Advances in technology have given new life to debates concerning privacy.' Specifically, issues surrounding increased access to personal medical records have recently garnered attention. On one side of the debate, healthcare providers and insurers support expanded access to medical records for treatment, research, and insurance claims purposes. At the same time, however, many patients legitimately expect their medical records to remain private. The advent of Internet access to patient records and electronic medical insurance claims submissions has heightened patients' concerns that computerized medical records will offer less protection and more potential for unauthorized disclosure than paper files in locked cabinets. …

Massena, New York, located on the St. Lawrence River in upstate New York, is a small industrial town of 16,000 persons. The St. Lawrence River forms the international border between New York State and Ontario, Canada. Three major industries, General Motors, Reynolds Metals, and the Aluminum Company of America, operate large, primary aluminum smelting operations in Massena. Massena was the site selected for the first United Stated aluminum smelters near the Alcoa plant on the St. Lawrence River. All three aluminum smelters continue to operate today. Immigration has been a subject of intense historical and contemporary debate in US political life. Proponents of immigration cite the important contributions immigrants have made and continue to make to the USA’s national development and evolution. Advocates of more restrictive immigration policies stress concerns over the USA’s ability to support immigrant residents and whether newer immigrants threaten the US national identity and social cohesion. Why Employers Use Flexible Staffing Arrangements: Evidence From An Establishment Survey, Susan N. Houseman

This paper examines which employers use flexible staffing arrangements, why they use these arrangements, and their implications for workers and public policy, drawing on a nationally representative survey of private sector establishments. Use of flexible staffing arrangements-including temporary help agency, short-term, on-call, regular part-time, and contract workers-is widespread and two-thirds of employers believe this use will increase in the near future. Traditional reasons concerning the need to accommodate fluctuations in workload or absences in staff are the most commonly cited reasons for using all types of flexible staffing arrangements. Genomic Structure Of Murine Mitochondrial Dna Polymerase-Gamma., Justin L. Mott, Grace Denniger, Steve J. Zullo, H. Peter Zassenhaus

We have sequenced a genomic clone of the gene encoding the mouse mitochondrial DNA polymerase. The gene consists of 23 exons, which span approximately 13.2 kb, with exons ranging in size from 53 to 768 bp. All intron-exon boundaries conform to the GT-AG rule. By comparison with the human genomic sequence, we found remarkable conservation of the gene structure; the intron-exon borders are in almost identical locations for the 22 introns. The 5' upstream region contains approximately 300 bp of homology between the mouse and human sequences that presumably contain the promoter element. This region lacks any obvious TATA domain …

Recently attention has been brought to trans-resveratrol's {TR) anticancer activity, as determined through a number of cultured cancer cell models. This activity was attributed to TR behaving as an estrogen, and the orientation of TR' s hydroxyl groups. Based on this work it was of interest to determine whether TR would also be toxic in prostate cancer cells; if toxic, did TR induce necrosis or apoptosis in the cells; was it toxic through hormone mediated pathways; and were TR's hydroxyl groups responsible for its biological activity. Presented here are the first data on copper complexation and speciation in the South Atlantic Ocean. Samples were collected at six vertical profile stations and along the surface (1m) transect during the 1996 IOC Baseline Contaminant Survey. Copper complexation measurements were made for the surface transect (1m) samples and three of the six vertical profile stations (1m - ~500m) using differential pulse anodic stripping voltammetry (DPASV) with a thin mercury film, rotating glassy carbon disk electrode (TMFRGCDE).

Women's Work And Economic Development, Kristin Mammen, Christina Paxson

Using a cross-country dataset and microdata from India and Thailand, we examine how women's work status changes with economic development. Several clear patterns emerge: women's labor force participation first declines and then rises with development; women move from work in family enterprises to work as paid employees; fertility declines; and gender gaps in education narrow. Women's education levels, and those of their spouses, appear to be important determinants of women's labor market activities. Broad welfare indicators, such as mortality rates and education levels, indicate that women's well-being improves on average with development, both in absolute terms and relative to men.

Extension is rapidly being drawn into a competitive knowledge marketplace. This article looks at why and proposes a way to meet the resulting new audience expectations. Technology and escalating audience demands for anytime/anyplace access conspire to place Extension--a large, traditional organization--at a disadvantage. The authors propose a new and distinct virtual Extension competitor designed to meet dot.com's head on: e-CES. As Tom Peters says, "Become distinct, or become extinct."

Delivering Short Seminars And Workshops To Farmers And Ranchers With Low Bandwidth On-Line Technologies, Bart Beaudin

One hundred fifty-three designers of on-line instruction completed a 35-item on-line questionnaire and recommended the following top three low bandwidth technologies to use when delivering short seminars and workshops: 1) Web page (text and < 50K graphics) with asynchronous discussion. 2) Web page only, and 3) Web-based training (user submit and feedback) with asynchronous discussion. There were no significant differences between how females and males rated the various technologies or when the data was controlled for age of the respondents. Experience designing on-line courses produced a significant finding at the .05 level for the rating of "Web Page with Asynchronous Discussion" (p = .032 and df = 145).

Helping Children Succeed After Divorce: Building A Community-Based Program In A Rural County, Diane E. Johnson

The devastating effect of divorce on children has led to the development of parenting programs across the country. Extension in rural Darke County, Ohio, played a key role initiating and conducting such a program by working in collaboration with the courts and mental health agencies. Through exit evaluations, more than 1,400 participants in 5 1/2 years indicated that they learned how to focus on their children through the grieving process and learned the importance of a low-conflict relationship with their child's other parent.

A Comparative Study Of Positive Pressure Ventilation Via Laryngeal Mask Airway And Endotracheal Tube, A Idrees, F A. Khan

Objective: To investigate the use of Laryngeal Mask Airway (LMA) and its comparison with the endotracheal tube for positive pressure ventilation.SETTING: A tertiary care teaching hospital.Methods: Fifty adult ASA I and II patients undergoing peripheral limb surgery were randomly allocated to 2 groups for LMA or endotracheal tube insertion. A standardized anaesthetic technique was used. The groups were then compared regarding haemodynamic changes on insertion as well as removal of LMA and ETT and any complications that occurred were noted.Results: The haemodynamic response to insertion was significantly attenuated (p < 0.05) in LMA group as compared to ETT group. The cardiovascular response to extubation was not significantly different between the groups. A higher incidence of coughing and mild hypoxaemia at extubation was noted in ETT group as compared to LMA group (p < 0.05) and blood was detected in 4 cases after LMA removal.CONCLUSION: It is concluded that the use of LMA during positive pressure ventilation is safe in selected cases. There is an attenuated haemodynamic response to insertion of LMA as compared to endotracheal tube which will be beneficial in certain patients e.g., those with ischaemic heart disease, vascular disease and hypertensives.

The response of the Vincentian and Charity Families to the attacks of 9/11 is chronicled, using people’s own words wherever possible. On the day itself and during the aftermath, Sisters of Charity and Vincentians prayed with and counseled grieving people, first responders, and rescue workers. Personnel from Saint Vincent’s Hospital and Medical Center were among the earliest to arrive at Ground Zero, and the Saint Vincent Catholic Medical Centers treated over one thousand victims. Among other things, Sisters of Charity served as New York Police Department chaplains, answered calls to hospital information lines, and worked at a DNA collection site …
