In this licensee holdover involving an OMH-regulated supportive housing community residence, landlord served a Notice to Quit based on tenant's discharge from the program, then subsequently executed a new Occupancy Fee Agreement with tenant. The court dismissed because landlord's execution of a new agreement after service of the predicate notice vitiated the Notice to Quit, rendering it a nullity. A landlord who renews an occupancy agreement after service of a termination notice cannot thereafter rely upon the prior notice as the basis for a summary proceeding.

Despite the increased risk of invasive pneumococcal disease among pediatric hematopoietic cell transplant (HCT) recipients, pneumococcal colonization has not been evaluated in this population. We prospectively assessed longitudinal pneumococcal colonization in 22 pediatric HCT recipients from conditioning therapy to 100 days post-HCT. A higher likelihood of pneumococcal colonization was observed in the late post-HCT period (weeks 9-14) compared with the early post-HCT period (weeks 1-8), with odds increasing with each additional week after conditioning.IMPORTANCEThis is the first study to longitudinally evaluate pneumococcal colonization in pediatric hematopoietic cell transplant (HCT) recipients from the onset of conditioning therapy to 100 days post-HCT. …

The onset of epilepsy in adulthood occurs most commonly after 55 years of age. Given the ageing global population, this disorder represents an increasing burden on healthcare and society. The bidirectional link between epilepsy and dementia is a focus of intense research with underlying tau pathology highlighted as a potential mechanistic link. In this review, we examine the evidence for tau-related neurodegenerative processes in epilepsy beginning with how changes in biochemical and structural properties of the tau protein can lead to abnormal phosphorylation and pathological aggregation. We consider the role of tau in seizure occurrence and cognitive difficulties in experimental …

Background: Chronic limb-threatening ischemia (CLTI) caused by infrapopliteal disease is a major therapeutic challenge, with percutaneous transluminal angioplasty (PTA) associated with high rates of restenosis and reintervention. The LIFE-BTK trial previously demonstrated superior efficacy of the Esprit (Abbott Vascular) BTK drug-eluting resorbable scaffold (DRS) over PTA at 1 year, with sustained benefits at 2 years, with comparable safety at both time points. This report presents the 3-year outcomes.
Objectives: We aimed to compare the 3-year safety and efficacy of DRS vs PTA in patients with CLTI and infrapopliteal artery disease.
Methods: In this randomized trial, 261 patients with CLTI were …

BACKGROUND: Non-small cell lung cancer (NSCLC) remains the leading cause of cancer-related mortality in the United States, but survival outcomes are shifting with evolving therapies and screening. Recent findings suggest that survival improvements vary across health-care systems, with favorable trends observed in the Veterans Health Administration (VHA). We sought to compare NSCLC survival trends across US health systems.
METHODS: We conducted a retrospective cohort study of patients with NSCLC (2007-2019) using data from the VHA and the National Cancer Database (NCDB), representing the non-VHA US general population. The primary exposure was health system (VHA vs non-VHA), and the primary outcome …
Dual-Pathway System For Low-Latency Pii Redaction In Generative Image Modification, Mira Leung
Dual-Pathway System For Low-Latency Pii Redaction In Generative Image Modification, Mira Leung
Defensive Publications Series
Some personally identifiable information (PII) redaction methods for generative image applications can have high latency and computational costs, which may limit their use in interactive contexts. A dual-pathway architecture may be used to process image modification requests. A synchronous, low-latency pathway can detect and obfuscate potential PII before a generative model concurrently inpaints the obfuscated areas and applies user modifications. Simultaneously, an asynchronous, offline pathway can generate a more thoroughly redacted version of the source image and store it for future use. This hybrid approach can provide a low-latency response suitable for on-demand image generation, while progressively creating higher-quality redacted …

Evaluating personalized artificial intelligence (AI) systems may be challenging, as the use of detailed user context can conflict with user privacy considerations. A system is described for generating synthetic user populations via a world simulation. The system can create generative agents with demographic and behavioral characteristics based on aggregated, anonymized statistical data. These agents can interact over time within a simulated environment, producing a stream of longitudinal user behavior captured in isolated test accounts. The resulting dataset can be used for scalable, automated testing and debugging of AI models, offering a contextually rich, privacy-respecting alternative to using real user data. …

Organizations in regulated sectors may face challenges using large-scale, cloud-based artificial intelligence (AI) services because of data sovereignty and compliance mandates. A system may provide a decentralized, agent-aware mesh network that functions as an infrastructure layer to bridge on-premise environments and external AI services. This network can intercept and semantically understand complex agent communications, allowing an active enforcement layer to perform actions such as sanitizing data, pausing high-risk operations for human review, and maintaining a sovereign audit log. This approach can allow organizations to leverage advanced AI reasoning capabilities from external providers while assisting in the maintenance of local control …

