Medical Necessity In Private Health Plans: Implications For Behavioral Health Care, Sara J. Rosenbaum, Brian Kamoie, D. Richard Mauery, Brian Walitt

This report addresses how the term "medical necessity" is defined in private health insurance coverage decisions. It summarizes a review of the literature, an extensive review of legal cases that challenge insurer decisions, materials prepared by the insurance industry, consultation with experts in the field, a review of investigations conducted by State departments of insurance and attorneys general, and interviews with health care executives regarding the decision-making process itself. The report does not explore factors that can affect access to care that might be considered clinically necessary by treating professionals or the effects of medical necessity decisions on therapeutic outcomes.

LSAY Cohort Reports

This report provides details of the experiences of the 1998 cohort of the Longitudinal Surveys of Australian Youth (abbreviated to LSAY Y98) at a single point in time. Information on this cohort was first collected in 1998, when these young people were Year 9 students in Australian schools. The reference period for this report is October 2002. Most members of the cohort were in their first post-school year, having completed their secondary schooling at the end of 2001. Information about the cohort's activities in previous years is available in earlier reports in this series.

In the few years following promulgation of the Restatement (Third) of Torts: Products Liability, several courts have reaffirmed their allegiance to the consumer expectations test for product design defect liability, while rejecting the Restatement's contrary recommendation to adopt a design defect test that focuses primarily on technical features regarding the risk and utility of alternative product designs. In this Article, Professor Kysar reviews the post-Third Restatement decisions, identifying within them a common failure to articulate a coherent, independent doctrinal role for the consumer expectations test, despite the courts' clearly expressed desire to do so. Using Zero-Inflated Count Regression Models To Estimate The Fertility Of U. S. Women, Dudley L. Poston Jr., Sherry L. Mckibben

In the modeling of count variables there is sometimes a preponderance of zero counts. This article concerns the estimation of Poisson regression models (PRM) and negative binomial regression models (NBRM) to predict the average number of children ever born (CEB) to women in the U.S. The PRM and NBRM will often under-predict zeros because they do not consider zero counts of women who are not trying to have children. The fertility of U.S. white and Mexican-origin women show that zero-inflated Poisson (ZIP) and zero-inflated negative binomial (ZINB) models perform better in many respects than the Poisson and negative binomial models. …

This article concerns the testing and estimation of a dose-response effect in medical studies. We study the statistical test of homogeneity against umbrella alternatives in a sequence of Poisson distributions associated with an ordered dose variable. We propose a test similar to Cochran-Armitage’s trend test and study the asymptotic null distribution and the power of the test. We also propose an estimator to the vertex point when the umbrella pattern is confirmed and study the performance of the estimator. Non-Gaussian point-referenced spatial data are frequently modeled using generalized linear mixed models (GLMM) with location-specific random effects. Spatial dependence can be introduced in the covariance matrix of the random effects. Maximum likelihood-based or Bayesian estimation implemented via Markov chain Monte Carlo (MCMC) for such models is computationally demanding especially for large sample sizes because of the large number of random effects and the inversion of the covariance matrix involved in the likelihood. We review three fitting procedures, the Penalized Quasi Likelihood method, the MCMC, and the Sampling-Importance-Resampling method. Monte Carlo methods were used to examine Type I error and power rates of 2 versions (conventional and robust) of the paired and independent-samples t tests under nonnormality. The conventional (robust) versions employed least squares means and variances (trimmed means and Winsorized variances) to test for differences between groups.

Random Regression Models Based On The Elliptically Contoured Distribution Assumptions With Applications To Longitudinal Data, Alfred A. Bartolucci, Shimin Zheng, Sejong Bae, Karan P. Singh

We generalize Lyles et al.’s (2000) random regression models for longitudinal data, accounting for both undetectable values and informative drop-outs in the distribution assumptions. Our models are constructed on the generalized multivariate theory which is based on the Elliptically Contoured Distribution (ECD). The estimation of the fixed parameters in the random regression models are invariant under the normal or the ECD assumptions. For the Human Immunodeficiency Virus Epidemiology Research Study data, ECD models fit the data better than classical normal models according to the Akaike (1974) Information Criterion. Poisson regression is useful in modeling count data. In a study with many independent variables, it is desirable to reduce the number of variables while maintaining a model that is useful for prediction. This article presents a variable selection technique for Poisson regression models. The data used is log-linear, but the methods could be adapted to other relationships. The model parameters are estimated by the method of maximum likelihood. The use of measures of goodness-of-fit to select appropriate variables is discussed. On Treating A Survey Of Convenience Sample As A Simple Random Sample, W. Gregory Thatcher, J. Wanzer Drane

Threat of bias has kept many from using data gathered in less than optimal conditions. We maintain that when convenience sampling represents race and gender at nearly correct proportions and can be beneficial, as these two variables are quite often used as stratification variables. We compared a convenience sample with a proven sample. Race and Sex were nearly proportional as was found in the proven sample. We conclude that the convenience sample can be used as though it is simple random.

Depressed hog prices in 1998 and 1999 were due to not enough shackle space (kill capacity) and not to a glut of slaughter swine. However, many swine producers were put out of business in California. In 2000, market prices rebounded due to increased shackle space and reduced numbers of slaughter swine available as a result of the sellouts of the previous two years. The economic roller coaster ride emphasized the need for maximizing profit while streamlining operations. Increasing numbers of pigs per litter, and thereby reducing the cost per pig produced, is one aspect of streamlining. A significant step in …

Alcohol And Anxiety: Subtle And Obvious Attributes Of Abuse In Adults With Social Anxiety Disorder And Panic Disorder, Lindsay S. Ham, Debra A. Hope

Previous research has found a relation between social anxiety disorder and alcoholism, but recent work found no differences in drinking levels among socially anxious individuals, dysthymics, and normal controls. Using a more sophisticated measure of substance abuse may further explicate the relation between social anxiety and drinking. We examined aspects of substance abuse in treatment-seeking individuals with social anxiety disorder or panic disorder (psychiatric control group) as well as nondisordered individuals (normal control group). We used the Substance Abuse Subtle Screening Inventory–3 because it includes both face valid and subtle items to control for social desirability. Contrary to the hypotheses, …

Silica Based Solid Phase Extraction Sorbent With A Removable Alkyltrimethylammonium Surfactant Phase, Tony Oakes

Presented is a solid phase extraction (SPE) sorbent having a removable "stationary phase." This removable phase that consists of alkyltrimethylammonium surfactant is initially immobilized onto hydrophilic strong cation exchange resin. The surfactant chain through hydrophobic interactions extracts hydrophobic analytes in the same manner as conventional bonded alkyl moieties on silica based non-polar sorbents. For the extraction of very hydrophobic species with conventional sorbents, solvents used for analyte elution are generally toxic and not directly instrument compatible. The chosen solvent must be strong enough to efficiently break hydrophobic interactions between the analyte and sorbent. Using a removable "stationary phase," hydrophobic interactions …

The challenges of managing interpersonal conflicts or performing internal peacemaking are as real and relevant as the challenges of our professional undertaking in handling “outside” disputes.
