRACT

Purpose: To review studies which support the hypothesis that allograft inflammatory factor-1 (AIF-1), a protein initially identified in chronically rejected cardiac allografts, may be involved in the pathogenesis of the progressive fibroproliferative vasculopathy which is a hallmark of systemic sclerosis (SSc).

Recent findings: Recent findings demonstrated elevated AIF-1 expression both in SSc affected tissues and peripheral blood mononuclear cells. A detailed immunohistopathologic study examined the tissue and cellular localization of the protein in affected SSc tissues and demonstrated its expression in the endothelium of dermal and pulmonary vessels, in the pulmonary parenchyma, and in relevant inflammatory cells including T …

The Role Of Training Variables In Effective Dissemination Of Evidence-Based Parenting Interventions, Alexandra C. Seng, Ronald J. Prinz, Matthew R. Sanders

From a public health perspective, mental health in parents and children can be promoted through population-based dissemination of parenting and family support interventions. However, it is critical that service providers who are acquiring evidence-based parenting interventions complete the training regimen to optimize dissemination and impact. This article examines training completion and its relationship to individual service provider characteristics, barriers to program use and subsequent implementation of an evidence-based program, the Triple P - Positive Parenting Program. In this study, 83.7% of the service providers completed the two-part training. Individual-level variables did not predict training completion. Service providers from diverse backgrounds …

Hospitals increasingly reference published evidence for any given procedure or guideline as part of the push towards evidence-based nursing. Medical centers and universities can develop a collaborative model to help support evidence-based practice. The authors describe a model of collaboration between a university and medical centers to enhance nurses’ knowledge and research skills and provide research opportunities for faculty.

Medical Students’ Assessments Of Skill Development In Rural Primary Care Clinics, M. L. Hunsaker, M. L. Glasser, K. M. Nielsen, Martin S. Lipsky

Introduction: This study assessed the impact of a rural primary care preceptorship on medical students’ self-perceived ability to provide acute, chronic, and preventive care, to perform procedures, to communicate with patients, and to understand the community and healthcare system.
Methods: Students were surveyed about their self-assessed skills on 11 major components (97 items) immediately before and after a 16 week preceptorship in a rural primary care clinic. Responses were analyzed for 96 medical students using a paired comparisons t-test and univariate statistics.
Results: Students’ skills significantly increased on all components and items. The skills most highly assessed post-preceptorship were …

Independent data from the Gulf of Mexico are used to develop and test the hypothesis that the same sequence of physical and ecological events each year allows the toxic dinoflagellate Karenia brevis to become dominant. A phosphorus‐rich nutrient supply initiates phytoplankton succession, once deposition events of Saharan iron‐rich dust allow Trichodesmium blooms to utilize ubiquitous dissolved nitrogen gas within otherwise nitrogen‐poor sea water. They and the co‐occurring K. brevis are positioned within the bottom Ekman layers, as a consequence of their similar diel vertical migration patterns on the middle shelf. After trauma to the low back, a decrease in intervertebral joint stiffness can occur, which can lead to inability of the motor-control system to compensate for the tasks imposed on the tissues, resulting in spinal instability. Intervertebral joint stiffness can be thought of as a tightening of a corset around a spine segment, facilitated through coactivation of small and large spinal muscles. The motor-control system might be able to adapt initially with cocontractions of the intrinsic muscles, but an unbalanced demand between the small and large muscles can lead to muscle fatigue and poor task performance. The Lipogenic Enzymes Dgat1, Fas, And Lpl In Adipose Tissue: Effects Of Obesity, Insulin Resistance, And Tzd Treatment, Gouri Ranganathan, Resat Unal, Irina D. Pokrovskaya, Aiwei Yao-Borengasser, Bounleut Phanavanh, Beata Lecka-Czernik, Neda Rasouli, Philip A. Kern

Acyl-coenzyme A:diacylglycerol transferase (DGAT), fatty acid synthetase (FAS), and LPL are three enzymes important in adipose tissue triglyceride accumulation. To study the relationship of DGAT1, FAS, and LPL with insulin, we examined adipose mRNA expression of these genes in subjects with a wide range of insulin sensitivity (SI). DGAT1 and FAS (but not LPL) expression were strongly correlated with SI. In addition, the expression of DGAT1 and FAS (but not LPL) were higher in normal glucose-tolerant subjects compared with subjects with impaired glucose tolerance (IGT) (P < 0.005). To study the effects of insulin sensitizers, subjects with IGT were treated with pioglitazone or metformin for 10 weeks, and lipogenic enzymes were measured in adipose tissue. After pioglitazone treatment, DGAT1 expression was increased by 33 ± 10% (P < 0.05) and FAS expression increased by 63 ± 8% (P < 0.05); however, LPL expression was not altered. DGAT1, FAS, and LPL mRNA expression were not significantly changed after metformin treatment. The treatment of mice with rosiglitazone also resulted in an increase in adipose expression of DGAT1 by 2- to 3-fold, as did the treatment of 3T3 F442A adipocytes in vitro with thiazolidinediones. These data support a more global concept suggesting that adipose lipid storage functions to prevent peripheral lipotoxicity.

During 2003, an estimated 906 million visits were made to physician offices in the United States (Hing, Cherry, & Woodwell, 2005).

Overall, 42% of visits to outpatient settings were attended by a registered nurse (Middleton & Hing, 2005).

Despite ambulatory care being the fastest growing site for care, it is the least studied.

The purpose of this article is to provide an overview of the role of the RN in ambulatory care and describe the direct and indirect economic value of RNs in ambulatory care settings.
