Cephalosporin Resistant Shigella Flexneri From A Clinical Isolate--A Rare Finding, Nasim Sabir, Afia Zafar

Shigellosis is an important public health problem, especially in developing countries. Antibiotic treatment of dysentery aimed at resolving diarrhea or reducing its duration, and to prevent transmission to other close contacts. Isolates with resistance to first line drugs have been reported throughout world, third generation cephalosporins and quinolones are mainstay of treatment. Shigella flexneri resistant to third generation cephalosporins in a clinical isolate is a rare finding, and this has been reported for the first time in Pakistan.

Frequency And Sensitivity Pattern Of Extended Spectrum Beta Lactamase Producing Isolates In A Tertiary Care Hospital Laboratory Of Pakistan, Kauser Jabeen, Afia Zafar, Rumina Hasan

Objective: To determine frequency, distribution and sensitivity pattern of Extended-Spectrum ß Lactamase (EBSL) producing organism at a tertiary care hospital in Pakistan.
Methods: All members of enterobacteriacae isolated between April and August 2002 were studied. Isolates were speciated according to standard biochemical tests. Susceptibility testing was performed by Kirby-Bauer method. ESBL was detected using double disc method using cefotaxime versus cefotaxime plus clavulanate according to NCCLS. Statistical analysis was performed by SPSS version 10. Test of significance were calculated using chi-square test.
Results: During the study period, 1137/2840 (40%) of the isolates tested were found to be ESBL producing. …

Frequency Of Isolation Of Shigella Serogroups/Serotypes And Their Antimicrobial Susceptibility Pattern In Children From Slum Areas In Karachi, Afia Zafar, Nasim Sabir, Zulfiqar Bhutta

Objectives: To assess the frequency of serogroups and serotypes, as well as the antimicrobial susceptibility pattern of Shigella species isolated from known cases of diarrhoea and dysentery from Karachi, Pakistan.
Methods
: A cross-sectional study was conducted between January 2002 and March 2003 at Aga Khan University on stool samples received from children with diarrhoea and dysentery from four low socio-economic areas (Sultanabad, Rehri Goth, Hijrat and Sherpao colony) of Karachi. Stool samples yielding growth of Shigella species, were further identified for serotypes by slide agglutination. Antibiotics susceptibility was performed by Kirby Bauer disk diffusion method

Solid And Cystic Epithelial Neoplasm Of Pancreas With Metastasis: Report Of A Highly Unusual Case, Zubair Ahmad, Nausheed Yaqboob, Suhail Muzaffar, Nadeem Ahmad

Solid and cystic papillary epithelial neoplasms of the pancreas are uncommon tumors occurring predominantly in young women. These tumors have excellent prognosis and after complete surgical resection, more that 95% patients are cured. Occasionally, they invade the surrounding pancreatic parenchyma. These tumors can recur even many years after resection, so long-term follow up is essential. Very few cases metastasize. We present a case of a middle aged woman with metastases to liver and omentum

Better Beginnings is an early intervention family literacy program that has been developed by the Public Library Services Directorate, at the State Library of Western Australia. Its stated purpose is to provide positive language and literacy influences for children in their first three years of life. The program is thought to be the first of its kind in Australia and has recently been taken up by another Australian Territory State. Palliative care is the relief of symptoms, regardless of their impact on the underlying disease process. The philosophical underpinning of current international paediatric palliative and supportive care models is that palliative and supportive care should be offered to all children with life threatening or chronic illnesses/disabilities with complex care needs. This approach allows the integration of cure-directed treatment and palliative care. allowing children to benefit from both philosophies of care. This report assesses the wellbeing of the professions of policing, nursing and teaching in Western Australia. The findings are derived from surveys of how individuals think about their occupations, their colleagues and employers. The level of wellbeing affects the ease of attracting and retaining staff and the quality of delivered services.

We present and discuss summary results of responses to questionnaires sent in 2005 to 5,180 police, 6,000 nurses and 9,000 teachers. Each of the professions is here regarded as a single group. Hope is considered to have a positive influence upon health. Cancer patients may enter hospice care after a rigorous course of medical treatment, having hoped for a cure or long remission. While the hope for cure is important, hope is no less important at the end of life when the goal of care is quality of life. This study examined the relationship between hope and quality of life in hospice patients with cancer.

Spousal bereavement has been consistently demonstrated in the literature to be one of the most highly stressful experiences in an individual’s lifetime. In addition many deaths in the United States are preceded by a period of caregiving, which is also believed to be highly stressful and have a profound impact on bereavement. Since the mid-1970s, top managers, politicians, and officials in both public and private institutions have promoted contracting-out services (privatization) as a means of increasing efficiency, flexibility, and quality. The privatization trend has occurred in many public sector organizations particularly in city services and prisons. Public health services are not immune to this trend. Today many county health departments have contracted the provision of public health services like womens health, primary care, and laboratory services. Understanding the motives for sunbathing and indoor tanning is an extremely important public health issue. Skin cancer rates have increased dramatically in recent years and UV exposure via sunbathing and utilization of sun lamps and tanning beds are considered important risk factors. Motives for sunbathing and tanning salon use have been thought to be related to appearance concerns, yet little research has examined the specific tan appearance attitudes that may contribute to use of these behaviors. In 2001, the Coral Reef Evaluation and Monitoring Program (CREMP) began monitoring the abundance and area covered by three clionid sponges (Cliona delitrix, C. lampa, and C. caribbaea). Subsequently, monitoring has been conducted annually at all 40 CREMP sites throughout the Florida Keys National Marine Sanctuary (FKNMS) and the Dry Tortugas. Between 2001 and 2002, mean clionid area decreased significantly from 7.6 cm2 /m2 to 4.6 cm2 /m2 (Wilcoxon; p= 0.035). Between 2002 and 2003, the decline to 4.5 cm2 /m2 was not significant. Approximately 80% of all clionid colonies recorded at the CREMP stations covered less than 50 cm2 …

Mapping The Major Axis Of Tephra Dispersion With A Mesoscale Atmospheric Model: Cerro Negro Volcano, Nicaragua, Marc A. Byrne

Models of tephra fallout are used to assess volcanic hazards in advance of eruptions and in near-real-time. Current models often approximate the wind field using simplistic assumptions of the atmosphere that cannot account for typical variations in wind velocity that occur in time and three-dimensional space. Here, a widely used mesoscale atmospheric model is used to improve forecasts of the location of the major axis of dispersion for erupting plumes. The Pennsylvania State University-National Center for Atmospheric Research fifth-generation Mesoscale Model (MM5) specializes in atmospheric prediction for regions on the order of ten to hundreds of kilometers on a side. …
