The Decision Of Do Not Resuscitate In Pediatric Practice, Mohammed M. Jan
The Decision Of Do Not Resuscitate In Pediatric Practice, Mohammed M. Jan
Saudi Medical Journal
Cardiopulmonary resuscitation (CPR) is now routinely performed on any hospitalized patients who suffer cardiac, or respiratory arrest. Children with irreversible, or progressive terminal illness may benefit temporarily from CPR, only to deteriorate later on. Painful and invasive procedures may be performed unnecessarily, and the child could be left in a poorer condition. A do not resuscitate (DNR) order indicates that the treating team has decided not to have CPR attempted in the event of cardiac or pulmonary arrest. While there is relatively ample literature on this topic in general, there is comparatively little focus on DNR orders as they pertain …

Degenerative changes in the spine can result in the formation of osteophytes on the anterior surface of the cervical spine. Depending on their site, osteophytes can bring on clinical manifestations such as dysphagia, hoarseness, and stridor. We discuss an interesting case of a young adult patient who presented with dysphagia along with neck discomfort, and on investigation was found to be suffering from diffuse idiopathic skeletal hyperostosis. Here, we briefly portray the presenting features, radiographic findings, and management options.

Carnitine-acylcarnitine translocase (CACT) deficiency (McKusick 212138) is a rare life threatening disorder characterized by hypoketotic hypoglycemia, hyperammonemia, encephalopathy, cardiomyopathy hepatopathy, and myopathy. Here, we present a detailed clinical course of 3 Saudi siblings with a severe phenotype. The third patient was described in more detail. Early medical intervention in the form of 25% dextrose intravenous infusion and carnitine supplement followed by a gradual introduction of a high carbohydrate low fat special formula resulted in a good clinical and biochemical response to the treatment in our patient. However, early nephrocalcinosis, severe hypotonia, and subsequently intravascular cerebral accident could not be prevented. …
