Treatment Of Plummer-Vinson Syndrome With Savary-Gilliard Dilatation, Mohamed I. Yasawy
Saudi Medical Journal
The association of postcricoid dysphagia, iron deficiency anemia and upper esophageal webs is known as the Plummer-Vinson syndrome (PVS). It is predominantly a disease of middle aged females. Four patients with cervical web of the esophagus were studied, dysphagia was mostly for solids, which were non-progressive and were present for years before they presented. Initial treatment in 3 patients consisted of esophageal dilatation with savary bougies after initial rupture of the web by endoscope. In one case, due to failure to visualize the esophageal lumen by fluoroscopy guidance, guide wire was introduced and breaking of the web was performed by …

Sickle Cell Disease In Pregnancy. Obstetric And Anesthetic Management Perspectives, Khalil E. Rajab, Jonathan H. Skerman
Saudi Medical Journal
Recent advances in the pathophysiology, clinical investigations, and management of sickle hemoglobinopathies enables all physicians to better manage these disease states and their sequelae. Patients with sickle cell disease SCD are living longer and are thus more likely to contract unrelated diseases that require surgery and anesthesia. Patients with SCD continue to be a challenge to all branches of medicine particularly in obstetrics, surgery and anesthesia; however, the armamentarium of new knowledge and practice places a different perspective on the care of this old disease. In general, the literature to date suggests that neither prophylactic transfusion of pregnant sicklers nor …

Cancer-Related Anemia, Hikmat N. Abdel-Razeq
Cancer-Related Anemia, Hikmat N. Abdel-Razeq
Saudi Medical Journal
Anemia is the most common hematological abnormality in cancer patients, unfortunately, it is often under-recognized and under-treated. The pathogenesis of cancer anemia is complex and most of the time multifactorial; involving factors related to the tumor itself or its therapy. While anemia can present in a wide range of symptoms, involving almost every organ, it is believed that it contributes much to cancer-related fatigue, one of the most common symptoms in cancer patients. In addition, there is increasing evidence to suggest that anemia is an independent factor adversely affecting tumor response and patient survival. While blood transfusion was the only …

Ambulatory Blood Pressure Monitoring. A Pediatric Review, Aftab S. Chishti
Ambulatory Blood Pressure Monitoring. A Pediatric Review, Aftab S. Chishti
Saudi Medical Journal
Hypertension is one of the major contributors to cardiovascular, renal and central nervous system morbidity and mortality. Although it is more prevalent in the adult population, hypertension and its sequelae are being seen in the pediatric population with increasing frequency. Blood pressure (BP) is not a static phenomenon. It is highly variable, changing constantly in response to various activities, stimuli, and stresses. Consideration of all of these factors make intermittent clinic BP measurements less effective in accounting for BP rhythmicity so ambulatory BP monitoring has emerged as a useful tool to give a detailed analysis of BP patterns during the …
