Posaconazole is a triazole antifungal agent used for prophylaxis and treatment of invasive fungal diseases. Posaconazole has high inter-individual variability in dose-exposure relationship and benefits from therapeutic drug monitoring (TDM). Adult studies report particularly high rates of subtherapeutic exposure among intensive care unit (ICU) patients related to changes in physiology during critical illness. The rates of subtherapeutic posaconazole exposure among critically ill children in ICU's are unknown. This single-center retrospective analysis at a large freestanding children's hospital used TDM results from 2010 to 2025 obtained while patients were undergoing care in a pediatric ICU. Affordable Thermal Control For Cell Culture Chambers: An Open-Source, Pid-Based Heating Module, Majid Rostami, Sesha Sai Ramineni, Sangyoon J. Han

Commercial environmental control systems for live cell imaging chambers frequently cost $10,000–20,000. This paper presents an open-source, PID-controlled heating module built entirely from off-the-shelf components for approximately $500. The system employs four adhesively mounted polyimide heater pads driven at 12 VDC through a closed-loop chain: a K-type thermocouple feeds a Delta DTB4848LV controller, whose analog output is converted to PWM and applied to the heaters via an opto-isolated MOSFET switch. The required heating power was determined through analytical thermal sizing and verified by finite-element simulation (ANSYS Mechanical), confirming that the cell culture region reaches 37°C at steady state. To alleviate the issue of low condensation efficiency of mixed vapor in the presence of high air concentration, this study employs extended condensation surfaces. Ribbed condensation surfaces with rib spacings of 1 mm, 2 mm, and 4 mm are investigated for air concentrations ranging from 30% to 90%. The results indicate that at an air concentration of 30%, the ribs weaken the condensation heat transfer; however, when the air concentration is greater than or equal to 50%, longitudinal ribs enhance the condensation heat transfer by about a factor of two. This memorandum discusses the possible collateral damage that Propositions 41 and 42 (on the ballot in California on November 3, 2026) might cause.  The two propositions were concocted very quickly by billionaires in order to trick voters that will vote for Proposition 40 (the billionaire tax) also to vote for these propositions that are designed to undermine the billionaire tax.

The basic trick is that these propositions have plausible seeming veneers but hide a poison pill that aims to frustrate the functioning of the billionaire tax.  The veneers are “plausible seeming” because they address problems that are not problems.  For …

BACKGROUND: Non-trauma center (NTC) emergency departments (EDs) play regular but underappreciated roles in the management of severely injured patients, yet many lack formal protocols to guide care. We describe the implementation and outcomes of a structured trauma evaluation and transfer protocol at an urban NTC emergency department.

METHODS: A multidisciplinary trauma protocol was implemented on April 1, 2023, at an urban NTC, supported by a Level I trauma center, 2.5 miles apart. The study period ran from January 1, 2019, to June 30, 2025. Using pre-post study design, 62 patients treated prior to implementation were compared with 49 post-intervention patients. …

OBJECTIVE: The Tetanus, Diphtheria, Pertussis (Tdap, "tetanus") vaccine is recommended for adults every 10 years. In a national sample of adult emergency department (ED) patients, we sought to determine (1) knowledge and up-to-date status of the tetanus vaccine; (2) reasons for nonreceipt of the vaccine; (3) vaccine acceptability if offered in the ED; and (4) reasons for nonacceptance in the ED if offered.

METHODS: This was a cross-sectional survey study among non-critically ill adults at 10 US EDs, asking questions about knowledge, receipt, and acceptability of the tetanus vaccine. Vaccine receipt was determined by self-report. Aggregate responses are reported as …

Despite the lack of evidence of a mechanism-based drug interaction, some professional organizations have advised caution coadministering apixaban and levetiracetam. Eligible patients taking apixaban with or without levetiracetam for any indication for at least 72 h were screened from the electronic medical records. Plasma samples from 77 patients were retrieved from salvaged clinical blood collections. Apixaban minimum and maximum concentrations (Cmin and Cmax) were compared using descriptive statistics in R. While the median Cmin and Cmax appear to be marginally elevated, there were no apparent differences relative to apixaban alone. These findings suggest no apparent interaction of levetiracetam and support …

