Assessing Depression Treatment And Outcomes Using Insurance Claims Data, Gregory Andrew Allard

This retrospective, longitudinal, quasi-experimental study uses insurance data to compare long-term outcomes among an adult population by depression history and treatment level. Treatment levels were assigned based on claims history correspondence with Agency for Health Care Policy and Research (AHCPR) depression outpatient treatment guidelines. Thirty-month outcomes included depression resurgence and total health care utilization (total utilization). Recurrent depressives (recurrents) were expected to experience resurgence at higher rates than single episode depressives (singles). Additionally, cases with AHCPR-recommended (preventative) treatments were expected to have lower resurgence rates compared to cases that received lower-level treatments. A Synthetic Approach To Forecasting Fleet Costs And Earnings In The Northeast Regions Of The United States, Philippe D. Lallemand

This study describes the New England Otter Trawl Fishery between 1982 and 1998 and the potential economic impacts of regulations on the Northeastern Multispecies Groundfish Fishery (MGF). The study uses primary data collected by the author and secondary data obtained from the National Marine Fisheries Service (NMFS). This research required extensive econometric analysis of both value landed and cost data. The landed value is influenced by the vessel's physical characteristics, port of landing, seasonal choices of fishing gear-species combinations, days at sea and distance from shore, and the captain's skill (also referred to as the "skipper effect"). The river continuum concept (RCC) asserts that the biological community in a stream system is largely regulated by geomorphological factors (Vannote, et al., 1980, 131). The concept states that community structure in natural stream systems is structured in order to capitalize on the most efficient energy utilization throughout. The objective of this research is to test the RCC in the Wheeling Creek watershed of northern West Virginia and southwestern Pennsylvania and to compare the results with those hypothesized under the RCC. This allowance for ambiguity also reminded me of my fondness for the labyrinths and forking paths of Borges, and I realized that this duality was probably closely related to my travels between cultural frontiers and my frequent passages between mania and depression. Japan's Ise Shrine And Selected Norwegian Stave Churches: An Examination Of The Definition Of Vernacular Architecture., H. Gary Gillespie

Japan’s Ise Shrine (300-600 C.E.) and Norway’s stave churches (1100-1250 C.E.) are examples of vernacular architecture. Analyses of their functions, building materials, construction techniques, structural forms, and the impact of local philosophical, mythological, and folk influences upon their forms and decorations show that the Ise Shrine and selected Norwegian stave churches reflect cultural values and idea systems common to both cultures. By definition, vernacular architecture implies a regional, provincial character wherein architectural style is perpetuated by native traditions as opposed to cross-cultural influences or universal expression. On May 19, 1920, gunshots rang through the streets of Matewan, West Virginia, in an event soon known as the “Matewan Massacre.” Most historians of West Virginia and Appalachia see this event as the beginning of a long series of events known as the second mine wars. This dissertation argues that this event was, rather, the culmination of an even longer series of events that unfolded in Mingo County, dating back at least to the Civil War, and setting the stage for the second mine war. The frontier period of the Kanawha Valley provides an interesting example of the sometimes dramatic change that occurred on the fringes of American civilization as it inched across the continent. Within twelve years of the creation of the county, it had developed from an embattled military buffer region into a quiet backwoods town. For the first six years, Indian attacks constantly assailed the settlements along the Kanawha River. My dissertation tracks the development of London's built environment through the seventeenth and eighteenth centuries, identifying the shared conceptual and narrative techniques employed in building literature (construction manuals, architectural treatises, and urban improvement tracts) and in literature about buildings (plays, poems, and novels) to discuss the evolution, dissemination, and significance of contemporary debates over urban expansion, urban planning, and spatiality. The first two chapters examine how literary texts proved instrumental in shaping debates over London's urban expansion at key historical junctures: the late-sixteenth century when London's population explosion triggered a series of anti-building proclamations and during the rebuilding of London …

