A multiplex amplification and detection platform for the diagnosis of Mycobacterium bovis and Brucella abortus infection simultaneously in bovine milk and nasal secretions was developed. This system (designated the bovine pathogen detection assay [BPDA]-PCR) consists of duplex amplification of species-specific targets (a region of the BCSP31K gene of B. abortus and a repeat-sequence region in the hsp65 gene of M. bovis, respectively). This is followed by a solid-phase probe capture hybridization of amplicons for detection. On the basis of spiking experiments with normal milk, the analytical sensitivity of the assay was 800 CFU equivalents/ml of milk for B. abortus and …

Balancing Public Health Against Individual Liberty: The Ethics Of Smoking Regulations, Thaddeus Mason Pope

Ten years ago, philosopher Robert E. Goodin published "No Smoking: The Ethical Issues." Goodin argued that the liberty of smokers can be justifiably limited for two reasons: to prevent harm to third persons and to prevent harm to smokers themselves under circumstances which make their decision to smoke substantially non-autonomous. In this article Thaddeus Pope reexamines the harm principle and the soft paternalism principle in light of more recent legal developments, gives them additional content, and carefully demarcates the justificatory scope of each. Pope also defines and defends a third liberty-limiting principle, hard paternalism, arguing that the liberty of smokers …

Nutritional status is the best global indicator of well-being in children. Although many surveys of children have been conducted since the 1970s, lack of comparability between them has made it difficult to monitor trends in child malnutrition.
Cross-sectional data from 241 nationally representative surveys were analysed in a standard way to produce comparable results of low height-for-age (stunting). Multilevel modelling was applied to estimate regional and global trends from 1980 to 2005.
The prevalence of stunting has fallen in developing countries from 47% in 1980 to 33% in 2000 (i.e. by 40 million), although progress has been uneven according to …

The purpose of this literature review is: to describe the historical context of Native American health, identify current challenges to Native American health and propose strategies to deal with the adverse effects of one of these challenges.
The historical context of Native American health discussed includes a history of Indian Health service and it's role as provider of American Indian health care since 1830. Challenges to the Indian Health Service population outlined include diabetes, cardiovascular disease and alcoholism. This Article describes potential pitfalls for employers arising from the ADA protections afforded to employees with no prior record of disability returning to work after medical certification of a disability requiring accommodation. The Article describes Federal laws protecting disabled employees from unnecessary intrusion in their private lives (such as the Americans with Disabilities Act of 1990, the Family Medical Leave Act of 1993, and the Occupation Health and Safety Act of 1970) and sets out the requirements imposed on employers. The hepatitis C virus (HCV) infection is a significant cause of morbidity and mortality worldwide. Infection with HCV becomes chronic in more than 80% of cases and it accounts for 20% of all cases of acute hepatitis. The hepatitis C virus was first identified by the molecular cloning of the virus genome in 1989. It is an enveloped, positive strand RNA virus with a genome size of around 9.5 kilobases. The single-stranded RNA genome of the virus contains a large open reading frame that encodes a large polyprotein of 3,010 to 3,033 amino acids shown to be processed by a …

The evolutionary causes of small clutch sizes in tropical and Southern Hemisphere regions are poorly understood. Alexander Skutch proposed 50 years ago that higher nest predation in the south constrains the rate at which parent birds can deliver food to young and thereby constrains clutch size by limiting the number of young that parents can feed. This hypothesis for explaining differences in clutch size and parental behaviors between latitudes has remained untested. Here, a detailed study of bird species in Arizona and Argentina shows that Skutch's hypothesis explains clutch size variation within North and South America. However, neither Skutch's hypothesis …

This study explored the level of knowledge related to breastfeeding among maternal-infant nurses. It also explored whether there was a relationship between nurses' breastfeeding knowledge and level of education, type of clinical experience, years of experience and personal experience with breastfeeding.
A convenience sample of 67 maternal-infant nurses completed a modified version of Leadley's (1994) Breastfeeding Knowledge and Attitudes Survey.
Descriptive and correlational statistics were used to analyze the relationship between the variables and the nurse's breastfeeding knowledge. Data analysis revealed a mean score of 13.2 out of 20 questions on nurses' breastfeeding knowledge. There was no significant difference in …

Objective: The goal of this study was to report the molecular analysis of antibiotic-sensitive and multidrug-resistant (MDR) strains of Salmonella typhi, using pulsed-field gel electrophoresis (PFGE), with a particular emphasis on the coexistence of these strains in a typhoid-endemic region of Karachi, Pakistan. Methods: One hundred isolates of S. typhi in humans (50 MDR and 50 antibiotic-sensitive isolates) from sporadic cases of typhoid fever were analyzed by Vi-phage typing, antibiograms and PFGE. Results: The MDR S. typhi strains were resistant to ampicillin, chloramphenicol, and trimethoprim-sulfamethoxazole. Analysis by PFGE showed that 50 MDR isolates of S. typhi had a single, homogenous …

The outbreak of enterovirus 71 infection in Taiwan, reported by Ho et al. (Sept. 23 issue), 1 occurred almost a year after the outbreak in Malaysia. Though both outbreaks occurred in Asia and both involved large numbers of deaths, it was not known whether the two outbreaks were related. We studied the nucleotide sequence and secondary RNA structure of some of the isolates, using the sequence of the 5' untranslated region (UTR). Epidemiological studies have failed to show any important impact of human immunodeficiency virus (HIV) infection and disease on leprosy, in contrast to the well documented increase in incidence and morbidity with other mycobacteria in these patients, notably Mycobacterium tuberculosis. The early events following exposure and repeated exposure to Mycobacterium leprae (ML) have not still been studied. Previous studies of first inoculations of ML in the skin of rhesus monkeys have indicated delayed recruitment of CD4+ cells and delayed interleukin (IL) 2 secretion in skin inoculation sites in simian immunodeficiency (SIV) positive animals. Research suggests that individuals with Posttraumatic Stress Disorder (PTSD) have significant dysregulation in a number of physiological indices, especially the sympathetic nervous system (SNS) and hypothalamic-pituitary-adrenal (HPA) axis. Biological theories of PTSD propose that prolonged SNS and HPA axis dysregulation places individuals with PTSD at risk for development of medical morbidity and impairments in health status. The present study examined the following research questions: (a) what is the prevalence of traumatic life events and PTSD in a low-income primary care population? Bartonella henselae is a zoonotic agent in which the domestic cat serves as the natural reservoir, and humans acquire potentially serious infections associated with this microorganism. The purpose of this research is to contribute to the understanding of the pathogenesis of B. henselae in the domestic cat using a molecular approach. Using sequence differences in a portion of the I 6S rRNA gene between B. henselae genotype I, and B. henselae genotype II, a nested polymerase chain reaction (nPCR) was designed and used to investigate various phases of feline bartonellosis. Research has revealed the increase in the number of individuals suffering from co-occurring illnesses since the early 1980's. Because this condition has become the norm rather than the exception, it becomes necessary to focus more clearly on individuals that are diagnosed with this illness and on the most effective treatment in order to assist the individual in recovery.
