Managed Care And Managed Sentencing — A Tale Of Two Systems, Ronald Weich

The daily injustices mount. The front line professionals who administer the system cry out for more discretion to depart from the rigid rules that bind them, Congress finally hears their call, and is poised to enact sweeping reforms.

Are improvements in federal sentencing law on the way? Probably not in the near future. But the new Congress will surely take up proposals to regulate the managed health care industry, and the impending debate over a proposed "Patients' Bill of Rights" law offers important lessons for federal sentencing policy.

Exoy, An Adenylate Cyclase Secreted By The Pseudomonas Aeruginosa Type Iii System, Timothy L. Yahr, Amy J. Vallis, Michael Hancock, Joseph T. Barbieri, Dara W. Frank

The exoenzyme S regulon is a set of coordinately regulated virulence genes of Pseudomonas aeruginosa. Proteins encoded by the regulon include a type III secretion and translocation apparatus, regulators of gene expression, and effector proteins. The effector proteins include two enzymes with ADP-ribosyltransferase activity (ExoS and ExoT) and an acute cytotoxin (ExoU). In this study, we identified ExoY as a fourth effector protein of the regulon. ExoY is homologous to the extracellular adenylate cyclases of Bordetella pertussis (CyaA) and Bacillus anthracis (EF). Mycobacterium Avium Complex Augments Macrophage Hiv-1 Production And Increases Ccr5 Expression, S. M. Wahl, T. Greenwell-Wild, G. Peng, H. Hale-Donze, T. M. Doherty, D. Mizel, J. M. Orenstein

Infection with HIV-1 results in pronounced immune suppression and susceptibility to opportunistic infections (OI). Reciprocally, OI augment HIV-1 replication. As we have shown for Mycobacterium avium complex (MAC) and Pneumocystis carinii, macrophages infected with opportunistic pathogens and within lymphoid tissues containing OI, exhibit striking levels of vital replication. To explore potential underlying mechanisms for increased HIV-1 replication associated with coinfection, blood monocytes were exposed to MAC antigens (MAg) or viable MAC and their levels of tumor necrosis factor α (TNFα) and HIV-1 coreceptors monitored. Evaluation Of The Macrocyclic Antibiotic Avoparcin As A New Chiral Selector For Hplc, K. Helen Ekborg-Ott, John P. Kullman, Xiande Wang, Kyung Gahm, Lingfeng He, Daniel W. Armstrong

Avoparcin is a macrocyclic glycopeptide antibiotic structurally related to vancomycin, teicoplanin, and ristocetin A. When attached to 5-μm spherical silica gel, the avoparcin proved to be an effective chiral stationary phase (CSP) that could be used in the reversed-phase, normal- phase, and polar-organic modes. The avoparcin CSP was complimentary to the other macrocyclic glycopeptide CSPs in that it could resolve some racemates that the others could not, and vice versa. Some important compounds resolved on the avoparcin CSP include verapamil, thyroxine, mephenytoin, and 2- imidazolidone-4-carboxylic acid. The use of this CSP and the optimization of separations on it are discussed. …

Pastors For Peace And The Martin Luther King Jr. Memorial Center: A Case Study Of Cooperation Between Two Nonprofit Organizations From Two Countries Without Diplomatic Relations, And With An Economic Embargo Enforced By One Country Against The Other, Alicia Beatriz Jrapko

This thesis is a case study about an American nonprofit organization, Pastors for Peace, working with a Cuban counterpart, the Martin Luther King Jr. Memorial Center. This study is about cooperation between nonprofit organizations in two countries without diplomatic relations. In addition, there is an economic embargo imposed by the United States against Cuba. The research demonstrates that when an American nonprofit organization working abroad aims to change U.S. policy, the work will be controversial and difficult, and there will be some legal implications for the organization as an entity as well as for staff members and volunteers individually.

Assessing The Accuracy Of A New Diagnostic Test When A Gold Standard Does Not Exist, Todd A. Alonzo, Margaret S. Pepe

UW Biostatistics Working Paper Series

Often the accuracy of a new diagnostic test must be assessed when a perfect gold standard does not exist. Use of an imperfect test biases the accuracy estimates of the new test. This paper reviews existing approaches to this problem including discrepant resolution and latent class analysis. Deficiencies with these approaches are identified. A new approach is proposed that combines the results of several imperfect reference tests to define a better reference standard. We call this the composite reference standard (CRS). We evaluated the side effects induced by injection of Freund's adjuvant (FA)and alternative adjuvants combined with different antigens. Rabbits and mice were injected subcutaneously, intramuscularly (rabbits) and intraperitoneally (mice)with different adjuvants (FA, Specol, RIBI,TiterMax, Montanide ISA50)in combination with several types of antigens (synthetic peptides, autoantigen, glycolipid, protein, mycoplasma or viruses). The effects of treatment on the animals' well-being were assessed by clinical and behavioural changes (POTand LABORASassays) and gross and histopathological changes. In rabbits, treatment did not appear to induce acute or prolonged pain and distress. Mice showed behavioural changes immediately after (predominantly secondary) immunization. This report, produced jointly by the British Union for the Abolition of Vivisection and Compassion in World Farming, fills a number of significant gaps in the current debate about xenotransplantation.

In this report we also summarise the ethical and welfare issues concerning experiments on animals for xenotransplant research and their possible use as source animals for organs. Both these aspects are responsible for much pain and distress caused to many animals. We prefer the term “source animals” to “donor animals”, because animals do not choose to donate their organs for xenotransplantation.