This study analyzes food security and coping mechanisms in semi-arid West Pokot between 1920–1995. Based on a wide array of primary and secondary sources, the study analyzes the historical role of indigenous irrigation, specifically furrow irrigation, rain-fed farming and livestock keeping, in food production and food security among the Pokot of northwest Kenya. Besides farming and herding, the study also analyzes trade, hunting and gathering, as well as lending and borrowing (for example of food) based on reciprocity, and their contribution to food security in the area. Despite the rapid industrialization and high rates of economic growth that Japan experienced for much of the postwar period, there have been relatively few attempts by English-language researchers to investigate Japan's domestic space-economy at the regional scale. This dissertation examines the process of uneven development in Japan from 1965 to 1994. The conceptual framework for the analysis is a regional political economy approach, combining theories of uneven development with recent work by Michael Webber and David Rigby on regional profit rates in manufacturing and the determinants of profit rate change over time. The purpose of this study was to determine the efficacy of a residential science research experience in changing participants' attitudes and understanding of the nature of science and their view of themselves as science researchers. Data from interviews, journal writings, classroom observations and two pre-post instruments were used in the evaluation plan. As participants of this study, 16 inservice teachers (K–16) attended a two-week residential institute at the National Radio Astronomy Observatory (NRAO) in Green Bank, West Virginia. The purpose of this study was to investigate the impact of gender dynamics and culture on interaction in the adult ESOL, from the teachers', students', and observer's points of view. Using an ethnographic methodology, the researcher drew from class observations, videotaped segments of classroom interaction, teachers' self-evaluations of gender equity in instruction, and their responses to gender dynamics scenarios, student responses to classroom photographs, as well as student, teacher, and administrator interviews. Although few would initially consider an airport a community, these sprawling sites of aviation technology do qualify according to the definition in the Wordsworth's Concise English Dictionary. Community: common possessions or enjoyment: people having common rights: a body of persons in the same locality: leading a common life: a common affinity. Certainly the thousands that enter daily any major hub share common possessions, rights, a shared locality, and an affinity or need for travel. More than a few frenzied business flyers might admit that their base airport is a more familiar location than their suburban or urban neighborhood.

Today, there are an estimated 150 securities exchanges trading stocks around the world. Tomorrow (or at least within the reasonably foreseeable future), this number is likely to shrink radically. The two great forces reshaping the contemporary world – globalization and technology – impact the world of securities markets in a similar and mutually reinforcing fashion:

  1. they force local and regional markets into more direct competition with distant international markets;
  2. they increase overall market capitalization and lower the cost of equity capital, as issuers are enabled to access multiple markets; and

In this review, I will first briefly address Professor Litman's evocation of the copyright law-making process. Her discussion of legislative history presents a valuable and compelling account, especially for those unfamiliar with copyright law. Nonetheless, it is not a principal focus of this review. The "archival turn" that characterizes much new and recent work in the disciplinary history of sociology is institutionally situated and replete with professional obligations and scholarly expectations, some less visible than others. Unlike our colleagues in academic departments of history, we are relative newcomers to archives and their riches. Pete Seeger, Ella Jenkins, Suni Paz and six other leading children's musicians in the U.S contributed their experience to this study about the crucial role of music and singing for social justice in children's lives and its shrinking presence in U.S. pre-school and elementary schools. The study built its theoretical framework on arts and music education, transformative education, and folk and socially conscious children's music. Participatory research, based on the revolutionary education theories of Paulo Freire, guided the dialogues with the participants. In Search Of The Qualitative Clear Majority: Democratic Experimentalism And The Quebec Secession Reference, Cristie Ford

This article attempts to marry direct democratic deliberation with the enforcement of important constitutional norms in the context of a real-life policy question. The question is the secession of Quebec from Canada. The article argues that a referendum is neither the most legitimate nor the most effective way to address the issue. The debate over Quebec's future must be reoriented by reference to the broad normative framework set out by the Supreme Court of Canada in the Reference Re Secession of Quebec, [1998] 2 S.C.R. 217, combined with a new democratic process.