DHS Science & Technology Directorate (S&T) tasked the NCITE Center, in support of Customs and Border Protection (CBP), to develop a framework for understanding the forms and underlying processes through which Mexico-based drug cartels innovate and adapt tactically. This rapid review report identifies the processes, drivers, and constraints that shape tactical innovation within cartel ecosystems. The framework adapts and extends multilevel frameworks of tactical innovation established in the terrorism literature, most of which were developed through the study of traditional terrorist organizations whose ideological constraints, resource profiles, and internal structures often differ substantially from those of Mexican drug cartels.

This report provides a framework for how analysts can leverage observable activities of foreign terrorist organization-designated transnational criminal organizations (FDTCOs) as indicators for underlying decision-making processes around organizational stress, adaptation, and other strategic tradeoffs. Building on our understanding of FDTCOs as operating within entrepreneurial ecosystems, we apply a decision domain framework to three groups through case studies: Sinaloa Cartel, Jalisco New Generation Cartel, and MS-13. These groups pose a persistent as well as growing threat to American communities and the broader Western Hemisphere.

As the U.S. State Department increasingly designates drug cartels as Foreign Terrorist Organizations (FTOs), scholars and policymakers face a pressing need to better understand the structure and behavior of these groups. This report argues that drug cartels, particularly the thirteen currently designated as FTOs, are more accurately conceptualized as criminal entrepreneurial ecosystems than as traditional terrorist organizations.

BACKGROUND & AIMS: The genetic basis of impaired intestinal barrier function in preterm infants is poorly understood. Variants in single immunoglobulin interleukin1-related receptor, a negative regulator of Toll-like receptor signaling, have been identified in preterm infants with necrotizing enterocolitis. We hypothesized that single immunoglobulin interleukin1-related receptor variants associated with necrotizing enterocolitis impair gut barrier function to pathobionts. The aim of this study was to determine how single immunoglobulin interleukin1-related receptor genetic variants disrupt neonatal intestinal barrier integrity and promote susceptibility to Gram-negative bacteria implicated in sepsis and necrotizing enterocolitis pathogenesis.

BACKGROUND: The 2023 US Anaphylaxis Practice Parameter indicated that some patients receiving epinephrine in the community might avoid emergency department (ED) visits; however, the criteria and safety of this approach remain unproven.

OBJECTIVE: To derive and validate a clinical prediction model for identifying children at risk of receiving epinephrine after arriving at the ED.

Skin carotenoid score (SCS) as measured using pressure-mediated reflection spectroscopy has been validated among adults with normal and overweight body mass index, with little validation research among adults with obesity. Therefore, the purpose of this exploratory study was to examine the validity of SCS as an approximation of fruit and vegetable intake among adults with obesity. Evolution of central nervous systems is a long-debated topic. Similar mechanisms of conditional neural specification linked to dorsal-ventral (D–V) axis formation have been used to support homology across several taxa. We tested for autonomous versus conditional neural specification in two distantly related annelids, Capitella teleta and Platynereis dumerilii, using blastomere isolations and ablations. Our results demonstrate autonomous specification of anterior neural tissue in isolated first-quartet (1q) micromeres for both annelids. In both annelids, partial larvae derived from isolation of the somatoblast, micromere 2d, also resulted in the formation of trunk neural tissue (ventral nerve cord or VNC), suggesting that this …

Candida (Candidozyma) auris has emerged as a hospital-acquired pathogen and was first reported in East Asia in 2009. Infective endocarditis due to C. auris has been rarely reported, while other Candida species account for < 2% of infective endocarditis (IE) cases. Herein we report three cases of endocarditis caused by C. auris , including a native aortic valve, a native tricuspid valve, and a prosthetic valve. Each of these cases had been in healthcare settings months to years earlier that were the likely sources of their infec- tions. These cases suggest the possibility for indolent but life-threatening infections from C. auris . ©2026 The Authors.