An On-Vessel Signal Discrimination Layer For Bandwidth-Constrained Maritime Decision Support, Dev Mohanty
An On-Vessel Signal Discrimination Layer For Bandwidth-Constrained Maritime Decision Support, Dev Mohanty
Defensive Publications Series
The DeepBlue protocol sentry is an on-vessel signal-discrimination independent layer that filters high-frequency NMEA telemetry streams into compact, information-dense feature vectors. Unlike AIS broadcasts, rather than transmitting all raw sensor data (which is bandwidth-expensive and mostly redundant), it:
- Normalizes raw NMEA sentences across GPS, propulsion, environmental sensors
- Derives features (fuel consumption, course/speed changes, propulsion efficiency drift, loitering detection) over rolling 1-5 min windows
- Scores materiality — only flags features that deviate from the vessel's own baseline
- Assembles vectors of tens-to-hundreds bytes for transmission over satellite/mesh uplinks
The key insight: filter redundancy at source before transmission, not downstream. It's a …

Automated Multi-Stream Audiovisual Synchronization Using Coordinated Markers, Avichal Rakesh
Defensive Publications Series
When an audio and/or video recording of a scene is captured using multiple recording devices, synchronizing frames captured by the different devices is a challenge. While movie clappers and timecode generators are used for this purpose, these are expensive solutions that are not always available, and in the case of timecode generators, require compatible recording devices. This disclosure describes techniques that use devices such as a smartphone, tablet, etc., that are commonly available to generate audio and/or visual markers using a speaker and display (or camera flash) of the devices. The markers are captured by the different recording devices and …

Ψ-Theory: Living Core. Ontological Development And Formalization. Comments 1–21, Valerii Y. Vorobiev
Defensive Publications Series
This document compiles comments 1–21, which chronologically develop and formalize Ψ-Theory — an ontological framework where time (Ψ-field) is the primary metric of reality, and gravity, mass, and quantum phenomena emerge from its dynamics. It addresses the crises of modern cosmology (dark matter, dark energy, Hubble tension) and quantum mechanics (measurement, non-locality) by replacing ad-hoc entities with a single causal principle. The comments include testable predictions (BH shadow shift, underground chronometry, Tully-Fisher derivation) and propose a phase diagram of reality from (4,2) to (0,-2).

Defensive Specifications Disclosure: Token-Free, Non-Linear Boundary, And Wave-Interference Based Architecture For Distributed Autonomous Ai Agent Frameworks, Christopher L. Eckes
Defensive Publications Series
The Distributed Agent Networks Integrated Efficiency Layer (DANIEL) is a token-free, hardware-agnostic framework for distributed autonomous AI agents that replaces symbolic communication with a three-layer, analog-modeled mathematical system. The framework utilizes phase-matrix integration to execute scalar consensus loops via trigonometric wave equations. Security is enforced by a passive, non-linear activation gate that leverages input gradient vectors to dynamically harden against high-amplitude adversarial prompt injections. Network state synchronization is maintained across a decentralized peer-to-peer mesh mapped onto a continuous toroidal ring topology, employing a rolling average vortex matrix to passively filter localized agent hallucinations and single-node failures. This integrated architecture removes …

The Dimensionally Extended Holographic Projection Model: A Viscoelastic Unified Field Specification, Christopher L. Eckes
Defensive Publications Series
This technical disclosure formalizes the geometric synthesis of the Dimensionally Extended Holographic Projection (DEHP) model. We present a particle-free, mathematically cohesive alternative to standard general relativity and non-baryonic dark matter. The framework replaces independent fundamental forces with a single fluid-dynamic mechanism: the topological metric embedding of a continuous, two-dimensional (2D) phase fluid substrate (z=0) operating in absolute harmonic resonance (the "Garden State").
Three-dimensional (3D) physical matter (z > 0) is modeled as a localized, high-tension wave crest ("knot") forced outward along a spatial z-axis by an original inflationary puncture flux (\(J_{z}\)). By defining antimatter as a zero-volume, 2D negative wave trough …

Healthcare sustainability is a multidisciplinary field that seeks to mitigate healthcare’s environmental consequences. Antimicrobial stewardship and healthcare sustainability both aim to reduce wasteful resource use while maximizing patient safety. However, a strong partnership between antimicrobial stewardship and healthcare sustainability has yet to develop. To facilitate environmentally sustainable decision-making in antimicrobial prescribing, we developed a web-based calculator (EcoRxChoice (www.ecorxchoice.com)) where users can enter different antimicrobial regimens and compare how much plastic waste each regimen creates. This work was supported in part by the Society of Infectious Diseases Pharmacists (SIDP), reflecting SIDP’s commitment to strengthening sustainability as a practical extension of antimicrobial …
