Saudi Medical Journal

Objectives: To uncover the predictive value of systemic immune-inflammatory index (SII) and systemic inflammatory response index (SIRI) on early pregnancy loss. Methods: A total of 535 individuals were enrolled in this retrospective analysis. The early pregnancy losses (EPL) group included patients between 18-35 years old who experienced EPL. The control group comprised healthy pregnant women who gave birth at ≥37 weeks. Results: The EPL group had significantly lower plateletcrit ( p =0.04), platelet distribution width (PDW, p <0.0001), and RDW ( p <0.0001) and higher monocyte ( p <0.0001) and SIRI ( p <0.0001) values than the control group. The hemoglobin, white blood cells, platelet count, neutrophil count, lymphocyte count, mean platelet volume, neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), and SII values were not significantly different between the EPL and control groups ( p >0.05). The cut-off value for the SIRI that offers the best sensitivity/specificity balance was 1.48 (sensitivity of 63%; specificity of 63%) in the …

Thrombocytopenia Following Iron Repletion With Ferrous Gluconate, Muneerah R. Alromaizan, Muath A. Alkhunizan

Saudi Medical Journal

Immune thrombocytopenia (ITP) is an autoimmune disorder marked by low platelet counts that puts patients at risk for spontaneous bleeding. A rare trigger for ITP is iron repletion, which has only been reported in a few cases. In this article, we present a unique case of a 54-year-old male with a history of recurrent ITP who experienced rapid thrombocytopenia following iron repletion with ferrous gluconate. Discontinuation of ferrous medications resulted in platelet counts returning to the normal baseline. Following more than 30 years of the patient’s clinical timeline, this case demonstrates the chronic nature of ITP and the complexity of …

The Protagonist Of Contemporary And Emerging Nanotechnology-Based Theranostics And Therapeutic Approaches In Reshaping Intensive Care Unit, Ling Xie, Yun Chen

Saudi Medical Journal

To maintain a clean and hygienic environment in the intensive care unit (ICU) is crucial for ensuring patient safety, preventing infections, and reducing healthcare-associated complications. With the increasing prevalence of infections and the emergence of viral and bacterial resistance to standard antiseptics, there is a pressing need for innovative antiseptic solutions. Nanotechnology is increasingly being employed in medicine, particularly focusing on mitigating the activities of various pathogens, including those associated with hospital-acquired infections. This paper explores the current impact of nanotechnology, with a particular focus on bacterial infections and SARS-CoV-2, which significantly strain healthcare systems, and then discusses how nanotechnology …

Saudi Medical Journal

Autosomal dominant hyper immunoglobulin E (IgE) syndrome is a rare inborn error of immunity that affects approximately one in a million individuals worldwide. It presents with various symptoms owing to multisystem involvement (immunological and non-immunological). Recurrent infections (mainly in the skin and lungs) are common presentations. A 5-year-old Middle Eastern boy presented with symptoms suggestive of obstructive uropathy secondary to multiple large pelviabdominal abscesses and acute kidney injury with hyperkalemia that necessitated admission to the intensive care unit. Upon further investigation, the patient’s genetic test (whole exome sequencing) demonstrated a heterozygous missense variant in the STAT3 gene. The patient completely …

Retrospective Analysis Of The Clinical Utility Of Multi-Cytokine Profiles In Smear-Negative Pulmonary Tuberculosis, Xuemei Li, Weidong Zhao, Hong Han, Zhi Yang, Fengqing Bi, Yingchun He

Saudi Medical Journal

Objectives: To evaluate cytokine profiles and interferon-gamma release assay (IGRA) for their diagnostic capabilities in the differentiation of tuberculosis (TB) from non-TB conditions, as well as smear-negative pulmonary tuberculosis (SNPT) from smear-positive pulmonary tuberculosis (SPPT). Methods: A total of 125 participants were included, 77 of whom had TB and 48 who didn’t, and demographic, clinical, and laboratory data were collected, including cytokine levels and IGRA results. The TB patients were further divided into 2 subgroups: SNPT (n=42) and SPPT (n=35). Results: Compared to non-TB, the TB group had lower BMI, higher WBC, neutrophils, monocytes, ESR and CRP ( p <0.05). TB patients showed higher IL-2, IL-6, IFN-γ, IL-8 ( p <0.001) and higher IGRA positivity (88.3% versus [vs.] 29.2%, p <0.001). Between SNPT and SPPT, moderate effect sizes were observed for IFN-α, IL-2, IL-10, IL-8 (Cohen’s d 0.59-0.76), with lower IGRA positivity in SNPT (81.0% vs. 97.1%, p =0.015). ROC analysis indicated IFN-α, IL-2, IL-10, IL-8 had moderate accuracy for SNPT diagnosis (AUCs 0.668-0.734), and combining these improved accuracy (AUC 0.759, 80% sensitivity, 64.2% specificity). Conclusion: A multi-biomarker approach combining these cytokines demonstrates enhanced diagnostic accuracy for tuberculosis.

Incorporating Ultrasonography To The Nutritional Assessment Process In Intensive Care Settings To Improve The Prescription Of Enteral And Parenteral Nutrition: Benefits, Practicality, And Challenges, Sara Zaher

Saudi Medical Journal

The validity of the traditional nutritional assessment tools in intensive care settings might be compromised when the patient has conditions such as oedema and inflammation. Ultrasound (US) is considered a non-invasive, bedside tool that can be utilized to assess changes in muscle mass. Hence, US could guide healthcare practitioners in identifying the varying degrees of malnutrition and adjusting the nutritional prescription accordingly. This review discusses the currently available data regarding the feasibility and practicality of using US measurements in intensive care settings. Overall, the data suggest that using US as part of the standard anthropometric assessment for critically ill patients …
